Accumulation of cholesteryl ester in atherosclerotic lesions.

This article reviews aspects of the molecular pathology of cholesteryl ester accumulation in atherosclerotic lesions. 1. Transcytosis of lipoproteins through a cultured endothelial monolayer. 2. Effects of platelets and PGI2 on intercellular transport of endothelial cells. 3. Transformation of macrophages to foam cells. 4. Cholesteryl ester deposition in the extracellular space of atherosclerotic lesions. The development and use of novel monoclonal antibodies recognizing atherosclerotic lesions and peroxidized lipoproteins prepared from then are also discussed.

Study of chronic schizophrenics using 31P magnetic resonance chemical shift imaging.

Phosphorus-31 chemical shift imaging showed regional abnormalities of in vivo 31P NMR spectra in the brains of chronic schizophrenic patients. In the left temporal region, the level of % phosphodiesters (PDE) was increased and the level of % gamma alpha beta-ATP (obtained by summation of gamma-ATP, alpha-ATP, and beta-ATP) was decreased. In the basal ganglia, the levels of % PDE were decreased and the level of % phosphomonoesters was increased. The levels of % gamma alpha beta-ATP were increased in the right basal ganglia. The level of % phosphocreatine was decreased in the frontoparietal region. These findings may represent different patterns of dysfunction of membrane phospholipid bilayers and high-energy phosphate metabolism in the specific cerebral regions.

Quantitative Southern blot analysis in the dystrophin gene of Japanese patients with Duchenne or Becker muscular dystrophy: a high frequency of duplications.

Eighty-four unrelated patients with Duchenne or Becker muscular dystrophy in Japan were studied by quantitative Southern blot analysis with dystrophin cDNA probes. We found partial deletions and duplications in 47 (56%) and 12 (14%) cases respectively by HindIII digestion. The duplications were confirmed by BglII digestion and densitometric scanning. The frequency of duplications in this study is significantly higher than those previously reported. This may be because of the small sample number, the racial difference, or our quantitative methods. Our results suggest that attempts to detect duplications are important for a precise diagnosis. Both deletions and duplications clustered at the two hot spots as reported previously. Six cases were exceptions to the 'reading frame hypothesis'. We detected three types of HindIII RFLP. Based on the results of one duplication case, we propose a revised sequential order of exons in the cDNA10 region of the dystrophin gene.

Monoclonal antibodies recognising pol and env proteins of mammalian retroviruses.

Mouse monoclonal antibodies were generated for three 20- to 23-mer synthetic peptides whose amino acid sequences were highly conserved in the pol or env regions of 14 mammalian and two avian retroviruses. By Western-blot analysis, these antibodies recognised the epitopes in major and minor pol or env proteins of six mammalian type-C and -D retroviruses. One of the anti-env monoclonal antibodies was found to recognise all the env transmembrane proteins of six mammalian retroviruses examined.

Hydrolysis of lipid droplets in acid cholesteryl-esterase-deficient fibroblasts.

The mechanisms of hydrolysis and accumulation of cholesteryl oleate-lipid droplets prepared in vitro were studied in acid cholesteryl-esterase-deficient fibroblasts (GM00863, GM03111). Acid cholesteryl esterase activity was reduced in both GM00863 and GM03111 (8.9% and 17.4% of the normal level, respectively), while neutral cholesteryl esterase activity was highly stimulated in GM03111. The hydrolysis of [14C]-cholesteryl oleate-lipid droplets in GM00863 was almost as efficient as in normal cells, while that in GM03111 was highly stimulated. When viewed by polarized microscopy the lipid droplets which had accumulated in the mutant cells showed anisotropic liquid crystalline structures. As in normal cells, some of these lipid droplets were observed by transmission electron microscopy as membrane-free lipid inclusion bodies in the cytoplasm. These results suggest that lipid droplets internalized into phagolysosomes of these mutant cells transferred to the cytoplasm, and were hydrolyzed there probably by neutral cholesteryl esterase.

Homeostasis as regulated by activated macrophage. VII. Suppression of serum cholesterol level by LPSw (a lipopolysaccharide from wheat flour) in WHHL (Watanabe heritable hyperlipidemic) rabbit.

The effect of LPSw (a lipopolysaccharide from wheat flour) on cholesterol catabolism was examined using WHHL (Watanabe heritable hyperlipidemic) rabbit, which is an experimental model of familial hyperlipidemia. The serum cholesterol level of the animal decreased by the addition of LPSw to drinking water. Following cessation of the addition of LPSw to the drinking water, the cholesterol level was decreased for 30 to 40d and then gradually elevated. The serum level of apolipoprotein B, which is a constituent of apolipoprotein of low density lipoprotein (LDL), also decreased in accord with serum cholesterol at a nearly coincident rate. Conversely, the level of apolipoprotein A-I, which is a constituent of apolipoprotein of high density lipoprotein (HDL), did not change, nor did HDL-cholesterol. Furthermore, the atherosclerosis risk factor, expressed as the ratio of apolipoprotein B to apolipoprotein A-I, was decreased by LPSw administration.

Paroxysmal atrial fibrillation and flutter associated with acute myocardial infarction: hemodynamic evaluation in relation to the development of arrhythmias and prognosis.

The hemodynamic background associated with the occurrence of paroxysmal atrial fibrillation and flutter (PAF), in patient with acute myocardial infarction (AMI) were evaluated. Sixty-seven of 381 consecutive AMI patients (17.6%) were noted to have PAF in the acute phase of infarction. These 67 patients with PAF (group 1) were compared with 60 randomly selected patients without PAF (group 2). The hospital mortality rate was 25.4% in group 1, and 11.7% in group 2 (p less than 0.01). The hemodynamic variables measured before the onset of PAF in group 1, showed significantly more unfavorable values than those in group 2, which were measured at the time of admission. The 67 patients in group 1 were divided into 50 patients who survived (group S) and 17 patients who died in the hospital (group D). The hemodynamic status in group D demonstrated significantly larger deterioration before the onset of PAF than in group S. Hemodynamic variables were compared before and during PAF in groups D and S, cardiac index (CI) decreased significantly, and stroke index (SI) decreased by 46% in group D, with no decrease in CI and less decrease in SI (28. p less than 0.05) in group S. In conclusion, not only the occurrence of PAF, but the prognosis of patients with PAF is dependent on the severity of hemodynamic disturbance imposed by AMI. Atrial contribution to ventricular filling has great importance in the maintenance of the cardiac output in this patient population.

Cloning and nucleotide sequence of the maltopentaose-forming amylase gene from Pseudomonas sp. KO-8940.

The gene coding for the maltopentaose-(G5)-forming amylase of Pseudomonas sp. KO-8940 was cloned into Escherichia coli and its nucleotides were sequenced. It was expected that a long open reading frame composed of 1,842-bp that encoded 614 amino acid residues for secretory precursor polypeptide including the typical signal sequence with an NH2-terminal was the gene. An extract of Escherichia coli carrying the cloned G5-forming amylase gene had amylolytic activity with which produced only G5 from starch, the same as that of the donor strain enzyme. In the deduced primary structure of this enzyme, the four conserved regions of many alpha-amylases were found, and the COOH-terminal portion of this enzyme showed high homology with other raw starch digesting amylases.

[Implementation rate of periodic health checkup at manufacturing factories in Kawasaki].

A questionnaire survey on periodic health checkup was conducted on 4,432 manufacturing factories in Kawasaki and the following results were obtained. 1) Implementation rate of periodic health checkup was 100% in factories with 100 workers or more. In smaller factories employing less than 100 workers, the implementation rate decreased with decrease in their total work force. The rate was 47.5% and 37.7% in factories with 9-4 and 3-1 workers, respectively. 2) The proportion of workers less than 30 years of age decreased and that of 60 or more years of age increased with decreasing size of the total work force. Distribution of both age groups was 12.6% and 12.9%, respectively in factories with less than 10 workers. The proportion of female workers increased from about 10% in factories with 300 workers or more to 26.8% in factories with less than 10 workers. 3) In large factories with 500 workers or more periodic health checkups were implemented mostly at their own or related medical facilities and in factories with 499-10 workers, almost 50% were implemented in independent health service facilities. In factories with less than 10 workers periodic health checkup were implemented mainly at health centers (33.1%) and at hospitals or clinics (26.9%). 4) The main reasons why small factories could not implement periodic health checkup were: (1) they could not afford the time for the implementation (50.0%) and (2) ignorance of the law of mandatory periodic health checkup for workers (23.6%).

Interaction of flufenamic acid on ethanol metabolism in rat.

Flufenamic acid (FA) is a widely used non-steroidal anti-inflammatory drug. It is also known to be an uncoupling agent of oxidative phosphorylation in mitochondria. The interaction of FA and ethanol has been of concern in the occupational health field, since alcohol consumption is a common habit among members of the working population. Thus, we investigated the effects of FA on ethanol metabolism in the rat. In the first experiment, FA and ethanol were administered intragastrically to male Wistar rats. Ethanol and acetaldehyde were measured in blood samples collected from the tail vein by head-space gas chromatography. In the second experiment, the interaction of FA and ethanol was observed in the perfused rat liver. The following items were monitored in perfused livers from both fed and fastd rats: uptake rate of ethanol, production rate of acetaldehyde, level of reduced pyridine nucleotides, and oxygen consumption rate. In the first experiment, the rats with FA showed significantly higher concentrations of both ethanol and acetaldehyde in blood after ethanol intake than the rats without FA. In the perfused liver, FA suppressed ethanol uptake, and increased acetaldehyde concentration in the effluent. FA decreased the level of intracellular reduced pyridine nucleotides which had been elevated by ethanol. FA caused an increase in the oxygen consumption rate, which was not altered by the coexistence of ethanol. It was concluded that FA suppressed ethanol metabolism due to suppression of acetaldehyde oxidation in the liver, despite its uncoupling effect on oxidative phosphorylation in mitochondria.

Acute inhibition of lipid transport in rat intestinal lymph by ethanol administration.

To observe the effect of acute ethanol ingestion on stable lipid transport in intestinal lymph, three groups of mesenteric lymph fistula rats were prepared. In group A rats, lipid emulsion containing 120 mumol/hr of oleate (control meal) was infused intraduodenally for 8 hr. Group B rats were infused with another lipid emulsion, which had the same basic composition as the control meal but included 0.75 g/kg body weight/hr of ethanol (alcohol meal), for 8 hr. In group C, rats were infused with the control meal for the first 4 hr, then with the alcohol meal for the next 4 hr. Lymph flow increased just after the infusion of ethanol, that is, lymph flow in group B was significantly higher than that of group A at 1 and 2 hr, and lymph flow in group C was significantly higher than that of group A at 5 hr. In group A, lymphatic triglyceride (TG) output reached a plateau of more than 30 mumol/hr after 3-hr infusion. TG output in group B was significantly lower than that in group A from 2 hr to 8 hr and was two-thirds that of group A at the plateau level. In group C, TG output was the same as in group A until 5 hr, but decreased to two-thirds of that in group A at 6 hr, 2 hr after replacing the control meal with the alcohol meal. Lymphatic phospholipid output exhibited a tendency similar to that of TG output. Comparisons of group A vs. B and A vs. C clearly demonstrated an inhibitory effect of ethanol on stable lipid transport in intestinal lymph. A 2-hr infusion was enough for ethanol to exhibit its inhibitory effect. In conclusion, stable lymphatic transport of intestinal absorbed lipid was suppressed by acute ethanol ingestion.

[Diagnostic role of fiberoptic bronchoscopy for mycobacterial diseases].

From 1981 to 1991, 5,572 patients underwent bronchoscopic examination, in this series 3,754 patient's samples were sent for determination of mycobacterial isolation. In the 506 samples, mycobacterial bacilli were detected with smear or culture. In the low tuberculous incidence area like northern part of America, many reporters supported the opinion that routine bronchial aspirate detection for tuberculosis was not cost-effective. Inversely, in the high tuberculous incidence area like Hong Kong, M. lp et al. claimed the usefulness of routine bronchial aspirate examination for mycobacterial bacilli. Japan was regarded as moderate tuberculous incidence area. We review a ten-year experience with fiberoptic bronchoscopy for mycobacterial isolation. Our results suggest that in an area with a moderate prevalence of tuberculosis, bronchoscopic examination for mycobacterial disease is useful in the selected hospitals for the selected cases. In Japan, the smear positive prevalence has been on the increase from 1982. We estimate that the Japanese popularization of fiberoptic bronchoscopy may influence a part of the Japanese epidemic statistical analysis of mycobacteriosis.

[Management of type A acute aortic dissection--results of the cases with thrombosed false lumen].

By reviewing the outcome, we studied the propriety of our principles for the treatment of type A acute aortic dissection in 45 patients, encountered during the 10-year period between 1981 and 1990. We conducted a comparative study of patients with a thrombosed false lumen (type T) and a patent false lumen (type P) to examine the effect of an acutely thrombosed false lumen on the prognosis of this disease. For the 25 patients treated in the first 6 years (1981-1986), operation was performed as soon as exact diagnosis was made, regardless of the presence of complications and the type or severity of the disease. Early death occurred in 9/20 operated cases and in 4/5 unoperated cases, so 13/25 patients died for a 52.0% mortality rate. For the 20 patients who received treatment in the latter period (1987-1990), we gave priority to conservative treatment for type T cases that were free from complications, and adopted a treatment method attaching greater importance to the resection of intimal tears. As a results, early deaths were observed in only 4 type P patients (20.0%) who underwent operation, a significant better result (p < 0.05). For patients in whom we were able to excise the intimal tear (30.0%, early mortality rate), the results were better than in those in whom the intimal tear were left alone (53.8%). The results were particularly good in type T patients (25.0% of them underwent intimal tear resection and 71.4% underwent no operation for the intimal tear).(ABSTRACT TRUNCATED AT 250 WORDS)

Detection and characterization of mRNA and proteins encoded by human rab2 low molecular weight GTP-binding protein gene.

Three RNA species of 3.5, 2.4 and 1.4 kb were detected in all human hematopoietic, fibroblastic and tumor cell lines examined, as the mRNA of human rab2 gene presumably involved in intracellular transport. The mRNA differed in the length of the 3'-non-translated regions due to termination and/or processing at the alternative polyadenylation sites. The 1.4-kb rab2 RNA was predominant in Molt-4 and U-937 of mononuclear cell origin, while the 2.4-kb RNA was dominant in the other cells examined. The degradation of all rab2 RNA was similar and as slow as beta-actin RNA. The rab2 proteins were ubiquitously detected in the human cells as two phosphorylated peptides of major pp24rab2 and minor pp25rab2. pp25rab2 was slightly more phosphorylated than pp24rab2. Both rab2 proteins were abundantly detected in neural PC12 and NB3 cells, and mostly pp24rab2 was produced in the other human and rodent cells.

[The immunosuppressive effects of 0.025% cyclosporin eye drops in alpha cyclodextrin on rabbit corneal allografts].

We reported the ocular penetration of cyclosporin (CYA) and the immunosuppressive effect of rabbit corneal allograft using 0.025% CYA eye drops in alpha-cyclodextrin (alpha-CD). Local application using 0.025% CYA in alpha-CD showed the concentration of 4,133 ng/gr in the cornea, but no detectable levels in aqueous humor and serum. All eyes (10/10) in the CYA eye drop group remained clear for 100 days after corneal allografting. CYA eye drops halted and suppressed the corneal allograft's immune reaction when the treatment was begun early in the initial phase of rejection. These results indicate that 0.025% CYA eye drops in alpha-CD penetrate the cornea 5 to 10 times more than CYA eye drops in lipophilic vehicles. Furthermore, they are extremely effective in suppressing the immune reaction of corneal grafts.
