Isolation, characterization, and primary structure of three major proteins obtained from Mytilus edulis sperm.

Acrosomal proteins from Mytilus edulis sperms were separated into 11 fractions by reverse phase HPLC. The three major proteins, named M3, M6, and M7, showed strong egg vitelline coat lysin and first polar body releasing activities. The amino acid sequences of these proteins were determined. M6 and M7 were composed of 180 amino acid residues and showed high sequence homology (76%), while M3 was composed of 149 residues and showed 26% homology with M6 and M7. The disulfide linkage motif of the three proteins was similar and resembled the carbohydrate recognition domain (CRD) of C-type lectin. The C-terminal half of these proteins showed sequence homology with CRD of C-type lectins, but no homology with vitelline coat lysins of other mollusks. The proteins bound to asialofetuin-Sepharose in the presence of Ca2+ and were eluted with EDTA, indicating that they are Ca(2+)-dependent carbohydrate-binding proteins.

Ten-year survival and prognostic factors in multiple myeloma. Japan Myeloma Study Group.

Among 1119 Japanese patients with symptomatic multiple myeloma diagnosed between 1965 and 1981, 38 (3.4%) survived more than 10 years. Younger age, low tumour mass (absence of severe anaemia, hypercalcaemia, renal failure, and multiple bone lesions), low plasma cell percentage in bone marrow, mature and intermediate myeloma according to Greipp's criteria, and a positive response to subsequent treatment, were related to long-term survival according to univariate analysis. Multivariate logistic regression analysis indicated younger age and low tumour mass as pretreatment characteristics to be related to long-term survival. Prognostic factors proposed applicable to myeloma were also related to 10-year survival.

Changes in serum immunoreactive inhibin during ovulation induction in women with amenorrhea.

Changes in serum immunoreactive (IR)-inhibin were measured by RIA in two studies, in order to elucidate, firstly whether the pattern of IR-inhibin secretion is similar to that of estradiol (E2), and secondly, whether inhibin suppresses endogenous FSH release. Study 1: Purified urinary FSH (pFSH) or human menopausal gonadotropin (hMG) were daily injected intramuscularly into women with hypogonadotropic amenorrhea at 12 to 14 week intervals. PFSH and hMG stimulated IR-inhibin release in a similar fashion in the ovulatory cycles, but the increase in estradiol (E2) during pFSH administration was delayed and lower than that during the hMG cycles. This suggests that E2 and IR-inhibin are secreted independently from the granulosa cells. Study 2: Ovulation induction was performed in 18 cycles of 9 women with polycystic ovarian disease (PCOD) by the step-down administration of pFSH. The serum FSH concentration in cycles with premature LH release increased even after the dose of pFSH was reduced, and were significantly higher than those of cycles without premature LH release. It was also found that the serum IR-inhibin concentration in cycles with the premature LH release was 2 to 4 times as high as in cycles without premature LH release. This suggests that IR-inhibin does not suppress endogenous FSH release associated with premature LH release.

[Relation of gestational age, maternal body weight and age or serum alpha-fetoprotein and human chorionic gonadotropin at second-trimester].

Serum levels of alpha-fetoprotein (AFP) and human chorionic gonadotropin (hCG) were measured in serum samples of 1,964 pregnant Japanese women whose gestational age and singleton pregnancy were confirmed by ultrasound examination during the first trimester of pregnancy. Statistical analysis of log-linear regression to calculate multiples of the median (MoM) was accomplished by the SAS statistical method. The levels of the two analytes noticeably decreased as maternal body weight increased. However, maternal age did not have a significant effect on either of them. The MoM formulae were as follows: MSAFPMoM = AFP/exp(0.861 + 0.0685 x gestational age(weeks) - 0.00572 x body weight(kg)). MShCGMoM = hCG/exp(6.12 - 0.787 x gestational age(weeks) - 0.00613 x body weight(kg)). Gestational age and maternal body weight should be considered as regression functions for the adjustment of serum levels in risk estimation of fetal anomalies and fetal demise in Japan.

[A trial of setting of the reference intervals of thrombin antithrombin III complex and fibrin.fibrinogen degradation products in pregnant women].

Activity of coagulation and fibrinolysis during pregnancy differed greatly from that during a nonpregnant period. We examined the reference intervals by mean +/- 2 standard deviations for thrombin antithrombin III complex (TAT), total FDP (T-FDP), FDP-E, D-dimer before and after delivery. The reference intervals obtained were as follows: TAT (micrograms/l): 1.0-4.8 for 1st trimester, 2.0-10.2 for 2nd trimester, 2.8-14.2 for 3rd trimester, 2.6-16.3 for full term pregnancy, 0-10.5 for post partum. T-FDP (micrograms/ml): 1.5-4.2 for 1st trimester, 1.8-5.6 for 2nd trimester, 1.9-8.5 for 3rd trimester, 2.9-9.7 for full term pregnancy, 2.4-9.0 for post partum. FDP-E(ng/ml): 26.4-98.0 for 1st trimester, 42.4-176.7 for 2nd trimester, 53.1-222.5 for 3rd trimester, 82.4-297.9 for full term pregnancy, 56.9-240.6 for post partum. D-dimer (micrograms/ml): 0.2-2.0 for 1st trimester, 0.6-3.8 for 2nd trimester, 0.6-4.3 for 3rd trimester, 0.7-6.8 for full term pregnancy, 0.6-5.7 for post partum. Further studies are needed to determine the borderline values to determine the clinical abnormality, with a comparison of the reference intervals with the pathologic laboratory data.

[Late phase II study with 21-consecutive-day oral administration of etoposide for malignant lymphoma].

We conducted a multi-institutional (33 institutes), late phase II study with a 21-consecutive-day oral administration of etoposide for malignant lymphoma. Patient entry criteria were either those refractory to standard therapies or those for whom no appropriate therapy was available. A once-daily dose of 50 mg/body was administered for 21 consecutive days. Of the evaluable 83 among 88 entry patients, the overall response rate was 53.0% (44/83), including 10 CR; 52.5% (42/80, 9 CR) with non-Hodgkin's lymphoma and 100% (2/2, 1 CR) with Hodgkin's disease. Regarding abnormal laboratory findings, myelosuppression was observed; the incidence rates of leukopenia (23.3% with Grade 3), neutropenia (32.6%), hemoglobin decrease (17.4%) and thrombocytopenia (4.7%) were 70.9%, 65.1%, 54.7% and 19.8%, respectively. Major adverse reactions and their incidence were: anorexia 43.0%, alopecia 37.2%, nausea/vomiting 32.6%, fatigue 18.6%, stomatitis 15.1%, fever 7.0% and diarrhea 5.8%. Therefore, a 21-consecutive-day oral administration of 50 mg/body/day or 75 mg/body/day appears to be effective for the treatment of malignant lymphoma.

Experimental induction of arthritis in rats immunized with Escherichia coli 0:14 lipopolysaccharide.

OBJECTIVE: Escherichia coli 0:14 (E. coli 0:14) induces arthritis in rabbits, mice and rats. This study was designed to investigate the effects of lipopolysaccharide (LPS) on the rat arthritis model induced by systemic injections of E. coli 0:14. METHODS: The induction of arthritis in the ankles of rats immunized by subcutaneous injections with heat-killed E. coli 0:14 and its LPS was studied. The appearance and levels of serum IgM rheumatoid factor-like substance (RFLS) was also investigated. The localization of interleukin 1 (IL-1) and LPS in the ankle joints were investigated immunohistochemically. RESULTS: The induction rate of arthritis in rats immunized with LPS was the same as that in rats immunized with E. coli. LPS and IL-1 were detected in synovial cells, infiltrating cells and some cells on pannus in arthritic joints. Anti-LPS IgM levels in rats immunized with E. coli were as high as those in rats immunized with LPS. RFLS levels in rats immunized with LPS increased more gradually than those in rats immunized with E. coli. CONCLUSION: Our findings suggest that LPS induces arthritis resembling rheumatoid arthritis in rats. The detection of IL-1 in synovial cells in conjunction with LPS suggests that local stimulation of IL-1 production may play an important role in the induction of this experimental arthritis.

Malignant pheochromocytoma accompanied by microangiopathic hemolytic anemia: a case report.

A 63-year-old woman was in hospital for persistent backache. Four months prior to admission she had been pointed out as having hypertension for the first time. On admission, she had anemia (hemoglobin 7.0 g/dl) with reticulocytosis, and a blood smear showed fragmented erythrocytes. A bone marrow aspirate disclosed erythroid hyperplasia and invasion of cancer cells. The chest roentgenogram showed a coin-lesion of the right lung and left pleural effusion. A diagnosis of microangiopathic hemolytic anemia (MAHA) associated with carcinomatosis was made, but the primary site of the cancer was unknown. Respiratory failure developed and the patient died a month later. Surprisingly, the autopsy revealed a malignant pheochromocytoma arising from the right adrenal gland with massive metastases to the lungs, liver, lymph nodes and systemic bones, and also disseminated intravascular coagulation (DIC). The DIC would probably account for the MAHA in this case. To our knowledge, this is the first reported case of malignant pheochromocytoma accompanied by MAHA.

Combination chemotherapy with MCNU, vindesine, melphalan, and prednisolone (MCNU-VMP therapy) in induction therapy for multiple myeloma. Japan Myeloma Study Group.

From January 1988 until December 1990, 99 previously untreated patients with multiple myeloma (MM) were enrolled in a randomized prospective study comparing two combination chemotherapies with and without MCNU as induction therapy for MM; 49 patients with vindesine, melphalan and prednisolone (VMP therapy) versus 50 patients with MCNU, vindesine, melphalan and prednisolone (MCNU-VMP therapy). Seventy-two evaluable patients (34 patients in the VMP group, 38 in the MCNU-VMP group) were analyzed. The response rate was slightly higher with MCNU-VMP than with VMP (81.6% vs. 64.7%). In 43 responders (21 patients in the VMP group and 22 in the MCNU-VMP group) who were treated with the same regimen as the induction therapy to maintain remission, the remission duration was significantly longer in patients treated with MCNU-VMP than in those treated with VMP (median > 10.1 vs. 8.0 months, P = 0.018), particularly in patients with PS 3-4. The remission duration in the MCNU-VMP group was also slightly longer in the patients with stage III disease and who were older than 65 years. The median survival time showed no significant difference between the VMP group (20.3 months) and the MCNU-VMP group (> 15 months). Leukopenia, thrombocytopenia and nausea/vomiting were found to be somewhat severe in the MCNU-VMP group. In summary, MCNU-VMP therapy is effective as induction therapy for MM.

Non-invasive Doppler technique for detection of flow velocity in left internal mammary artery grafts.

The capability of Doppler to detect patency of left internal mammary artery grafts through the supraclavicular approach was evaluated. A total of 37 patients with a left internal mammary artery graft to the left anterior descending artery were divided into three groups, according to postoperative coronary angiographic studies. Group 1 comprised 20 patients with no significant stenosis of the left internal mammary artery and subtotal or total obstruction in the left anterior descending coronary artery proximal to the anastomotic site. There were ten patients with no significant stenosis of the left internal mammary artery and 75% stenosis of the left anterior descending coronary artery in group 2. Seven patients with significant stenosis of the left internal mammary artery formed group 3. Analysis of the flow velocity pattern in the left internal mammary artery graft included: (1) systolic peak velocity (SPV), (2) diastolic peak velocity (DPV), (3) systolic time velocity integral (SI), (4) diastolic time velocity integral (DI), (5) diastolic peak velocity/systolic peak velocity and (6) diastolic time velocity integral/diastolic time velocity integral+systolic time velocity integral. There were no significant differences in systolic peak velocity and systolic time velocity integral between the three groups. The diastolic peak velocity and diastolic time velocity integral were greater in group 1 than in groups 2 (P < 0.05, P < 0.01) and 3 (P < 0.05). The diastolic peak velocity/systolic peak velocity and diastolic time velocity integral/diastolic time velocity integral+systolic time velocity integral ratios in group 3 were significantly smaller than in groups 1 and 2 (P < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

The impact of diabetes mellitus on the prognosis of alcoholics.

In this study, the mortality of clinically treated Japanese alcoholics with diabetes mellitus was analysed. Fifty-one diabetic alcoholics without liver cirrhosis (DM), 23 diabetic and cirrhotic alcoholics (DM.LC), 44 cirrhotic alcoholics without diabetes (LC), and 354 alcoholics without either complication (AL) admitted to the National Institute on Alcoholism in 1985 were studied. Thirty-seven diabetics required insulin treatment, and 12 oral hypoglycemic agents. The 4.4-year survival and drinking status after discharge were studied in 1990. Stepwise logistic regression analysis showed that the estimated odds for death increased 8.10, 4.38, 3.70, and 3.27 times for the subjects with the alcohol misuse after discharge, DM, DM.LC, and LC, respectively. The 4.4-year survival rate of alcoholics who continued misusing alcohol was much lower in DM (26%,P < 0.0005) and LC (35%, P < 0.0001) than in AL (73%). The survival rate of those who stopped misusing alcohol was significantly higher in DM (90%, P < 0.0001), LC (88%, P < 0.0001) and AL (94%, P < 0.0005) than those who continued misusing alcohol. There was no significant difference in the survival rate between the alcoholics with DM.LC who continued misusing alcohol (50%) and those who stopped misusing alcohol (73%). In the dead patients, 56% of DM died unexpectedly or suddenly, whereas 71% of LD died of liver failure after hospitalization. These results suggest that diabetic alcoholics should be intensively educated for abstinence.

[Association of autoimmune disease in primary biliary cirrhosis].

Association of autoimmune disease in patients with primary biliary cirrhosis (PBC) was described. Forty-one patients with asymptomatic PBC (a-PBC) and 13 patients with symptomatic PBC (s-PBC) were examined. The overall prevalence of autoimmune disease in association with PBC was 63%. There was no significant difference in the frequency of autoimmune disease association between a-PBC (63.4%) and s-PBC (61.5%). The most frequent complication was Sjögren's syndrome (31.7% in a-PBC and 38.5% in s-PBC). Raynaud's phenomenon was seen in 7 patients (12.2% in a-PBC and 15.4% in s-PBC). Five of these 7 patients showed the CREST syndrome having the anti-centromere antibody. Association of 3 polyarthritis, 2 autoimmune hepatitis, 2 Hashimoto's disease, 1 systemic lupus erythematosus, 1 polymyositis and 1 temporal arteritis was seen in a-PBC patients.
