[Clinical experience with daily doses of misonidazole (author's transl)].

In this pilot study daily low doses of misonidazole (in the range of 1 to 2 g) up to cumulative doses between 7 and 19 g/m2 were used. Serum levels were analysed at different times after administration and according to several dose regimens. We related the cumulative doses to the incidence and severity of the observed peripheral neuropathies. The aim was to find an effective daily low-dose schedule of misonidazole with a clinically acceptable incidence of side effects. Some impressive clinical responses were observed.

[Combination therapy of local tumour recurrence in patients with breast cancer (author's transl)].

Treatment of 54 patients with extensive loco-regional recurrence of breast cancer is reported. Combined radio-chemotherapy was given to all patients with the aim of reducing the irradiation dosage and also decreasing the side effects of chemotherapy by reduced drug dosage. In this way a significant improvement in the quality of life could be achieved. The remission rate in patients with their first recurrence was 87.5%, while for subsequent recurrences the rate decreased to 50%. The median time of remission in both groups of patients was 7 months. There was complete loco-regional tumour control in 51% of all patients (with and without distant metastases). A significant prolongation of the survival time was not accomplished by this treatment.

[Radiosensitization with metronidazole. Pharmacocinetical comparison by means of blood level curves after administration of "metronidazol" in the form of tablets, suppositories, and enemas (author's transl)].

Severe acute gastrointestinal side-effects following high oral doses of metronidazole (6 g/m2) could be avoided by a combined oral and rectal application. With tablets and an enema (1 g metronidazole/2 ml) in a ratio 1:4 and an increased dosage of 10 g/m2, maximal serum concentrations of about 200 micrograms/ml are obtained like after an oral dosage of 6 g/m2. The maximal radiosensitizing effect on hypoxic tumor cells is seen five hours after oral application and seven hours after the combined oro-rectal application. No longterm toxicity was found following six high doses of metronidazole.

[Clinical evaluation of the antioestrogenic agent tamoxifen in the treatment of metastatic breast cancer (author's transl)].

The endocrine management of advanced breast cancer is of great clinical importance. Apart from the known therapeutic approach with oestrogens, androgens, gestagens and steroids, the oestrogen antagonist, tamoxifen appears to be a promising therapeutic agent. In this study from October 1st, 1975, to October 1st, 1976, the value of tamoxifen was examined in a controlled clinical trial. An objective remission after 3 months' treatment was achieved in 41% of the cases. The criteria of Karnofsky were used to evaluate clinical success. The median time of remission was 5,5 months. The determination of oestrogen receptors could provide a better selection of patients and, hence, help to achieve an increase in the remission rate. An interesting observation was a probably immunostimulative effect, but further investigations are necessary. The side-effects of treatment are detailed and the indications for the use of tamoxifen are discussed. More astonishing than the objective remission was the subjective improvement with relief of pain due to generalized bone metastases. On the basis of our observations it is concluded, that the use of the oestrogen antagonist, tamoxifen presents a potent alternative in the treatment of advanced and incurable breast cancer, especially in postmenopausal patients.

Further clinical experiences of a phase I study with the hypoxic cell radiosensitizer misonidazole.

Since April 1976 we have performed clinical investigations with multiple doses of the hypoxic cell radiosensitizer misonidazole in 21 patients. A significant side effect of the drug was the development of peripheral sensory neuropathies in 13 patients (8 mild, 5 severe) and of a transient acute organic psychosyndrome in 2 of the 5 patients with a severe polyneuropathy. The severity of the polyneuropathies is related to the total dose of misonidazole and the overall time of drug administration. Treatment schedules designed to obtain the desired sensitizing effect without neurological side effects are under investigation.

[Effect of the estrogen antagonist tamoxifen in the treatment of advanced mastocarcinoma (author's transl)].

Today the endocrin therapy of the advanced mastocarcinoma is in common use. Besides the already known therapy by estrogens, androgens, gestagens, and steroids, Tamoxifen, and estrogen antagonist, is a very promising therapeutic drug. In the presented study, Tamoxifen was submitted to a critical clinical control during a period of one year from 1st October 1975 until 1st October 1976. After a three months' treatment, a rate of 41% of objective remissions could be obtained. The criteria of success were estimated according to the scheme of Karnofsky. The average remission time is 5,5 months. By a determination of the estrogen receptors it would be possible to realize a therapeutic selection and to achieve a higher remission rate. The authors made an interesting observation, i.e. a probably immuno-stimulating effect which, however, still has to be submitted to further examinations. The side effects are described in detail and the indications are established. Its is astonishing that the subjective ameliorations, i.e. cessation of pains in case of generalized formation of metastases in the bones are much more frequent than the objective remissions. We came to the conclusion that the treatment by Tamoxifen is a valuable alternative in the therapy of the mastocarcinoma, above all in the postmenopausal period if the disease is advanced and incurable.

Egocentric mental rotation in Hungarian dyslexic children.

A mental rotation task was given to 27 dyslexic children (mean age 9 years, 2 months) and to 28 non-dyslexic children (mean age 8 years, 8 months). Pictures of right and left hands were shown at angles of 0, 50, 90 and 180 degrees, and the subjects were required to indicate whether what was shown was a right hand or a left hand. It was found that, in this task, the dyslexics did not show the normal pattern of response times at different angles, and also, that they made more errors than the controls. It is argued that this result is compatible with hypothesis that, in typical cases of dyslexia, there is a malfunctioning in the posterior parietal area.
