Two groups of male workers who were exposed to formaldehyde, the first group in phenol-formaldehyde-plastic foam matrix embedding of fiberglass (batt making), and the second in the fixation of tissues for histology, were studied for work-related neuro-behavioral, respiratory, and dermatological symptoms; and for pulmonary functional impairment. Forty-five male fiberglass batt makers who were studied across the initial work-shift after a holiday had average frequencies of combined neurobehavioral, respiratory, and dermatological symptoms of 17.3 for the hot areas and 14.7 for the cold areas of the process. Their symptom counts were significantly greater than those for 18 male histology technicians who averaged 7.3, and for 26 unexposed male hospital workers who averaged 4.8. During their first workshift after holidays, 58% of the batt makers had a decrease in one or more tests of pulmonary function. Nine nonsmokers had decreases more frequently than did 35 smokers; forced expiratory volume in one second FEV1.0 decreased in 16%, diffusing capacity for carbon monoxide (sb) decreased in 30%, forced expiratory flow 25-75 decreased in 16%, and forced expiratory flow) 75-85 decreased in 36%. Thirty-five percent of all 44 men had drops in FEV1.0, forced vital capacity, or in diffusing capacity (sb).
Viet Nam has an increasing need for building materials, including refractory bricks. Little is known regarding the occupational hazards incurred in brick manufacturing. To determine the occupational health hazards posed by a refractory brick operation in Ha Noi, Viet Nam, a cross-sectional occupational risk survey was conducted, focusing on respirable dust hazards. It included an industrial hygiene walk-through and gravimetric dust analysis. Noise, heat, lack of head protection, and dust exposure were hazards identified at the brick-manufacturing site. Respiratory protection involved a three-layer cloth mask that had not been tested for efficacy. Silica dust exposure levels exceeded the Vietnamese permissible exposure level by almost fivefold based on gravimetric assessment and estimated silica content. This brick manufacturing site contains significant injury and respiratory illness hazards. Further investigations are necessary to begin to develop occupational safety measures at the site.
To expand the base of knowledge regarding perceptions about potential environmental threats to children's health, a survey was conducted in the Northwest United States. Samples of Head Start parents, PTA presidents, public health officials, school nurses, naturopathic physicians, family practitioners, and pediatricians were mailed a questionnaire inquiring into the nature and degree of concerns about pediatric environmental health. The response rate was 24%. Trends in the data showed disparities in perceptions regarding levels of concern and exposure concerns between respondent categories. Disparities also existed regarding information resources used for children's environmental health. Recognizing differences in perceptions of children's environmental health, as demonstrated in these results, may be useful for risk communication and resource allocation, especially in the context of the wide variety of health belief models. Such knowledge may help clarity situations with environmental health risk concerns, including clinical, public health, and educational circumstances.

An unusual complication of persistent lymph fistula occurred three months postoperatively following axillo-femoral and femoro-femoral bypass graft. The lymphangiograms demonstrated the precise site of lymph extravasation. Multiple aspirations and local drainage were of no avail and it finally responded to prolonged continuous local pressure over the area of lymph extravasation.

Closed chest cardiac massage is simple and usually a safe procedure for cardiac resuscitation. However, it is not innocuous and is occasionally associated with serious and even fatal complications. A case is reported in which fracture of the upper part of the left half of the sternum occurred following midline sternotomy. The sternum was re-approximated in the usual manner by interrupted wire sutures, but the fracture was not specifically repaired, because it was thought to be adequatley stabilized by this closure. Postoperatively, the patient required closed chest cardiac massage which caused a fatal laceration of the outflow tract of the right ventricle due to penetration of the sharp edges of fractured fragments. We believe fracture of the sternum which occurs while retracting it with sternotomy retractor during open heart surgery should be deliberately repaired to avoid this serious complication.
