The serum and bile tegafur (FT), 5-fluorouracil (5-FU) and uracil levels after administration of UFT were assessed in 13 cases of malignant biliary tumor accompanied by biliary obstruction in comparison with FT alone. The serum and bile FT and 5-FU levels showed almost the same transition pattern in both groups, reaching to the plateau in 1-2 weeks and revealing cumulative effect by continual administration. Correlation was obtained between serum and bile levels except for 5-FU level in UFT group (p less than 0.05). Correlation between 5-FU and uracil was obtained in the serum in both groups (p less than 0.05), but no effect of uracil was observed. In bile, correlation was seen only in UFT group (p less than 0.05), and the effect of uracil was observed in bile 5-FU level.
The 117 cases of two regional (thoraco-abdominal) lymph node dissection were compared to the 56 cases of three regional (with neck area in addition) lymph node dissection for esophageal carcinoma. As for operative death and postoperative complications, there were no difference between these two groups. The prognosis of three regional dissecting group was better than that of two regional dissecting group concerning about the cases that lymph node metastases were less than 1 or 2, the depth of invasion was limited to just near adventitia (a1) and the locations were in upper or midthoracic esophagus. But also the rate of postoperative recurrence to the neck and uppermediastinal lymph nodes were high in spite of three regional dissection. Putting together with all cases, significant improvement of prognosis were not obtained in the three regional group compared to two regional group. So we thought the usual three regional lymph node dissection might not be sufficient. Recently we added midsternotomy and made the lymph node dissection in neck-thoracic junction more complete. This procedure did not make the operative death and postoperative complications increase and the improvement of prognosis was expected by it.

One hundred seventy-three patients were treated 256 times with chemotherapy supported by autologous bone marrow transplantation during the past 9 years. The two most commonly used protocols were (cyclophosphamide 1,600-2,400 mg/m2 + adriamycin 80 mg/m2 + ACNU 3 mg/kg) and (cyclophosphamide 1,600-2,400 mg/m2 + adriamycin 100 mg/m2 + CDDP 100-150 mg/m2). Among 115 patients in the therapeutic setting in which two courses were usually given, 75 were evaluable and the overall response rate was 42.7% with 12.0% CR rate. Breast and pediatric groups responded well; the response rate in breast cancer was 67.9% with 21.4% CR rate. Two patients with breast cancer who had multiple distant metastases and 2 pediatric patients are now alive with NED after 5 years of the treatment and seem to have been cured. The results in adjuvant settings have also been quite promising, e.g., 79.4% 5-year survival probability among breast patients mainly in stage III. Although drops of blood cell counts to the nadirs (WBC counts: less than 100-300) could not be prevented, the periods of myelosuppression appeared to have been effectively shortened so that the patients could be safely managed with intensified general supportive measures. Platelet counts are usually less affected, but the recovery is slower than for WBC counts. There were 13 patients who died within 10 weeks of the initiation of the treatment. Two of them succumbed to sepsis, and progressive disease was the cause of death in 8 patients whose terminal phases were undoubtedly affected by some infectious problems. We have shown that there are inverse relationships between infused numbers of CFU-GM and marrow recovery judged by the duration of neutropenia and the time required for neutrophils to recover over 500. Our recent laboratory experiments testing CFU-E, BFU-E and CFU-Mk in addition to MNC counts and CFU-GM showed that vulnerabilities of marrow progenitors seem to differ from cell lineage to cell lineage. This must therefore be taken into careful consideration in pursuing marrow transplantation.
We developed a high-yield culture system, consisting of a culture bag on a rotator. The culture bag has two compartments, an inner compartment separated from an outer compartment by a semipermeable membrane. Cells in an appropriate culture medium are placed in the inner compartment, and medium without serum is placed in the outer compartment. The bag is rotated at an angle of 45 degrees between 0.5 and 5 rpm in a 37 degrees C incubator. The medium is changed in the outer compartment at intervals when needed. The outer compartment acts as the feeder as well as allows catabolites to diffuse through the culture. In addition, since the system is a closed-system, sterile conditions can easily be maintained. Using this system, we demonstrated that up to 20 X 10(6) cells/ml of PBMC could be cultured in IL-2 with sufficient recovery rate and cytotoxicity.
Pharmacokinetics and clinical studies of imipenem/cilastatin sodium (IPM/CS), a combined preparation of a new carbapenem antibiotic and a dehydropeptidase-I inhibitor, respectively, were carried out in neonates and premature infants in a joint study by a co-research group. 1. Peak blood levels of IPM/CS when administered at 10 mg/10 mg/kg or 20 mg/20 mg/kg by 30- or 60-minute intravenous drip infusion were achieved at the end of infusion. A dose response was clearly observed between the doses and the peak levels achieved. 2. The areas under the blood concentration time curve (AUC) of CS were greater than those of IPM in most patients. Blood half-lives of IPM and CS tended to be longer in younger neonates and premature infants than in older subjects. The blood half-life of CS tended to be longer than that of IPM. 3. Cumulative urinary recovery rates of CS were greater than those of IPM, cumulative urinary recovery rates tended to be greater in older neonates and premature infants than younger subjects. 4. One hundred and thirteen patients were treated for bacterial infections with IPM/CS and 32 patients were treated prophylactically. Daily doses of IPM/CS ranged from 9 mg/9 mg/kg to 150 mg/150 mg/kg. 5. Clinical efficacies of IPM/CS were evaluated in a total of 56 patients with identified etiologic pathogens. The efficacy rate was 98.2% with 33 patients rated as excellent, 22 patients as good and 1 patient as fairly good. (Diagnoses were sepsis in 10 patients and meningitis in 2 patients, etc.) Fifty-seven patients with no identified etiologic pathogens were rated as excellent for 22 patients, good for 34 patients and fairly good for 1. The efficacy rate in these patients was 98.2%. Thirty-two patients were treated prophylactically and the results obtained were satisfactory. 6. Bacteriologically, the eradication rate was 94.5% in 56 patients; i.e., 52 were eradicated, 2 were decreased, 1 persisted and 1 was unknown. 7. Adverse effects were observed in 7 (4.4%) of 160 patients, i.e., 2 patients had diarrhea and 2 patients had rash, etc. Abnormal laboratory data considered related to the therapy occurred in 28 (17.6%) of 159 patients, with 10 patients with eosinophilia (6.3%) and elevation of GOT and/or GPT, etc. All these were non serious, and all values returned to normal after discontinuance of therapy. An abnormal prothrombin (PIVKA II) was observed in 1 of 10 patients tested.(ABSTRACT TRUNCATED AT 400 WORDS)
Pharmacokinetic, bacteriological and clinical studies on imipenem/cilastatin sodium (IPM/CS) were performed in neonates. The results were as follow: 1. A total of 27 patients consisting of 17 mature and 10 immature infants were treated with IPM/CS. Each dose was 20 mg/20 mg/kg, and it was administered 2 approximately 3 times daily, in a 1-hour intravenous drip infusion for 3 approximately 12 days. The clinical efficacy of IPM/CS in 10 patients with bacterial infections (2 with sepsis, 3 with suspected sepsis, 2 with pneumonia, 2 with urinary tract infection and 1 with acute omphalitis) was evaluated as excellent in all patients, with an efficacy rate of 100%. All 5 causative organisms found in 5 patients (Staphylococcus aureus in 1, Staphylococcus epidermidis in 1, Escherichia coli in 2 and Flavobacterium meningosepticum in 1) were eradicated. Among 27 patients administered IPM/CS, adverse reactions were observed in 2 patients. These were rash and diarrhea. As for abnormal laboratory test values, elevations of GOT and GPT were observed. 2. MICs of IPM against 14 clinical isolates (S. epidermidis 1, S. aureus 6, Streptococcus agalactiae 4, E. coli 1, Enterobacter cloacae 1 and F. meningosepticum 1) from neonatal patients with bacterial infections were examined. IPM showed good antibacterial activity comparable to that of cefotaxime against S. agalactiae; however, the activity against methicillin-resistant S. aureus was poor. 3. Serum levels of IPM and CS were investigated in a total of 22 patients consisting of 15 mature and 7 immature infants after 20 mg/20 mg/kg of IPM/CS was administered. IPM and CS produced peak serum levels at the end of the drip infusion. In mature infants, peak serum levels of IPM and CS were 31.8 micrograms/ml (17.1 approximately 59.0 micrograms/ml) and 59.9 micrograms/ml (35.6 approximately 99.0 micrograms/ml), respectively. In low birth weight infants, these were 25.0 micrograms/ml (16.8 approximately 41.8 micrograms/ml) and 55.2 micrograms/ml (33.8 approximately 82.4 micrograms/ml), respectively. Half-lives of IPM and CS were 1.0 approximately 2.7 hrs. and 0.9 approximately 7.4 hrs. in mature infants, and 1.6 approximately 3.0 hrs. and 1.3 approximately 9.7 hrs. in immature infants, respectively. Generally the longer half-lives were observed in the younger neonates. Serum levels of CS remained higher and half-lives of CS were longer than those of IPM. The pharmacokinetics in neonates were different from those in adults or children.(ABSTRACT TRUNCATED AT 400 WORDS)

Cefpodoxime proxetil (CPDX-PR, CS-807) was evaluated for its efficacy, safety and pharmacokinetics in children. CPDX-PR was effective in 93.6% of 47 cases with respiratory tract, middle ear, skin or urinary tract infections. Twice or 3 times daily administration of 3 mg/kg each was sufficient to treat streptococcal pharyngitis and Haemophilus influenzae infections. No severe adverse reaction was encountered in 52 cases treated with CPDX-PR. The serum half-life was approximately 2.17 +/- 0.24 hours after oral administration.
Cefteram pivoxil (CFTM-PI, T-2588), a new oral cephalosporin antibiotic, was evaluated for its safety and efficacy in children. Fifty-three patients with bacterial infections were treated with 8.3 to 21.8 mg/kg/day of CFTM-PI. The drug was very effective (efficacy rate 98.1%) in pharyngitis, otitis media, tracheobronchitis, pneumonia and skin infections. The antibacterial potency was very good against Haemophilus influenzae, Streptococcus pyogenes and Streptococcus pneumoniae, and was comparable with cephalexin against Staphylococcus aureus. No severe adverse reaction was encountered with the CFTM-PI therapy. The data suggest that CFTM-PI is a safe and effective antibiotic when used in children with susceptible bacterial infections.
The effect of halothane and enflurane on tracheal tone were studied in 21 patients during the induction of anesthesia. Endotracheal tube cuff pressure was used to measure tracheal tone. Anesthesia, maintained by nitrous oxide 70% in oxygen, was supplimented with succinylcholine drip infusion to immobilize the patient. Ventilation was controlled by a Volume-preset ventilator. In the halothane group, the initial cuff pressure was 14.8 +/- 1.3 (mean +/- SE) cmH(2)O but 10 min after 0.15 mg/kg of pancuronium injection, it increased to 21.7 +/- 2.3 cmH(2)O (control). Ten min after inhalation of 0.75% of halothane, cuff pressure decreased to 14.7 +/- 2.3 cmH(2)O (34 +/- 11% decrease from the control value). In the enflurane group, the initial cuff pressure was 17.6 +/- 1.8 cmH(2)O and it increased to 21.0 +/- 1.7 cmH(2)O (control) 10 min after pancuronium injection. Ten min after 1.7% of enflurane inhalation, cuff pressure decreased to 17.1 +/- 2.3 cmH(2)O (23.9 +/- 6% decrease from the control value). Halothane and enflurane produced similar tracheal dilatation in healthy individuals.
We isolated a mouse monoclonal antibody that disrupts Ca2+-dependent cell-cell adhesion of amphibian (Xenopus laevis) cells. When added to culture medium, the monoclonal antibody completely disrupted cell-cell adhesion of amphibian cells in monolayer culture and specifically inhibited Ca2+-dependent cell-cell adhesion of dissociated cells in reaggregation experiments. The monoclonal antibody recognized a 140 kDa cell surface glycoprotein antigenically different from the previously reported Ca2+-dependent cell-cell adhesion molecules (cadherins).

For breast tissue characterization, three acoustic parameters, integrated attenuation coefficient(IAC), frequency-dependent attenuation coefficient(FDAC) and instantaneous frequency(IF), were estimated in vitro and processed to make images of these parameters for resected human breast specimens. Experimental system with a 5 MHz center frequency was used and the same estimating and imaging system was connected with a clinical diagnostic device with a 7.5 MHz annular array probe (Aloca SSD 270) for in vivo type studies. To estimate these parameters, fast Fourier transform and spectral difference method were adopted for IAC and FDAC and the Hilbert transform for IF. Large statistical variances were found in the estimated values of these parameters. These dispersions of data may be due to insufficient size and tissue heterogeneity of the breast specimens. Additional miscellaneous problems may be involved. However, all of these can be, in part, overcome by averaging procedures. The significance of estimating and imaging these parameters for breast tissue characterization was discussed.
Experiments were performed to investigate the effects of mecobalamin (CH3-B12) on disorders of testicular function. Male rats received subcutaneously an injection of doxorubicin (ADR) three times a week for 3, 5 and 7 weeks. Thereafter, they were periodically sacrificed for the examination of cauda epididymal sperm profiles and the morphology of the testis. ADR treatment caused a decrease in sperm counts, sperm motility and the diameter of the seminiferous tubules, and an increase in the percentage of abnormal sperms. These damaging effects depended on the dosage and period of treatment with ADR. Based on these findings, rats with testicular dysfunction induced by the treatment with ADR at a daily dose of 0.25 mg/kg, three times a week for 5 weeks were prepared to investigate the effects of CH3-B12 on oligozoospermia. CH3-B12 was given intraperitoneally to the oligozoospermic rats six times a week for 5 and 10 weeks. The results obtained indicated that 1,000 micrograms/kg of CH3-B12 caused a significant increase in both the sperm counts and the diameter of the seminiferous tubules. These results suggest that CH3-B12 is a candidate drug for oligozoospermia.
The influence of indometacin farnesil (IMF), a prodrug of indomethacin, on blood coagulation was compared with indomethacin (Ind) in normal rats and warfarin treated rats. In normal rats, 30 mg/kg of Ind very markedly shortened the extrinsic coagulation time and decreased the hematocrit, GOT, GPT and ALP in plasma at 24 hr after administration, but 3 mg/kg of Ind and both 10 and 100 mg/kg of IMF did not influence any parameter. In warfarin treated rats, 2.5 mg/kg of Ind decreased the normal prothrombin level at 48 hr, and 10 mg/kg of Ind prolonged the blood coagulation time, decreased the normal prothrombin level and hematocrit, and increased the PIVKA-II level. Moreover, at 48 hr, 3 of 6 rats in the 10 mg/kg Ind-administered group died due to intestinal bleeding. IMF at all dosages examined did not affect any of the above parameters.
The influence of aztreonam (AZT), a new monobactam antibiotic, on blood coagulation was compared with latamoxef (LMOX), cefoperazon (CPZ), cefotetan (CTT) and ampicillin (ABPC). These antibiotics were administered intraperitoneally once a day for 7 days to rats fed a vitamin K-deficient diet. On the 8th day, blood coagulation activity and bacteria in the caecum were observed. 1) One hundred mg/kg and 300 mg/kg of LMOX prolonged the blood coagulation time and decreased normal prothrombin in a dose-dependent manner. CPZ, CTT and ABPC (300 mg/kg each) decreased both of these parameters, but not as much as LMOX did. AZT (300 mg/kg) had no effect on these parameters. 2) Three hundred mg/kg of ABPC and LMOX decreased the viable cell counts of all aerobic G (+), aerobic G (-), and anaerobic bacteria and yeast, while AZT decreased only aerobic G (-) bacteria. These results suggest that AZT may be less likely to induce hypoprothrombinemia than other antibiotics in clinical use.
