Changes in cytosolic 3,5,3'-tri-iodo-L-thyronine (T3) binding activity during administration of L-thyroxine to thyroidectomized rats: cytosolic T3-binding protein and its activator act as intracellular regulators for nuclear T3 binding.

Changes in the amount of cytosolic 3,5,3'-tri-iodo-L-thyronine (T3)-binding protein (CTBP) and its activator during administration of L-thyroxine (T4) to thyroidectomized rats were investigated. Thyroidectomy decreased the amount of CTBP in the kidney, whereas the activator was not significantly modified by thyroidectomy. The activator was increased by administration of T4 to thyroidectomized rats. The amount of CTBP was also increased by administration of T4. The activator increased the maximal binding capacity (MBC) without changes in the affinity constant for T3 binding in CTBP. A T4-induced increase in MBC in cytosol inhibited nuclear T3 binding in vitro by competition of T3 binding between CTBP and the nuclear receptor. These results suggest that thyroid hormone increases the capacity for cytosolic T3 binding through increasing the amount of CTBP and its activator, and that these increases play a role in regulating the amount of T3 that binds to its nuclear receptor.

Compensation of otolith function.

Horizontal eye displacement in total darkness during lateral tilting about the body axis was examined in 14 patients with unilateral loss of labyrinth function, including 8 cases of positional vertigo or dizziness and 6 symptom-free cases. The change in eye displacement following labyrinthectomy was also studied in 2 cases of labyrinthitis and one case of delayed hydrops. Seven out of 8 subjects suffering from positional vertigo or dizziness exhibited abnormal eye displacement on tilting to the affected side, while 5 out of 6 symptom-free subjects showed normal eye displacement. The correlation between anomalous eye displacement and the presence of subjective symptoms was statistically significant (p less than 0.05). Abnormal eye displacement following labyrinthectomy was readily noted within 2 weeks of surgery, but became almost undetectable 3 to 4 weeks postoperatively, in close accordance with a decrease in positional vertigo or dizziness. It was concluded that the measurement of eye displacement during lateral body tilt, although previously reported to be useful in the assessment of function, might also be employed to examine compensation for the loss of otolith function.

[Age and efficacy of new long acting calcium blocker, nisoldipine, is essential hypertension].

The relationship between the age and the clinical efficacy of a new long acting calcium entry blocker, nisoldipine, was investigated in essential hypertension using two double blind group studies, in which nifedipine or atenolol was used as a control drug. Following 4 weeks of the observation period, nisoldipine was administered 2.5 mg once a day and an increase in dose was available up to 5 to 10 mg per day for 12 weeks of the treatment period. One hundred fourty nine cases who received nisoldipine were divided into the following two groups; 97 cases of the middle aged patients under 59 years old (mean age of 49.5 years) and 62 cases of the elderly patients more than 60 years old (mean age of 64.8 years). Average dose of nisoldipine at 12 weeks of treatment period was 4.8 mg in middle age and 5.8 mg in elderly group. The positive antihypertensive effect was observed in 82.2% in the middle aged patients and in 83.6% in the elderly patients; there was no significant difference between the two groups. In each group, significant fall in blood pressure was observed after the treatment, although no significant difference in the fall of systolic and diastolic blood pressure in the treatment period was observed between the two age groups. The side effect was observed in 16 cases (16.5%) of the middle aged patients and in 7 cases (11.3%) of the elderly patients, but no significant difference in the incidence was observed. The significant deterioration of the clinical laboratory findings was not observed in both groups. The rate of usefulness (useful or above) was 74.7% in the middle aged patients and 81.5% in the elderly patients, and there was no significant difference between the two groups. These results indicate that a new long acting calcium entry blocker, nisoldipine, is effective and safe antihypertensive drug for the elderly as well as for the middle aged patients.

[Effects of interferon on the cell cycle of human hepatoma HLF cells analyzed by flow cytometry].

Growth inhibition by interferon (IFN) was investigated in human hepatoma HLF cells by use of flow cytometry to study the cell cycle. INF-alpha or -beta inhibited growth more than IFN-gamma. Use of either IFN-alpha or -beta and IFN-gamma at the same time inhibited growth more than with any one kind of IFN, but use of IFN-alpha and -beta together did not cause much inhibition. IFN inhibited growth by causing cells to accumulate in the S phase instead of moving on to the G2 phase. Accumulation in the S phase was less with IFN-gamma than with -alpha or -beta. It increased with the combination of IFN-alpha or -beta with IFN-gamma, but not with the combination of IFN-alpha and -beta.

[Exercise induced hypoxemia and exercise tolerance in patients with COPD and the benefits of oxygen supplementation].

In order to evaluate the benefits of O2 supplementation during exercise, slowly incremental treadmill exercise tests were performed twice with 30 minutes interval rest in fourteen patients with severe COPD. The patients breathed room air. 31/min of compressed air by nasal prongs, and 31/min of supplemental oxygen in single blind fashion at random. The patients who developed arterial desaturation below 88% on exercise, group D, showed slight but significant increase in walked distance (397 m vs 424 m) and significant decrease in breathlessness (22.9 vs 16.9) on oxygen as compared to on air. On the other hand in patients without significant arterial desaturation, group S, there was no improvement in those parameters. The increase in walked distance on oxygen was closely related with the decrease in mean inspiratory flow (VT/Ti), blood lactate level, and CO2 production at identical work load. Plasma human atrial natriuretic peptide (h-ANP) levels in group D increased with exercise from a resting value of 27.6 +/- 6.9 to 44.0 +/- 9.0 on compressed air whereas the increase was significantly suppressed to 35.4 +/- 9.0 on oxygen. In group S there was no difference in the increase of plasma h-ANP levels between air and oxygen breathing during exercise (33.1 +/- 5.1 vs 31.9 +/- 9.6). A close correlation (r = 0.908) was found between mean pulmonary artery pressures and plasma h-ANP levels at rest and during exercise performed in four patients breathing air and oxygen. Those findings suggested that arterial plasma h-ANP levels reflected the right ventricular afterload and that they could be used to evaluate the effectiveness of O2-supplementation during exercise.(ABSTRACT TRUNCATED AT 250 WORDS)

Immune responses in newly developed short-lived SAM mice. III. Genetic control of defective helper T-cell activity in in vitro primary antibody response.

Immune activities of newly developed, short-lived SAM-P/1 mice declined sharply after a few months of age. As early as 2 months of age, the activity of T-helper (Th) cells ('Th2'-like) in the in vitro primary antibody response was profoundly impaired, in contrast to normal activity of Th cells ('Th1'-like) engaged in cell-mediated immune responses. Thus, young SAM-P/1 mice show a functional heterogeneity of Th cells. To determine how such a 'Th2' abnormality is inherited in SAM-P/1 mice, immune activities of their hybrids and backcrosses between MHC-identical, high responder B10.BR mice were statistically assessed. The distribution of responses did not support the Mendelian single-gene determination for low responsiveness. Moreover, involvement of a single gene which exhibits incomplete dominance was ruled out because of a continuous distribution pattern of antibody response in the F2 generation. Such an analysis strongly suggests that the impaired 'Th2'-like activity of SAM-P/1 mice is under control of two genes, based on the proportion of low responders in F2 hybrids (29 out of 267, 10.8%) and on calculation according to Wright's formula (n = 1.72). Further linkage analyses suggest that one of the genes is closely linked to albino coat-colour (c) locus on chromosome 7. The putative two genes are likely to control 'differentiation' or 'maturation' of Th2-like cells defectively, but the defect is not refractory, because in vivo-primed Th cells function in vitro as do those in ordinary strains of mice. Possible mechanisms and biological significance in relation to loss of immune activity with ageing are discussed.

[Experimental model of respiratory distress syndrome by lung lavage with fluorocarbon].

The purpose of this study was to determine how much alveolar surfactant is washed out by lung lavage with fluorocarbon and also to find out whether or not instillation of artificial surfactant can restore pulmonary function after lung lavage in rats. The lung lavage was performed manually, administering a tidal volume 5 mg (2 mg/100g body weight) of fluorocarbon for about eight seconds. This process was repeated more than 40 times. In the study group, 4 ml/kg (120 mg/kg) of artificial surfactant was instilled into the trachea, and the same amount of normal saline was instilled in the control group. The amount of phospholipids extracted in lung lavage was 16.6 +/- 3.6 mg/kg body weight in the control group and 18.9 +/- 3.7 mg/kg body weight in the experimental group. Immediately after instillation of the surfactant, arterial oxygen pressure increased from 84 +/- 17 mmHg to 195.1 +/- 26.7 mmHg, and remained high, at about 170-260 mmHg. In contrast the physiological saline treated group did not show any change. We conclude that lung lavage with fluorocarbon is an adequate ideal experimental model of respiratory distress syndrome and suggests that combination of artificial surfactant treatment after lung lavage with fluorocarbon might be a new pulmonary washing method for severe lung disease.

[Malignant thymoma detected by thallium-201 myocardial scintigraphy].

We present two cases of malignant thymoma which showed a remarkable accumulation of thallium-201 chloride on myocardial scintigraphy. A 69 year-old man underwent stress 201Tl scintigraphy to evaluate myocardial ischemia and abnormal accumulation of thallium activity was observed in the anterior mediastinum by chance. It was more clearly visualized on delayed image. Moreover, abnormal uptake of 67Ga citrate was also observed in the same region. In another 68 year-old woman, there was high uptake on 201Tl scintigraphy, but no abnormal uptake using 67Ga. The diagnosis of malignant thymoma was confirmed by operation in each patient. These two cases had no abnormality in the mediastinum on the chest X-ray film and one of them had no uptake of 67Ga, 201Tl scintigraphy was more useful to detect malignant thymoma.

[The relationship between N-isopropyl-p-[123I]iodoamphetamine accumulation in the brain and its dynamics in other organs].

Single photon emission computed tomography of the brain and dynamic scintigraphy of the lung and liver with N-isopropyl-p-[123I]iodoamphetamine (IMP) were performed in 21 patients with and without cerebrovascular diseases to determine the relationship between IMP accumulation in the brain and its dynamics in other organs. The D/E ratios of the lung, liver and brain were calculated dividing the counts of the delayed image by that of the early image. The brain D/E ratio in the normal hemisphere was well correlated with the lung D/E ratio. So cases which showed more rapid lung washout tended to washout faster from the brain. We thought that this fact was affected by the systemic circulation. And the values of the brain D/E ratio of the cerebrovascular disease group were larger than that of the non-cerebrovascular disease group. This suggest the frequent existence of arteriosclerotic changes in patients of the cerebrovascular disease. Because the local brain washout was depended on the amount of the cerebral blood flow there. On the other hand, the lung D/E ratio was inversely correlated with the liver D/E ratio. In other words, more washout from the lung caused more accumulation of IMP in the liver. Also the brain D/E ratio was inversely correlated with the liver D/E ratio. In conclusion, there was a close relationship between IMP dynamics in the brain and that in other organs.

[Experimental focal cerebral ischemia produced by embolization with silicone cylinder in normotensive (NTR) and spontaneously hypertensive rats (SHR): comparison of neurological and pathological findings].

A focal cerebral ischemic model was produced by occlusion of the intracranial main cerebral artery with a silicone cylinder in normotensive (NTR) and spontaneously hypertensive rats (SHR). Main cerebral artery could be successfully occluded in approximately 90%. The most frequent embolized site was the distal part of the internal cerebral artery (ICb) and less frequently the horizontal segment of the anterior cerebral artery (Al). Mortality rate of NTR with ICb occlusion (NTR-ICb) was 43% at 72 hours after embolization and that of SHR with ICb occlusion (SHR-ICb) was 67% at 24 hours after embolization. NTR-ICb showed neurological signs (i.e. circling movement, hemiparesis, poor response to pain stimuli) and histologically, showed infarction in the deep cerebral structures (i.e. thalamus, hypothalamus, hippocampus, and internal capsule) accompanied with mild disruption of blood-brain barrier (BBB). SHR-ICb showed more serious neurological signs and more severe cerebral infarction in the deep cerebral structures with severe disruption of BBB. In SHR-ICb, ischemic cerebral edema was more prominent which may deteriorate symptoms and pathological findings compared to NTR-ICb. This embolization model is proposed to be useful for studying the pathophysiology of focal cerebral ischemia, especially, early ischemic edema.

[An epidemiologic study on patients with lung disease, using a data base management system and the histopathologic diagnosis].

A new pathologic data base management system was introduced in the Section of Pathology, Clinical Laboratory, of this hospital for an efficient sorting and searching past data and for the rapid epidemiological study of them. By the use of this system, the incidence and the age and sex distribution of various diseases were studied in a total of 1,264 patients received pathological examination for surgically operated specimens or biopsied materials at the Chest Disease Research Institute Hospital, Kyoto University, from January, 1986 to December, 1988. A gradual increase in the number of frozen section samples of surgically removed organs and of open lung biopsies was noted during this period. Adenocarcinoma, squamous cell carcinoma, small cell carcinoma and large cell carcinoma amounted to 42.5%, 32.9%, 12.2%, and 7.5% of primary lung cancers, respectively. The male-to-female ratio of patients with primary lung cancers was 2.9:1 but there was no significant difference in the age distribution among four types of the primary cancers in both sexes. Two hundred and ninety nine (32.6%) patients were diagnosed as non-neoplastic lung diseases: 121 infectious (13.2%), 69 granulomatous (7.5%), 62 pulmonary fibrotic (6.8%) and 47 miscellaneous non-neoplastic (5.1%) diseases.

[Clinical and laboratory studies in seven patients with pre-B cell leukemia in children].

We have experienced and treated seven patients of pre-B cell leukemia in childhood. Clinical, cytological and ultrastructural characteristics of them were studied. Most of them had higher counts of white blood cells, hepatosplenomegaly, high value of lactic dehydrogenase and various karyotype abnormalities at onset. The chromosomal translocation t (1; 19) that is supposed to be specific to pre-B cell ALL was found in four of seven of our cases. In the seven patients, survival was studied in comparison to that of 27 common ALL patients at our hospital that are common in childhood acute leukemia. Although no difference in remission duration and survival time between pre-B cell ALL patients and common ALL group, there have been seen the tendency that remission and survival were of shorter duration for patients with pre-B cell ALL.

Isoproterenol stress thallium scintigraphy for detecting coronary artery disease.

The value of exercise thallium scintigraphy in detecting coronary artery disease is well established. However, there are at times situations in which the exercise test cannot be readily used. Isoproterenol (ISP) stress ECG (ISP-ECG) is reportedly a useful method in diagnosing coronary artery disease. In the present study, we assessed the diagnostic value of ISP thallium scintigraphy, comparing it with those of ISP-ECG and exercise thallium scintigraphy. The study population consisted of 24 patients who had histories of chest pain without previous myocardial infarction. ISP was given at increasing doses of 0.02, 0.04, 0.08 micrograms/kg/min at 3-minute intervals, and was terminated for any of the following reasons: angina, significant arrhythmia, significant ST segment depression (greater than or equal to 0.1 mV) or target heart rate. Thallium scintigrams were obtained immediately after terminating ISP infusion, and after a 3-hour delay, redistribution scans were obtained. Scintigrams were considered positive when a reversible defect was present. In nine patients who underwent exercise tests, exercise thallium scintigraphy was also performed. After the stress tests, coronary angiography was performed. According to the presence or absence of significant coronary artery stenosis (greater than or equal to 75%), all subjects were divided into two groups: coronary artery disease (CAD) group (n = 12) and so-called normal coronary (NC) group (n = 12). 1. Among 12 patients in the CAD group, ISP induced anginal pain in six (50%), and ISP-ECG and ISP thallium scintigraphy were positive in 10 (83%) and in 11 (92%), compared with four (33%), four (33%) and two (17%) in the NC group.(ABSTRACT TRUNCATED AT 250 WORDS)

[Quantitative evaluations of left ventricular function obtained by electrocardiographically-gated magnetic resonance imaging].

Using electrocardiographically-gated magnetic resonance imaging, regional cardiac function was evaluated in 12 normal volunteers and in 10 cases of old myocardial infarction. The optimal short axis of the left ventricle was selected at the chordae tendineae level. The left ventricle was divided into 12 segments using a computer-aided system, and percentile shortening fraction (%SF) and percentile wall thickening (%WT) were calculated in each segment by the fixed coordinate method. In the normal volunteers, heterogeneity of both %FS and %WT was observed, ranging from 25 +/- 13% and 37 +/- 13%, respectively in the septal segment, to 49 +/- 13% and 60 +/- 21%, respectively in the posterior segment. In the cases of myocardial infarction, decreased %FS and %WT were detected at the affected regions. The abnormal regions revealed by %WT tended to be narrower than those revealed by %FS. Thus the MR technique at the optimal axis may be useful for quantitative evaluations of regional cardiac function.

[Cutaneous malignant lymphoma in childhood].

Two cases of malignant lymphoma in childhood were studied. The first case was a Japanese girl aged 8, in whom the primary site was skin of the right temple. The second case was a 4-year-old Japanese boy, who had metastases to the abdominal skin. Histochemical findings indicated B-cell lineage in both cases. Primary cutaneous lymphoma is extremely rare in childhood. Fourteen such cases that have been reported in Japan and our case added to them, were reviewed. The relationship between their morphologic, immunohistochemical and clinical findings were summarized and discussed. Although the prognosis of lymphoma confined to skin in childhood has been reported not to be bad as compared with other types of lymphoma, our first such case was fatal. This suggests that appropriate initial treatment is very important. Recent advances in science may clarify the clinical and biologic characteristics of this tumor in the near future.

Intrauterine depressed skull fracture. A report of two cases.

Two cases of intrauterine depressed fracture are presented. In each case, the mother had no history of abdominal trauma during pregnancy, the children were delivered normally without the use of forceps, but a round depression was present in the left frontal bone at time of delivery. CT scans demonstrated a depressed fracture of the left frontal bone and stagnation of cerebrospinal fluid around the fracture. Elevation of the depression was therefore made through a small incision along the rear edge of the frontal bone using a periosteal elevator. A review of intrauterine depressed skull fracture is made and the concept, mechanism, surgical indications and operative methods of this rare fracture are discussed.

[BCR rearrangement and cytogenetic findings in Philadelphia-positive chronic myelocytic leukemia in children].

Eight children with Philadelphia (Ph1) chromosome positive chronic myelocytic leukemia (CML) were available for cytogenetic studies and breakpoint cluster region (bcr) rearrangement analysis as compared to the features of adults with Ph1-positive CML. In chronic phase additional abnormalities other than Ph1 chromosome were found in none of our cases. On the other hand, in blastic crisis all of 6 cases had additional chromosomal abnormalities like as i(17q), double Ph1 and +8. The frequency of the appearance of additional chromosomal abnormalities, especially i(17q), is higher in children than in adult cases. In the DNA of 7 of 7 examined patients, rearrangement of bcr could be demonstrated by Southern blot analysis. These findings were similar to those observed in adults. An analysis of the location of the bcr breakpoint indicated that 5' breakpoints were found in four cases who were long-term survivors, and two of the other cases had blastic crisis from the onset of the disease. These findings showed the cytogenetic findings of children with Ph1+CML were different from those of the adult cases in the frequency of the appearance of the additional chromosomal abnormalities, and the location of the bcr breakpoint in children cases might be different from that in the adult cases and influence its prognosis.
