The fine structure of the intercellular junctions of the hen parathyroid gland was studied using freeze-fracture replicas and thin sections. In the conventional thin sections, desmosomes, intermediate junctions (maculae adherentes) and gap junctions were observed, and in the lanthanum-fixed sections, tight junctions (maculae occludentes) were demonstrated as well. In the freeze-fracture replicas, desmosomes, gap junctions, tight junctions and combination forms of gap and tight junctions occurred, but intermediate junctions were not identified. Junctional complexes (zonulae occludentes) were not encountered in any preparations. The gap junctions varied in size and shape; they ranged from irregularly shaped, minute assemblages of particles to large aggregations of a round or elliptic outline. Both the tight junctions and the combination forms of gap and tight junctions also exhibited a variety of shape and dimension, and, depending on the form of the tight junctional strands, they were classified into three types: type I consisted of a simple line of strands; type II consisted of a closed network of strands; and type III consisted of an open network of strands. The combination forms were more numerous than the typical tight junctions. The possible significance of these junctions is discussed in relation to the function of the parathyroid parenchymal cell.

Color-functional imaging of intrahepatic blood flow was developed using the 133Xe clearance method and a gamma camera with a computer system. During the 2 min after intrasplenic injection of 133Xe in saline solution, 24 sequential gamma images were obtained. After setting the hepatic region, 133Xe clearance curves were extracted from the serial images every 6 X 6 mm element and regional blood flow for each element was calculated. The calculated regional hepatic blood flow values were displayed as a color images in eight color steps. Eleven patients with and without liver diseases were studied. In all the patients studied, heterogeneous intrahepatic distribution of blood flow was clearly demonstrated by the functional image of regional hepatic blood flow. Although a consistent pattern of intrahepatic distribution of blood flow was not obtained, greater-flow regions were frequently observed in the right lobe. Repeat studies in two patients demonstrated that the intrahepatic distribution of blood flow varied. The 10-15 s scintiphotosplenoportograms also showed the existence of restricted or preferential intrahepatic distribution of splenic flow. These results strongly suggest that intrahepatic distribution of blood flow in the human liver is heterogeneous and variable.
The lipoperoxide level in liver as well as serum in alcoholic patients was measured by using the thiobarbituric acid method. Heavy drinkers with deposition of fat in liver had a significantly (p less than 0.001) higher level of hepatic lipoperoxide examined in about 7 days after abstinence than did nonalcoholic individuals with histologically normal liver. Of these heavy drinkers, cases with a remarkably high level of liver lipoperoxide showed a high incidence of hepatic cell necrosis. The hepatic lipoperoxide level was correlated with the serum lipoperoxide level in these heavy drinkers (r = 0.750, p less than 0.001). In chronic alcoholics, high levels of serum lipoperoxide were usually observed on admission, and a significant positive correlation was found between the serum lipoperoxide level and serum GOT activity (r = 0.541, p less than 0.01). After a 2-wk abstinence, the levels of serum lipoperoxide decreased, which was associated with improvement of liver function tests. These results suggest a possible involvement of lipid peroxidation in alcohol-induced liver damage in humans.

Scintiphotosplenoportography (SSP) was performed in 190 instances in 161 patients. No significant complications, such as severe pain or splenic haemorrhage, were encountered in any of the patients studied. Flow patterns of SSP were classified into nine groups according to the direction of collateral flow. Hepatopetal collateral flow was frequently observed in pancreatic cancer. In portal hypertension, cephalic collateral flow was more frequently observed than caudal flow. In detecting oesophageal varices, both SSP and endoscopy were performed in 81 patients. In four out of 46 patients with cephalic flow, oesophageal varices could not be observed by endoscopy. Conversely, in three out of 44 patients with oesophageal varices, SSP failed to show cephalic flow. The combined use of SSP and endoscopy would therefore provide more useful information in the management of patients with portal hypertension.
