Bronchioloalveolar carcinoma with bronchorrhoea treated with erythromycin.

Recent reports have shown that erythromycin inhibits bronchial hypersecretion, and, accordingly, erythromycin was administered to a patient with bronchioloalveolar carcinoma with bronchorrhoea. After administration of erythromycin, marked reduction in the volume of sputum was observed, and the patient's quality of life was improved. It is concluded that erythromycin may reduce hypersecretion in bronchioloalveolar carcinoma.

Retreatment of chronic hepatitis C with interferon.

OBJECTIVE: We analyzed the retreatment of chronic hepatitis C with interferon to get the standpoint for the selection of patients to receive it. METHODS: A complete response was defined as continuous normalization of the serum alanine aminotransferase (ALT) level and continuous disappearance of serum hepatitis C virus RNA (HCV-RNA) during interferon administration and more than 6 months after. Patients without complete response were classified as noncomplete responders. From August 1990 to May 1993, we retreated 23 noncomplete responders on initial treatment with interferon and studied the factors that alter the effectiveness. RESULTS: Complete response was achieved in eight (34.8%) patients; the other 15 patients did not achieve this degree of response. Three patients were of genotype II and five were of genotype III in these eight complete responders. All complete responders were patients with relapse who had had normalized serum ALT levels during the initial administration and undetectable HCV-RNA at the end of the period. No patient who failed to achieve normalization of the serum ALT level on initial treatment achieved a complete response on retreatment. HCV-RNA concentrations before retreatment were significantly less than before initial treatment in the eight complete responders. CONCLUSION: Interferon retreatment of patients who do not achieve a complete response may be effective in relapsed cases with undetectable HCV-RNA at the end of initial treatment. Selection of patients to receive interferon retreatment requires careful review and consideration of genotype, HCV-RNA concentration, and the clinical response on initial treatment.

[Clinical study of patients with idiopathic interstitial pneumonia accompanied by diabetes mellitus].

We retrospectively analyzed the clinical presentation of non-insulin-dependent diabetes mellitus (NIDDM) in patients with idiopathic interstitial pneumonia (IIP). Between 1977 to 1993, 75 patients with IIP were admitted to our hospital. None had previously received corticosteroids. To compare the incidence of NIDDM with other pulmonary diseases and the degree of obesity, we randomly selected 200 patients without IIP who were admitted to our hospital in 1992 and 160 healthy subjects. We analyzed weight indices using two methods, body mass index and weight as a percentage of ideal body weight. Eighteen patients (24.0%) with IIP and 9 patients (4.5%) in the disease control group had NIDDM (p < 0.001). As the incidence of NIDDM is approximately 10% in Japan, it was suggested that these IIP patients showed a high prevalence of accompanying NIDDM. Among females, patients with IIP showed a strong tendency toward obesity compared with the healthy control subjects (p < 0.001), but this was not noted among males. Female patients with NIDDM showed a strong tendency toward obesity compared to those without NIDDM, but this was not noted among males. Although the correlation between IIP and NIDDM is unclear, we noted recent reports that advanced glycation and products can stimulate macrophages to secrete tumor necrosis factor and interleukin-1 in vitro, and can stimulate free radical generation by early glycation products in vitro. These cytokines and oxygen radicals may play a role in the pathogenesis or induction of IIP.

[Study on intratumor administration of lentinan--primary changes in cancerous tissues].

Lentinan was administered for gastric cancer in order to determine what type of changes would occur as a consequence. In an experimental study, the intraperitoneal administration of lentinan on cancerous tissues caused a marked development of reticular fibers, along with an enhanced interstitial response. Findings which support the relationship between a marked development of reticular fibers and an anti-tumor effect, were also obtained. Furthermore, lentinan was administered for human gastric cancer to evaluate whether or not an increase in interstitial response would be observed. As a result, reticular fibers developed in tumor sites with a resultant fragmentation of cancer cell nests. Many T lymphocytes infiltrated cancer sites where the lentinan was administered. These findings suggest that the intratumor administration of lentinan enhanced an interstitial response and also activated an anti-tumor immune response in the human gastric cancer tissues.

[Analysis of methods of administration on local arterial infusion chemotherapy for severe advanced hepatocellular carcinoma].

Between 1986 and 1994, hepatic local arterial infusion chemotherapy with implanted reservoir was performed for 51 patients with hepatocellular carcinoma (HCC) who were not indicated for surgery, PEIT and/or TAE because of the advanced stage of tumors and impaired liver function. We compared direct effects, survival rates, and rates of outpatients by dividing these 51 patients into 3 groups according to the methods of administration--repeated one-shot arterial infusion group, continuous arterial infusion group and intermittent high-dose arterial infusion group. There were no significant differences among these three groups in terms of background factors. The continuous infusion group had significantly better survival rates than the repeated one-shot infusion group, and the survival rate of the intermittent high-dose infusion group was similar to that of the continuous infusion group, although there were no significant differences in effectiveness among the three groups. Patients in the continuous infusion group and intermittent high-dose infusion group could receive almost all of their treatment as outpatients. Furthermore, more patients in the intermittent high-dose infusion group could receive whole treatment without hospitalization than patients in the continuous infusion group. Because intermittent high-dose arterial infusion of 5-FU showed about the same survival rate as continuous arterial infusion and because it can maintain high quality of life, it is suggested to be a standard method for local arterial infusion of 5-FU for severe advanced HCC.

[A case of Klippel-Trenaunay-Weber syndrome associated with intracranial multiple angiomas].

A case of Klippel-Trenaunay-Weber syndrome associated with multiple intracranial angiomas was reported. Intracranial angiomas with Klippel-Trenaunay-Weber syndrome is very rare. Only 4 cases are known to this day. A 24-year-old man fell at his workplace and suffered a head injury. He was admitted to our hospital. Neurological examination revealed no abnormality. However, physiological examination demonstrated hyperplasia and cutaneous hemangiomas of left lower leg, malformation of the left eyelid and clouding of the left cornea. CT showed left hemispheric atrophy and subdural effusion. MRI demonstrated multiple flow voids at the left cerebellar hemisphere and at pons. Cerebral angiogram demonstrated left cerebellar AVM and pontine angioma. Feeding arteries of the AVM were left superior cerebellar artery and left posterior inferior cerebellar artery. Those of the pontine angioma could not be identified. No spinal angiomas were revealed. While spinal angiomas associated with this syndrome are known, association of intracranial lesions are rare. Reported anomalies are angiomas, hemi-hypertrophy of skull and brain, carotid occlusion, megadolicoanomaly of the basilar artery, meningioma and glioma. Our case report with cerebral angiomas is the first one in Japan. We postulate that anomaly of our case originated at the Streeter's 2nd-3rd stage or that multiple anomalies occurred. We emphasize that intracranial, spinal or visceral angiomas, which can be fatal if it bleeds, should be routinely examined in Klippel-Trenaunay-Weber syndrome.

[A case of parosteal osteosarcoma at the cranial vault].

A rare case of parosteal osteosarcoma of the cranial vault has been reported. A 32-year-old male was admitted complaining of a hard scalp mass at the left temporoparietal region for several years. On admission he showed no neurological abnormalities. The craniogram, CT and MRI revealed a calcifying mass attached by a small stalk to the skull, and a radiolucent cleft between the tumor and the skull. On operation, the tumor proved to be hard, and it existed between the temporal muscle and the periosteum. A small tumoral attachment existed at the parietal region. The tumor was detached from the skull and the surrounding cranial bone was removed. The histological diagnosis was parosteal osteosarcoma. There have been only 12 cases of parosteal osteosarcoma of the skull reported. The pathological, neuroradiological findings and treatment of parosteal osteosarcoma were the main topics of discussion. This disease should be considered as one of the possible etiologies when a patient with a tumor of the skull is encountered.

[Two cases of IIP which developed acute exacerbation after bronchoalveolar lavage].

We reported two cases of idiopathic interstitial pneumonia (IIP) who developed acute exacerbation after bronchoalveolar lavage (BAL). One case was a 67-year-old male who presented with dry cough and exertional dyspnea. He was diagnosed as IIP and transbronchial lung biopsy revealed alveolitis. BAL was performed after administration of prednisolone. He complained of severe dyspnea after BAL and was diagnosed as having an acute exacerbation of IIP. In spite of extensive treatment including pulse therapy with methylprednisolone, he died. The other case was a 57-year-old male noted to have a chest X-ray abnormality who presented with dry cough and dyspnea on exertion. He was diagnosed as having IIP and primary lung cancer. BAL was performed to evaluate the activity of IIP, and respiratory distress subsequently became severe. After BAL, he developed an acute exacerbation of IIP and died in spite of treatment. In both cases, peripheral white blood cell counts were increased just before BAL. It was suggested that this condition might participate in acute exacerbation of IIP. It should be kept in mind that some patients with IIP may develop acute exacerbation after BAL.

Comparison of the effects of various vasodilators on the rat portal vein and mesenteric artery.

The effects of various vasodilators on isolated helical strips of rat portal vein and mesenteric artery were examined. Dilazep, ibudilast, nifedipine, verapamil and papaverine relaxed the KCl- and norepinephrine-induced contractions of the portal vein to a greater extent than they relaxed those of mesenteric artery. Dibutyryl cyclic AMP and NKH477 did not show any significant difference in terms of the concentrations that produced 50% inhibition (IC50) of the KCl-induced contraction of the portal vein and mesenteric artery. The relaxant effects of nitroprusside and isosorbide dinitrate on the KCl- and norepinephrine-induced contractions of the portal vein were less potent than their effects on contractions of the mesenteric artery. The agents that inhibited the contractions of the portal vein more potently than those of the mesenteric artery were associated with parallel shifts to the right of the concentration-response curves for CaCl2 in both preparations. These results suggest that drugs with a Ca2+ channel blocking action may be preferable to those that cause a decrease in portal pressure for treatment of portal hypertension.

Quantitation of HTLV-I provirus among seropositive blood donors: relation with antibody profile using synthetic peptides.

To study the pathophysiological state of HTLV-I carriers and the quantitative aspect of the risk of HTLV-I infection, we measured the absolute amounts of HTLV-I provirus in 39 seropositive blood donors. The amount of provirus varied from less than one to more than 10(4) molecules per 10(5) peripheral-blood mononuclear cells (PBMC). The average amount of HTLV-I provirus among seropositive blood donors was estimated as 3.7 x 10(3) molecules per 10(5) PBMC. The titer of the antibody against only the synthetic peptide (aa 100 to 130) of the gag p19 region of the HTLV-I provirus showed a weak but significant correlation with the amount of the provirus.

Ca2+(-)dependent and Ca2+(-)independent vasorelaxation induced by cardiotonic phosphodiesterase inhibitors.

Cardiotonic agents that belong to a group of phosphodiesterase inhibitors--vesnarinone, amrinone, enoximone, pimobendan, MS-857 and E-1020--inhibited the 35 mM KCl- and 10(-7) M norepinephrine-induced contractions of helical strips from rat thoracic aorta in a concentration-dependent manner. In the absence of extracellular Ca2+, 10(-7) M phorbol 12,13-dibutyrate caused a sustained contraction of the muscle strip without an increase in intracellular Ca2+ level ([Ca2+]i). The phorbol 12,13-dibutyrate-induced contractions in Ca2+(-)free buffer were also inhibited by the cardiotonic phosphodiesterase inhibitors. A cyclic GMP-inhibited cyclic nucleotide phosphodiesterase was partially purified from rat aorta and the activity of the phosphodiesterase was inhibited by the cardiotonic agents. The inhibitory effect of these agents on the KCl-, norepinephrine-and phorbol 12,13-dibutyrate-induced contractions showed good correlations to the concentrations of the agents producing 50% inhibition (IC50) of cyclic GMP-inhibited cyclic nucleotide phosphodiesterase. Vesnarinone inhibited the norepinephrine-induced contraction with a decrease in [Ca2+]i, but inhibited the phorbol 12,13-dibutyrate-induced contraction in Ca(2+)-free buffer without significant changes in [Ca2+]i. Dibutyryl cyclic AMP and NKH477 also inhibited the phorbol 12,13-dibutyrate-induced contraction in Ca(2+)-free buffer without significant changes in [Ca2+]i. The six cardiotonic phosphodiesterase inhibitors increased the cyclic AMP contents of the muscle strips. These results suggest that the inhibitory actions of these cardiotonic phosphodiesterase inhibitors on cyclic GMP-inhibited cyclic nucleotide phosphodiesterase may cause vasorelaxation through a decrease in [Ca2+]i and an inhibitory effect on a Ca(2+)-independent contractile process (or a decrease in Ca(2+)-sensitivity of contractile elements).

Induction of peroxisomal beta-oxidation by structural analogues of dehydroepiandrosterone in cultured rat hepatocytes: structure-activity relationships.

The structural requirements of dehydroepiandrosterone (DHEA) for the induction of peroxisomal beta-oxidation were studied in cultured rat hepatocytes. The hepatocytes were incubated for 5 days with various steroids, including 3- and 17-substituted analogues and 5-hydrogenated analogues of DHEA, and the activities of peroxisomal beta-oxidation and carnitine acetyltransferase were measured. Among the steroids examined, DHEA, DHEA sulfate (DHEAS), dehydroandrosterone sulfate, androstenediol 3-sulfate, epiandrosterone sulfate and androsterone sulfate significantly induced the enzymes; 4.6- to 14.2-fold for beta-oxidation and 5.1- to 10.9-fold for carnitine acetyltransferase at 50 microM. All of the sulfated steroids were more effective than the corresponding unsulfated forms. DHEAS was the most potent inducer. The 3-sulfuric group was required for the marked induction of peroxisomal beta-oxidation, and the 17-carbonyl group was also important. Furthermore, the relatively planar conformation of the steroidal hydrophobic backbone was crucial for inducing the enzyme. The configuration of the 3-sulfuric group (beta-configuration) and the presence of a double bond at position C5 were not primary determinants for the action of DHEAS. On the other hand, the introduction of bulky substituents to position C17 or aromatization of ring A led to a loss of inducing activity. Thus, there are strict structural requirements for the DHEA induction of peroxisomal beta-oxidation, suggesting the presence of a certain specific recognition site in the cell for DHEAS, which mediates the peroxisome proliferator action of DHEA.

GABA receptor subtypes involved in the neuronal mechanisms of baroreceptor reflex in the nucleus tractus solitarii of rabbits.

The role of GABAA and GABAB receptors in neuronal mechanisms of the baroreceptor reflex in the nucleus tractus solitarii (NTS) of the anesthetized and immobilized rabbits were investigated using a microiontophoretic technique. Baroreceptive neurons (BRNs) activated or depressed by baroreceptor stimulation (phenylephrine, 10 micrograms/kg, i.v.) were identified in the NTS (activated BRN (A-BRN) and depressed BRN (D-BRN), respectively). The GABAA antagonist bicuculline (40-80 nA) increased spontaneous activities of these neurons, but the GABAB antagonist phaclofen (80-160 nA) did not. Evoked responses of A-BRNs were potentiated by bicuculline and phaclofen, while the responses of D-BRNs were not clearly affected by these drugs. These results suggest that most of A- and D-BRNs are tonically inhibited by endogenous GABA acting on GABAA receptors, but not on GABAB receptors, and that GABAergic mechanisms suppressively modulate the baroreceptor reflex acting on GABAA and GABAB receptors of A-BRNs, but not of D-BRNs.

Individual diversity of IL-6 generation by human monocytes with lentinan administration.

Effect of in vivo administration of lentinan on IL-6 generation by human peripheral blood monocytes was examined using 5 healthy volunteers. Monocytes isolated from them before and 3 days after intravenous administration of 2 mg lentinan per person were cultured for 2 days and then the levels of IL-6 in the supernatants of cultured monocytes were determined. In vivo lentinan administration elicited an increase in IL-6 generation by monocytes in 3 of 5 cases. Failure to increase IL-6 generation by monocytes in two cases is characteristic of lentinan, since these monocytes generated an apparent level of IL-6 in response to lipopolysaccharide, a potent monocyte activator. Thus, it was demonstrated that lentinan was capable of augmenting IL-6 generation by human monocytes and that there was individual difference in their responsiveness to lentinan.

Antitumor activity of a new series of platinum complexes: trans(+/-)-1,2-cyclohexanediammineplatinum(II) conjugated to acid polysaccharides.

Complexes with trans(+/-)-1,2-cyclohexanediammineplatinum(II) conjugated to acid polysaccharides were synthesized and their antitumor activities were tested in female CDF1 mice with intraperitoneal leukemia L1210 cells. Platinum was released from the polymers under physiological conditions, with half-lives from 3.3 to 19.3 h. A hyaluronic acid-supported complex was the most effective against the tumors (all six mice survived for 60 days). The group given a chondroitin polysulfate-supported complex had five survivors, the chondroitin sulfate A group also had five, the chondroitin sulfate C group had three and the heparan sulfate group had two. Part of the antitumor activity was due to increased efficacy of the polymers. The bioavailability of these complexes is high. Therefore, acid polysaccharides should be a good system for delivering antitumor platinum complexes.

Lack of stereoselectivity in rat liver peroxisomal enzyme induction by optically active phenoxyacetic acids.

To ascertain if there is stereoselectivity in peroxisomal proliferation induced by chiral peroxisome proliferators, induction by stereoisomers of 2-methyl-4'-chlorophenoxyacetic acid and 2-methyl-2-(2'-4'-dichlorophenoxy)acetic acid were studied in rat in vivo and in vitro with isolated rat hepatocytes. No significant differences in the inducing potencies of the stereoisomers of the above two phenoxyacetic acid derivatives were found for cyanide-insensitive fatty acyl-CoA oxidizing system, fatty acyl-CoA oxidase, carnitine acetyltransferase or carnitine palmitoyltransferase. There was also no significant difference in the degree of hepatomegaly or lipid-lowering effect between the isomers. The findings with cultured rat hepatocytes agreed with those of the studies in vivo.
