Elevation of laminin and beta-subunit of prolyl 4-hydroxylase in the sera of human subjects with Schistosomiasis mansoni.

Serum levels of several markers for liver fibrosis were measured utilizing three groups of human subjects related with schistosomiasis mansoni in northeast Brazil; (1) 20 Schistosoma mansoni egg-positives, who have never been administered with anti-schistosomal drugs, (2) 29 egg-negative inhabitants in the endemic area of schistosomiasis, and (3) 23 egg-negative Japanese immigrants in the non-endemic area. None of these sera were positive for antibody to the surface antigen of human hepatitis B (HBs) and circulating HBs antigen. There was no significant difference in the serum levels of N-terminal peptide of procollagen type-III between the egg-positive subjects and either of the egg-negative Brazilian or Japanese immigrants, whereas the mean value of serum laminin significantly increased in the egg-positive subjects. A significantly higher concentration of serum immunoreactive beta-subunit of prolyl 4-hydroxylase (IR beta PH) was also observed in the egg-positive subjects only in comparison with that of the egg-negative Brazilian. Serum laminin and IR beta PH concentrations of the egg-positive subjects did not correlate with the absorbance of enzyme-linked immunosorbent assay (ELISA) which utilized crude antigens isolated from schistosome adults or eggs. No significant difference in these two parameters was observed between two subgroups of the egg-negative Brazilian or Japanese immigrants divided according to the serological data by ELISA. These findings suggest that serum laminin and IR beta PH levels are worth further evaluation for their usefulness as the marker for liver fibrosis in schistosomiasis.

[Localization of pancreatic insulinomas by combined radiographic methods].

Seventeen cases with a total of eighteen insulinomas were examined by ultrasonography (US), computed tomography (CT), angiography, percutaneous transhepatic portal and pancreatic venous sampling (PTPS) and intraoperative ultrasonography (IOUS). We discussed about diagnostic accuracy of insulinomas on these diagnostic modalities. Tumor localization was achieved by US in 39%, CT in 72% and angiography in 81%. Although total 15 tumors (83%) can be correctly detected, multiple lesions were not completely ruled out. We recommended the criteria in PTPS as follows: Insulin concentration level above 200 mu U/ml. Or, both of insulin concentration ratio (step-up site/control) above three and elevation of the CPR concentration level on the PTPS. According to these criteria, the accuracy rate was 91%. Overall preoperative diagnostic accuracy was 100%. IOUS allowed us to detect the localization of the tumors completely.

Synthesis of imidazo[4,5-d][1,3]thiazine ribosides and related compounds of biological interest.

Several tri-O -acetyl-1-beta-D -ribofuranosyl- and 2-acetoxyethoxymethylimidazo[4,5-d][1,3]thiazine derivatives 5-10 were synthesized from imidazo[4,5-d][1,3]thiazine 4 and tetra-O -acetyl-beta-D -ribofuranose or 2-acetoxyethoxymethyl acetate by fusion method. In each case, depending on the substituent on position 5 of thiazine skeleton, only N3 isomer or both N1 and N3 isomers (in different ratios) were obtained.

ESR behaviour of spin-labelled oligoDNAs.

Oligodeoxyribonucleotides (oligoDNAs) were spin-labelled at the 5'-end internucleotide linkage, directly or through the spacers of different lengths, with TEMPO and the labelled oligoDNAs were explored for the ESR spectroscopical behaviour in solution. Oxidation of H-phosphonate intermediates in CCl4 solutions of TEMPO and diamines was taken for the introduction of TEMPO and spacers, respectively. ESR lines of TEMPO attached to oligoDNAs, directly or through spacers, were already broadened compared to those of free TEMPO, and the broadening decreased with increasing chain length of the spacer and increased with increase in the chain length of the oligoDNAs. The ESR lines of the oligoDNA labelled directly were further broadened in the presence of a DNA complementary to the labelled oligoDNA. In addition, the broadened lines were further broadened in the presence of a larger DNA. This observation implies that spin labelling at the internucleotide linkage is useful in the DNA probe method to avoid B/F separation.

Mercury level and histochemical distribution in a human brain with Minamata disease following a long-term clinical course of twenty-six years.

Details are given of the mercury level and deposition in a human brain with Minamata disease following a twenty-six year clinical course after the first severe attack in 1956. This is the first report of a case in which the methylmercury level within the brain has returned to normal limits in a severely affected victim. However, the total mercury remained high in the brain, and mercury was clearly demonstrated histochemically in microglial cells or macrophages over wide areas of the brain and in neurons of specific brain areas. Bergmann's glial cells also contained mercury deposits, whereas Purkinje's cells had relatively little or no deposition. Mild deposition of the metal was demonstrable in the epithelial cells of the choroid plexus.

[Erythropoiesis and hemolysis in hemodialysis patients].

In order to investigate the pathogenesis of renal anemia, erythroid marrow cellularity, factors affecting erythropoiesis and hemolysis, hemolysis starting point by Parpart method and red cell life-span were studied in 21 patients undergoing hemodialysis (HD). Mean value of serum erythropoietin level (EPO) in HD patients was 28.4 mU/ml, which value was nearly equal to that in healthy subjects. Total erythroblast count was higher than normal up to 25.2% in HD patients with Ht below 25% (A group), on the other hand, in HD patients with Ht above 25% (B group) it was 21 6%, nearly equal to normal. Total erythroblast counts positively correlated to EPO level, but did not correlate to ribonuclease, aluminium and parathyroid hormone. Red cell life-span was 23.4 days in A group, and it was 19.8 days in B group Hemolysis starting point was observed at 0.61% NaCl in B group, and at 0.56% in A group. Hemolysis starting point negatively correlated to red cell life-span, but did not correlate to BUN, serum creatinine and serum guanidino compound. Hb level negatively correlated to nuclear cell counts of bone marrow in HD patients, and positively correlated to hemolysis starting point. These results suggested that erythroblast count was controlled by both erythropoietin and hemoglobin levels in HD patients. Hemoglobin level in HD patients was maintained by balance of counteracting factors between erythropoiesis and hemolysis.

[A case of advanced bladder cancer successfully treated with intraarterial chemotherapy].

A 79-year-old Japanese male came to our hospital with complaints of protraction and macroscopic total hematuria. The medical imaging methods revealed a large papillary tumor in the left lateral wall of the bladder with the staging of T3N0M0. The pathological diagnosis was transitional cell carcinoma, G3. He received intraarterial chemotherapy with CDDP, ADM and MTX, which was called IA-MAC regimen. After two IA-MAC, no medical imaging method could reveal the tumor. The biopsied specimen, which was taken using the transurethral resection of the bladder mucosa, demonstrated no malignant cells. The new chemotherapeutic regimen "IA-MAC" is useful for the treatment of localized advanced bladder cancer in clinical use and enables one to preserve the organ.

[A case of multiple sclerosis with pure alexia].

We report here a case of multiple sclerosis (MS) showing pure alexia. The patient was a 28 year-old, right-handed male student of medicine. He had been healthy prior to college, but when he tried to read the newspaper one morning in January of 1983, he suddenly became aware that he could not. He could speak fluently and had no disturbances of auditory comprehension. No signs of abnormality were noted in his writing ability. Thereafter, the patient occasionally experienced difficulty in reading, together with right homonymous hemianopsia. These symptoms, however, usually vanished following sleep. In April, 1984, the patient experienced headache and ataxia when walking, followed by repeated remission and exacerbation of the symptoms. Corticosteroid therapy produced complete disappearance of the headache and ataxia. In September of 1985, he was hospitalized at our department due to the clouding of consciousness accompanied by convulsions. Neurologically, no abnormalities of the ocular fundus were noted and Babinski's sign was negative, but left hemiplegia was observed. Neuropsychologically, his intelligence was normal and he had no difficulty in writing, but paralexia was noted. There were no disorders of spontaneous speech or auditory comprehension. Examination by CT scan and magnetic resonance imaging revealed a number of lesions in the white substance of the cerebrum, including the left occipital lobe. The abnormal signs seen radiographically vanished when cortical hormones were administered. It has been thought that the symptoms of multiple sclerosis are due mainly to disorders of the white substance, and that MS seldom produces symptoms of aphasia, due to the fact that the lesions in this disease are generally small.(ABSTRACT TRUNCATED AT 250 WORDS)

[Prognostic estimation of cerebral arterio-venous difference of oxygen content (c-AVDO2) on severe head injury patients].

UNLABELLED: Cerebral arterio-venous difference in oxygen content (c-AVDO2) was calculated in 10 patients suffering from severe head injury with brain contusion as the chief morbid condition [consciousness on admission: under 10 on the Glasgow Coma Scale (GCS); 5 survived and 5 died), in order to re-evaluate the importance of c-AVDO2 as a factor of prognosis evaluation in patients with severe head injury. Indwelling catheters were placed in the femoral artery and jugular vein on the side of the brain contusion in all patients on admission, and blood samples were collected when necessary for calculation of c-AVDO2. Whenever blood samples were collected, GCS-based evaluation of consciousness and determination of auditory brainstem response (ABR) (a V-wave latency period of 5.67 + 0.40 msec. or more was considered to indicate prolongation) were performed. In 6 patients (2 survived and 4 died) who underwent monitoring of epidural pressure (EDP), this was confirmed on blood sample collection. c-AVDO2 was (1) compared between patients who survived and those who died, and studied in terms of (2) consciousness, (3) ABR and (4) EDP. RESULTS: (1) c-AVDO2 was 8.23 +/- 0.78 ml% in patients who survived and 4.94 +/- 0.71 ml% in those who died. (2) c-AVDO2 in patients with GCS values of 15-11, 10-7, and 6-3 was 7.85 +/- 1.41 ml%, 7.70 +/- 1.53 ml% and 5.09 +/- 1.27 ml%, respectively. (3) c-AVDO2 when the V-wave latency periods of ABR were less than 6.07 msec. and greater than or equal to 6.07 msec. was 7.13 +/- 1.74 ml% and 6.51 +/- 2.29 ml%, respectively, and when the latency disappeared c-AVDO2 was 3.74 +/- 0.62 ml%. (4) c-AVDO2 at EDP values of 10-40 mmHg, 41-70 mmHg and 71 mmHg or more was 6.99 +/- 1.53 ml%, 6.23 +/- 2.36 ml% and 4.15 +/- 0.35 ml%, respectively. CONCLUSIONS: (1) The c-AVDO2 in the patients who died was significantly lower than that in the patients who survived. (2) All of the patients who showed a c-AVDO2 of 5 ml% or less at least once dies, suggesting that the critical point of poor absolute prognosis is 5 ml%.

Suppression of the activities of T-lymphocyte-related enzymes in spleen by administration of an immunosuppressant, 15-deoxyspergualin.

We investigated the effects of both (-) and (+)-enantiomers of 15-deoxyspergualin (DSG), an immunosuppressant, on enzyme networks in spleens of ICR mice. Of the 14 hydrolytic enzymes tested, the activity of dipeptidyl aminopeptidase IV (DAP-IV) was found to be significantly suppressed by the administration of (-)-DSG but not by that of (+)-DSG. In contrast, the activity of N-acetyl-beta-D-glucosaminidase (GlcNAc-ase) was suppressed by the administration of both enantiomers of DSG. Judging from the previous reports of ours and others which suggested specific relations of these enzymes to T-lymphocytes, the above findings may warrant further studies of these enzymes in relation to immunologic functions of the body.

[Malignant melanoma of male urethra: a case report].

A case of male urethral melanoma is reported. A 85-year-old male with a 2-month history of progressive, severe obstructive urinary symptoms and bloody urethral discharge was referred to us after an unsuccessful management at a local doctor. Physical examination revealed an ill looking old man with no evidence of nevi or other cutaneous pigmentation looking like malignant melanoma. Neither palpable periurethral mass nor inguinal lymphadenopathy was noted. RUG showed an irregular shadow defect in bulbous urethral regions. In cystourethroscopy, a raised nodular reddish black lesion in the urethra without adjacent satellite lesions was found. Histologic examination revealed that the tumor was made up of closely spaced, anaplastic, spheroidal or polyhedal cells. Intracellular brown pigment was richly present, gave a negative reaction for iron, but stained black with Masson-Fontana's method. Further examination for evaluating metastases including bone scintigraphy, computer tomographic scan, chest X-ray film were negative. Due to his poor risk, radical operation such as cystourethrectomy might be undesirable. We performed TUR to relieve urethral obstruction, because the patient refused cystostomy. He died of wide spread metastases at 6 months after the operation. This case seems to be the second report in the Japanese literature.

[Hemagglutinating activity of Escherichia coli isolated from the respiratory tract in comparison with those isolated from urine feces and blood].

Various bacteria with pili are able to agglutinate human and animal red blood cells. Hemagglutinating activity of 131 strains of Escherichia coli isolated from respiratory tract (24 strains), urine (64 strains), feces (36 strains) and blood (7 strains) were tested using human type A, guinea pig, bovine and chicken erythrocytes. Concerning the hemagglutination activity for erythrocytes from at least one of four species (human, guinea pig, bovine, chicken), the strains isolated from respiratory tract showed a higher level than those isolated from feces (p less than 0.01) or those isolated from blood (p less than 0.1). Agglutination of human erythrocytes: Of 131 strains, mannose-sensitive agglutination was observed in 31 strains, mannose-resistant agglutination in 40 strains and non-agglutination in 60 strains. More agglutinated strains were isolated from the respiratory tract than those isolated from blood but not with statistical significance (p less than 0.1). Agglutination of guinea pig erythrocytes: Of 131 strains, mannose-sensitive agglutination was observed in 56 strains, mannose-resistant agglutination in 10 strains and non-agglutination in 65 strains. More agglutinated strains were isolated from the respiratory tract than from urine, feces and blood (p less than 0.01), and of those, 89% were mannose-sensitive agglutination. Agglutination of bovine erythrocytes: Of the 131 strains, mannose-sensitive agglutination was observed in 4 strains, mannose-resistant agglutination in 6 strains and non-agglutination in 121 strains. [A case of testicular tumor presenting as acute scrotum].

A case of teratoma of the testis presenting as sudden spontaneous hemorrhage without previous injury is described. A 25-year-old male was admitted with right scrotal pain and swelling. Though acute epididymitis or testicular torsion could not be neglected on physical examination, tumor-like echogram was obtained. High orchiectomy was performed subsequently. Macroscopically, testicular tumor with subcapsular hematoma was evident. Histopathological diagnosis was mature teratoma (pT1, No, Mo). The patient, after combined chemotherapy including cisplatin, vinblastine and peplomycin, is alive well without metastases for 15 months after operation.

[Antenatal detection of developing nervous system abnormalities and perinatal surgical management].

As the use of prenatal ultrasound sonographic study becomes more popular, it is likely that several fetal anomalies of the central nervous system will be diagnosed in utero more frequently and accurately. Obstetric sonogram correlates well with the results of postnatal CT scanning. Now, with improved diagnosis and support facilities for the newborn, neurosurgical intervention is being requested, as more concern is expressed for such fetus. We have experienced seven cases of fetal disorders of the central nervous system-meningocele, meningoencephalocele, myeloschisis, arachnoid cyst and maximal hydrocephalus. When diagnosed late in gestation (after the 2nd trimester), it may be reasonable to consider early delivery at the time of pulmonary maturity, and extrauterine management to decrease secondary damage to the developing central nervous system. The method of delivering a fetus must be tailored to suit each case. We present our program for the management of fetal hydrocephalus-placement of a subcutaneous low profile Ommaya's reservoir for neonate immediately after birth, and then subsequently conversion to a ventriculoperitoneal shunt three or four weeks later. As advances are made in antenatal diagnostic techniques and appropriate therapy, prognostic data on congenital abnormalities of the central nervous system will increase and become more pertinent.

[Intrahepatic portal vein aneurysm: report of two cases].

We present two cases of intrahepatic portal vein aneurysm. The lesions were seen as low density mass on precontrast CT, hypoechoic mass with connecting vascular structures on ultrasonogram and dilatated branch of portal vein on MRI. Definite diagnosis were obtained by arteriogram. Only 15 cases of portal vein aneurysm were reported in Japan up to now in the literature.

[Blind-ending bifid ureter: report of a case].

A case of blind-ending bifid ureter is presented. A 65-year-old man was admitted with the complaint of dysuria. He had no past episode of left flank pain or pyelonephritis. Digital examination and urethrography suggested benign prostatic hypertrophy. Drip infusion pyelography showed an abnormal cavity at the lower portion of the left ureter. He was diagnosed as benign prostate hypertrophy and left blind-ending bifid ureter. During suprapubic prostatectomy, the bifid ureter was resectd. The related reports are reviewed in the Japanese literature.
