[A case of adult T cell leukemia complicated with strongyloidiasis and amplification of pneumocystis carinii DNA in bronchoalveolar lavage fluid].

The patient was a 75-year-old male, who simultaneously showed symptoms of bacterial meningitis during steroid treatment for erythroderma and symptoms of respiratory failure. Based on ground-glass shadows in both lungs on chest X ray, bronchoalveolar lavage (BAL) was carried out and strongyloides was detected. In addition to strongyloidiasis, the patient was shown to have the complication of pneumocystis carinii (PC) pneumonia after PC DNA was detected in BAL fluid using a PCR assay. When other causes for immunodeficiency affecting the incidence of opportunistic infection were investigated, the ATL virus was detected in peripheral blood cells and monoclonal amplification was indicated, though the presence of anti-ATL antibody was negative. According to the results, this patient was found to have early stage adult T cell leukemia. In conclusion, we treated this adult T cell leukemia patient who had strongyloidiasis and amplification of PC DNA in BAL and for which the PCR assay, a new technology used for diagnosing PC pneumonia, was considered to be effective.

[Effect of growth hormone on fetal and placental growth of spontaneous dwarf rat].

The effect of growth hormone on fetal and placental growth was studied in the spontaneous dwarf rat (SDR). The SDR is a newly established experimental model for pituitary growth hormone (GH) deficient dwarfism with an autosomal recessive mode of inheritance. The growth of heterogenic fetuses which were made by crossing female SDRs with normal male rats was similar to that of homogenic SDR fetuses on day 16 of gestation (Day 16), but on Day 20 heterogenic fetuses which had GH in their serum were significantly heavier than homogenic fetuses. There was no significant difference between the two groups in placental weight. The daily infusion of ovine-GH into pregnant SDR caused a significant increase in placental weight on Day 20, but had no effect on fetal growth. These results suggest that fetal GH plays a role in the fetal growth without affecting placental growth, and that maternal GH affects placental growth without affecting fetal growth.

Diffuse and broad podocytic detachment in a case of nephrotic syndrome associated with Kimura's disease.

We report a 49-year-old Japanese male with nephrotic syndrome associated with Kimura's disease. Renal biopsy revealed diffuse podocytic detachment from the glomerular basement membrane (GBM). He had an episode of nephrotic syndrome when complete remission was induced with steroid therapy six years prior to the present admission. However, complete remission of the nephrotic syndrome was not achieved by the steroid on this admission and massive proteinuria (5 g/day) persisted. We suggest that steroid-resistant proteinuria is closely related to podocytic detachment from the GBM and that the production of extracellular matrix by epithelial cells may participate in segmental sclerotic lesions in the patient.

[Progress of preoperative diagnosis of pancreatic tumor--with special reference to extension of pancreatic cancer].

Preoperative diagnosis of the extension of pancreatic cancer is very important for determining the range of resection. We evaluated the preoperative imaging diagnosis of the tumors and compared it with the pathological diagnosis. Out of 68 pancreatic head cancers, the percentage of correct diagnosis of serosal invasion was 69.1%, that of over-estimate was 23.5% and that of under-estimate was 7.4%. In the case of retroperitoneal invasion, a correct diagnosis had been made in 79.1%, over-estimate in 4.5% and under-estimate in 16.4%. In the case of portal vein invasion, these percentages were 52.8%, 39.6% and 7.5%, respectively. In the case of lymph node involvement, these percentages were 85.3%, 2.9% and 11.8%, respectively. The percentage of preoperative over-estimate was higher for serosal invasion and that of preoperative under-estimate was higher for retroperitoneal invasion. In the case of portal vein invasion, the percentage of preoperative over-estimate was relatively high. Of the 12 patients who had paraortic lymph node metastases, 6 patients had a single metastatic node in the paraortic area. The minimum size of these nodes was 2 x 2 mm. Only 5 cases could be diagnosed preoperatively. We evaluated the usefulness of abdominal CT scan and superior mesenteric arteriography for the diagnosis of superior mesenteric arterial wall invasion. In both methods, the over-estimate rate was 0%, but the under-estimate rate was relatively high. Conclusively the preoperative diagnosis of the extension of the pancreatic cancer by conventional imaging methods has some limits.

Changes in manganese content of mononuclear blood cells in patients receiving total parenteral nutrition.

We measured the manganese content of mononuclear blood cells and the manganese concentrations in whole blood and plasma from 31 healthy volunteers and 25 patients receiving total parenteral nutrition (TPN) (i.e., no dietary manganese). The manganese content (mean +/- 2 SD) of mononuclear blood cells and the manganese concentration in whole blood and plasma in the healthy volunteers were 8.84 +/- 4.18 ng/10(8) cells, 11.21 +/- 4.68 micrograms/L, and 1.21 +/- 0.72 micrograms/L, respectively. The manganese content of mononuclear blood cells from patients receiving TPN gradually decreased, falling below the lowest limit of the normal range by 21 weeks (95% confidence limits, 13-30 weeks) after starting TPN. Manganese concentrations in whole blood and plasma were within normal limits during our study period. These results suggest that the manganese content of mononuclear blood cells may better indicate manganese nutritional status than whole-blood or plasma concentrations.

Age-associated decreases in the messenger ribonucleic acid level and the rate of synthesis of apolipoprotein A-II in murine senile amyloidosis.

BACKGROUND: Apolipoprotein A-II (apoA-II), an apoprotein of serum high density lipoprotein, is the serum precursor of murine senile amyloid protein fibril. Three types of apoA-II protein variants (type A, B, and C) were found among inbred strains of mice. We reported the decreased concentration and the accelerated clearance of serum apoA-II with advancing age in the senescence-accelerated mouse-prone (SAM-P/1) mice, a strain with a high incidence of severe senile systemic amyloidosis and type C apoA-II. EXPERIMENTAL DESIGN: Age-related changes in hepatic mRNA levels and rates of synthesis of apoA-II were investigated in SAM-P/1 and SAM-R/1, the latter of which has a low incidence of amyloidosis and type B apoA-II. RESULTS: At age 2 months, both strains had the same levels of hepatic apoA-II mRNA. However, in SAM-P/1 after age 4 months, we observed a remarkable age-associated decrease in apoA-II mRNA levels and the level at age 14 months was about 50% of that seen at age 2 months. On the other hand, in SAM-R/1, the level at age 17 months was still 77.4% of the level at age 2 months. No age-related decrease in mRNA levels of apoA-I, another major apolipoprotein of high density lipoprotein, was observed in either strain. Slight age-associated decreases in ApoE mRNA levels and age-associated changes in apoB mRNA levels were observed, with the same profiles for both strains. The rates of hepatic synthesis of apoA-II protein decreased significantly in SAM-P/1 and decreased slightly in SAM-R/1, with advancing age. The parallel changes observed between mRNA levels and rates of synthesis of apoA-II indicate that decrease in the rate of apoA-II synthesis reflects age-associated decreases in mRNA levels. CONCLUSIONS: These findings suggest that the decreased concentration of serum apoA-II protein with advancing age may be caused by a decrease in the level of its mRNA.

[A clinical study of fluconazole in the treatment of systemic mycosis associated with immunocompromised children].

Efficacies of fluconazole (FLCZ) were evaluated in 8 cases of systemic mycosis (1 case each of candidemia, candiduria, 4 cases of pulmonary candidiasis, and 2 cases of suspected fungemia) complicated in immunocompromised children. Enrolled in the study were 3 patients with acute leukemia, 1 with malignant lymphoma, 2 with congenital immunodeficiency syndrome, 2 with other disorders. The clinical efficacies were good in 6 out of 7 cases. No side effects were observed. FLCZ is a very good and safe agent for the treatment of systemic mycosis complicated with immunocompromised children.

[A study of thymidine kinase activity in lung cancer tissue].

In order to investigate the usefulness of thymidine kinase, which acts to salvage thymidine to its nucleotide for DNA biosynthesis, as a marker of malignancy in lung cancer, the enzyme activities in extracts of tumors and uninvolved lungs of 83 resected cases of lung cancer were determined. The mean value of thymidine kinase activity in tumors was as much as 4.3 times higher than that of uninvolved lungs. There was no significant correlation between the activity in tumors and clinicopathological findings such as histological type, the grade of histological differentiation, pT factor, pN factor, pTNM stage and tumor size. The tumor doubling time did, however, show a significant inverse correlation with activity when compared logarithmically (r = 0.879, p = 0.00002). Moreover, there was a significant relationship between enzyme activity and post-operative recurrence of lung cancer. In conclusion, thymidine kinase was found to be a useful marker of malignancy in lung cancer.

Measurement of portal systemic shunting by oral and per-rectal administration of [123I]iodoamphetamine, and clinical use of the results.

UNLABELLED: OBJECTIVE. We used a method by which the portal shunt index via the superior mesenteric vein (SI-S) and that via the inferior mesenteric vein (SI-I) could be evaluated simultaneously. The clinical usefulness of the method was studied. METHODS: Scintigraphy was done 3 h after subjects took an enteric-coated capsule containing [123I]iodoamphetamine. At that time, the radionuclide was injected into the rectum. Counts for the lungs and the liver were used to calculate the two shunt indices. RESULTS: The mean SI-S and SI-I tended to be higher for disorders that were more severe, increasing in the order of chronic persistent hepatitis, chronic aggressive hepatitis, and cirrhosis. In cirrhotic patients, the SI-I was higher than the SI-S (p < 0.001). The SI-S and SI-I were both higher in cirrhotic patients with esophageal varices than in such patients without varices (p < 0.05 and < 0.001). The SI-S and SI-I were higher in cirrhotic patients with ascites than in such patients without ascites (p < 0.001 and < 0.01). Correlation between either of these indices and classical indicators of functional reserve was high. The correlation of SI-S with the Child-Turcotte classification and total bilirubin level was higher than the correlation of the SI-I and these indicators of functional reserve. CONCLUSIONS: This method permits assessment of the portal hemodynamics in a relatively noninvasive way, and is clinically meaningful.

[Incidence of lymph node involvement in pN2 non-small cell lung carcinoma and reevaluation of the lymph node dissection method].

We reported post operative survival rate of 134 pN2 squamous cell carcinoma and adenocarcinoma patients in our institute, and analyzed the level of the mediastinal lymph node metastasis to evaluate the standard lymph node dissection method (R2b). Post operative survival rate showed no difference between the left and right lung carcinoma but it showed significant difference between the upper lobe and lower lobe of the both side. Compared with the upper lobe lung cancer, the lower lobe cancer showed higher incidence of metastasis to #7, #8 or #9 lymph node. It is suspected that lymph node dissection of ipsilateral #12 (U), contralateral hilar and #4 lymph node should be considered for the lower lobe cancer especially with #7 lymph node metastasis.

[Experience with a operated cervico-mediastinal neuroblastoma in infant].

Cervical neuroblastoma is relatively rare and shows very good prognosis. We reported a case of surgically resected cervico-mediastinal neuroblastoma. A 7-month-old boy was pointed out to have Horner syndrome (right ptosis and anhidrosis) and an abnormal shadow at right cervico-mediastinum on chest X-ray film during his stay in another hospital because of acute bronchitis. On examination after admission in our hospital, 3 x 4 cm size mass was palpated at the right neck and urine VMA and HVA level were elevated. The close examination demonstrated no sign of tumor in any other organ and tissue, which suggested that the tumor was cervical origin. 9 x 5, 5 x 3 cm tumor was almost removed and patient received chemotherapy. He is now free from disease at 1 year after operation.

[Clinical assessment of 99mTc-MAG3 scintigraphy for impaired renal function].

The renal scintigraphy and clearance of 99mTc-mercaptoacetyltriglycine (MAG3) and 123I-ortho-iodohippuric acid (OIH) were compared in 12 patients with impaired renal function. The patients were first studied with MAG3 and then reinvestigated with OIH two days later. The MAG3 renal scintigraphy gave better quality and functional information in comparison with OIH. The MAG3 and OIH renogram showed identical relative kidney uptake and Tmax, but T1/2 of MAG3 was longer (p < 0.001). The plasma clearance of MAG3 was lower than that of OIH, but they correlated significantly (r = 0.87, p < 0.005). Also, the plasma clearances of MAG3 and para-aminohippuric acid (calculated by the standard clearance technique) showed good correlation (r = 0.77, p < 0.05). The urinary excretion during the first 60 min after injection was the same for the two radiopharmaceuticals. We conclude that 99mTc-MAG3 is a suitable alternative to 123I-OIH in renal scintigraphy.

cAMP-induced changes of intracellular free Mg2+ levels in human erythrocytes.

To examine the role of cAMP in the regulation of intracellular free magnesium concentration ([Mg2+]i), we measured [Mg2+]i in human erythrocytes by 31P-NMR spectroscopy. (-)-Isoproterenol, forskolin, Bt2cAMP and 8-bromo-cAMP decreased [Mg2+]i in human erythrocytes. Bt2cAMP did not increase the efflux rate of Mg2+ from erythrocytes. HA1004, a potent inhibitor of cAMP-dependent kinases, markedly increased the [Mg2+]i in a Mg(2+)-free buffer solution. Addition of 8-bromo-cGMP or 12-O-tetradecanoylphorbol 13-acetate (TPA) did not affect the [Mg2+]i. These results suggest that beta-adrenergic stimulation and cAMP play an important role in the regulation of [Mg2+]i in human erythrocytes.

Increased endothelial gene expression of angiotensin AT1A receptor in cyclosporine induced hypertensive rats.

We examined the gene expression of angiotensin AT1A receptors in aortic endothelial cells of cyclosporine induced hypertensive rats using reverse transcribed polymerase chain reaction method. The gene expression of angiotensin AT1A receptors in aortic endothelial cells of cyclosporine induced hypertensive rats showed a two-fold increase when compared with that in normotensive control rats. These results raise a possibility that the up-regulation of endothelial angiotensin AT1A receptors participates in the pathogenesis of cyclosporine induced hypertension.

Evidence for superantigen production by Yersinia pseudotuberculosis.

Yersinia pseudotuberculosis is an enteric pathogen that induces a variety of clinical symptoms, fever, scarlatiniform rash, diarrhea, vomiting, and arthritis. Characteristic histopathologic findings in Y. pseudotuberculosis infection such as lymphoid hyperplasia, typically seen in mesenteric lymph nodes, suggest that the stimulation of a large proportion of T lymphocytes may be involved in the pathogenesis of this infection. In this study, we assessed the mitogenic activity of culture supernatants of the clinical isolates of Y. pseudotuberculosis and investigated the mechanism by which these culture sups activate T cells. The culture sups, as well as partially purified fractions obtained by gel filtration, were found to selectively stimulate T cells bearing V beta 3, V beta 9, V beta 13.1, and V beta 13.2 compared with stimulation by anti-CD3. Furthermore, fibroblasts transfected with different HLA class II molecules, either HLA-DPw9, -DQw6, -DR1, or -DR4 Dw15, were capable of presenting Y. pseudotuberculosis culture supernatants to purified T cells. The T cell response to this sup was not restricted by donor HLA-DR types and was not neutralized by antibodies against the known staphylococcal superantigens, Staphylococcal enterotoxin (SE)A, SEB, SEC2, SED, SEE, and TSST1. These results suggest that Y. pseudotuberculosis produces superantigenic toxins that may mediate some of the systemic illnesses associated with infection by this organism.

Lymphatic flow in carcinoma of the distal bile duct based on a clinicopathologic study.

BACKGROUND: Nodal status is one of the most important prognostic factors for distal bile duct cancer. The pattern of lymphatic spread of distal bile duct cancer was analyzed by determining the frequency of involvement of various lymph nodes. MATERIALS AND METHODS: From 1973 to 1991, 29 patients with distal bile duct cancer underwent pancreaticoduodenectomy at Kanazawa University Hospital. A precise evaluation of their nodal involvement and the relationship among the lymph nodes was determined by histopathologic examination. RESULTS: Twenty of the 29 (68.9%) patients had nodal involvement. The lymph nodes with a high metastatic rate were those around the lower portion of the hepatoduodenal ligament (number 12abp2), the superior posterior pancreaticoduodenal lymph nodes (number 13a), and the superior mesenteric artery (number 14) (12abp2, 24.1%; 13a, 51.7%; 14, 34.5%). At least one para-aortic lymph node was involved with cancer in two (6.9%) patients. All of the patients except one who had one or more positive number 14 lymph nodes also had positive number 12abp2 or 13a lymph nodes. CONCLUSIONS: Lymph nodes number 12abp2 and 13a are important in lymphatic metastasis to superior mesenteric lymph node for distal bile duct cancer. Nodal dissection around the superior mesenteric artery should be performed in all patients except those without pancreatic invasion.
