[Telemedical support in emergency medicine].

In July 1997 we began support of paramedics who would respond to emergency scenes using a telecommunications system based in the hospital emergency unit. However, since no telecommunications system that could be employed on the actual clinical site was available, we had to develop a new system. Our telecommunications system (TMS-6101, NIHON KOHDEN WELLNES Co., Tokyo, Japan) can transmit vital signs in real time and interactively transmit several types of moving images. Because the current infrastructure for radio communications cannot provide adequate telemedical support for paramedics, we are using the new system for clinical support between our unit and associated hospitals. Based on 57 cases of clinical telemedical intervention, the system has enabled high-quality decision making by specialists without the need for them to travel to the scene or transport X-rays films, ultrasonographic reports, or endoscopic results. If this newly developed telecommunications system is employed for telemedical interventions in medical facilities in remote areas or on remote islands, medical consultations for Japanese individuals overseas, night-time first aid in urban areas, and in disaster situations, the physicians on both side of the line will be able to obtain a wealth of timely information, greatly influencing outcome in both emergency and nonemergency cases.

Evaluation of acute aortic dissection by cine-MRI.

UNLABELLED: We investigated operative indication of acute aortic dissection by analyzing the movement of the intimal flap using cine-MRI. METHODS: Ten consecutive cases of acute aortic dissection diagnosed between December 1997 and September 1998 participated in this study. Six of these patients had in Stanford type A and four Stanford type B aortic dissection. The diagnosis of aortic dissection was based on demonstration of two aortic channels with each cardiac cycle. After obtaining SE(spin-echo) images, gradient echo sequences (FFE) were obtained such that the false lumen was maximally imaged and the intimal flap was depicted perpendicularly. During one RR-interval of ECG, 12 to 16 measurements were recorded and combined in a cineloop mode. We defined the intimal flap, the site of the entry and thrombosis in the false lumen, and evaluated the movement of the intimal flap as the flap movement index (FMI). Maximum and minimum aortic diameters (ADmax, ADmin), as well as maximum and minimum false lumen diameters (FDmax, FDmin) were measured. FMI was defined as[(FDmax/ADmax) - (FDmin/ADmin)]/(FDmin/ADmin) x 100(%). RESULTS: Total thrombosis was noted in four cases, partial thrombosis in 3, and in the remaining 3 no thrombosis of the false lumen was noted. The FMI ranged between 4% and 87% (mean 26.3 +/- 8.6%). Among the 4 cases with the large FMI of greater than 30%, 3 underwent emergency surgery and were confirmed to have no thrombosis in the false lumen. The fourth case, who had partial thrombosis, underwent medical therapy, but since complete thrombosis in the false lumen never did occur in this case, surgery became absolutely necessary. The other 6 cases with a FMI of less than 30% showed a tendency toward gradual reduction of the false lumen. CONCLUSION: FMI is a potentially useful parameter in cases with acute aortic dissection to determine whether or not emergency surgery is indicated.

Strategy for prevention of myonephropathic metabolic syndrome (MNMS): comparison of cooling and perfusion.

UNLABELLED: To evaluate the usefulness of cooling and perfusion methods for acute ischemic legs, we performed the experimental study using adult mongrel dogs. As creation of acute ischemic legs, the abdominal aorta was cross-clamped below the renal arteries, where all branches were ligated, for 6 hours. After release of clamp, the various parameters were examined for 18 hours. All dogs were divided into four groups according to the suppressive methods. CONTROL GROUP: cross-clamping for 6 hours with no suppressive method, Group 1: cooling legs using crushed ice during 6 hour-clamping, Group 2: perfusion by heparinized saline of 2.5-10.0 ml/kg before declamping, Group 3: cooling during latter half of 6-hour clamping and perfusion same as Group 2. The serum levels of metabolites and the tissue pressure of the thigh showed significantly lower in the suppression groups than CONTROL GROUP after declamping. In addition, GOT, CPK and aldolase revealed significantly lower values in Group 3 than Group 2. The tissue blood flow of the thigh recovered to the same extent as before ischemia in Groups 2 and 3, while it did not so in CONTROL GROUP and Group 1. Microscopic findings in hematoxylin and eosin (H-E) staining indicated a marked destruction of muscle fiber and interstitial edema in the striated muscle in CONTROL GROUP. These changes were not found so much in Group 2, and almost no changes in Group 1 and 3. The immunostaining for Mb in the striated muscle showed negative in CONTROL GROUP and Group 2, while it showed positive staining in Groups 1 and 3. The immunostaining for Mb in the kidneys showed the most dense deposits of Mb in the renal tubules in CONTROL GROUP. These changes were less in Group 2, little in Group 3, and no change in Group 1. In conclusions, cooling method revealed the minimal metabolic change, and combined method with perfusion was still more effective for prevention of MNMS.

Assessment of left ventricular function using dobutamine stress echocardiography and myocardial scintigraphy in valvular heart disease.

To assess the left ventricular (LV) function in valvular heart disease, we employed the preoperative dobutamine stress echocardiography and the myocardial scintigraphy. During the past 13 years, 37 of 324 the patients showed LV dysfunction with the % fractional shortening (%FS) of 25% or less in the preoperative echocardiogram. These patients were retrospectively divided into two groups; Group A (n = 21): %FS has improved late after operation; Group B (n = 16): %FS has deteriorated or LV failure occurred. The mean follow-up period was 84 +/- 54 months after valve surgery. No significant differences were observed in the preoperative characteristics and operative variables between these two groups. The dobutamine stress test had been performed in 8 patients in Group A and 9 patients in Group B preoperatively, and the maximum increase ratio of %FS (delta %FS) was used for assessment. Seven patients in Group A had showed delta %FS of more than 9%, while all patients in Group B had showed delta %FS of less than 9%. Myocardial scintigraphy was performed in 11 patients of them, and another 22 patients with %FS of above 25% acted as the control group. The Defect Score, which was defined as the sum of defect scales in 25 LV segments, showed a significant difference between 11 patients with LV dysfunction and control group. The distribution of the Defect Score in each myocardial segment, showed significantly higher in the posterior and inferior LV segments. In addition, the perfusion defect on myocardial imaging was initiated in the junction between the septal and LV free wall, and extended from the posterior to the lateral wall along with deterioration of LV function. In conclusion, preoperative dobutamine stress echocardiography proved to be very useful for prediction of the postoperative LV function, and myocardial scintigraphy might be indicative of LV function even in valvular heart disease.

Paramalleolar bypass concomitantly with extended endarterectomy for limb-threatening ischemia: A case report.

A 74-year-old male was admitted to our university hospital with a refractory ulcer of the left third toe. The ankle pressure index was 0.43. On his angiogram, the popliteal artery was totally occluded in the distal site, while the peroneal artery was solely patent and inflowed into the distal posterior tibial artery. At surgery, endarterectomy of 7 cm in length was performed on the tibioperoneal trunk of the occluded popliteal artery following patch repair using a saphenous vein to restore the genicular arterial network and infrapopliteal arteries. Thereafter, the bypass surgery was performed using the in situ saphenous vein from the patent proximal popliteal artery to the distal posterior tibial artery. The postoperative angiogram showed patency of the graft as well as restoration of the genicular arterial network and infrapopliteal arteries. The ankle pressure index improved to 1.04, and the refractory ulcer was completely cured one month after revascularization.

Surgical assessment of tricuspid valve replacement for severe tricuspid regurgitation without stenosis.

Eight patients undergoing tricuspid valve replacement (TVR) for severe tricuspid regurgitation (TR) without stenosis were subjected to the study. Four patients had primary TR due to trauma, infective endocarditis, or Ebstein's anomaly, and the other 4 had secondary TR associated with mitral valve lesions. The right heart pressure as well as clinical manifestations due to right heart failure showed deteriorated values in both the primary and secondary TR groups, though pulmonary arterial pressure was higher in the latter group. In 2 patients, initial annuloplasty procedures could not reduce TR sufficiently and were instantly abandoned for TVR. These 2 patients, one with congenital and one with secondary TR, died of intractable right heart failure early postoperatively, while the remaining 6 patients are alive to date, in the New York Heart Association (NYHA) functional class I, during the follow-up period of 6-192 (mean, 104) months. The 2 patients who died had shown a longer morbid duration and a lower preoperative right ventricular systolic function indicated by stroke work to pulmonary artery resistance. This paper might suggest that an earlier surgical intervention in severe TR is recommended before the right ventricular function deteriorates.

Quantitative assessment of peripheral occlusive arterial diseases by color duplex sonography and blood endothelin level.

UNLABELLED: To evaluate peripheral occlusive diseases quantitatively, we performed color duplex sonography and measured the blood endothelin (ET-1) level. MATERIALS AND METHOD: We measured the systolic velocities of the dorsal pedial and the posterior tibial arteries as well as the brachial artery. We also calculated the flow volume, and the ratio of systolic velocities and flow volume of the lower to upper extremity (AVI, AFI). Furthermore we measured the blood ET-1 level and investigated the relationship between this value and clinical symptoms. RESULTS: The value of AVI as well as AFI decreased in the order of Fontaine class I, II, III and IV. In four limbs with a Fontaine class greater than II with a normal ankle pressure index, the values of AVI were low. On the other hand, three limbs with normal values of peak-AVI (> 0.9) and lower API (< 0.75) were in Fontaine class I. The value of the ET-1 level was higher in Fontaine class III and IV than in class II, and decreased after revascularization along with improvement of clinical symptoms. CONCLUSIONS: The new AVI and AFI values showed a better correlation with clinical symptoms than API. The ET-1 level was significantly higher in Fontaine class III and IV, and showed marked regression after arterial reconstruction. Thus, the new AVI and AFI values may provide a novel means of identifying patients with the peripheral occlusive diseases, and the measurement of ET-1 level may be potentially useful in identifying the severity of arteriosclerosis.

Pharmacokinetics of NS-105, a novel cognition enhancer. 2nd communication: distribution and transfer into fetus and milk after single administration, and effects of repeated administration on pharmacokinetics and hepatic drug-metabolizing enzyme activities in rats.

The tissue distribution and transfer into the fetus and milk of NS-105 ((+)-5-oxo-D-prolinepiperidinamide monohydrate, CAS 110958-19-5), a novel cognition enhancer, were investigated in rats after single oral administration of 14C-NS-105. The effects of repeated oral administration on the pharmacokinetics of NS-105 and hepatic drug-metabolizing enzyme activities also were investigated in rats. The radioactivity concentration in most tissues of male rats reached a maximum of 0.5 h after the single oral administration of 14C-NS-105, indicating rapid absorption and distribution, 0.5 h after the administration, the highest concentrations were present in the kidney and stomach, and the lowest in the white fat. The concentrations in the remaining tissues were moderately lower than the plasma value. The radioactivity concentrations in all the tissues tested decreased along with the plasma concentration, and were below or near the detection limit 24 h after the administration. Most of the radioactivity in the plasma, liver, kidney and cerebrum was due to unchanged NS-105. The tissue distribution patterns of radioactivity in female (non-pregnant) and pregnant rats after the oral administration of 14C-NS-105 did not differ from the pattern in male rats, revealing neither sex- nor pregnancy-related differences in NS-105 distribution. In pregnant rats, the maximum concentration in the fetus was 66% of that in the maternal plasma. In lactating rats, the radioactivity concentration in the milk was similar to that in the plasma. During and after the repeated oral administration of 14C-NS-105, the plasma concentrations and cumulative urinary and fecal excretions of radioactivity did not change with the number of administrations and were similar to the corresponding values after the single administration. The radioactivity concentrations in most tissues 8 h after the 7th, 14th and 21st administrations were about twice the corresponding values after the single administration, indicating that there is no marked accumulation of radioactivity in the tissues and that a steady state level was reached within 1 week. Repeated oral administration of NS-105 (10 mg/kg) to male rats did not affect hepatic drug-metabolizing enzyme activities.

[The role of microsomal triglyceride transfer protein in metabolism of apo B-containing lipoprotein].

Microsomal triglyceride transfer protein (MTP) plays a central role on secretion of lipoprotein from the liver and the intestine. MTP catalyzes the transfer of triglyceride, cholesteryl ester and phosphatidylcholine between membranes and lipoproteins. In human, defect of MTP activity, result from mutations encoding the MTP large subunit, is the primary cause of abetalipoproteinemia. To investigate the association between hyperlipidemia with obese and MTP, We used Otsuka Long-Evans Tokushima Fatty rat, an animal model of obesity with visceral fat accumulation, hyperlipidemia. In animals, very-low density lipoprotein-triglyceride levels were elevated compared with the control rats. Hepatic mRNA levels of acyl-coenzyme A synthetase, and MTP were also elevated. These results suggest that the enhanced expression of both ACS and MTP genes associated with visceral fat accumulation may be involved in the pathogenesis of hyperlipidemia in obese animal models.

[A case of neuroleptic malignant syndrome associated with reversible leukoencephalopathy].

We report a case of neuroleptic malignant syndrome associated with reversible leukoencephalopathy. The patient was a 60-year-old woman. Soon after ingesting an antipsychotic drug, the patient developed neuroleptic symptoms. After hydration and dantrolene sodium were administered, muscular rigidity gradually improved and serum levels of CK became normal. On the 7th hospital day, however, she fell into coma and showed tetraplegia. Although brain CT was normal on admission, diffuse low density areas were observed on the parieto-occipital cerebral white matter. The same lesions were observed on T2-weighted MRI. On the 20th hospital day, after giving her glycerol and adrenocorticosteroid, not only abnormal neurological findings but also abnormal CT and MRI findings disappeared. There were only two case reports of leukoencephalopathy with malignant syndrome in the literature. This is a rare case of reversible leukoencephalopathy with neuroleptic malignant syndrome due to the antipsychotic drug.

Clinical results of total cystectomy for 92 patients with bladder cancer.

The survival rate of 92 patients with primary bladder cancer who had undergone total cystectomy during a 13-year period from 1984 to 1996 was examined. The mean follow-up period was 1,886 days. The 5-year survival rate was 67.9% and the 10-year survival rate was 55.1%. When survival rates were compared pathohistologically, with 81 patients with transitional cell carcinoma divided into two groups, a high-stage group including T3 and T4 patients and a low-stage group with all other patients, the cancer-specific 5-year survival rate of the low-stage group was 88.9% while that of the high-stage group was 45.4%; this difference was significant (p = 0.0002). There were also significant differences in survival rate between those with and those without regional lymph node metastasis, those with and those without lymphatic infiltration, and those with and those without vascular infiltration. However, there was no significant difference in survival rate for the 34 patients with T3 or T4 disease when those with or without chemotherapy and/or radiation therapy were compared.

Increased excretion of nitric oxide in exhaled air of patients with chronic renal failure.

Nitric oxide exerts multiple effects on renal function. It remains unclear whether endogenous nitric oxide production is increased or decreased in patients with chronic renal failure. To evaluate endogenous nitric oxide production in these patients we studied exhaled nitric oxide output by an ozone chemiluminescence method and plasma NO2(-)/NO3(-) levels by the Griess method in 40 patients with end-stage chronic renal failure who underwent regular continuous ambulatory peritoneal dialysis (n=30) or haemodialysis (n=10), and in 28 healthy subjects. Patients with chronic renal failure had a higher exhaled nitric oxide concentration [39+/-3 versus 19+/-1 parts per billion, (mean+/-S.E.M.), P<0.0001], a greater nitric oxide output (177+/-11 versus 96+/-7 nl.min-1.m-2, P<0.001) and a higher plasma NO2(-)/NO3(-) concentration (96+/-14 versus 33+/-4 micromol, P<0.01) than controls. These values did not differ between patients on haemodialysis and those on continuous ambulatory peritoneal dialysis. Patients with chronic renal failure had significantly higher plasma concentrations of both interleukin-1beta and interferon-gamma than controls. The exhaled nitric oxide output did not correlate with plasma NO2(-)/NO3(-) or with peritoneal dialysate NO2(-)/NO3(-), but plasma NO2(-)/NO3(-) correlated with dialysate NO2(-)/NO3(-) in patients who underwent continuous ambulatory peritoneal dialysis (r=0.77, P<0.01). Haemodialysis for 4 h acutely decreased plasma NO2(-)/NO3(-) (92+/-17 versus 50+/-8 micromol, P<0.05) and cGMP concentration (16.5+/-4.3 versus 5.1+/-1. 7 pmol/ml, P<0.01), but did not decrease exhaled nitric oxide output. The increase in exhaled nitric oxide with the simultaneous increase in circulating cytokines suggests that nitric oxide synthase seems to be induced significantly in patients with chronic renal failure. Increased endogenous nitric oxide production may have a pathophysiological role in patients with uraemia.

Postnatal changes in Bax-immunoreactivity and apoptosis of the rat trigeminal primary neurons.

The developmental changes of Bax protein-like immunoreactivity (Bax-ir) and naturally occurring cell death were investigated in the trigeminal primary neurons of rats, in the first 2 postnatal weeks. At 1 day postpartum, the trigeminal primary neurons exhibited intense cytoplasmic Bax-ir. A densitometric analysis indicated >95% of cells exhibited an ir-density (the staining intensity as compared to the background level) higher than 1.5. By 2 weeks the ir-density significantly decreased with >95% lower than 1.5, that was similar to the adult level. A nick-end labeling method revealed DNA fragmentation in apoptotic trigeminal primary neurons in newborn rats. During the first 11 days the percentage of apoptotic cells was highly correlated to the postnatal days with a regression line y = -0.083x + 0.877 (r = 0.976). A possible role of Bax in neonatal rat primary neurons is discussed.

Cerebellar cell lines established from a p53-deficient adult mouse.

p53-Deficient mice are prone to develop spontaneous tumors [L.A. Donehower, M. Harvey, B.L. Slagle, M.J. McArthur, C.A. Montgomery Jr. , J.S. Butel, A. Bradley, Mice deficient for p53 are developmentally normal but susceptible to spontaneous tumors, Nature 356 (1992) 215-221], but brain neoplasms are rare and difficult to culture in vitro. Here we describe cloning and long-term culture of heterogeneous neural cell lines from a multifocal cerebellar neoplasm that arose in an adult p53-/- mutant mouse. These lines may be useful for studies of neoplastic transformation and cell lineage in cerebellar development.

Mechanism of the Shape and Structure Control of Monodispersed alpha-Fe2O3 Particles by Sulfate Ions.

The characteristics of the adsorption of sulfate ions to hematite (alpha-Fe2O3) particles have been studied to elucidate the anisotropic growth of hematite particles in the presence of sulfate ions. Sulfate ions were actually found to be most strongly adsorbed to crystal faces parallel to the c-axis of the hexagonal crystal system from the adsorption isotherms of sulfate to hematite particles of different crystal habits, in accord with the retardation of their growth in the direction normal to the c-axis. It was found from FTIR spectroscopy that the adsorbed sulfate ions on the faces parallel to the c-axis, such as the {110} and {100} faces, or on {012} faces took the bidentate structure to Fe ions on the surfaces, while a monodentate structure was suggested for sulfate ions on the c-planes ({001} faces). The adsorbed amount of sulfate ions to the {012} faces of hematite at 100 degreesC decreased with increasing pH and became almost zero at pH >/=5, suggesting that the shape control with sulfate is possible only in the acidic media at pH < 5. EDX and chemical analysis revealed that sulfate ions were almost uniformly incorporated into the ellipsoidal or peanut-type particles during their growth in the presence of sulfate ions. On the other hand, sulfate ions in the solution phase with a high concentration of chloride ions such as 3.0 mol dm-3 were likely to be present in the form of free ions without forming a complex with Fe3+, as suggested from UV spectroscopy at 25 degreesC. When the concentration of sulfate ions in the solution phase exceeded the level for shape control of hematite, precipitation of acicular alpha-FeOOH (goethite) was observed. If hematite particles were present in such a system, alpha-FeOOH was grown as whiskers on the hematite particles. When [SO2-4] in the solution phase was higher than 0.1 mol dm-3, precipitation of basic ferric sulfate was also observed in addition to the growth of alpha-FeOOH whiskers on hematite. Posttranscriptional control of adipocyte differentiation through activation of phosphoinositide 3-kinase.

Differentiation of adipocytes is an important aspect of energy homeostasis. Although the transcriptional regulation of adipocyte differentiation is relatively well characterized, the subsequent molecular events remain unclear. The activity of phosphoinositide (PI) 3-kinase precipitated with antibodies to phosphotyrosine has now been shown to increase transiently during adipocyte differentiation of 3T3-F442A and of 3T3-L1 cells. PI 3-kinase activity precipitated with antibodies to insulin receptor substrate 1 (IRS1) and association of subunits of PI 3-kinase with IRS1 were also increased at this stage of differentiation, suggesting that IRS1 contributes to PI 3-kinase activation. Inhibition of the activation of PI 3-kinase by expression of dominant negative mutant subunits of the enzyme prevented adipogenesis, as assessed by lipid accumulation and expression of key adipocyte proteins such as GLUT4, adipsin, and aP2, suggesting that PI 3-kinase activation is essential for adipocyte differentiation. However, these mutant proteins did not affect either the expression of the transcription factor PPARgamma at the mRNA or protein level or the increase in the abundance of mRNAs encoding the adipocyte marker proteins. These results demonstrate that adipocyte differentiation is regulated at the posttranscriptional level and that activation of PI 3-kinase is required for this regulation.

Prognostic discrimination among neuroblastomas according to Ha-ras/trk A gene expression: a comparison of the profiles of neuroblastomas detected clinically and those detected through mass screening.

BACKGROUND: Neuroblastomas (NBs) exhibit a wide variety of clinical behavior. It is important to determine the biology of NB before treatment is instituted. METHODS: One hundred six NBs detected clinically (clinical NBs) were classified according to immunohistochemical expression of the Ha-ras and trk A genes. Association of the two-gene expression with patient outcome was examined retrospectively, and the possibility of prognostic prediction was evaluated. The profile of the expression of the two genes in 85 NBs detected through mass screening (mass NBs) was compared with that in clinical NBs. RESULTS: Ha-rasltrk A expression in clinical NBs was associated with disease free survival, even when the NBs had no amplification of the N-myc gene. Multivariate analysis demonstrated that the expression of Ha-rasl/trk A was a significant prognostic factor that was independent of stage, age at diagnosis, and N-myc amplification. Favorable outcomes of patients with advanced NB were distinguished by high Ha-ras and high trk A expression, and unfavorable outcomes were distinguished by low Ha-ras and low trk A expression. A profile of the two genes in mass NBs was different from that in clinical NBs. Greater than 50% of the mass NBs were detected as localized tumors with high Ha-ras and high trk A expression. The mass screening detected NBs with favorable and unfavorable biology. CONCLUSIONS: The expression of Ha-ras and trk A is an excellent predictor of both favorable and unfavorable biology in NBs. The information it provides can be important in determining the appropriate therapeutic intervention for each patient.
