Cloning and sequencing of trehalose biosynthesis genes from Rhizobium sp. M-11.

The two genes encoding maltooligosyl trehalose synthase and maltooligosyl trehalose trehalohydrolase, which are related to biosynthesis of alpha, alpha-trehalose, were cloned from Rhizobium sp. M-11. Sequence analysis showed that the synthase gene composed of 2316 bp was connected with the hydrolase gene of 1788 bp by an overlap of one nucleotide. The deduced amino acid sequences of both enzymes have several regions common to amylolytic enzymes belonging to an "alpha-amylase family".

[Characteristics of atrial activation sequence of atrial fibrillation and flutter in a canine model].

In order to induce either atrial fibrillation (af) or atrial flutter (AF) in 22 dogs under pentobarbital anesthesia, a right atriotomy parallel to the sulcus terminalis was performed, but closed with running sutures. Effective refractory periods (ERP) were measured in the sites of the appendages and the bodies in both atria. Further, we developed a computerised mapping system which enabled us to reveal sequential activation of various parts of the heart. As a result, we could assess the circuit of excitation which gave rise to AF. AF occurred in 7 of the 22 dogs, and af in the remaining 15 dogs. It was revealed that during AF there was a counterclockwise (5 dogs) or clockwise (2 dogs) reentrant pathway involving the right atrial free wall and superior vena caval orifice with consequent left atrial activation. Although standard limb lead electrocardiogram showed af, all dogs with sustained af had a regular activation sequence in the left atrium [at a cycle length (period) of 140.1 +/- 16.9 msec, arising either in the left atrial appendage (8 dogs) or adjacent to the left pulmonary vein (7 dogs)]. However, 12 of the 15 dogs had a chaotic activation sequence whereas the other 3 dogs showed a regular activation sequence in the right atrium. Statistically, during AF there was no significant difference between RA ERP (153.2 +/- 12.6 msec), LA ERP (146.4 +/- 14.4 msec) and LA cycle length (period) (140.1 +/- 16.9 msec). During af, there was no significant difference between LA ERP (131.7 +/- 15.2 msec) and LA cycle length (period) (125.7 +/- 9.1 msec), but RA ERP (165.4 +/- 15.5 msec) was significantly longer than the LA cycle length (period) (p < 0.0001). Although in the present experiment regular left atrial activation and complex right atrial activation were observed predominantly, the activation sequence of af was different in each dog. These results clearly indicate that to employ a mapping system like ours is necessary to determine relevant surgical procedures to stop af and AF.

Growth and metastasis of two canine mast cell tumors in SCID mice.

Intermediately differentiated mast cell tumors in two dogs were subcutaneously xenotransplanted into severe combined immunodeficiency (SCID) mice. Both tumors primarily grew and were serially transplantable in SCID mice. The histological features of the xenografts were similar to those of original tumors in dogs. Both of these subcutaneous tumors were judged as connective tissue mast cells by toluidine blue stain. One of the two xenografts metastasized to the tracheobronchial lymph nodes, omentum, mesentery, subpleural region and retroperitoneum of the SCID mouse. These canine mast cell tumor xenografts in SCID mice may be valuable tools for investigating the growth and metastatic behaviors of the tumor.

[Immunohistological study on proliferative activities and expression of c-erbB-2 oncoprotein in salivary gland tumors].

The proliferative activity of tumors and the expression of c-erbB-2 oncoprotein were investigated in 86 salivary neoplasms (54 pleomorphic adenomas and 32 malignant tumors), and in 17 normal salivary glands. Numbers of cells that stained positive to anti-proliferating cell nuclear antigen (PCNA) antibody and the nucleolar organizer region associated protein (AgNOR) were used as an index of the proliferative activity. The results obtained in this study were as follows: 1) PCNA-positive and AgNOR-positive cells were elevated in malignant tumors relative to pleomorphic adenomas and normal salivary glands. 2) Salivary gland tissue around the pleomorphic adenoma with high proliferative activity demonstrated higher activity than normal gland tissue. 3) The proliferation of pleomorphic adenoma was not affected by the age or size of the tumors. 4) Expression of c-erbB-2 oncoprotein was not observed in pleomorphic adenoma and normal gland but was observed in malignant tumors. 5) In one case of a carcinoma in pleomorphic adenoma, the pleomorphic adenoma region also revealed c-erbB-2 positive cells.

[Squamous cell carcinoma of the parotid gland in children].

A five-year-old boy with squamous cell carcinoma of the parotid gland is presented. The patient had a painless mass in his left parotid region with left facial palsy. Squamous cell carcinoma of the parotid gland shows high grade malignancy and is extremely rare in children, especially under five years old. After neo-adjuvant chemotherapy, he was given a total parotidectomy and radical neck dissection. The left facial nerve could not be preserved. Radiotherapy was also administered postoperatively. There was no evidence of local recurrence or distant metastasis 18 months after the surgery. Careful long-term follow-up is mandatory for this patient.

[Autoradiographic studies of oxalate distribution in rat kidney].

BACKGROUND: We performed macro and micro-autoradiographic studies using 14C-oxalate in normal and hyperoxaluric rats in order to elucidate intrarenal distribution of oxalate and to determine where calcium oxalate crystal can be adhere in the kidney. METHODS: Macro and micro-autoradiographic studies of the kidney in normal rats were carried out at intervals of 15, 30 and 90 min after intravenous administration of 14C-oxalate (37 MBq/kg BW). Hyperoxaluria was induced by vitamin B6 deficient diet. The procedure of macro and micro autoradiography is the same as that in normal rat. RESULTS: In normal rats, macroautoradiogram showed that the radioactivity of 14C-oxalate exists in the whole kidney, and microautoradiogram identified this radioactivity mainly in the extraluminal space of renal tubules at 15 min. Macro and micro-autoradiograms taken 90 min after the injection showed practically no radioactivity of 14C-oxalate in the cortex or the medulla, but it was located in the extraluminal space of the papilla. In hyperoxaluric rats, macroautoradiogram showed some spotty accumulations of 14C-oxalate in the inner medulla and papilla. Microautoradiogram revealed that these accumulations are mainly seen in the extraluminal space. CONCLUSION: These results indicate that in normal rats the injected 14C-oxalate remains in the renal papilla, especially in the extraluminal space, when nearly all oxalate was excreted by urine, and in hyperoxaluric rats 14C-oxalate exists there as calcium oxalate crystal or microlith.

Transient suppression of pancreatic endocrine function in patients following brain death.

To investigate pancreatic endocrine function in brain-dead patients, an intravenous glucose tolerance test (i.v. GTT) was performed in 8 brain-dead subjects maintained using the combined administration of vasopressin (ADH) and catecholamines during the first 3 days following the onset of brain death. Ten healthy adults served as control subjects. Although plasma glucose concentrations markedly increased and exceeded 300 mg/dl in most subjects just after the onset of brain death, they decreased to less than 200 mg/dl in most subjects after 24 h. The early insulin release also was significantly lower in brain dead subjects compared to controls (p < 0.01). The late insulin release was not decreased compared to controls but rather increased, which was accompanied by decreased glucose disappearance rate. The early insulin release recovered rapidly during the first 3 days following brain death without significant changes in the plasma hormone concentrations such as epinephrine, human growth hormone, thyroid-stimulating hormone, triiodothyronine, thyroxine, cortisol, and glucagon. The early insulin release and the plasma glucose concentration just before i.v. GTT was negatively correlated (R = -0.55, p < 0.05). We suggest awaiting recovery from hyperglycemia and early insulin release provides a reasonable approach to transplantation of the pancreas.

[Low-dose combination cytarabine and etoposide for myeloid crisis transformed from unclassified chronic myeloproliferative disorder].

A 65-year-old man visited our department for further leukocytosis examination. Hematological examinations disclosed elevation of the leukocyte count with left deviation. The neutrophilic alkaline phosphatase score was reduced. Bone marrow was hypercellular and consisted almost entirely of granulocytic cells in all stages of maturation. However, cytogenetic analysis revealed no Philadelphia chromosome, and genotypic analysis disclosed no bcr rearrangement. He was ultimately diagnosed as having unclassified chronic myeloproliferative disorder. He had been followed without chemotherapy, and he developed blastic crises (CD10+, CD13+, CD24+). Chemotherapy was effective, and then he was followed with carboquone. However, myeloid crisis (CD13+, CD33+) developed again. Standard chemotherapies had no effect, he developed pneumonia, and he was in poor general condition. Therefore, low-dose combination with cytarabine and etoposide was performed. The result was that the blasts disappeared, his general condition improved and infection was reduced. Major side effects were absent; however, the blasts proliferated again after the treatment was discontinued. In conclusion, this combination may be useful treatment for myeloid blastic crises even if the patient is in poor condition. But the modification of the administration schedule requires some consideration.

Pharmacokinetics of the new thyrotropin releasing hormone analogue montirelin hydrate. 1st communication: plasma concentrations, metabolism and excretion after a single intravenous administration to rats, dogs and monkeys.

The plasma concentrations, metabolism and excretion were studied in rats, dogs and monkeys after a single intravenous administration of 14C-montirelin hydrate (CAS 90243-66-6, 14C-NS-3). 1. The plasma concentration of radioactivity and the area under curve (AUC0-infinity) increased with the dose after intravenous administration of 0.25, 1 and 4 mg/kg to male rats. 2. Until 4 h after intravenous administration of 0.25 mg/kg, the plasma concentration of radioactivity decreased with half-lives of 0.512, 0.771 and 0.786 h in male rats, dogs and monkeys, respectively. The plasma concentration of radioactivity in rats increased 6-10 h after administration, suggesting enterohepatic circulation. 3. The plasma concentration of NS-3 in various animal species decreased biphasically after intravenous administration. The elimination half-lives, t1/2 beta, in rats, dogs and monkeys were 0.324, 0.679 and 0.682 h, respectively, and the steady state volumes of distribution, Vdss, were 0.248, 0.319 and 0.306 l/kg, respectively. 4. The binding of NS-3 to serum protein was less than 3% in rats, dogs, monkeys and humans. 5. Within 48 h after intravenous administration of 1 mg/kg to male rats, the excretion of radioactivity in urine, feces and expired air was 67.3, 15.0 and 14.8% of the dose, respectively (total 97.1%). The biliary excretion of radioactivity was 26.6% within 24 h after dosing. 6. No sex-related difference was found in plasma concentration of radioactivity or the excretion of radioactivity in urine, feces and expired air after intravenous administration of 1 mg/kg to male and female rats. 7. In male dogs, the excretion of radioactivity in urine and feces was 92.3 and 5.6% of the dose, respectively, during 72 h after intravenous administration of 0.25 mg/kg (total 97.9%). 8. In male monkeys, the excretion of radioactivity in urine and feces was 91.7 and 2.9% of the dose, respectively, during 72 h after intravenous administration of 0.25 mg/kg (total 94.6%). 9. The urinary excretion of NS-3 and its main metabolite CNK-6004 (deamidation product) during 24 h after intravenous administration was 31.4 and 25.3% of the dose in male rats, 45.4 and 30.1% in male dogs, and 20.2 and 48.0% in male monkeys, respectively. The biliary excretion of NS-3 and CNK-6004 in male rats was 1.1 and 17.4%, respectively, during 24 h after administration. 10. The radioactivity in the bile collected from donor rats that had received 14C-NS-3 was easily absorbed from the intestinal tract of recipient rats after intraduodenal administration of the donor's bile.

Pharmacokinetics of the new thyrotropin releasing hormone analogue montirelin hydrate. 2nd communication: distribution and transfer into the fetus and milk after a single intravenous administration and pharmacokinetics and enzyme induction after repeated intravenous administration to rats.

Pharmacokinetics of 14C-labeled montirelin hydrate (CAS 90243-66-6, NS-3) in rats was studied after single or repeated intravenous administration. 1. Radioactivity concentrations in tissues after single administration to male and female rats were highest in the kidney followed (in this order) by plasma, liver, blood, pancreas, uterus (female rats), lung and skin and low in various brain sites 5 min after administration. The concentrations in most tissues were practically parallel to those in plasma over the 24-h period after administration. After decreasing rapidly the concentrations rose slightly for 10 h and then decreased gradually. 2. Five min after single administration to male rats, the concentration of the main metabolite CNK-6004 (deamidated product) was lower in the plasma, but higher in the liver and kidney than the NS-3 concentration. From 0.5 to 2 h after administration, the concentration of CNK-6004 was higher than that of NS-3 in the plasma, liver and kidney, accounting for 33-64% of the radioactivity concentration. 3. After administration to rats on the 18th day of pregnancy, the radioactivity concentrations in the fetal whole body and fetal tissues peaked later than those in the maternal plasma, tissues and placenta. The maximum concentration in the fetal tissues was 2% or less of that in the maternal plasma. 4. After administration to lactating rats, the radioactivity concentration in milk reached the maximum 10 h after administration, and decreased gradually in parallel with the concentration in the plasma 24 to 168 h after administration. 5. During repeated once daily administration for 10 days, the radioactivity concentration in the plasma 24 h after each administration reached practically steady state level after the 7th administration and decreased with a half-life of 38.1 h after the last administration. 6. The radioactivity concentrations in most tissues after the last administration were not significantly different from those after a single administration. Decreased elimination of the radioactivity was observed in the white fat and skin, in which radioactivity levels were higher than those in the other tissues a long time after the last dose. 7. Excretion of the radioactivity in the urine and feces during repeated administration was constant after the 2nd administration. The excretion by 168 h after the last administration was 66.9 and 14.3% of the cumulative dose in the urine and feces, respectively (total: 81.2%). 8. The composition of metabolites in the plasma, liver, kidney and urine after the last administration did not differ markedly from that after a single administration. Once daily repeated administration for 7 days had no effect on the liver drug metabolizing enzyme activities.

Pharmacokinetics of the new thyrotropin releasing hormone analogue montirelin hydrate. 3rd communication: identification of metabolites in rat urine.

The metabolism of montirelin hydrate (CAS 90243-66-6, NS-3) was studied in rats after intravenous administration of 14C-labeled or unlabeled NS-3. 1. Four radioactive metabolites (M-1 to M-4) were found in the urine after administration of 14C-NS-3. M-3 (major metabolite) and M-2 showed the same Rf values as (-)-N-[[(3R,6R)-6-methyl-5-oxo-3-thiomorpholinyl]carbonyl]-L-histi dyl-L- proline (CNK-6004) and (+)-N-[[(3R,6R)-6-methyl-5-oxo-3- thiomorpholinyl]carbonyl]-L-histidine (CNK-6001), respectively. 2. M-3 and M-2 were purified from the urine after administration of unlabeled NS-3, and their chemical structures were identified by mass spectrometry, 1H-nuclear magnetic resonance spectroscopy, thin-layer chromatography and high performance liquid chromatography. Consequently, M-3 was identified as CNK-6004 formed by deamidation at a prolinamide moiety of NS-3, and M-2 as CNK-6001 formed by deprolination of CNK-6004.

[Evaluation of concomitant use of cyclosporin and percutaneous isolated liver perfusion under complete venous isolation and charcoal hemoperfusion].

It is difficult to administer an effective dose of cyclosporin A (CsA), a potent inhibitor of P-glycoprotein, to prevent multiple drug resistance due to its side effect. We herein evaluated the efficacy of concomitant use of this agent and complete hepatic venous isolation and charcoal hemoperfusion (HVI.CHP). Dogs were divided into two groups; group I (n = 4), intraarterial infusion of only adriamycin (ADM) and group II (n = 4), intraarterial infusion of CsA and ADM. In both groups, ADM was intraarterially administered for 10 minutes under HVI.CHP. In addition, in group II, CsA infusion (0.3 mg/min.kg) was initiated 20 min prior to the start of ADM infusion and maintained for 30 min. The AUC (micrograms.min/ml) of ADM were 21.2 +/- 8.6 (mean +/- SD) and 28.4 +/- 10.3 in groups I and II, respectively, at prefilter (hepatic venous level) and 8.1 +/- 4.6 and 4.8 +/- 3.8, respectively, at postfilter, showing an effective drug elimination in both groups. The Cmax (micrograms/ml) were 14.1 +/- 2.2 at prefilter, 2.4 +/- 0.5 at postfilter, and 3.4 +/- 1.2 in systemic level. These results indicated that HVI.CHP allowed the high-dose CsA infusion required for P-gp inhibition in the liver and could reduce extraregional CsA leakage.

[The effect of high-dose intraarterial chemotherapy with percutaneous hepatic venous isolation and charcoal hemoperfusion (HVI.CHP) for unresectable multiple hepatocellular carcinoma--comparison with other therapeutic modalities].

We evaluated the therapeutic effect of HVI.CHP in 11 patients with Stage III and IVA multiple hepatocellular carcinoma (group H) in comparison with conventional intraarterial chemotherapy (group S, 12 patients) and transarterial embolization (TAE; group T, 15 patients). The patients in each group had the same background as group H in terms of liver function tests. The Vp3 positive rates in group H (37%) and group S (47%) were significantly higher than group T (0%). One-year survival rates of groups H, S and T were 63. 6%, 8.3% and 46.7%, respectively, and 2-year survival rates were 63.6%, 0% and 20%, respectively. A significant difference was seen between the survival curves of groups H and S (p < 0.01), whereas no significant differences were detected between those of groups H and T. The survival rates for stage IVA patients in groups H, S and T were 55.6%, 10% and 40%, respectively, for 1 year and 55.6%, 0% and 10%, respectively, for 2 years. The difference of survival curves between Stage IVA patients of each group was analogous to those between overall patients of each group. Although 4 patients with Vp3 were included in group H but none in group T, the survival rates of group H were rather higher than in group T at 2 years. These data suggest that HVI.CHP could be the treatment of choice for unresectable multiple hepatocellular carcinoma.

[Evaluation of a single catheter technique for percutaneous isolated liver perfusion].

We have established a single catheter technique of percutaneous isolated liver perfusion using a 4-lumen-2-balloon (4L - 2B) catheter for treatment of unresectable malignant liver tumors. Herein reported are the technique, safety and pharmacokinetics of the system in comparison with the original double-balloon technique. This study included 19 patients with malignant liver tumors treated by adriamycin at a dose of 100 mg/m2. Seven patients had the double-balloon technique (group D), in which filtered hepatic effluent and the rest of the inferior vena caval blood were separately drawn and returned to the left axillary vein. The other 12 patients had single catheter technique (group S). In group S, hepatic effluent was solely isolated and directed to CHP filters. All patients except for one in group S showed good hemodynamic stability. The hepatic venous flow rate of group S was significantly higher than in group D (p < 0.05). Although the mean area under the time concentration curve at systemic serum was significantly lower in group S compared to group D, the rate of side effects was similar in both groups. A 4L. 2B single catheter allowed safe and repeated percutaneous isolated liver perfusion for technical simplification of the treatment.

[A case of advanced hepatoma cured by repeated percutaneous isolated liver perfusion using hepatic venous isolation and charcoal hemoperfusion].

We have reported the treatment results of percutaneous isolated liver perfusion using hepatic venous isolation and charcoal hemoperfusion (HVI.CHP) for unresectable liver cancers. This is a case of multiple advanced hepatoma cured completely by repeated per cutaneous isolated liver perfusion. The patient was a 58-year-old woman who was referred to our hospital for a hepatic tumor detected by abdominal computed tomograpy (CT). On admission, she showed HBs antigen positive, mild anemia and liver dysfunction, and elevation of tumor markers. Abdominal CT demonstrated nodular tumors in segment 4. In addition, hepatic angiography additionally revealed multiple bilobar metastases. We treated this case with high-dose intraarterial adriamycin (150 mg/body) using HVI.CHP. There after, the patient received intermittent intraarterial low-dose epirubicin infusions (30 mg/body, 5 times) via an implantable catheter system. Furthermore, she was given a second high-dose of adriamycin (130 mg/body) under HVI.CHP 7 months after the first treatment. Despite repeated high-dose treatments, she had no severe side effects. The levels of tumor markers, including AFP and PIVKA-II, decreased to normal range, and all tumor nodules have disappeared in abdominal CT studies at present, 20 months after the initial treatment. In conclusion, our experience suggests that advanced hepatoma with multiple bilobar lesions, as in this case, would be cured by repeated percutaneous isolated liver perfusion using HVI.CHP.

[A case of advanced cholangiocellular carcinoma treated successfully by percutaneous isolated liver perfusion with cisplatin].

Herein reported is a case with unresectable intrahepatic cholangiocellular carcinoma (CCC) treated successfully with high-dose cisplatin infusion under hepatic venous isolation and charcoal hemoperfusion (HVI.CHP). The patient was a 43-year-old woman, who was investigated for liver dysfunction and subsequently referred to our institution with a diagnosis of unresectable intrahepatic CCC. We gave this patient a total of 2 treatments of percutaneous isolated liver perfusion with HVI.CHP (a total dose of cisplatin, 400 mg and adriamycin 30 mg). Although she had a slight elevation of serum GOT levels, leukopenia and renal dysfunction did not occur throughout the posttreatment course. The level of DUPAN-2 after the first treatment showed a marked reduction to 910 U/ml, from a pretreatment level of 2,700 U/ml. Abdominal CT scan also demonstrated a remarkable regression of liver tumors (PR: volume reduction, 76%). In conclusion, in percutaneous isolated liver perfusion using HVI.CHP, we have safely accomplished a dose intensification of intraarterial cisplatin. The treatment resulted in a marked tumor regression. Therefore, we consider that this method offers an effective therapeutic option for unresectable CCC.
