[Clinical investigation of carbohydrate antigen CA-50].

The CA-50 enzyme immunoassay kit (EIA kit) that has been developed with the use of C-50 monoclonal antibody prepared by L. Lindholm et al. was evaluated for diagnosis of human cancer. The levels of CA-50 in the sera were determined using this kit supplied from Mitsui Pharmaceuticals, Inc. Co. in 759 healthy donors, 728 patients with benign disease and 1,263 untreated patients with cancer. A CA-50 concentration of 40 U/ml of serum was used as the cut-off value. Patients with pancreatic cancer and patients with bile duct cancer had high positive incidence of 75% and 68%, respectively, compared with a low positive incidence of under 40% in patients with other cancers. On the other hand, positive rates in patients with benign disease were as low as 13%. Comparison of the serum levels of CA-50 with CA19-9 in the same samples did not exhibit complete positive correlation in patients with pancreatic cancer, patients with bile duct cancer and patients with liver cancer. These findings indicated that C-50 antibody reacted with two epitopes of CA19-9 and sialosyllactotetraose. From the above results, the usefulness of CA-50 as a tumor marker for pancreatic cancer and bile duct cancer was recognized with this EIA kit.

Idiopathic portal hypertension associated with polyclonal hyperimmunoglobulinemia. A case report and review of the literature.

We report a case of idiopathic portal hypertension associated with prominent polyclonal hyperimmunoglobulinemia and plasmacytosis in the bone marrow and the spleen. Microscopic examination of the liver showed chronic inflammation in the portal area and abnormal vasculature adjacent to the portal tracts. In the spleen, polyclonal plasma cell proliferation was demonstrated histochemically in addition to the presence of remarkable sinus hyperplasia and periarterial fibrosis. In the present case, a chronic inflammatory state such as connective tissue disease was strongly suspected to exist in the liver, since pancytopenia and hyperimmunoglobulinemia persisted even after splenectomy. It is suggested that the pathogenesis in the present case may have been due to this chronic inflammatory state.

[A case of primary psoas abscess with paravesical extension].

We report a case of primary psoas abscess with paravesical extension. A 16-year-old man who had suffered from remittent fever for two months was admitted to our clinic. He had no urological symptoms or walking disturbances, but ultrasonography showed a well-defined echoic mass at the right paravesical space. Computed tomography confirmed this finding, and right psoas muscle appeared to be enlarged with irregular low density areas. Percutaneous drainage for right paravesical abscess was performed, and 15 ml of pus was obtained. The bacterial and fungal culture of the specimen resulted in no growth of any organisms. Six days after percutaneous drainage, he received en bloc resection of right paravesical abscess with segmental resection of bladder wall and peritoneum adherent to it. Furthermore, the affected region of the right psoas muscle was salvaged as highly upward as possible. Surgical specimen disclosed, pathologically, a non-specific granuloma with stellate necrosis. This finding suggested a feature of cat-scratch disease, tularemia, lymphogranuloma venereum and Yersinia infection. We reviewed the related literature on diagnostic images such as CT, ultrasonography and 67Ga scintigraphy. Moreover, bacterial causes and therapy of primary psoas abscess are discussed.

[Chemolysis of cystine calculi incarcerated in the ureter by the alkaline organic amine, tromethamine-E].

We dissolved the cystine calculi incarcerated in the ureter by irrigation with tromethamine-E via percutaneous nephrostomy. Case 1: A 25-year-old man with a past history of cystinuria and staghorn calculi in the right kidney, suffered from obstructive renal failure caused by the incarceration of a stone (18 x 15 mm) in the left middle ureter. After recovering promptly from renal failure by left percutaneous nephrostomy, he received continuous irrigation with tromethamine-E through an angiographic catheter percutaneously placed in the ureter. The ureteral calculus was reduced by 70% in size after the irrigation for 35 days, and then extracted percutaneously. Case 2: A 3-year-old child had multiple cystine calculi in the right renal pelvis and ureter. Although a calculus in the renal pelvis was extracted by the percutaneous ultrasound lithotripsy, two calculi incarcerated in the middle ureter were impossible to remove by a percutaneous approach. The irrigation with tromethamine-E was initiated through the catheter placed percutaneously in the right ureter. Both calculi completely dissolved 47 days later. The chemolysis by irrigation with tromethamine-E was greatly valuable in the treatment of cystine calculi. This dissolution could be an alternative to surgery especially in the treatment of ureteral cystine calculi, which might be difficult to be extracted by the percutaneous or transurethral approach.

Spergualin: a new antitumour antibiotic.

Spergualin was isolated from the culture filtrate of Bacillus laterosporus as an antitumour substance. It had a unique structure and was shown to have chemotherapeutic effects on mouse transplantable leukaemias such as L-1210, P-388, P-815, C-1489, EL-4 and RL male 1. It was especially effective to L-1210 leukaemia and the leukaemia-bearing mice were even curable by the optimal dose of this drug. When the spergualin-treated cured mice were inoculated again by L-1210 cells, those leukaemic cells did not grow in the animals suggesting that specific immunity to L-1210 had been induced. In this induction of immunity cytotoxic T lymphocytes were suggested to be involved. Cytostatic effect of spergualin in cell culture was dependent on the content of amine oxidase in serum. In the study of structure-activity relationship, the 15-hydroxy group was found to be not necessary, while the spermidine moiety was essential for antitumour activity. 15-Deoxy derivative of spergualin was found to be more potent in antitumour activity.

Salivary immunoreactive human epidermal growth factor (IR-hEGF) in patients with peptic ulcer disease.

EGF is reported to have a potent protective effect on peptic ulcer formation in rats. In this study, we measured the IR-hEGF concentrations in the saliva of normal human subjects and patients with peptic ulcer disease or non-peptic ulcer gastroduodenal disease. In normal subjects, the level of salivary IR-hEGF was highest in the early morning, and the values in individuals on different days showed small variations. There were no sex differences or age-related changes in the salivary IR-hEGF levels. The concentrations of the peptide were lower in patients in the active (0.96 +/- 26 ng/ml, mean +/- SE, n = 4) and healing stages (1.06 +/- 0.24 ng/ml, n = 8) of peptic ulcer disease as compared with those in normal subjects (3.19 +/- 0.46 ng/ml, n = 47). No significant differences in salivary IR-hEGF levels were observed between normal subjects and patients in the scaring stage of peptic ulcer disease (2.40 +/- 0.42 ng/ml, n = 21), or those with gastric cancer (2.44 +/- 0.27 ng/ml, n = 21) and atrophic or superficial gastritis (2.31 +/- 0.34 ng/ml, n = 28). Although the pathophysiological significance of these lower salivary IR-hEGF levels in patients with peptic ulcer disease is unclear, it is possible that the low level of hEGF in saliva may decrease the resistance of the mucosa to physicochemical stress, and thus participate in the development of the diseases.

In situ inhibition of tyrosine protein kinase by erbstatin.

Erbstatin inhibited the kinase activity of the receptor for epidermal growth factor in cultured A431 cells, while it did not alter turnover of the receptor protein. It also inhibited autophosphorylation of the src gene product p60src in Rous sarcoma virus-infected normal rat kidney cells. Erbstatin did not inhibit the binding of epidermal growth factor to its receptor but did inhibit internalization of epidermal growth factor-receptor complexes. It did not inhibit the epidermal growth factor-stimulated phosphatidylinositol turnover in A431 cells. Thus, erbstatin inhibited two oncogene product-related tyrosine protein kinases in situ and thus is a useful tool to study the role of tyrosine protein kinase.

Effect of forphenicinol, a low molecular weight immunomodifier, on the growth of and the immune responses to murine lymphoma EL4.

Oral administration of forphenicinol, S-2-(3-hydroxy-4-hydroxymethylphenyl)glycine, inhibited the growth of EL4 lymphoma by both pre- and posttreatment. Although some mice given this drug rejected the tumor, they could not obtain acquired immunity against the tumor. Macrophages taken from normal mice did not inhibit the growth of EL4 cells in vivo (tumor neutralization assay) or in vitro (cytostasis assay). Forphenicinol induced effector activity in such macrophages neither in vitro nor in vivo. Forphenicinol administration to mice bearing EL4 activated macrophages to inhibit the growth of EL4 in vitro and in vivo. Moreover, it stimulated the tumor growth-inhibitory activity of splenocytes in both tumor-free and tumor-bearing mice.

[Spergualin, a new antitumor antibiotic].

Spergualin is a new antitumor antibiotic which has a very unique structure. It was shown to have chemotherapeutic effects on mice inoculated with various mouse leukemias such as L-1210 (IMC), L-1210, P388, P815, C1489, EL4 and RL male 1. Among these, spergualin was especially effective against L-1210 (IMC) tumors, where the mice were completely cured at an appropriate drug dosage. These cured mice rejected the second challenge of L-1210, but not other tumors, indicating that specific immunity had been induced by spergualin. Preliminary data suggested that killer T cells were acting as effector cells. There was some correlation between the cytocidal activity of spergualin and the concentrations of amine oxidases in the serum. These results suggest that the induction of tumor-immunity plays an important role in the antitumor activity of spergualin.
