Clinical study of asthma in adolescents and in young adults. Correlation between laboratory findings and severity of asthma.

Fifty-eight mite-allergic asthmatic patients ages 15 to 22 years were examined. Low respiratory threshold to acetyl-choline (RT-Ach), high maximal histamine release (HR) by mite and anti-IgE, high serum IgE levels and high eosinophil counts were highly associated with the severity of asthma. RT-Ach correlated inversely with HR to mite and anti-IgE. The maximal histamine release from leukocyte caused by anti-IgE also correlated highly with the severity of asthma.

Immunologic analysis of steroid-dependent asthma.

In order to analyze steroid-dependent asthma immunologically, IgE antibodies to mite (Dermatophagoides farinae), Candida albicans, and Aspergillus fumigatus were measured in 112 asthmatic patients. IgG and IgG subclass antibodies to mite were also measured. The rate of patients who were positive to candida IgE RAST was higher in atopic steroid-dependent patients than in atopic steroid-independent patients (P less than .01). The rate of mite-sensitive patients who had not received immunotherapy with mite or house dust was higher than in the atopic steroid-dependent patients than in atopic steroid-independent patients (P less than .05). IgG1 and IgG4 antibodies to mite were higher in mite-sensitive steroid-independent patients than in mite-sensitive steroid-dependent patients. IgE antibodies to A. fumigatus were detected only in patients with allergic bronchopulmonary aspergillosis (ABPA). Based on these results, we were encouraged to try immunotherapy with house dust mite or C. albicans if patients were steroid-dependent and sensitive to these allergens except when the patients had ABPA.

Modulation of human basophil histamine release by hemopoietic growth factors.

To determine what kinds of CSF modulate human basophil function, recombinant or purified hemopoietic growth factors were tested for the effect on histamine release from basophils. Both granulocyte (G)-macrophage (M) CSF and IL-3 markedly enhanced histamine release upon stimulation with anti-IgE in a dose-dependent manner (maximal enhancement 25.5% by GM-CSF and 30.8% by IL-3 as expressed as percent increase against total cellular histamine content), whereas G-CSF, M-CSF, and IL-4 had no effect. Enhancing action of these factors was still observed in the highly enriched basophil population, suggesting that this action was not via contaminating cells. Enhancement of histamine release by both factors was not necessarily IgE mediated, because they also amplified histamine release upon stimulation with FMLP and ionophore A23187. The enhancement by both factors was temperature dependent, and took place rapidly and reached plateau levels in 15 min. GM-CSF and IL-3 achieved the similar plateau level of augmentation and no additive effects were observed between them. This finding suggests that they enhance histamine release by sharing the same pathway in the release reaction.

Role of protein kinase C in histamine release from human basophils.

In this study, we investigated the role of calcium and phospholipid-dependent protein kinase (protein kinase C, PKC) in the modulation of histamine release from human basophils. A novel and potent inhibitor of PKC, K-252a, inhibited the release of histamine induced by anti-IgE in a dose-dependent manner with ID50 (the dose required for 50% inhibition of histamine release) of 2.2 x 10(-8) M. Histamine release stimulated with 12-0-tetradecanoyl-phorbol-13-acetate(TPA) was also suppressed by K-252a with maximal inhibition of 48.0 +/- 9.3% at 10(-7) M. In contrast, K-252a did not inhibit the release of histamine in response to FMLP and ionophore A23187. Another inhibitor of PKC, H-7, exhibited a dose-dependent inhibition of anti-IgE-induced histamine release with ID50 of 8.6 x 10(-4) M. H-8 and HA1004, which closely resemble H-7 in chemical structure but are less potent in inhibiting PKC, did not inhibit histamine release stimulated with anti-IgE, but rather enhanced the release at higher concentrations. These results strongly suggest that PKC activation plays a crucial role in the mediation of IgE-mediated histamine release from human basophils.

Identification of variants of the basophilic leukemia (RBL-2H3) cells that have defective phosphoinositide responses to antigen and stimulants of guanosine 5'-triphosphate-regulatory proteins.

Antigen immunoglobulin E-mediated secretion of histamine from RBL-2H3 cells is associated with substantial hydrolysis of membrane inositol phospholipids and a rise in the concentration of cytosol Ca2+ (calcium signal). Such responses differed among cloned variant lines of the RBL-2H3 cell line from undetectable (1A3 bromodeoxyuridine-resistant (BUDRR), 2B1 BUDRR, and 1B3 BUDRR lines) to about 80% of those in the parent RBL-2H3 cells. In all but one line (1B3 thioguanine-resistant (TgR)), the intensities of the phosphoinositide response and of the calcium signal were correlated with the secretory response. The 1B3 TgR line had no detectable calcium signal (as measured by quin 2 fluorescence or uptake of 45Ca2+) but paradoxically showed modest rates of hydrolysis of inositol phospholipids and of secretion. The responses of the 1B3 TgR line were, however, dependent on the presence of external Ca2+ ions. The induction of secretion with antigen, therefore, was invariably associated with the hydrolysis of inositol phospholipids, but it was not necessarily associated with a change in concentration of cytosol Ca2+. All antigen unresponsive clones could secrete when synergistic signals were induced by exposure to the Ca2+ -ionophore, A23187 and the phorbol ester, 12-O-tetradecanoylphorbol 13-acetate. These lines, otherwise, had immunoglobulin E receptors and had no obvious defect in their capacity to synthesize the inositol phospholipids or in their phenotypic expression of phospholipase C as measured in cell extracts. One finding of possible relevance to the role of guanosine 5'-triphosphate-regulatory proteins in the activation of phospholipase C was the inability of one antigen-nonresponsive line to respond to NaF (in intact cells) or to guanosine 5'-(3-O-thio)triphosphate (in electrically permeabilized cells).

Capsaicin-induced beta-adrenergic action on energy metabolism in rats: influence of capsaicin on oxygen consumption, the respiratory quotient, and substrate utilization.

The mode of action of capsaicin on energy metabolism was investigated in rats. The oxygen consumption was higher when capsaicin (6.0 mg/kg) was intraperitoneally injected than when it was not injected. The respiratory quotient (R.Q.) increased and then decreased after the administration of capsaicin. The levels of serum glucose and immunoreactive insulin rapidly increased after the administration of capsaicin. Also, liver glycogen rapidly decreased, in contrast to the serum glucose concentration which rapidly increased. The serum-free fatty acid level gradually increased after the administration of capsaicin. These alterations in energy metabolism on the administration of capsaicin were similar to those in the metabolism of epinephrine, and were specifically inhibited by various beta-adrenergic blockers. On the other hand, the alterations were not affected by pretreatment with alpha-adrenergic or ganglion blockers. These results suggest that the mode of action of capsaicin on the enhancement of energy metabolism in rats comprises a direct (as an agonist) and/or an indirect (via catecholamine) beta-adrenergic action. Therefore, it was speculated that the adrenergic action of capsaicin resulted, at least in part, in a decrease in the perirenal adipose tissue weight and serum triglyceride concentration in rats fed a high fat diet supplemented with capsaicin (T. Kawada et al., J Nutr 116:1272-1278, 1986).

Yuta (shaman) and community mental health on Okinawa.

This study reports on the relations between the yuta (shaman) and the community mental health activities on Okinawa, Japan. Focus is on the process of initiation of the yuta and its meanings from the mental health viewpoints, the functions of the yuta in the particular cultural background of the island, and the importance of admitting the existence of the yuta in its relations to the psychiatric treatment in a mental hospital. The discussion is based on the authors' research findings which were obtained mostly through their therapeutic activities and their field studies. The authors' assumption is that each culture has in it a certain social background that is unique in that culture by which a stress or insanity is increased or brought about, but in the same culture there are ways to decrease the stress and to cure the insanity.

Auranofin, an oral chrysotherapeutic agent, inhibits histamine release from human basophils.

The effect of auranofin, an oral chrysotherapeutic agent, on histamine release from human basophils was studied. Auranofin inhibited IgE-mediated, anti-IgE-induced histamine release in a dose-dependent fashion with a concentration of drug required to produce 50% inhibition of 1.3 micrograms/ml. This compound also inhibited calcium ionophore A23187 and 12-0-tetradecanoyl-phorbol-13-acetate-induced histamine release with a concentration of drug required to produce 50% inhibition of 1.7 micrograms/ml and 0.9 micrograms/ml, respectively. Auranofin was less active for formyl-L-methionyl-L-leucyl-L-phenylalanine-induced release of histamine with 32 +/- 10% (mean +/- SEM) inhibition at 2 micrograms/ml. Auranofin effects were reversible when leukocytes preincubated with auranofin was washed with buffer. In contrast, gold sodium thiomalate enhanced IgE-mediated histamine release, suggesting that these two compounds exert their actions at different levels. These results suggest that auranofin may be beneficial to patients with allergic diseases such as bronchial asthma.

IgG4 antibodies in patients with house-dust-mite-sensitive bronchial asthma: relationship with antigen-specific immunotherapy.

Sera from 40 patients with house-dust-mite (Dermatophagoides farinae)-sensitive bronchial asthma were evaluated by solid-phase radioimmunoassay for their mite-specific IgG4 antibody levels. Asthmatic patients undergoing specific immunotherapy possessed a significantly higher mean value of IgG4 antibodies than normal controls and asthmatics without immunotherapy (p less than 0.01 and p less than 0.05, respectively). Moreover, evaluation of 11 patients before and after immunotherapy showed that IgG4 antibodies tend to increase during immunotherapy. The clinical significance of the IgG4 subclass as blocking antibodies in immediate allergic reactions is also briefly discussed.

Studies on a new antibiotic M-92 produced by Micromonospora. II. Isolation and physicochemical properties of M-92 and its components.

A new antibiotic complex, M-92 was isolated from the whole fermentation broth of Micromonospora verruculosa MCRL 0404. The whole broth was mixed with talc and filtered. The filter cake thus obtained was extracted with acidic methanol to give a crude powder of M-92 complex, which was then separated into A (acidic) and N (neutral or weakly acidic) groups. The A group components are soluble in alkaline water (pH 8.5), while the N group components are not. These components were further separated into six major components designated VA-2, BA-4, BA-5, BN-1, BN-2 and BN-3 by silica gel column chromatography. Components with the letter "V" are reddish violet and those with "B" are blue. The IR and UV spectra of these components suggest that their chromophores may be the same or very closely related to one another. The molecular formula of BN-3 was determined to be C29H23NO9 by mass spectrometry. The results of various spectroscopies on BN-3 suggest that M-92 components consisted of chromophores in which juglone (5-hydroxynaphthoquinone) is conjugated with naphthazarin (5,8-dihydroxynaphthoquinone).
