Clinical demonstration of vibration-induced depression of left ventricular function.

In experimental studies, minute sinusoidal vibration has been reported to induce functional depression of the left ventricle and to be an index for evaluation of myocardial crossbridge kinetics. Therefore, to examine whether or not this vibration-induced functional depression could also be observed in the human ventricle, motion of the left ventricular (LV) wall or LV pressure was measured by echocardiography (n = 16) or by left heart catheterization (n = 4), in which 100 Hz, 2.07 mm-amplitude sinusoidal vibration was applied to the subject's precordium. In the echocardiographic study, left ventricular wall shortening did not change by vibration in nine healthy volunteers, but was depressed in two patients with aortic regurgitation (AR) and in one patient with ischemic heart disease (IHD). In measurement of LV pressure, the decrease in LV systolic pressure caused by vibration was obviously observed in two patients (AR and IHD) but was not observed in two patients with hypertrophic cardiomyopathy. These results suggest that we might be able to extend previously proposed experimental idea on early detection of the abnormality in myocardial crossbridge kinetics to the clinical setting.

Human left ventricular wall vibration responded to precordial minute vibration.

We examined, noninvasively, the response of the left ventricular (LV) wall to precordially applied vibration in seven normal subjects. The LV posterobasal wall vibration was detected by an esophageal vibration sensor. Even though the amplitude of input vibration was maintained at a constant level, a sharp change in amplitude of the LV wall vibration was observed during the cardiac cycle, especially when 1) the subject was lying in a 45 degree left anterior oblique supine position, 2) the sensor was positioned at approx. 40 cm from the incisor teeth and 3) breath was at expiration. The pattern of this amplitude modulation changed with the input frequency. We concluded that the change in amplitude of output signal must have resulted from LV contraction and relaxation.

Mechanism of cerebral vasospasm--contractile proteins in smooth muscle of bovine cerebral artery.

Understanding of the pathogenesis of cerebral vasospasm requires basic study of the contraction mechanism of cerebral arteries. In this study, the morphological and biochemical properties of actomyosin from bovine cerebral arteries were examined. Arrays of thick and thin filaments were clearly seen on electron micrographs of a cross section of a muscle strip stretched to 1.5 times its normal length in Krebs-Ringer solution. The diameters of the thick and thin filaments were 110.0 +/- 19.9 A and 51.0 +/- 8.0 A, respectively. The average ratio of thick to thin filaments was about 1:15 to 1:16. The gel pattern of Ca2+-sensitive actomyosin extracted from bovine cerebral arteries revealed the presence of myosin, actin, and tropomyosin, but not skeletal troponin. The Mg-ATPase activity of actomyosin was greatly enhanced in the presence of Ca2+. Rapid superprecipitation occurred after the clearing phase, when 1 mM Mg-ATP was added to a suspension of actomyosin with 10(-5) M Ca2+. This superprecipitation did not occur with a lower Ca2+ concentration of 10(-8) M. Intracellular free Ca2+ appears to play a crucial role in the regulation of contraction and relaxation of cerebral arterial smooth muscle.

[Massive hemorrhage within an acoustic neurinoma. Case report].

A 39-year-old male experienced unilateral right hearing loss and tinnitus for 7 years and was hospitalized after he suddenly developed severe headache, vertigo, and right facial paralysis. Computed tomography (CT) showed a round, high-density are in the right cerebellopontine angle. Magnetic resonance (MR) imaging demonstrated a crescent-shaped region of high signal intensity, representing hemorrhage, in the superior aspect of the tumor, surrounded by edema. The right internal auditory canal was enlarged. Four-vessel angiography disclosed neither an aneurysm nor an arteriovenous malformation. A right suboccipital craniectomy revealed an encapsulated mass 3 cm in diameter in the right cerebellopontine angle. The tumor was totally removed. Histological examination revealed a typical neurinoma composed of Antoni type A and B cells. After undergoing anastomosis of the right hypoglossal and facial nerves, the patient was discharged in good condition. In this case MR imaging demonstrated intratumoral hemorrhage (which is rare in cases of acoustic neurinoma) and the surrounding tissue more clearly than did CT scanning.

[Clinical study on the relationship between middle cerebral artery lesions and Binswanger type infarction].

To investigate the relationship between middle cerebral artery (MCA) trunk lesions and the etiology of Binswanger type (B type) infarction, which was demonstrated as a diffuse subcortical low density area/high intensity area by CT/MRI, patients with both MCA lesions and B type infarction were studied clinically. Eighteen patients with B type infarction were diagnosed among 224 patients with MCA occlusion/stenosis on angiography accounting for 8%. The incidence was as high as 25% in M2 stenosis. The mean age of B type infarction patients was 64 years and 16 of them were men. Chronologically stepwize/slowly-progressive deterioration of clinical manifestations were observed in 14. All patients had hemiplegia, though half of them were mild or moderate in severity. Furthermore, aphasia, Gerstmann syndrome and dementia were present in 10, 1 and 2 patients, respectively. Twelve had a history of hypertension, while 11 showed transient decreases with marked changes (more than 31 mmHg in mean arterial blood pressure) in arterial blood pressure during their clinical course. Out of 9 patients in whom cerebral blood flow (CBF) was measured by 133Xe injection method/inhalation method, 7 demonstrated mild to moderate decreases in mean CBF (more than 30 ml/100 g/min) with no relation to the severity of MCA lesions. These findings suggested that hemodynamic mechanisms associated with hypoperfusion due to marked fluctuations in blood pressure are accelerating factors of B type infarction and MCA lesions, even though ischemia in the subcortical area due to leptomeningeal anastomosis may be mild or moderate.

Studies on anti-liver cell membrane antibodies in sera of patients with autoimmune chronic active hepatitis.

In using the rat hepatocytes isolated and treated with formalin as antigen, IgG class antibodies to the liver cell membrane (LM-Abs) in sera of patients with chronic active hepatitis were examined by both indirect immunofluorescence (IF) and solid-phase enzyme immunoassay (EIA). Specificity of the reaction in IF and EIA was confirmed by an absorption test using different tissues and by inhibition test using the liver specific protein. Indirect IF test using isolated hepatocytes showed the linear staining in eight of 14 sera of patients with autoimmune chronic active hepatitis (CAH). Mean binding values and positive frequency of LM-Abs to the isolated hepatocytes which were detected by EIA were 4.18 +/- 2.99 and 71.4% (10/14) in autoimmune CAH, 3.16 +/- 2.50 and 50% (6/12) in primary biliary cirrhosis, 1.40 +/- 2.12 and 28.6% (4/14) in non-A, non-B, chronic hepatitis (NANB-CH) with serum gamma-globulin showing more than 2.0 gr/dl, 1.02 +/- 1.30 and 0% (0/13) in NANB-CH with gamma-globulin showing less than 2.0 gr/dl, 1.04 +/- 1.20 and 0% (0/15) in HB-CH, and 1.80 +/- 3.60 and 25.0% (3/12) in SLE. Titers of LM-Abs in serum of a autoimmune CAH patient decreased significantly after treatment with prednisolone, but their titers did not correlate with the activities of anti-nuclear antibodies or anti-smooth muscle antibodies. From these results, it was considered that some of NANB-CH patients with both LM-Abs and higher serum gamma-globulin more than 2.0 gr/dl might have some autoimmune characters similar to CAH.

[Red cell hypoplasia following autoimmune hemolytic anemia associated with T-CLL: report of a case and review of the literature].

A 78-year-old woman, who had axillary lymphadenopathy but no hepatosplenomegaly, was admitted because of lymphocytosis. The leukocyte count was 18.1 x 10(9)/l with 72% abnormal cells. Neither anemia nor thrombocytopenia was present. Many abnormal cells and erythroblasts were seen in the bone marrow. These abnormal cells had irregular nuclei but no granules in the cytoplasm. The surface markers of these cells were positive for E-rosette, CD 2, CD 3, and Leu 7 but negative for CD 4, CD 8, CD 11 (OKM 1), CD 16 (Leu 11), and HLA-DR. The DNA analysis revealed the rearrangement of T-cell receptor beta-chain genes. Direct Coombs test was positive and red-cell life-span (51Cr) was T 1/2 = 19.5 days. The patient was diagnosed as having T-CLL with mild autoimmune hemolysis and was followed without treatment. Seven months later, the leukemia cells of peripheral blood increased to 62.6 X 10(9)/l and the frank autoimmune hemolytic anemia developed. After prednisolone, vincristine and cyclophosphamide were administered, leukemia cells of blood decreased. Anemia with reticulocytopenia, however, persisted and direct Coombs test became negative. In the bone marrow at that time, many neutrophils and megakaryocytes besides leukemia cells were preserved, but erythroblasts were hardly seen, namely a pattern of red cell hypoplasia was observed. The patient deteriorated rapidly and died 26 months after initial recognition of lymphocytosis. When complement was added, the patient's serum obtained during red cell hypoplasia but not during autoimmune hemolysis inhibited BFU-E and CFU-GM in in vitro colony assays. This case indicates that not only B-CLL but also T-CLL is accompanied by immune hematocytopenia.

Thyroid antigen-mediated glomerulonephritis in Graves' disease.

We describe a patient with Graves' disease in whom marked proteinuria, microhematuria and hypoalbuminemia were associated. Renal biopsy demonstrated electrondense deposits in the capillary basement membrane, a finding consistent with immune complex glomerulonephritis. Indirect immunofluorescent examination with rabbit antihuman thyroglobulin indicated that these electron-dense deposits were thyroid antigen-mediated immune complexes.

[Alteration of prostaglandin metabolism in alcoholic liver disease: its association with platelet dysfunction after chronic alcohol ingestion].

Recently, hepatic microcirculation has been focused on as an important pathogenic factor in progression of alcoholic liver disease (ALD). Therefore, blood levels of several prostaglandins, which are associated with organ microcirculation, were determined in various liver diseases, including ALD. Blood thromboxane B2 (TXB2) level was significantly increased in ALD, when compared to other types of liver diseases, whereas both 6-keto prostaglandin F1 alpha (6-keto PGF1 alpha) and prostaglandin E were not changed. These consequences resulted in the imbalance of 6-keto PGF1 alpha to TXB2, which might promote platelet aggregation and blood vessel contraction. Indeed, the increase of beta-thromboglobulin and platelet factor 4 in blood was observed in ALD. Furthermore, in ALD, the rate of arachidonate-induced platelet aggregation was prominently enhanced, and malondialdehyde production in platelet, which was well correlated with blood TXB2 levels, significantly increased. Thus, the present study may indicate that, in ALD, hyper-aggregability of platelet is produced, because of the derangement of prostaglandin metabolism and platelet dysfunction.

Ultrastructure of basal cell adenoma in the parotid gland.

Basal cell adenoma of the parotid gland was studied with electron microscopy. The cells constituting this tumor were divided into three types of epithelial cells; ductal, myoepithelial, and squamous cells. The ductal cells, which were polygonal and cuboidal in shape, formed a sometimes distinct lumen. Glycogen were recognized in the cytoplasm of these cells. The myoepithelial cells appeared as plasmacytoid cells which contained abundant microfilaments. The squamous cells were characterized by the presence of well-developed tonofilaments and desmosomes. However, no secretory cells could be found, although small, electron dense granules were detected in the cytoplasm of the ductal cells. The granules were unlike secretory granules in their size, number and location. In consideration of the presence of secretory and myoepithelial cells, we reviewed previously reported literature and discussed the identification of secretory granules. From our and other reported results, we tentatively concluded that the electron dense granules described as secretory granules are not intrinsic secretory granules. Further, we suggested that the cell types and the histogenesis of basal cell adenoma are analogous to those of both pleomorphic and clear cell adenomas.

[Out-of-hospital sudden cardiac death: a comparative study spanning 10 years].

Incidence, etiology and time zones of sudden cardiac deaths were compared for 1986 and 1976. Totals of 1,140 cases of acute endogeneous deaths, 590 in 1986 and 550 in 1976, were sent for coroner's inquest in Kanagawa Prefecture. These were the materials for the present study. Sudden cardiac deaths included 239 (46.1%) in 1986 and 137 (37.4%) cases in 1976 in males, and 81 (47.1%) in 1986 and 74 (40.0%) cases in 1976 in females. There were 129 (21.9%) and 163 (29.6%) cases with cerebral hemorrhages in 1986 and 1976, respectively. The acute cardiac death was the most frequent cause among acute endogenous deaths, and it approximately doubled among males during an interval of 10 years. It was related to a marked increase in ischemic heart disease (from 89 to 170 cases) in males compared to a slight increase among females (from 58 to 76 cases). Non-ischemic acute cardiac deaths were frequently noted in males; 38 (27.7%) and 27 (11.4%) cases in 1976 and 1986, respectively. In ischemic heart disease, deaths most frequently occurred about midnight (from 12 a.m. to 1 a.m.) or in the evening (from 5 p.m. to 6 p.m.), and deaths due to acute cardiac failure occurred during sleep. Time zones of evening deaths in ischemic heart disease corresponded to the report of Muller et al., but, the peak about midnight was not reported. This difference may be explained by the circadian rhythm theory, however, heavy alcohol intake and spasmogenicity in the Japanese people may also play roles in midnight deaths.

[Aclarubicin-combined combination chemotherapy in patients with refractory or relapsed non-Hodgkin's lymphoma].

Patients with lymphoma who became refractory or resistant to standard chemotherapy including anthracyclines were treated with aclarubicin-combined chemotherapy including VP-16, ifosfamide, and carboquone in a multicenter study. Twenty-one patients were entered in this study, and 18 of them were evaluable. The median age was 52 years old (range 27-74), and there were 17 male and 3 female patients. The vast majority of patients were diagnosed as having diffuse lymphoma, of which 10 cases had large cell type. Surface markers were measured in 8 patients, of whom 4 had T-cell lymphoma. Remission was attained in 3 of 18 patients (17%) with one complete and lasting remission with T-cell lymphoma. In conclusion, the response rate in this study was poor, but this type of combination chemotherapy might be considered in patients with T-cell lymphoma.

[Chemotherapy of stage III-IV Hodgkin's disease. A retrospective analysis of the 53 cases collected from 9 institutions in Japan].

The clinical data of 53 patients with Stage III-IV Hodgkin's disease collected from 9 institutions in Japan were analyzed for the efficacy of chemotherapy. CR rate (78%) and five-year relapse-free survival (RFS) rate (61%) were higher in the patients treated with VEPA/CHOP regimen than those in patients with VEMP/BONP or MOPP/C-MOPP regimen, although the difference was not statistically significant because of the small number of the patients. As to 14 patients treated with CHOP regimen, CR rate was 87% and RFS curve trend toward plateau at 67% after 2 years and 3 months from the initiation of chemotherapy. Salvage therapy with adriamycin-based combination chemotherapy achieved CRs in 9 of 14 (64%) patients who had been treated with VEMP, BONP, modified MOPP or C-MOPP regimen. The CHOP regimen was effective in the treatment of III-IV Hodgkin's disease but an alternative multidrug chemotherapy with ADM, CPM, VCR, BLM, etoposide and procarbazine is recommended for achieving a higher CR rate and a better RFS. Prospective study is needed to establish the standard chemotherapy for Hodgkin's disease in Japan.

[Follicular lymphoma in Japan: retrospective study on natural history and therapeutic results in 50 cases collected from 4 institutions].

Clinical data of the 50 patients with nodal follicular lymphoma were collected from 4 institutions in Japan and were analysed retrospectively. A frequency of follicular lymphoma was 9.3% in the 364 patients with nodal lymphoma registered between 1981-1986. Twenty-one patients (42%) were classified as follicular medium-sized cell type (F-medium; identical with follicular small cleaved cell type by Working Formulation criteria), 12 (24%) as follicular mixed (F-mixed), and 17 (34%) as follicular large (F-large). There were 31 (62%) patients with Stage III-IV disease involving bone marrow in 6 patients, liver in 2, bone marrow and liver in 2, stomach in 1. Overall five- and ten-year survival rates were 65% and 32%, respectively, in the 49 patients whose median follow-up period was 3 years, ranging from 2 to 11 years. Adriamycin-based combination chemotherapy [ADM(+) therapy] produced 10 (100%) complete response (CR) of 10 patients with F-large lymphoma and 8 (80%) of them were free from relapse. But, in the patients with F-medium lymphoma, ADM(+) therapy produced one (17%) CR of 6 patients and their survivals were not superior to those treated with ADM(-) therapy, single agents or radiotherapy alone. There was no difference in the natural history or therapeutic results between the patients in Japan and United States, although the frequency of follicular lymphoma was extremely lower in Japan. ADM(+) therapy can be accepted as the first-line chemotherapy for the patients with F-large lymphoma, but the best modality of treatment should be investigated for the patients with F-medium lymphoma.

Intermediate-dose cytosine arabinoside in the treatment of recurrent or refractory non-Hodgkin's lymphoma.

Twelve patients with recurrent or refractory non-Hodgkin's lymphoma (NHL) were treated with an intermediate-dose of cytosine arabinoside (CA) by continuous i.v. infusion in combination with daunorubicin (DNR). Of the 10 patients evaluated, one obtained a complete response (CR) and three partial responses (PR), and an almost complete disappearance of the tumor for three weeks (minor response) was seen in two patients. Severe myelosuppression was a dose-limiting factor. Intermediate-dose CA is effective for recurrent or refractory NHL but it needs to be combined with the proper partner drugs to obtain better therapeutic results.

[Consecutive changes of R-R interval variation on ECG after abstinence in male alcoholics].

Heart rate variation were studied in 37 male alcoholics (age: 46.0 +/- 7.4 years old, duration of habitual drinking: 23 +/- 8 years.) who were admitted in Kurihama National Hospital within 48 hours of last drinking. The coefficient of variation (CV value = (standard deviation/mean R-R interval) X 100) of 100 successive R-R intervals in ECG during supine resting position was measured consecutively on admission, 7th, and 30th hospital day. The CV values on admission (2.50 +/- 0.81) and on 7th day (2.60 +/- 0.87) were significantly lower than those on 30th day (3.41 +/- 1.06, p less than 0.001). In 10 alcoholics, CV values were also measured on 60th day (3.32 +/- 1.18), which were not significantly different from that on 30th day (3.17 +/- 1.18). The ratio of CV to standard prediction value (CV/(-0.066 X age + 6.480)) were 0.67 +/- 0.24, 0.68 +/- 0.21 and 0.90 +/- 0.28 on admission, 7th and 30th day, respectively. The ratio on admission and on 7th day were significantly lower than those on 30th day (p less than 0.001). These findings are suggesting that the disturbance of heart rate variation in alcoholics is reflecting the deterioration of autonomic nervous system on alcohol withdrawal stage.

[A case of intracranial hemorrhage in a young infant due to vitamin K deficiency: comparison of MRI and CT findings].

We reported a case of intracranial hemorrhage due to vitamin K deficiency in a 43-day-old male infant whose MRI findings were compared with CT findings. The infant was vacuum delivered at the 40th gestational week and Apgar score was 9 at 5 min after birth. He weighed 2,750 g at birth and was breast-fed. His growth was normal after birth, but forty-one days after birth, the infant became febrile and vomited three times. Forty-two days after birth, right hemiconvulsions occurred and the infant vomited again. He was brought to our clinic forty-three days after birth, because cranial CT showed multiple intracranial hemorrhages. On admission, skin color was pale, and the patient was somnolent. The anterior fontanel was bulging and tense. Neurological examination revealed right hemiparesis. Since laboratory data indicated that intracranial hemorrhage resulted from vitamin K deficiency, administration of vitamin K and blood transfusion were carried out. MRI examination was made three days after admission, and demonstrated a posterior fossa subdural hematoma as well as a left frontal subdural hematoma associated with intracerebral hemorrhage. Five days after admission, left frontal subdural hematoma was removed by left frontal craniotomy, and twenty-six days after the operation he was discharged without any neurological deficits.(ABSTRACT TRUNCATED AT 250 WORDS)
