Decatromicins A and B, new antibiotics produced by Actinomadura sp. MK73-NF4. I. Taxonomy, isolation, physico-chemical properties and biological activities.

New antibiotics designated decatromicins A and B were isolated from the culture broth of Actinomadura sp. MK73-NF4. They were purified by butyl acetate extraction, silica gel column chromatography, silica gel TLC and Sephadex LH-20 column chromatography. Decatromicins A and B inhibited growth of Gram-positive bacteria including methicillin-resistant Staphylococcus aureus (MRSA).

Decatromicins A and B, new antibiotics produced by Actinomadura sp. MK73-NF4. II. Structure determination.

The structures of decatromicins A and B that strongly inhibit the growth of MRSA were elucidated by the analysis of various NMR experiments. The planar structure was determined by 1H, 13C, COSY, HMQC and HMBC NMR spectra. The relative configuration of aglycone was elucidated by NOESY experiments and the absolute structure was determined by application of the modified Mosher's method. The absolute structure of glycosyl moiety was determined by X-ray analysis of the O-(p-bromobenzoyl) derivative.

The decrease of cytochrome c oxidase activity by 15-deoxyspergualin results in enhancement of XTT reduction in cultured cells.

Exposure to immunosuppressant, 15-deoxyspergualin (DSG) induced enhanced formazan producing activity from XTT (2,3-bis(2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-5-carb oxanilide, sodium salt) in cultured cells, but not from MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide). Formazan generation from XTT is known to be stimulated by cytochrome c oxidase (COX) inhibitors such as KCN and NaN3, whereas MTT reduction is not affected by them. So, the effect of DSG on COX was examined. DSG did not directly inhibit the enzyme, but the cellular enzyme activity was decreased by exposure to DSG. It was thought that stimulation of XTT reduction by DSG resulted from the decrease of cellular cytochrome c oxidase activity.

Vinylamycin, a new depsipeptide antibiotic, from Streptomyces sp.

A new depsipeptide antibiotic, vinylamycin, was isolated from the culture broth of an actinomycete strain. The producing organism, designated MI982-63F1, was identified as a member of Streptomyces. Vinylamycin was isolated from the culture broth by extraction with EtOAc and purified by crystallization from EtOAc. The structure of vinylamycin was determined by spectroscopic analysis and degradation studies. Vinylamycin showed antimicrobial activities against Gram-positive bacteria including MRSA.

[Successful treatment of metastatic bone cancer from colon with combination treatment (radiation and 5-FU); a case report].

An 80-year-old woman with metastatic bone cancer from ascending colon showed a remarkable response to combination therapy of 5-FU and radiation. 5-FU (500 mg/day) was administered daily through intra-arterial catheter. The total dose of 5-FU was 15.5 g. Radiotherapy was performed (total radiation dose, 45 Gy). After therapy, metastatic lesions were remarkably reduced in size according to various imaging techniques, and the serum CEA level dramatically decreased from 249 ng/ml to 5.2 ng/ml. Other tumor markers such as CA19-9, NSE, and CYFRA were also dramatically decreased. Various tumor-related symptoms, especially the pain caused by tumor invasion, were also decreased. Drug toxicity caused slight nausea and leucopenia. These results suggested that the combination therapy of 5-FU and radiation is useful for the pain caused by metastatic bone cancer.

[Intra-arterial chemotherapy via reservoir system for advanced bladder cancer and prostate cancer].

A clinical study was performed on the efficacy of intra-arterial chemotherapy using a reservoir system for advanced urological malignancies. The reservoir system was indwelted in the femoral subcutaneous layer by Seldinger's method. Fifteen cases of inoperable complicated advanced bladder cancer and 10 cases of postoperative local recurrent bladder cancer were administered intra-arterial chemotherapy using a reservoir system. Then, 23 cases of local relapsed prostate cancer and two cases of endpocrine-resistant prostate cancer were administered the chemotherapy. The administered anti-cancerous agents were methotraxate, cis-platinum and adriamycin, then 5-FU or carboplatin was administered as maintenance therapy. The mean number of courses of chemotherapy was six for bladder cancer and four for prostate cancer. During stabilization of the local lesion, no distant deterioration was recognized. The overall clinical efficacy was a positive response (PR) and no change (NC): for 18 and 7 cases of bladder cancer, and 11 and 14 cases of prostate cancer, respectively. The median duration of stabilization was 23 months for bladder cancer and 12 months for prostate cancer. The adverse effects were fever than those with systemic chemotherapy.

[A case of spontaneous pyeloduodenal fistula].

A 61-year-old woman visited our hospital complaining of right flank pain and fever. The radiograph demonstrated multiple renal calculi. Radio renography showed no uptake in the right kidney. Therefore, we diagnosed her with pyonephrosis, and recommended open nephrectomy. However, she selected the conservative treatment with extra corporeal shockwave lithotripsy (ESWL). In spite of disappearance of multiple calculi, pyuria continued for 3 months after ESWL. Retrograde pyelography showed a fistula from the right pelvis into the duodenum. The patient was successfully treated by nephrectomy and duodeno-fistelectomy.
