[Application of fuzzy inference to personal identification--sex determination from teeth].

Fuzzy inference has received much attention recently as a new type of computer control technology for application in various fields. In this study I applied fuzzy inference to personal identification in medical jurisprudence. Sex determination program was constructed from teeth using the fuzzy inference technique and was compared with known discriminant analysis. The materials examined were mandibular and maxillary dental plaster models of 100 adult males and females, and 80 infant males and females. For the permanent teeth, measurements were made on crown width and thickness of the mandibular and maxillary central incisors, canines, first premolars, and first molars, and of the width and length of the maxillary dental arch. For the deciduous teeth, measurements were made on crown width and thickness of all teeth. Stepwise discriminant analysis was conducted using these values. The values from 5 selected upper-ranked items were set as input objects of the fuzzy inference program and the probability of maleness obtained from the discriminant analysis was set as the output object. Finally, sex determination program for both permanent and deciduous teeth was constructed using a fuzzy inference software development tool. Each measured value was input into this program and the output results were compared with those of discriminant analysis. The percentages of correct determinations for permanent teeth were 83.0% for males and 86.0% for females using stepwise discriminant analysis, however it increased to 93.0% for males and 89.0% for females using the fuzzy inference program. The percentages of correct determinations for deciduous teeth models were 67.5% for males and 75.0% for females using stepwise discriminant analysis, and increased to 86.3% for males and 81.3% for females using the fuzzy inference program. Among samples with probabilities between 40% and 60%, 3 out of 14 males and 4 out of 13 females were misjudged using stepwise discriminant analysis in cases of permanent teeth. However using fuzzy inference program, it reduced to 0 for males and 3 for females. In case of deciduous teeth, 9 out of 20 males and 9 out of 23 females were misjudged using stepwise discriminant analysis. But it reduced to 5 for males and 4 for females using the fuzzy inference program.

[Gallstone classification and analysis of their constituents].

Macroscopic classification of gallstones as proposed by the Japanese Society of Gastroenterology is based on the presence of characteristic structures on the cut surface. According to this classification, gallstones can be broadly divided into three types: cholesterol gallstones, pigment gallstones and rare gallstones. This classification is widely accepted in Japan because of its clinical applicability. A more accurate classification requires analysis of the constituents of gallstone. In particular, infrared absorption spectroscopy has been utilized for this purpose because it enables a rapid and easy analysis, requiring only small amounts of sample. In the selection of patients with gallstones for non-surgical treatment, such as ESWL and dissolution therapy, qualitative diagnosis is necessary. With this in mind, we have attempted to produce CT-Profile curves of individual gallstones, depicting changes in CT values assessed along cross-sectional lines passing from the center of the stone to their maximum diameters. This method allowed qualitative diagnosis almost of all gallstones.

[The limits for modified Fontan operations with AV valve regurgitation--the effect of AV valve regurgitation on the Fontan operation for single ventricle].

We performed a modified Fontan operation on 14 patients of single ventricle with AV valve regurgitation (single right ventricle: SRV 8, single left ventricle: SLV 6) until 1990. During the same period, 41 patients of single ventricle without AV valve regurgitation underwent the modified Fontan operation. To evaluate the effect of AV valve regurgitation on the Fontan operation, operative mortality, ventricular and pulmonary functions were compared between two groups. No significant differences were observed in operative mortality (7% vs 12%). There were also no significant differences of post-operative cardiac index (mean 2.6 L/min/m2 vs 2.6 L/min/m2), pulmonary vascular resistance (mean 2.3 U.m2 vs 2.2 U.m2) and preoperative ejection fraction (mean 54% vs 62%) between two groups. Therefore, the hemodynamic status of single ventricle following the Fontan operation remained almost unchanged irrespective of the presence of AV valve regurgitation. This suggested that AV valve regurgitation was not itself a contraindication of the Fontan operation, if the correct indices and time for operation were selected.

Role of endogenous prostaglandins in secretin- and plaunotol-induced inhibition of gastric acid secretion in the rat.

We investigated the role of endogenous prostaglandins in the inhibitory effect of exogenous secretin and the antiulcer agent plaunotol on gastric acid secretion in the rat. Intravenous infusion of secretin (0.05 CU/kg/h) and intraduodenal administration of the secretin-releasing agent, plaunotol (320 mg/h), resulted in significant inhibition of gastric acid secretion stimulated by intravenous infusion of pentagastrin (0.3 micrograms/kg/h), and this was accompanied by an increase in the prostaglandin E2 content of the gastric mucosa. Intraduodenal administration of plaunotol (320 mg/h) produced plasma secretin levels comparable to the levels achieved by intravenous infusion of secretin (0.05 CU/kg/h). Intravenous administration of prostaglandin synthesis inhibitor, indomethacin (2 mg/kg + 1 mg/kg/h), completely abolished both the inhibitory action of secretin and plaunotol on gastric acid secretion, and the increase in gastric mucosa prostaglandin E2 induced by secretin and plaunotol. The results indicate that endogenous prostaglandins play a significant role in the inhibitory action of exogenous and plaunotol-released endogenous secretin in the rat.

Expression of tyrosinase gene in transgenic mice: programmed versus non-programmed expression.

The transgenic experiment is a useful tool for the study of cell type-specific expression of genes during embryogenesis. We constructed a minigene, mg-Tyrs-J, by fusing a tyrosinase cDNA, Tyrs-J, with the 5' upstream region of genomic deoxyribonucleic acid (DNA) clone, G3L, and microinjected this minigene into fertilized eggs from albino BALB/c mice. It was expected that a melanin pigment would be produced and deposited in the melanosomes of melanocytes of BALB/c mice if the mg-Tyrs-J was expressed in a cell type-specific manner. In the transgenic mice, melanin pigments were observed only in melanocytes and in hair shafts of hair follicles, and in the choroid and pigmented epithelium of the eyes. However, Southern blot analysis of the genomic DNA of the transgenic mice showed that the transgene was present in all tissues examined. These results apparently indicate that the introduced transgene is integrated into a chromosome(s) and distributed among somatic cells, whereas the gene is expressed exclusively in melanocytes, i.e., the transgene is expressed in a cell type-specific manner. Some founder mice were crossed with BALB/c albino mice to establish transgenic lines. Each line and subline shows inherited characteristic phenotypes; in particular, offsprings from two founders exhibited a patched phenotype. The possible mechanism of this interesting expression of these transgenes is discussed in comparison with the results of other investigators who observed both programmed and non-programmed expression of the tyrosinase gene in transgenic mice.

Anomalous origin of the left coronary artery from the right pulmonary artery: report of a case.

A 1-year-old boy was referred to our hospital for further investigation of mitral regurgitation. Color-Doppler echocardiography revealed the site of the anomalous origin of the left coronary artery which was confirmed by a cineangiographic study. This case is interesting for several reasons: the first is that the anomalous left coronary artery originated from the right pulmonary artery; the second is that two-dimensional echocardiographic findings can be misleading, i.e., a vessel-like structure which seemed to be the left coronary artery arising normally from the aorta was visualized; and the last is that a two-dimensional color-Doppler echocardiogram was useful in the diagnosis of the anomalous origin of the left coronary artery. The patient underwent implantation of the anomalous left coronary artery as well as Kay's plasty of the mitral valve, and has been doing well since then.

[Successful two-staged Jatene operation for severe right ventricular dysfunction and tricuspid regurgitation after Senning operation--a case report].

We report a seven-year-old boy who underwent a successful Jatene procedure seven years after Senning procedure. The modified Senning operation was performed for the treatment of transposition of the great arteries with intact ventricular septum at three years of age. However, he gradually showed the progressive right ventricular failure with tricuspid regurgitation after operation. When he was referred to our hospital, he was in severe right ventricular failure and showed massive tricuspid regurgitation simultaneously with the moderate degree of left ventricular failure. Staged pulmonary artery banding was applied to prepare the left ventricle for Jatene procedure in spite of the presence of left ventricular failure. The systolic pressure ratio of left ventricle to right ventricle was elevated to 0.91 by this banding although the left ventricular ejection fraction decreased from 43% to 30%. And Jatene procedure was successfully performed after three months of preparation period. Catheterization study after Jatene procedure revealed improved right and left ventricular functions with decrease of tricuspid regurgitation. We conclude that the Jatene procedure should be an ideal alternative in patient with right ventricular failure and/or tricuspid regurgitation after an atrial switch operation; the left ventricle could be prepared by an effective pulmonary banding in most instances.

[The establishment of a monolayer culture system of guinea pig chief cells and an enzyme immunoassay system for guinea pig pepsinogen].

For the purpose of studying pepsinogen secretion from gastric chief cells, we established a monolayer culture system of guinea pig chief cells and an enzyme immunoassay (EIA) system specific for guinea pig pepsinogen. Dispersed chief cells were obtained from gastric mucosa of a guinea pig using collagenase, GEDTA, and Percoll solution, suspended in DMEM/F-12 (1/1 containing 10% FCS) media, and cultured for 70hr. Then the monolayer culture system was established. Pepsinogen was purified from gastric mucosa of a guinea pig using DEAE-Sephacel and Sephacryl S-200 columns. Antibody to pepsinogen was raised by immunizing rabbit with the purified pepsinogen. A two-site EIA system was then established using beta-galactosidase-labeled Fab' antibody. The EIA system showed sensitivity to measure above 1.5ng of guinea pig pepsinogen, and the monolayer culture system responded well to secretagogues. These systems are useful for studying pepsinogen secretion.

[Surgical treatment of double orifice mitral valve in atrioventricular septal defect].

Ten patients with double orifice mitral valve and atrioventricular septal defect (AVSD) were examined. There were four cases with incomplete type, three with intermediate type and three with complete type AVSD. All patients with incomplete type AVSD were doing well after the mitral cleft repair, Leaving the accessory orifice untouched. Various valve repair procedures and prosthetic valve replacement were performed in the patients with intermediate and complete types of AVSD with left-sided atrioventricular valve regurgitation. There were three operative deaths, which included one patient with the intermediate type and two with the complete type. All of them were one-month old babies and were operated on emergency. Their general operative findings were hypoplasia of the common atrioventricular valve leaflets, abnormal attachment of the chorda tendineae and papillary muscle dysplasia. These findings increased the difficulty of the valve plasty to control mitral regurgitation.

Strength reductions from trabecular destruction within thoracic vertebrae.

An in vitro model of metastatic lesions in thoracic vertebrae was used to determine if the reduction in vertebral cross-sectional area could be used to predict the associated strength reduction. Defects, entirely within the trabecular bone, were created in alternating vertebrae of unembalmed human thoracic spines. The adjacent vertebrae were tested intact and served as controls. Defect size was determined as the cross-sectional area of the defect divided by the nominal cross-sectional area of the vertebral body midplane. Vertebrae were tested to failure using combined axial-flexion loads. For each spine, a linear regression was determined between the cross sectional area of the superior endplate and the load at failure for the intact vertebrae. The intact strength of bodies with defects was estimated from this regression. The normalized strength of thoracic vertebrae with trabecular defects was linearly related to the reduction in cross-sectional area (normalized failure load = 1.0-Ad/Ai, r2 = 0.51; Ad = cross-sectional area of defect; and Ai = intact cross-sectional area at midplane). The data suggest that the strength reduction due to lytic defects within the centrum of thoracic vertebrae is proportional to the cross-sectional area of bone resorbed.

Cloning and sequencing of the cDNA encoding tyrosinase of the Japanese pond frog, Rana nigromaculata.

We cloned and sequenced the cDNA encoding tyrosinase (TYN) of the Japanese pond frog, Rana nigromaculata. The 3511-bp cDNA contained a 54-bp 5'-noncoding region, a 1596-bp open reading frame encoding TYN of 532 amino acids (aa), and a 1861-bp 3'-noncoding region. The aa sequence of frog TYN predicted from the cDNA sequence was homologous to that of mouse and human TYNs. The aa sequence including the copper-binding domain, which is likely the active center of TYN, was highly conserved among these three species and Neurospora crassa, Streptomyces antibioticus, and S. glaucescens. The frog TYN also contains possible glycosylation sites and conserved Cys at sites similar to those in the mouse and human TYNs. There are two hydrophobic regions at the N-terminus and near the C-terminus, which are likely the signal (leader) peptide and a transmembrane domain, respectively.

Processing of mutated proinsulin with tetrabasic cleavage sites to bioactive insulin in the non-endocrine cell line, COS-7.

The amino acid sequence, Arg-4-X-3-Lys/Arg-2-Arg-1 decreases X+1, is thought to be a consensus processing site for a constitutive secretory pathway in non-endocrine cells. We created a mutant proinsulin DNA with a peptide structure of B chain-Arg-Arg-Lys-Arg-C peptide-Arg-Arg-Lys-Arg-A chain, which compares to the native proinsulin structure of B chain-Arg-Arg-C peptide-Lys-Arg-A chain. When the mutant insulin was expressed in a monkey kidney-derived cell line, COS-7, approximately 60% of the total immunoreactive insulin appeared as mature insulin in the culture medium. This conversion to the mature form was strikingly facilitated by co-expressing the mutant proinsulin with furin, a homologue of the yeast endoprotease, Kex2.

Deficiency of fibrinolytic enzyme activities in the serum of patients with Alzheimer-type dementia.

Previously we reported that there is a kallikrein deficiency in the cerebral tissue of patients with Alzheimer-type dementia. The present study was performed to investigate protease changes in the serum of these patients. The results showed that the kallikrein activity was normal, but that the activities of plasmin and urokinase were significantly low. The present findings indicate a derangement in the clotting and fibrinolytic systems in Alzheimer patients.
