The present paper describes a patient who exhibited 'Alice in Wonderland' (AIW) syndrome as well as Lilliputian hallucinations. The patient regularly consumed a cough syrup that contained dihydrocodein phosphate and dl-methylephedrine hydrochloride over 3 years. At the age of 46, he developed AIW syndrome. The patient ingested a large dose of triazolam and exhibited delirium. Even after the disappearance of symptoms associated with AIW syndrome and delirium, the patient continued to experience Lilliputian hallucinations. We believe that these hallucinations were caused by some of the components of the cough syrup.
This study psychopathologically analyzes and compares the premorbid personalities associated with bipolar II disorder and unipolar depression. Using Cloninger's tridimensional personality theory, we evaluated 14 inpatients with bipolar II disorder and 14 inpatients with unipolar depression. The results indicate that the premorbid personality associated with bipolar II disorder is characterized as 'the reward-dependent, passive-avoidant/dependent tendency of personality' or 'the dependent tendency of personality'. We also clarified that atypical symptoms of bipolar II disorder, such as the hypomanic state and the mixed state, were induced by the mingling of this tendency with the melancholic personality type (Tellenbach) or the cycloid personality type (Kretschmer), both of which are based on syntony (Minkowski), categorized by the use of an obsessional defense mechanism to maintain stable social relations. When an insufficiency in the obsessional defense weakens the syntony, the dependent tendency engenders the symptoms stated above. Although few serious problems associated with the dependent tendency may have occurred previously, it may have resulted in conflicts with others after onset of the bipolar II disorder. Therefore, psychotherapy for these conflicts is necessary along with the administration of mood stabilizers.
To elucidate the role of oxygen extraction during exercise in chronic heart failure (CHF), 26 patients underwent the symptom-limited maximal exercise. Hemodynamics, oxygen uptake (VO2) and blood gas were examined during exercise. Group I (n = 16, VO2max less than 20 ml/min/kg) showed severe cardiac dysfunction during exercise compared with group II (n = 10, VO2max greater than or equal to 20 ml/min/kg). Though VO2 was similar in both groups at rest and with a 25-watt load, oxygen extraction was significantly higher in group I than in group II. At the maximal work load, VO2max was significantly lower in group I, whereas there was no significant difference in oxygen extraction between the two groups. With a 25-watt load, mixed venous partial oxygen tension was similar, however, oxygen saturation was significantly lower in group I. It was concluded that oxygen extraction during exercise rapidly increased in group I, and this may play an important role in the maintenance of exercise capacity during mild physical activity in CHF.
A 62-year-old Japanese female was admitted due to dyspnea. She showed a marked increase in CA19-9 (maximum, 192,000 U/ml) and monoclonal IgM-kappa type rheumatoid factor (RF) activity. The patient died of respiratory failure 3 months later. Autopsy findings revealed an infiltration of IgM-kappa-positive plasma cells in the pulmonary interstitium, and therefore a diagnosis of lymphoid interstitial pneumonia (LIP) was made. The bronchoepithelial cells were stained with monoclonal antibody-reactive with CA 19-9 antigen. This is the first documented LIP, associated with a marked increase in monoclonal IgM-kappa type RF and CA19-9 in the serum.
The expression of tumor suppressor gene, p53 and cyclin-dependent kinase inhibitor, p21 in oral epithelial dysplasia and oral squamous cell carcinoma (OSCC) was examined immunohistochemically and its relationship with clinicopathological findings was analyzed. Among 24 epithelial dysplasias, 4 cases (17%) expressed p53 protein and 23 cases (96%) expressed p21 protein. On the other hand, expression of p53 was observed in 64% of OSCCs, and expression of p21 was observed in 77% of OSCCs. In the analyses of the correlation between the expression of p53 and p21 in epithelial dysplasia and OSCC, 79% of epithelial dysplasias were p53-negative and p21-positive, compared to 25% of the OSCCs. p21 expression did not correlate with p53 expression. These results were also demonstrated in OSCC cell lines by western blot analysis. Cumulative survival rate of the patients p53-negative and p21-positive was higher than those p53-positive and p21-negative, those p53-negative and p21-negative and those p53-positive and p21-positive. These findings suggest that p53 expression and p21 negative expression may involve in neoplastic transformation of oral epithelium. In the present study, we did not observe correlation between the expression of p53 and p21 proteins in OSCC. p21 expression may be regulated by p53-independent pathways as well as p53-dependent ones. However, combination of the p21 and p53 expression may be useful as a prognostic marker.
To determine whether connective tissue attachment can occur on implant materials by repopulating periodontal ligament (PDL)-derived cells and whether the new connective tissue attachments formation is influenced by the type of implant materials, periodontal tissue regeneration on four materials implanted into root cavities of cat canines was observed under conditions where PDL-derived cells could populate the surface of the materials. New connective tissue attachment occurred on bioactive materials such as bioglass and hydroxyapatite, while little or no cementum deposition was seen on bioinert materials such as titanium alloy and partially stabilized zirconium. Results showed that PDL-derived cells can form new connective tissue attachments on implant materials and that the formation of new connective tissue attachment is influenced by bioactivity of the materials.
