Forensic diagnosis coding and retrieval by microcomputer.

A system is described for coding of anatomic--including forensic--pathologic diagnoses from necropsy cases using a microcomputer. The SNOMED software from the College of American Pathologists is utilized. RAM-disk software is employed to enhance the speed of operation of the system. The microcomputer can be operated by clerical staff versed in medical terminology. Speed of entry and retrieval, with minimal manual interaction, is an important aspect of operation. Equipment and storage costs are minimal.

Misrepresentation of stimulant street drugs: a decade of experience in an analysis program.

The findings of the Los Angeles County Street Drug Identification Program over the period from 1971 to 1980 for analysis of 1163 samples of street drugs represented as stimulants are presented. Of the 614 alleged cocaine samples, cocaine was found to be absent in 19.2%, found combined with stimulant substitutes in 22.5%, and found by itself in 58.3% of these samples. The validity for 473 alleged amphetamine samples was only 27.1%, with 23.0% of samples found to contain stimulant substitutes along with amphetamine, and 49.9% of the samples lacking any of the alleged drug. Seventy-six alleged methamphetamine samples showed 32.9% validity, with 3.9% of samples having mixtures of alleged drug and substitutes, and 63.2% of samples lacking any methamphetamine. A total of 15 different stimulant substitutes were found in 74 combinations. Caffeine, ephedrine, lidocaine, and procaine were the commonest substitutes.

Geographic forensic medicine and forensic sciences.

The necessity of learning more about the criminality and the culture of persons from overseas is upon us. As forensic scientists, we have to take a lead in presenting information to our colleagues that would facilitate their investigations. In this paper, we look at many of the different cultures that have been presented to American authorities, and the activities of the Milton Helpern International Center for the Forensic Sciences are discussed.

The determination of cocaine and its major metabolite, benzoylecgonine, in postmortem fluids and tissues by computerized gas chromatography/mass spectrometry.

An analytical procedure for the simultaneous determination of both cocaine and benzoylecgonine in postmortem fluid and tissue samples has been developed by using computerized gas chromatography/mass spectrometry and gas chromatography using a nitrogen/phosphorus (N/P) detector. Both methods are accurate and sensitive and allow the determination of tissue concentrations of cocaine and benzoylecgonine as low as 0.015 microgram/mL.

Anatomical distribution of human bite marks in a series of 67 cases.

A study was made of the anatomical distribution of human bite marks, as well as their distribution by age of victim and type of crime involved. Bite marks in this study occurred primarily in sex-related crimes, child abuse cases, and cases involving other types of physical altercations. Bite marks were found on virtually all areas of the body, with more than one bite mark on 40% of the victims. Female victims were most commonly bitten on the breasts, arms, and legs, while male victims were most commonly bitten on the arms and shoulders.

Hydromorphone detected in bile following hydrocodone ingestion.

Two similar cases are reported here in which Tussionex, a preparation containing hydrocodone and phenyltoloxamine, caused or contributed to death. Toxicological analyses revealed a high concentration ratio of hydromorphone to hydrocodone in the bile in both cases. It is postulated that the finding of hydromorphone is due to the metabolism of hydrocodone.

Demonstrative evidence developed at the autopsy.

A postmortem examination early during the investigative period of the murder of a young law enforcement officer on the edge of the Kansas Turnpike led to the development and application of a technique for the preservation of evidence by a visiting consultant. This resulted in the construction of a life-sized head which was used to demonstrate the entrance and exit sites of two bullets through the head and the site of the injury to the head from a pistol butt. This exhibit provided a dynamic demonstration of the relationship of the fatal weapon to the victim and contributed to the understanding of the paths of the bullets which passed through the victim's head into the ground. The technique is described.

The Milton Helpern International Center for the Forensic Sciences represents a unique self-supporting reference resource available at no cost to any forensic practitioner in the work or any student, faculty member or person in law, medicine, or law enforcement who has an interest in obtaining information in support of their requests for knowledge in the forensic sciences. The Registry of Unusual Cases represents an outstanding resource available at the Center for the collection, registration, and dissemination of information to anyone who reports or inquires about similar cases. This service is explained and illustrated in this article.

Analysis of dorsal pitting in the os publis in an extensive sample of modern American females.

Pubic bones from 486 modern American females between the ages of 13 and 99 were studied in an attempt to explore the relationship between the degree of dorsal pitting and the following variables: (1) number of full term pregnancies, (2) inteval since last pregnancy, (3) age of decedent.A statistical association was found between the number of full term pregnancies and the degree of dorsal pitting. However, the correlation is not strong; 17 females reported as being nulliparous have "medium to large" dorsal changes whereas 22 females having from one to five full term pregnancies have an absence of dorsal changes. Females who had their last child 15 or more years prior to death are more apt to have "medium to large" dorsal changes than are females who have more recently given birth. This effect appears to be independent of the number of full term pregnancies, but is probably related to age. When the data are analyzed by multiple regression analaysis, it is found that age and number of pregnancies are most important in predicting the degree of pitting and the effect of the interval on pitting is not significant. Age is found to be an important variable independent of the number of full term pregnancies. In nulliparas, an absence of dorsal pitting is far more frequently found in females younger than 30 than in those over 30.

Phencyclidine-related deaths in Los Angeles County, 1976.

Concentrations of phencyclidine in blood and liver are presented in five fatal cases occurring in Los Angeles County in 1976. Eleven other deaths in which phencyclidine contributed to death are described; acute psychotic reactions were observed in some of these cases. Two cases involved the drowning of individuals whose swimming capabilities may have been diminished from the effects of PCP. One case is presented in which a 20-year-old male took a massive overdose of phencyclidine for suicidal purposes.
