Inflammatory cell changes in haversian canals. A possible cause of osteoporosis in rheumatoid arthritis.

We studied the morphology of the haversian canals in the osteopenic cortical bone of the medial femoral neck from patients with rheumatoid arthritis and compared the findings with those in patients with osteoarthritis and with uncomplicated coxa valga. In the rheumatoid bone, the diameters of the canals were larger and many more contained osteoclasts. Fewer haversian canals showed only lining cells than in the osteoarthritic or coxa valga patients. In bone from rheumatoid patients, especially in canals with osteoclasts, small blood vessels were frequently lined by tall endothelial cells with an infiltration of mononuclear cells. These morphological differences are discussed with reference to the possible mechanisms of loss of cortical bone in rheumatoid arthritis and other conditions.

[Cerebral cysticercosis presenting as a solitary cyst. Diagnosis and treatment with CT-guided stereotactic biopsy--case report].

The patient, a 52-year-old male, experienced a generalized convulsive seizure. Computed tomography (CT) revealed a solitary, noncalcified cyst, 1 x 2 cm in diameter, in the left occipital lobe, without contrast enhancement. No other lesions were found in systemic soft tissues, skeletal muscles, or the eyes. A CT-guided stereotactic biopsy was performed through an occipital burr hole, and the diagnosis was cerebral cysticercosis. Although cysticercosis is still common in Mexico, Chile, and India, its occurrence has rarely been reported in Japan, especially since World War II. Intraparenchymal calcification and cystic lesions are common CT findings. However, in this case, a non-enhancing solitary cyst was the only CT finding. The authors review the literature and discuss the CT features and the usefulness of CT-guided stereotactic biopsy in the diagnosis and treatment of cysticercosis.

Seborrheic keratosis with possible central regression.

We describe a case of suspected superficial-type basal cell epithelioma. However, it showed the histologic features of pigmented seborrheic keratosis and, on the basis of the clinical and histologic findings and the developmental course, was presumed to have undergone central regression. Mononuclear cell inflammation was considered to be involved in the regression.

[Imaging diagnosis of hepatocellular carcinomas].

With the rapid progress of various imaging methods, including ultrasonography (US), computed tomography. (CT), digital subtraction angiography (DSA) and magnetic resonance imaging (MRI), it has become possible to detect small liver cancer less than 2 cm in diameter, and the prognosis of hepatocellular carcinomas is now improving rapidly. However, the accurate detection of smaller lesions about 1 cm in diameter and their differential diagnosis are difficult by conventional imaging methods such as US, CT and arteriography. For this purpose, we stressed the effectiveness of the combined use of CT and arteriography (the so-called CT arteriography), CT during arterial portography or Lipiodol CT. The promising future of MRI in this field is also discussed.

[Long-term effects of partial splenic embolization (PSE) for hypersplenism].

24 cases of hypersplenism were treated by PSE using Gelfoam particles. In cases with 90% embolized, the mean WBC and platelet counts were improved for 5.0 and 3.4 months. In cases with over 60% embolized, those were improved for 19.6 and 19.3 months. 9 cases with over 60% embolized remain in good condition more than 2 years, especially 2 cases of them remain more than 3 years. One patient with total embolized developed a splenic abscess and died after one month from PSE. But another severe complications were not shown.

[CT of the pelvic kidney].

Six cases of unilateral pelvic kidney are presented. CT features are summarized as follows: (1) an absence of a kidney in the renal fossa with normal sized opposite kidney: (2) a pelvic kidney whose hilus faces in various directions; and (3) homogeneous parenchymal enhancement and visualization of the pyelocalyceal system on post-contrast CT. In the study of suspected pelvic kidney, CT is a useful diagnostic tool.

[A case of pulmonary alveolar microlithiasis on the bone image of dual-energy subtraction radiography].

A 33-year-old female was evaluated for abnormality of her chest roentgenogram which showed diffuse fine granular micronodulation in the lower two thirds of both lung fields. The bone image (soft tissue cancelled image) of dual-energy subtraction radiography obtained with Fuji Computed Radiography (FCR) demonstrated that the abnormal shadows were characterized by calcified deposits.
