[Regional left ventricular diastolic dysfunction in patients with apical hypertrophy].

Differences in cardiac function between the apical and the chordal parts of the left ventricle in apical hypertrophy (APH) were investigated by M-mode echocardiography. The subjects consisted of 10 patients with APH (APH Group) and 10 normal controls (N Group). The M-mode echocardiograms of the interventricular septum (IVS) and left ventricular posterior wall (LVPW), both in the apical and chordal parts were simultaneously recorded with the electrocardiogram and phonocardiogram. There were no significant differences in the blood pressures, heart rates, left ventricular end-diastolic internal diameters, and left ventricular end-systolic internal diameters between the APH Group and the N Group. The hypertrophy was localized to the IVS and LVPW of the apical part in the APH Group. In the chordal part, there were no significant differences in the peak negative dD/dt (-dD/dt) and the time to the peak filling rate (TPFR) between the APH Group and N Group. In the apical part, -dD/dt of the APH Group tended to increase compared with that of the N Group. The TPFR of the APH Group was significantly longer than that of the N Group (APH Group: 167 +- 33 msec and N Group: 126 +- 19 msec, respectively).(ABSTRACT TRUNCATED AT 250 WORDS)

[The influence and outcome of acute infantile intracranial hematoma with hypoxemia].

Unusually broad areas of cerebral infarction were demonstrated by CT scan in three head injured infants with acute intracranial hematoma. They revealed very characteristic CT findings including contralateral hemispheric ischemic zone. Case 1 is a 5-month-old boy who had hit his head 4 days before. On admission he was semicomatose and his respiration had suffered from generalized seizures with arterial PO2 value of 43 mmHg. CT scan revealed right subdural hematoma, and bihemispheric ischemic low density was also demonstrable. Hematoma clot weighing 10 grams was removed through emergency craniotomy, followed by external decompression. There was a marked atrophic change in the right cerebral hemisphere and contralateral frontal base during the following few months, but the basal ganglionic region, brainstem and cerebellum were hardly affected. The patient developed comparatively well mentally for the next one and a half years. Case 2 was a 2-year-old boy who had a previous history of moderate head trauma 8 hours before admission. After a lucid interval, sudden epileptic attacks hospitalized him in a condition of cardiopulmonary arrest. CT scan revealed severe epidural hematoma on the patient's right cerebrum. Emergency craniotomy was performed and hematoma 95 g in weight was removed followed by decompression. Postoperative CT showed broad ipsilateral ischemic edema including the contralateral cerebral hemisphere and brainstem. One and a half years later, the patient shows decorticated posture with ataxic respiration and negative light reflexes. Case 3 was an 8-month-old boy who had fallen down and hit his head on the floor. Status epilepticus had attacked him, causing him to be admitted in a dyspneic state.(ABSTRACT TRUNCATED AT 250 WORDS)

Prostaglandins in type II collagen-induced ear diseases.

This study examines the role of prostaglandins in type II collagen-induced inner ear disease. Hartley guinea pigs were immunized with type II collagen in complete Freund's adjuvant. Auditory brain stem response was measured initially and at 2, 3, 4, and 6 months after immunization. The presence of PGE2 and 6-keto-PGF1 alpha in the perilymph was determined by radioimmunoassay. Perilymph from immunized animals contained about five times more PGE2 and about three times more 6-keto-PGF1 alpha than the perilymph from control animals. However, the two groups showed no difference in the level of either PGE2 or 6-keto-PGF1 alpha in cerebrospinal fluid and serum. Immunized guinea pigs also showed evidence of hearing loss. The temporal bones showed the following changes: spiral ganglion degeneration; mild to moderate degeneration in the organ of Corti; and infrequent and very mild endolymphatic hydrops. Thus, prostaglandins might play an important role in the pathogenesis of type II collagen-induced inner ear disease.

[Detection of left ventricular thrombi after acute myocardial infarction using Ga-67-DFO-DAS-fibrinogen].

Ga-67-DFO-DAS-fibrinogen (Ga-fbg) scintigraphy, a new radiopharmaceutical method, was performed for detecting intraventricular thrombi following acute myocardial infarction in five patients. The thrombi in four of them were detected by two-dimensional echocardiography (2DE) and that in the fifth patient was suspected during magnetic resonance imaging. Imaging of the heart was performed using a scinticamera with a medium energy collimator and multiple views (anterior, LAO 30 degrees, LAO 45 degrees, and lateral) three and four days after the intravenous administration of Ga-fbg. By Ga-fbg scintigraphy, intraventricular thrombi were detected in four patients. The size of the thrombi visualized by Ga-fbg appeared larger than those by 2DE. In one patient examined again after anticoagulant therapy, a thrombus was missed by 2-DE, but it was detected by Ga-fbg, though the radioactivity of the thrombus decreased. We concluded that Ga-fbg scintigraphy is a very simple method and sufficiently useful for detecting active left ventricular thrombi and for monitoring the effect of anticoagulant therapy. It could be more sensitive than 2DE for determining the extent of an active intraventricular thrombus.

[Therapy of advanced neuroblastoma by the protocol of the Welfare Ministry used by the Sawaguchi Group].

Five patients with advanced neuroblastoma were treated with the protocol of the Welfare Ministry used by the Sawaguchi group. This protocol consists of cyclophosphamide (CPM), vincristine (VCR), 4'-O-tetrahydropyranyladriamycin (THP-ADM) and cisplatinum (CDDP). Two patients treated with this protocol as the first therapy obtained partial remission. Two patients who had obtained complete remission before the treatment with this protocol remained in complete remission after the treatment. One patient who had shown poor response to the therapy given before treatment with this protocol also showed poor response to the protocol. The most significant side effect with this protocol was myelosuppression, the severity of which was closely related to the dose of THP-ADM. Damage to the liver, kidney and heart was not very significant. We advocate that patients with advanced neuroblastoma can obtain complete remission with the aid of surgery and radiotherapy, etc., if they are treated with the present protocol as the first therapy.

Effects of the mass screening of neuroblastoma in Sapporo City.

One hundred sixty-seven cases of neuroblastoma in the Registry of Childhood Malignancies in Hokkaido Prefecture from 1969 to 1984 were studied, using the age at diagnosis, the clinical stage at diagnosis, and the survival rate to assess the influence of the mass screening of neuroblastoma performed in Sapporo City since 1981. In Sapporo City, the capital of Hokkaido Prefecture, the condition of the three parameters improved significantly after the institution of mass screening (e.g., the 48-month survival rate improved--from 21.3% to 87.5%). In Hokkaido Prefecture (Sapporo City excluded), however, where the mass screening had not yet been performed, no significant change was observed in any of the three parameters (48-month survival rate changed only from 21.1% to 28.1%). Consequently, the improvement in Sapporo City was thought to be attributable to the effects of the mass screening.

Mode of disulfide bond formation of a heat-stable enterotoxin (STh) produced by a human strain of enterotoxigenic Escherichia coli.

To determine the modes of three disulfide linkages in the heat-stable enterotoxin (STh) produced by a human strain of enterotoxigenic Escherichia coli, we synthesized STh(6-18), which consists of 13 amino acid residues and has the same intramolecular disulfide linkages as native STh [(1985) FEBS Lett. 181, 138-142], by stepwise and selective formation of disulfide bonds using different types of removable protecting groups for the Cys residues. Synthesis of the peptide with different modes of disulfide bond formation provided three peptides consistent with standard STh(6-18) in their physicochemical and biological properties, thereby indicating that the disulfide bonds in STh(6-18) are Cys-Cys-Glu-Leu-Cys-Cys-Asn-Pro-Ala-Cys-Thr-Gly-Cys.

Decreased intracellular free magnesium in erythrocytes of spontaneously hypertensive rats.

Using 31p-NMR (the phosphorus nuclear magnetic resonance) spectroscopy, we measured intracellular free Mg levels in the erythrocytes of untreated (n = 7) and diltiazem-treated spontaneously hypertensive rats (SHR) (n = 8), and compared them with age-matched Wistar-Kyoto rats (WKY) (n = 10). The intracellular free Mg levels were significantly (p less than 0.01) decreased in untreated SHR compared with those in control WKY. A successful antihypertensive treatment with diltiazem increased the intracellular free Mg levels compared with untreated SHR (p less than 0.05). Furthermore, an inverse correlation was observed between intracellular free Mg levels and blood pressure levels in all groups (r = -0.48, p less than 0.01, n = 25). These observations suggest that abnormalities of intracellular Mg metabolism may be, in part, related to the development or the maintenance of hypertension in SHR.

Different karyotypic patterns in early and advanced stage neuroblastomas.

Of 23 untreated and 7 treated (relapsed) neuroblastomas, 14 (11 untreated, 3 treated) had modal chromosome numbers in the diploid (45 to 51), 9 (8 untreated, 1 treated) in the triploid (60 to 77), and 6 (3 untreated, 3 treated) in the hypotetraploid (81 to 88) range, and one (untreated) had hypertetraploidy (100). The near-or-pseudodiploid and hypotetraploid tumors were characterized by numerous structural abnormalities, most frequently of 1p, and frequent presence of double minutes or homogeneously staining regions. The near-triploid tumors were characterized by three almost complete haploid sets of chromosomes, and few structural abnormalities. N-myc amplification was found in five of the near-or-pseudodiploid or hypotetraploid tumors but in none of the near-triploid tumors. Most near-triploid tumors were found in infants at stage I or II, and the near-or-pseudodiploid or hypotetraploid tumors in children at stage II or IV mostly 1 year old or older. Among the untreated patients, all 8 with a near-triploid tumor were alive with no evidence of the disease, and the 11 with a near-or-pseudodiploid tumor had a median survival of only 376 days (P less than 0.05), 7 of the 11 being dead. Thus, the near-triploid patients had well recognized favorable prognostic factors and an excellent prognosis, and the near-or-pseudodiploid patients had unfavorable prognostic factors and a dismal prognosis. The hypotetraploid tumors seemed to have karyotypic and clinical features in common with the near-or-pseudodiploid tumors. We presume that the near-triploid tumors and the near-or-pseudodiploid or hypotepraploid tumors may constitute distinctly different subcategories within neuroblastomas.

Nuclear ribonucleoprotein immune complexes in pericardial fluid of a patient with mixed connective tissue disease.

We performed immunopathologic studies of the pericarditis present in a patient with mixed connective tissue disease. A large number of nuclear RNP (nRNP) immune complexes (ICs) were found in the pericardial fluid, but not in the serum. The pericardial small vessels had no deposits of IgG. These results suggest that locally formed nRNP ICs were closely associated with the pathogenesis of pericarditis in this patient.
