Inhibitory effect of progesterone on cell death of mouse uterine epithelium.

The protective effect of progesterone against cell death of mouse uterine epithelium was evaluated by examining the retention of 5'-[125I]iodo-2'-deoxyuridine [( 125I]IdUrd) incorporated into the whole uterus and the apoptotic index (percentage of apoptotic cells in total cells), which is a good index of physiological cell death. Castrated adult female mice were given a daily injection of oestradiol-17 beta for 3 days, and then an injection of [125I]IdUrd. They were then divided into 4 groups, which received a daily injection of vehicle only, oestradiol-17 beta (E), progesterone (P), or both oestradiol-17 beta and progesterone (EP), and were killed at intervals during these treatments for determination of 125I radioactivity retained in the whole uterus. On treatment with vehicle only, the 125I radioactivity retained in the uterus decreased rapidly, but treatment with E, P or EP reduced the loss of 125I radioactivity significantly. Progesterone did not antagonize the effect of oestradiol-17 beta on the 125I radioactivity retained in the uterus. The apoptotic index of uterine cells was examined by a similar experimental protocol, but without injection of [125I]IdUrd. In the group treated with vehicle only, the apoptotic indices of both luminal and glandular epithelia increased markedly, but the injection of E, P or EP suppressed these increases significantly. Progesterone did not antagonize the effect of oestradiol-17 beta on the apoptotic index. The apoptotic index of stroma was not affected by the injection of E, P or EP. On the other hand, progesterone completely inhibited the increase in the mitotic index of uterine epithelia induced by oestradiol-17 beta. These results show that progesterone alone or in combination with oestrogen reduced cell death in mouse uterine epithelium and that the effects of oestrogen and progesterone on uterine cell death were independent of their actions on cell division.

Aromatase activity in cultured ovaries from fetal and neonatal golden hamsters.

The production of oestrogen from androstenedione by ovaries of hamsters on day 14 of pregnancy and on days 0 and 3 after birth was investigated by culturing these ovaries for 48 h in serum-free medium containing insulin and [3H]androstenedione. Both oestrone and oestradiol-17 beta were produced in culture of ovaries on day 14 of pregnancy. The production of oestrogen increased about 2-fold in culture of ovaries on days 0 and 3. Dibutyryladenosine 3':5'-cyclic monophosphate stimulated the oestrogen production by ovaries on day 14 of pregnancy and on days 0 and 3. The present results indicate that the ovaries of fetal and neonatal hamsters have an aromatase activity and that these aromatase activities are responsive to adenosine 3':5'-cyclic monophosphate.

Postprandial plasma gastrin and secretin concentrations after a pancreatoduodenectomy. A comparison between a pylorus-preserving pancreatoduodenectomy and the Whipple procedure.

Based on the observation that patients given a pylorus-preserving pancreatoduodenectomy maintain higher gut hormonal levels than do patients who have received the classic Whipple surgical procedure, which seems most likely due to a postoperative difference in the remaining digestive tract, the postprandial plasma gastrin and secretin concentrations in patients who have received either surgery have been evaluated to examine this difference more fully. The subjects were 20 patients treated by a pylorus-preserving operation and 27 patients treated by the Whipple procedure whose concentrations were compared with those of 8 healthy control patients. The postprandial plasma gastrin concentrations were found to be similar in patients given the pylorus-preserving operation and the controls and were significantly lower in patients who underwent the Whipple procedure (p less than 0.05). Similarly, the postprandial plasma secretin concentrations did not differ in these two groups, whereas patients who underwent the Whipple procedure showed significantly lower concentrations at 60, 90, and 120 minutes (p less than 0.05). The above findings, as well as supportive data in the literature, indicate that the duodenal bulb and the gastric antrum, which are resected in the Whipple procedure and are kept in the pylorus-preserving operation, seem to play important roles in the gut hormonal release and that the pylorus-preserving operation is the superior surgical technique in terms of gastrin and secretin release.

[Clinical analysis of 17 cases of Japanese encephalitis experienced in the last ten years].

We studied on the prognosis-related factors on 17 cases of Japanese encephalitis experienced during the last ten years and compared the clinical features with those in previous reports, especially when the disease was prevalent. The patients, ranging from 33 to 91 years old, consisted of 7 men and 10 women. All of them showed an encephalitis type. 47.1% of the patients were completely cured, 35.3% cured with sequelae and 17.6% died. The mortality rate was decreased compared to that of previous reports. All of the death cases were women. There was no relationship between the mortality rate and the patients' age. As previously reported, the more severe the disturbance of consciousness, the higher the mortality was. However the rate of death was lower than that of previous reports. We conclude from this study that clinical features of the disease have changed when compared with that of the older days.

Pericholecystic abscess: classification of US findings to determine the proper therapy.

Sixteen patients with a pericholecystic abscess were classified into three groups according to the sonographic findings: Nine patients had an abscess in the gallbladder bed, three in an intramural location in the gallbladder, and four in an intraperitoneal cavity. In the gallbladder bed group, six patients with a localized abscess responded well to conservative therapy, followed by an elective operation. Two patients with a complicated abscess were initially treated with ultrasonically (US) guided percutaneous transhepatic gallbladder drainage and then with percutaneous transhepatic aspiration of the liver abscess, followed by an elective operation. A patient with a communicating abscess underwent an elective operation because the abscess completely disappeared after percutaneous transhepatic abscess drainage under US guidance. One of the three patients with an intramural abscess needed an immediate operation, and the other two were treated conservatively, followed by an elective operation. All four patients with an intraperitoneal abscess needed an emergency operation. Because each type of abscess received differing and successful treatments, the authors' method of classification of US findings seemed to provide a useful way to select the specific therapeutic procedure to achieve optimal results.

Fragile X chromosome in institutionalized male adults with mental retardation.

A clinical and cytogenetical study of mentally retarded male adults with an IQ of below 35 has been carried out in the Ranzan Colony for Mental Retardates, Saitama, Japan. The study showed one fragile X-positive retardate with IQ 23 in the 53 residential retarded patients, giving a hemizygote prevalence of 1.9% (1/53). Macroorchidism and elongated ears, practical markers for the clinical diagnosis of fragile X-linked mental retardation, were also detected in this 43-year-old subject with fra(X)(q27.3). He is amiable and no autistic behavior has been observed. The subject, however, has been suffering from epilepsy since childhood and has cataract in both eyes.

In vitro immunosuppressive properties of spergualins to murine T cell response.

Deoxyspergualin has strong immunosuppressive activity in animals. However, it shows less in vitro immunosuppressive activity at the therapeutic concentration used for in vivo administration. Recently, we reported that there are some technical problems with in vitro experiments. In this report, the effects of spergualins were examined under in vitro conditions which excluded these problems, and compared with cyclosporine A (CYA). Spergualins have suppressive effects on mixed lymphocyte response (MLR) and cytotoxic T lymphocyte induction. Furthermore, interleukin-2 (IL-2) induced proliferation of concanavalin A blasts and CTLL-2 were inhibited at low concentration. However, spergualins have little effect in the early stage of MLR or the mitogen response. These results suggest that spergualins act on the proliferation and differentiation of T cells which respond to growth factors, such as IL-2.

[The role of the Na, K-ATPase inhibitor in renal sodium handling in patients with essential hypertension].

The present study aimed to elucidate the role of Na, K-ATPase inhibitor in renal sodium metabolism in essential hypertension. Mean arterial pressure (MAP), heart rate(HR), urine volume (UV), urinary excretion of sodium (UNaV), endogenous creatinine clearance (Ccr), fractional excretion of sodium (FENa), plasma renin activity(PRA) plasma aldosterone concentration(PAC), plasma noradrenaline concentration (PNA) and urinary excretion of noradrenaline(UNA) were measured before and after intravenous injection of ouabain (0.1 mg/m2.BSA) in 12 normotensive(NT) and 22 mild-to-moderate essential hypertensive subjects(EHT). Following ouabain injection, UV, UNaV FENa significantly increased, but PRA decreased, in both NT and EHT. MAP, HR, Ccr, PNA, and UNA did not change significantly in either group. On the other hand, a significant decrease in PAC was observed in NT, but not in EHT. The changes of UNaV and FENa were significantly attenuated in EHT as compared to NT. No significant difference in change of MAP, HR, UV, Ccr, PNA, UNA, or PRA was demonstrated between NT and EHT. A significantly positive correlation was found between delta UNaV and delta FENa in both NT and EHT, while no significant correlation was observed between delta UNaV and delta MAP, delta UV, delta Ccr, delta PRA, delta PAC, delta PNA and delta UNA in either group. These results suggest that 1) Na, K-ATPase inhibitor clearly augments natriuresis by suppression of sodium reabsorption in renal tubules, 2) since this augmentation was attenuated, there is an elevation of endogenous Na, K-ATPase inhibitor(s) should be considered in EHT, and 3) an increase of the inhibitor might participate to the hypertensive mechanism in EHT.

[The effects of intravenous infused magnesium on hemodynamics and renal water-sodium metabolism in patients with essential hypertension].

Mean arterial pressure (MAP), heart rate (HR), urine volume (UV), urinary excretion of sodium (UNaV), endogenous creatinine clearance (Ccr), fractional excretion of sodium (FENa) were measured before and after intravenous infusion of 10% magnesium sulfate (an initial dose: Mg 13.5 mg/m2.BSA/15 min; a maintenance dose: Mg 2.7 mg/m2.BSA/105 min) in 6 normotensive subjects (NT) and 12 mild-to-moderate essential hypertensives (EHT). Following magnesium infusion, serum magnesium concentration (s-Mg) increased and reached the level of about 1.8 times basal value. Significant increases of UV, UNaV and FENa in both NT and EHT, and a similar tendency of Ccr in EHT were observed, while no significant change in MAP nor HR was found in the two groups. The changes in UNaV (delta UNaV) were positively correlated with those in FENa (delta FENa) and a similar tendency was shown between delta UNaV and change in Ccr (delta Ccr) in all subjects. While there was no significant percentage change of s-Mg (% delta s-Mg) nor of Ccr (% delta Ccr), those of UNaV (% delta UNaV) and FENa (% delta FENa) were significantly greater in EHT. It is concluded from these findings that magnesium infusion produces diuresis and the natriuresis which might result from suppression of renal tubular reabsorption of sodium, without any change in systemic hemodynamics in NT and EHT. The pronounced natriuretic response to magnesium in EHT might contribute to the hypotensive mechanism of magnesium loading in EHT.

[A case of thymic cyst with elevated CEA in the cystic fluid].

A 66 year-old male was admitted on October 31, 1987 because his chest X-ray film revealed a well-defined round shadow in the right inferior mediastinum. Chest CT showed a large homogenous mass in the right anterior mediastinum. The mass was identified as a cyst on echogram. Surgical removal of the cyst was done with right thoracotomy. The cyst was 15 cm in size and contained about 500 ml of grayish white fluid. Pathological examination showed a thymic cyst and no malignancy. Biochemical analysis of the cystic fluid revealed increased CEA value of 223.2 ng/dl in spite of normal CEA level in serum. CEA was demonstrated in the epithelium of the surface and Hassall's corpuscles by PAP stain. It is conceivable that this elevated CEA observed in the cystic fluid might be correlated with CEA-like antigens such as NCA, NFA and BGP.

The effects of 30% nitrous oxide on auditory evoked middle latency responses.

The effects of 30% nitrous oxide on auditory evoked middle latency responses (MLR) were studied in 11 healthy volunteers. The evoked potentials were averaged from electroencephalograms following stimulation by 2048 clicks at 10 Hz and of 0.5 ms duration. Latencies and amplitudes of the Pa-Nb wave were measured and compared with the control values. The mean amplitude after inhalation of nitrous oxide for 30 minutes nitrous oxide was 54.2 +/- 23.9 (SD) % of control value. This decrease was recognised as statistically significant (P less than 0.05). Latencies of the Pa-Nb wave did not alter during the inhalation of nitrous oxide. The results show that 30% nitrous oxide slightly suppressed the auditory cortex.

[Causes of death after an early gastric cancer operation].

From April, 1972 to December, 1984, 329 early gastric cancer case were operated on by this department. Results of these operations have revealed the following statistics: The 5-year survival rate of those with an m-cancer was 97.6% and for those with an sm-cancer, 92.6% and, the 10-year survival rate of m-cancer was 91.6% and sm-cancer, 89.9%, respectively. Twenty-nine postoperative deaths were recognized in this series, and the causes of death in each case have been analysed. Prophylactic measures to prevent postoperative death also are discussed.

Diagnostic evaluation of hepatobiliary scintigraphy using 99mTc-N-pyridoxyl-5-methyltryptophan in acute cholecystitis.

Hepatobiliary scintigraphy using 99mTc-N-pyridoxyl-5-methyl-tryptophan (99mTc-PMT) was carried out in 80 patients with benign biliary diseases, excluding cholecystectomized patients, for the diagnosis of acute cholecystitis. Using the conventional diagnostic criteria based on nonvisualization of the gallbladder in acute cholecystitis, the specificity, efficiency and positive predictability test results were 63%, 72% and 45%, respectively. When delayed intestinal transit of radioactive material was added to this criteria, the diagnostic value was significantly improved (p less than 0.01); the specificity, efficiency and positive predictability test results were 97%, 97% and 90%, respectively. Delayed intestinal transit of radioactive material should be considered in the diagnosis of acute cholecystitis.

Increase in collagen production with loss of androgen responsiveness in cultured androgen-responsive Shionogi carcinoma 115 cells.

The collagen production of androgen-responsive and -unresponsive Shionogi carcinoma 115 cells was investigated by culturing them in a medium with or without testosterone. Androgen-unresponsive cells were obtained by culturing a cloned androgen-responsive cell in a testosterone-free medium for 12 weeks. The collagen production of androgen-responsive cells slightly increased in the absence of testosterone, whereas testosterone did not affect the collagen production of androgen-unresponsive cells. Androgen-unresponsive cells produced 3-4 times more collagen than androgen-responsive cells. The major collagen produced by both androgen-responsive and - unresponsive cells migrated to the same position in sodium dodecylsulfate:polyacylamide gel electrophoresis. The present results indicate that the collagen production of androgen-responsive Shionogi carcinoma 115 cells increases with the loss of androgen responsiveness in culture.

Development of spindle-shaped cells and chondroid cells from androgen-dependent Shionogi carcinoma 115. A light and electron microscopic study.

Androgen-dependent Shionogi carcinoma 115 (SC115) is an undifferentiated medullary carcinoma consisting of compact round cells. However, when host male DS mice were castrated 2 weeks after tumor transplantation, tumors composed of compact round cells, spindle-shaped cells and chondroid cells grew 4 weeks after castration. Compact round cells with desmosomes were arranged in solid nests and exhibited immunoreactivity for keratin protein. Spindle-shaped cells had prominent rough endoplasmic reticulum, and appeared to secrete collagen. Chondroid cells had the characteristics of chondrocytes. The light and electron microscopic features were highly suggestive of a transition from compact round cells to spindle-shaped cells, and from spindle-shaped cells to chondroid cells. The histology of this tumor thus suggests that SC115 cells are able to change into chondroid cells via spindle-shaped cells.
