Elevation of the extracellular glutamate concentration in the hippocampus after total cerebral ischemia related to the deterioration of the recovery in EEG and evoked potentials in dogs.

The concentrations of extracellular glutamate (Glu), aspartate (Asp) and glycine (Gly) were measured by microdialysis method in the cortex and hippocampus before, during and after 15 min of total cerebral ischemia in dogs. The correlations between the concentrations of amino acids and the changes in EEG and evoked potentials (EP) after ischemia were evaluated. Total cerebral ischemia was achieved by occluding the ascending aorta and the caval veins. The concentrations of Glu in the hippocampus significantly increased from 1.73 +/- 0.59 (mean +/- SEM) nmol.ml(-1) at pre-ischemia to 5.46 +/- 1.34 (P < 0.05) during ischemia and 14.37 +/- 3.70 (P < 0.01) 0-15 min after ischemia, and returned to the pre-ischemic level 30 min after ischemia. The concentration of hippocampal Glu 0-15 min after ischemia had significant negative correlations with the EEG-EP scores (0 = serious deterioration of electrical function and 6 = normal electrical function) 30 min, 3 hr and 5 hr after ischemia (r = -0.69, P < 0.05 : r = -0.67, P < 0.05 : r = -0.70, P < 0.05, respectively). The increase of the extracellular Glu concentration in the hippocampus immediately after ischemia may aggravate the neurological outcome after total cerebral ischemia.

Molecular analysis of patient and carrier genes with congenital steroid 21-hydroxylase deficiency by using polymerase chain reaction and single strand conformation polymorphism.

Steroid 21-hydroxylase deficiency is a major cause of congenital adrenal hyperplasia and is caused by genetic impairment of this enzyme. Since approximately 80% of cases are caused by point mutations of the CYP21B (CYP21A2) gene, whereas the remaining 20% are due to deletion of this gene, we used the polymerase chain reaction single strand conformation polymorphism technique for rapid and accurate diagnosis of this disease. Of 23 patients examined, 1 had a hemizygous CYP21B gene. 18 patient's genes localized their harmful mutations or deletion on both the alleles, while 4 of them found their causative mutations on one of the two alleles, and 1 failed to find any responsible mutation. All the mutations (four nucleotide substitutions) detected are also found in the CYP21A (CYP21A1) pseudogene. A mutation at the intron 2 site is most prevalent in both salt-wasting and simple virilizing forms of the disease, and accounts for 37% of the patient's genes (17/46). Pedigree analysis of these mutations revealed that the mutations (at least four of them) occurred de novo at a considerable frequency on both the paternally and maternally inherited chromosomes. This result could explain occasional discordance of the diagnosis using HLA typing with the clinical symptoms.

de novo mutation causes steroid 21-hydroxylase deficiency in one family of HLA-identical affected and unaffected siblings.

Over 90% of congenital adrenal hyperplasia (CAH) results from 21-hydroxylase deficiency. Because the CYP21B gene is located within the HLA complex and is very tightly linked to HLA markers, HLA typing is widely used for prenatal diagnosis and identifying heterozygous family members. In the course of a study on identification of heterozygous family members with HLA typing, we recognized an unusual family case in which three siblings share the same HLA haplotype, and only one of them had the simple virilizing form; her two siblings did not have any endocrinological abnormalities. We investigated the mode of genetic transmission by using polymerase chain reaction and single stranded conformation polymorphism. The present study revealed that the proband was a compound heterozygote with the intron 2 mutation that causes aberrant RNA splicing and the missense mutation of exon 4, while the other siblings and the father had only one allele of a missense mutation in exon 4; the mother is a normal homozygote. This result together with DNA fingerprint analysis strongly suggest that the intron 2 mutation occurred de novo in the maternally inherited gene of the proband. This seems to be the first case of a de novo mutation of the CYP21B gene that causes CAH.

Effects of menatetrenone on bone loss induced by ovariectomy in rats.

The effects of menatetrenone, a vitamin K2 homologue, on bone loss induced by ovariectomy in rats were studied in 3 experiments. Menatetrenone was given as a dietary supplement. In experiment 1, at 2 weeks postovariectomy, menatetrenone (10 mg/kg/day given for 2 weeks) inhibited the decrease in bone density of the femoral metaphysis induced by the ovariectomy. In experiment 2, menatetrenone (3 or 30 mg/kg/day given for 6 months) inhibited the decrease in bone strength of the femur and the decrease in calcium and hydroxyproline content of the femoral diaphysis at 6 months postovariectomy. In experiment 3, menatetrenone treatment, at 30 or 100 mg/kg/day for 6 months, protected against the decrease in bone strength and calcium and hydroxyproline content in the bone loss model induced by ovariectomy and calcium-deficient diet. These findings suggest that menatetrenone protects against the bone loss induced by ovariectomy.

Application of enzyme-linked immunosorbent assay for the seroepizootiological survey of antibodies against porcine cytomegalovirus.

Enzyme-linked immunosorbent assay (ELISA) for the detection of antibodies against porcine cytomegalovirus (PCMV) was developed using the OF-1 strain of PCMV as an antigen. Results of the ELISA were compared to those of indirect fluorescent antibody (IFA) and serum neutralization (SN) tests. ELISA and IFA test were found to be extremely sensitive more than SN test. All of 11 tested sera were highly reactive in both ELISA and IFA test, but 6 of them were antibody-negative in the SN test. The retrospective survey of 436 fattening pig sera collected in Japan in 1981 showed that 433 (99.4%) of them were highly antibody-positive in ELISA.

[The effect of soluble fiber dietary supplement on constipation in 3 patients with dysphagia who suffered from cerebral infarction with special reference to serum diamine oxidase activity].

Dietary supplement with soluble fibers was given to 3 patients with stroke and dysphagia and obtained improved defecation condition. These clinical effects of soluble fibers should reduce the burden of the patients and families. Long-term inaction of the gastrointestinal tract or continuation of supplementary diet without fibers will induce atrophic intestinal mucosa and abnormal intestinal function. The addition of dietary soluble fibers into supplementary diet can improve the atrophy of the intestinal mucosa and the decline in function of the intestine, constipation and meteorism. The serum diamine oxidase activity, which is regarded as a parameter of intestinal mucosal atrophy, increased with the improvement of constipation and meteorism after addition of dietary soluble fibers. We think that dietary soluble fibers are necessary, especially for the patients who have low diamine oxidase activity. The measurement of serum diamine oxidase activity should be an effective method to evaluate intestinal mucosal atrophy and estimate dietary fibers. We selected a supplementary diet, Enrich-SF, which contains Fibaron, a kind of soluble fiber, galactomannan purified from guar, because this canned supplementary diet has only one kind of soluble fiber. Some consider that soluble fibers are fermented to short-chain fatty acids in the intestinal tract, and improve the disordered bacterial flora in the intestine, resulting in more regular intestinal function. Attention should be paid to dietary fibers in cases of long-term tube feeding.

[A case of gastric cancer with multiple liver metastasis treated with low dose CDDP therapy on an outpatient basis after palliative gastrectomy].

A 59-year-old-male with advanced gastric cancer and multiple liver metastasis was treated by intravenous administration of MMC (30 mg/body) and oral administration of HCFU (400mg/body/day) initially after palliative gastrectomy. CT scan revealed that liver metastatic lesions were undetectable at three months after operation. Seven months later, administration of HCFU was discontinued because liver metastatic lesions were observed again. Intravenous administration of MMC (10mg/body)+ACR (20mg/body) was performed four times weekly, but liver metastatic lesions were increasing in size. From 15 months after operation intravenous administration of CDDP (25mg/body/week) was started on an outpatient basis. After administration of CDDP 6 times, liver metastatic lesions were diminished in size. By CT scan the maximum percent reduction of liver metastatic lesions was 97% and duration of partial response was seven months. Eight months later, liver metastatic lesions regrew. At 28 months after operation, the patient died of liver metastatic lesions. This low dose regimen of CDDP on an outpatient basis is potentially useful in the treatment of advanced gastric cancer with multiple liver metastasis.

Estimation of the functional reserve of the human liver by urinary D-glucaric acid excretion after vitamin C administration.

The excretion of D-glucaric acid in the urine (uGA) correlates with the total liver content of hepatic cytochrome P-450, the metabolism of which depends on adenosine triphosphate (ATP) being produced by intrahepatic cellular mitochondria. Five cases of compensated liver cirrhosis group with less than 0.4 mg/kg/min of the maximum removal rate of indocyanine green (ICGR max), and 5 cases with normal hepatic function (control group), were monitored for uGA before and after P-450 activation induced by administration of 1 g of vitamin-C. Before vitamin-C administration, no differences in uGA excretion were observed comparing the cirrhosis with the control group. After administration of vitamin-C, the excretion of uGA was significantly lower in the cirrhosis group. The measurement of uGA is considered to represent vitamin-C induced activation by P-450, and is a new method for evaluation of the functional reserve of the liver.

[Pharmacokinetic, bacteriological and clinical studies on S-1108 in children].

Pharmacokinetic, bacteriological and clinical studies on S-1108 were performed in children. The results were as follows: 1. A total of 11 patients were treated with S-1108. Each dose was 3 mg/kg, orally administered 3 times daily for 4-14 days. The clinical efficacies of S-1108 in 10 patients with bacterial infections (1 with bacteremia, 4 with pneumonia, 1 with acute maxillary sinusitis, 1 with scarlet fever and 2 with streptococcal pharyngitis) were evaluated as excellent in 8 patients and as good in 2 patients with an efficacy rate of 100%. Only one patient with staphylococcal scalded skin syndrome due to methicillin resistant Staphylococcus aureus (MRSA) who received gamma-globulin was not evaluated. Fourteen causative strains of 5 species were found in 10 patients. Three strains of Streptococcus pneumoniae out of 5, 2 of 3 Branhamella catarrhalis strains, none of Staphylococcus aureus and all 3 strains of Streptococcus pyogenes were eradicated. No adverse reaction was observed in any of the 11 patients. 2. MICs of S-1108 against 5 clinically isolated S. pneumoniae from cases of infections were examined. All of them were relatively highly resistant to penicillins. S-1108 was compared with cefteram pivoxil, cefpodoxime proxetil, cefaclor and cefixime, and it showed better antibacterial activity or than other cephems. 3. Double peaks were obtained in plasma levels of S-1108 orally administered at a dose of 3 mg/kg at 30 minutes after meal and were 1.03 microgram/ml and 0.74 microgram/ml at 1 and 4 hours after administration, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Pharmacokinetic, bacteriological and clinical studies on cefditoren pivoxil in children].

Pharmacokinetic, bacteriological and clinical studies on cefditoren pivoxil (CDTR-PI, ME 1207) were performed in children. The results were as follows: 1. A total of 18 patients (19 infections) were treated with CDTR-PI. The doses ranged 2.1-3.2 mg/kg, and it was orally administered 3 times daily, for 4-10 days. Clinical efficacies of CDTR-PI in 18 patients with 19 bacterial infections (3 with tonsillitis, 1 with bronchitis, 7 with pneumonia, 1 with acute maxillary sinusitis, 4 with otitis media, 1 with urinary tract infection, 2 with skin and soft tissue infection) were evaluated as excellent in 13 infections and as good in 6 infections with an efficacy rate of 100%. Twelve causative strains of 5 species were found in 11 patients. Streptococcus pneumoniae in 2 cases out of 3, Haemophilus influenzae in 4/4, Staphylococcus aureus in 2/2, Haemophilus parainfluenzae in 2/2 and Escherichia coli in 1/1 were eradicated. Two patients had mild diarrhea but did not need specific treatment. Severe adverse reaction was not observed in any of the 18 patients. 2. MICs of CDTR were examined against 4 clinically isolated S. pneumoniae strains. Two strains of S. pneumoniae were relatively resistant to penicillins. 3. Pharmacokinetic studies Peak serum CDTR concentrations in 3 patients were 2.38 micrograms/ml, 0.72 micrograms/ml and 2.25 micrograms/ml at a dose of CDTR-PI 3 mg/kg orally administered at 30-minute after meal.(ABSTRACT TRUNCATED AT 250 WORDS)

Spectroscopic study of the interaction between poly-(9-vinyladenine) and single or multistrand RNA.

The interaction between poly(9-vinyladenine) (PVAd) and poly[r(U)] was investigated by means of uv, CD, 1H-, and 31P-nmr spectroscopies. The interaction was dependent on the molecular weight of PVAd determined by uv and CD spectroscopies. Based on imino proton nmr, it was clearly found that PVAd formed the complex with poly[r(U)] by complementary hydrogen bonding. The interaction of PVAd with double- and triple-stranded helices of RNA was also investigated by uv melting behavior and 31P-nmr spectroscopy. The results suggested that PVAd could not interact with the double-stranded poly[r(A)].poly[r(U)] but did with the triple-stranded RNA.

Glycosidically bound sialic acid levels as a predictive marker of postoperative adjuvant therapy in gastric cancer.

A group of 293 gastric cancer patients were examined to see if the preoperative value of glycosidically bound sialic acid is a predictor of prognosis and effectiveness of postoperative adjuvant therapy. All patients had gastrectomies and were histologically confirmed to have primary adenocarcinoma of the stomach. Some patients then received either postoperative adjuvant chemotherapy or immunochemotherapy. Patients with sialic acid levels less than 74.5 mg/dl survived significantly longer than those with sialic acid levels of 74.5 mg/dl or of 85.3 mg/dl and over. No significant differences in survival were found among patients treated by gastrectomy alone, gastrectomy plus chemotherapy and gastrectomy plus immunochemotherapy. However, patients with abnormally elevated levels of sialic acid survived significantly longer when they were treated with immunochemotherapy after gastrectomy than those treated by gastrectomy alone or with chemotherapy after gastrectomy. By using Cox's multivariate regression model, pTNM stages, postoperative adjuvant therapy (chemotherapy and immunochemotherapy) and preoperative serum levels of sialic acid were examined as prognostic variables. Postoperative therapy was a significant prognostic variable in patients with abnormally elevated levels of sialic acid. The preoperative serum level of sialic acid is a promising predictive marker of the response to postoperative adjuvant immunochemotherapy.

Rheumatoid wrists treated with synovectomy of the extensor tendons and the wrist joint combined with a Darrach procedure.

Forty-three rheumatoid wrists in 43 patients with bilateral wrist involvement were treated with synovectomy of the extensor tendons and wrist joint combined with a Darrach procedure in the period from 1966 to 1986. Clinical and radiologic assessment of the wrists was carried out after an average follow-up period of 11 years, with comparison of the treated and the opposite untreated wrists. The authors confirmed what others have concluded regarding the operation: pain was generally decreased, forearm rotation increased, and wrist extension and palmar flexion changed little. Radiologically, carpal collapse and palmar carpal subluxation progressed nearly parallel to the opposite wrists, but ulnar carpal shift was much greater in the surgically treated wrists. Therefore it is suggested that some measure to prevent ulnar carpal shift, such as Clayton's tendon transfer or radiolunate arthrodesis, should be included in this operation.
