In surgical treatment for lung cancer, increase of right ventricular afterload associated with decrease of pulmonary vascular bed after lung resection is serious problem in acute phase. Intensive care is necessary for the patients having major surgery, especially in aged. Until now, we have studied on pre and post operative extravascular lung water (EVLW) in addition to measurement of conventional hemodynamic changes on right ventricular afterload. And then, we have discussed when we should start to care intensively for degradation of compensatory hyperdynamic change. According to percent exchanges (delta %) between pre and post operative measurement, we established following criterion 1) delta %HR greater than 150% 2) delta SVI less than 60% 3) delta % PARI greater than 200% 4) observed postoperative EVLW greater than predicted postoperative EVLW. Furthermore, we investigated the correlation with these criterion and RVEF in 14 patients using a rapid computerized thermodilution method on right cardiac function in acute phase. We found that 1) changes of RVEF seem to depend on endsystolic volume 2) RVEF decreased just after operation and then slowly increased until 48 hours 3) PARI elevated to 194 +/- 117% in average at 6 to 24 hours showing significant inversed linear correlation to changes of RVEF (R = -0.54). The patients having prolonged elevation of PARI showed degradation of cardiac output and RVEF. RVSWI showed compensative acceleration gradually with increased right ventricular afterload.(ABSTRACT TRUNCATED AT 250 WORDS)
For the purpose of simplification of prediction of postoperative lung function, we studied to predict lung function by analizing the frontal and lateral view of chest plain roentgenogram and investigate the correlation to respiratory complication on 111 patients with lung cancer. According to TNM classification of lung cancer, prediction was performed as follows. Predicted postoperative lung function = [(42-number of resected subsegments)/(42-number of occupied subsegments)] x preoperative VC or FEV1.0. In this formula, 42 was the number of functioning subsegments of whole lung (right: 22, left: 20), and then preoperative occupied subsegments was ordered by T factor, where T1 lesion in lung field was prescribed as 1 subsegment and T2 was more than 2 subsegments respectively in plain chest roentgenogram. And also, on the patients having hilar lesions, it was required to calculate the number of subsegments in atelectasis, peripheral obstructive pneumonia and/or partial emphysematous change due to intrabronchial lesions. There was uniformly positive correlations in VC (R = 0.7949) and FEV1.0 (R = 0.8235) of the patients studied respectively. The patients having pneumonectomy showed tendency of over estimation, on the other hand, the patients having resection of a few segments showed under estimation. To predict the postoperative respiratory condition, we calculated the predicted post-operative %VC and %FEV1.0 for predicted preoperative normal VC and FEV1.0. Above the al, we tried to investigate the correlation with predicted postoperative %VC, %FEV1.0 and postoperative respiratory complications.(ABSTRACT TRUNCATED AT 250 WORDS)
A 68-year-old man was admitted to our hospital because of shock. CT scan revealed ruptured abdominal aortic aneurysm (AAA) with isolated left-sided inferior vena cava (L-IVC). After emergent laparotomy, maintaining blood pressure stable, we clamped the aorta at the level of the supra-celiac region. An AAA (8.5cm in diameter) leaking from the posterior wall with a retroperitoneal hematoma was found, and L-IVC pressed by AAA was identified. The AAA was dissected with caution so as not to injure L-IVC and replaced with a woven Dacron bifurcated graft. The postoperative course was uneventful. The embryonic development and clinical significance of this rare anomaly are discussed. This is the first successful case in the Japanese literature by our survey.
Assessment of the level of high-energy phosphates in the myocardium before, during and after ischemia was performed in 19 consecutive patients who underwent cardiac operation. The following results were obtained. 1) The levels of total nucleotides (ATP, ADP, AMP, CP) determined before, during and after aortic cross clamp were 14.94 +/- 4.12, 5.59 +/- 2.16 and 3.58 +/- 1.14 micrograms/mg. protein in the right atrial appendage. Those in the left ventricular myocardium were 18.22 +/- 4.90, 6.99 +/- 1.52 and 4.35 +/- 1.06 micrograms/mg. protein. The latter levels were higher than the former. 2) Rapidly decreasing after aortic cross clamp, the nucleotides dropped to 37-38% of preischemic level during aortic cross clamp and to 23% of preischemic level 30 minutes after reperfusion. At the termination of extracorporeal circulation, when circulatory dynamics stabilized, the nucleotides were only recovered to 50-53% of preischemic level. 3) Negative correlation was observed between the length of aortic cross clamp time and the content of nucleotides in the atrial muscle. (p less than 0.05) 4) Fluctuations in the nucleotide level indicated that the current method of myocardial protection using GIK solution produced unsatisfactory recover from ischemic damage and reperfusion injury.
We discussed the operative concept of revascularization of lower extremities in patients associated with severe, coronary artery disease (CAD). Those with symptomatic CAD may undergo coronary artery bypass (CABG) or percutaneous coronary angioplasty (PTCA) with or without intraaortic balloon pump (IABP). Special attention should be paid during operation in these patients in order to reconstruct the arterial catheter route, which gives us the best way to percutaneous transfemoral approach to the aorta or the coronary arteries. One should not choose arbitrarily extra-anatomical bypass, such as axillo-femoral or femorofemoral, in these cases. Also, artificial graft should not be applied in the common femoral arteries, which will make percutaneous approach difficult. Common femoral arteries, if needed, are best reconstructed by means of thromboendarterectomy. Attaining smooth, bilateral aorto-ilio-femoral continuity is the main goal of revascularization of lower extremities in patients with CAD.
In surgical treatment for lung cancer, large amount of intrabronchial sputum and secretion was occasionally encountered in patients, especially underwent bronchoplastic surgery. It has been discussed as causes which was affected from transection of pulmonary branches of vagal nerve and bronchial arteries. So we have measured pre- and postoperative hemodynamics and extravascular lung water (EVLW) at the bedside, as to investigate the correlation with formyl sputum and increase of right ventricular afterload which was presumed by lung resection and bronchoplasty. Respiratory complication was classified by the incidence of forced bronchial toilet in acute phase. Seven patients were investigated. Three of seven showed white sputum which was difficult to excrete. EVLW was 9.91 +/- 2.12 ml/kg in average before operation and then showed a tendency to increase within 102 +/- 29% of percent exchange (delta %) between pre- and postoperation. As to comparison of delta % of EVLW in acute period, the patients underwent bronchoplastic surgery showed a tendency to increase rather than the patients underwent lobectomy. According to former studies, we found the uniform correlation R = 0.7075 between observed EVLW and predicted postoperative EVLW estimated by prediction of residual functioning lung volume before operation. And also, the patients showing postoperative EVLW more than predicted EVLW before operation had a tendency to increase amount of sputum. Furthermore, concerning hemodynamic changes, pulmonary arterial resistance index (PARI) elevated to 196 +/- 165% (83-515%) in average associated with increased right ventricular stroke work index (RVSWI) as a result of compensatory changes for increased right cardiac afterload.(ABSTRACT TRUNCATED AT 250 WORDS)
An intrarenal infusion of (+/-)-1-(2,5-dimethoxy-4-iodophenyl)-2-aminopropane (DOI), a selective 5-HT2 agonist, at a rate of 5 micrograms/min in anesthetized dogs resulted in an increase in renal blood flow (RBF) without any transient decrease as usually observed during the infusion of 5-HT. During the infusion, urine flow (UF) and urinary sodium excretion rates (UNaV) increased along with RBF while the mean arterial pressure and glomerular filtration rate did not change. After pretreatment with ritanserin, a selective 5-HT2 antagonist, DOI failed to increase RBF, UF and UNa V. It is concluded that DOI produces renal vasodilatation mediated via 5-HT2 receptors and has a diuretic action.
We studied the effects of cromakalim, a potassium channel opener, on regional blood flow in conscious spontaneously hypertensive rats using microspheres. Cromakalim produced significant hypotension and increased blood flow in the stomach, skeletal muscle and skin. Calculated vascular resistances decreased in many organ vascular beds. Although nifedipine also showed a similar hemodynamic profile, differences were observed in the brain, gastrointestinal tract and skin.
We examined the inhibitory effects of a novel antiplatelet aggregating agent, E-5510, on cyclic flow variations (CFVs) of coronary blood flow (CBF) in anesthetized open-chest pigs. These CFVs, which are characterized by progressive declines in CBF followed by sudden restoration of flow, were initiated by electrical stimulation of the intimal surface of the left circumflex coronary artery (LCX). A reduction in CBF to zero during CFVs was accompanied by ischemic changes in the surface electrocardiogram and regional segment shortening of the left ventricular wall. Occlusive thrombi were detected postmortem in the coronary arteries of the animals in which CFVs had occurred. After CFVs had been observed for 1 h, E-5510 (0.01 or 0.1 mg/kg) or saline was administered intravenously. Once CFVs were initiated, both the frequency and the severity (the mean of the three lowest nadirs of CBF) were unchanged by the administration of saline. E-5510 at 0.01 mg/kg decreased the frequency of CFVs from 7.7 +/- 0.9 to 4.6 +/- 1.1 CFV s/h (n = 7, p less than 0.05), and increased the mean lowest nadir from 13.5 +/- 5.2% of the CBF level before electrical stimulation to 54.3 +/- 11.1% (n = 7, p less than 0.01). E-5510 at 0.1 mg/kg further decreased the frequency from 8.9 +/- 0.7 to 2.4 +/- 0.5 CFVs/h (n = 9, p less than 0.01), and increased the mean lowest nadir from 14.3 +/- 3.2% to 53.6 +/- 10.8% (n = 9, p less than 0.01). E-5510, however, showed no ameliorative effect on ischemia-induced myocardial dysfunction, as expressed by the decrease in regional myocardial shortening.(ABSTRACT TRUNCATED AT 250 WORDS)
In order to determine the influence of strain and sex on local carcinogenesis in rat mammary tissue, 1 mg of 7,12-dimethylbenz(a)anthracene (DMBA) was dusted directly onto the exposed mammary gland of 30-day-old Long-Evans (L-E) rats and Sprague-Dawley (S-D) rats. The experiment was terminated 28 weeks after application of the carcinogen. Tumors measuring between 1 and 2 cm in diameter were harvested from female L-E rats with high frequency (85%) and long latency (mean: 23.7 weeks after DMBA dusting), and from female S-D rats with extremely high frequency (98%) and short latency (16.7 weeks). Male rats of both strains were almost identically much less susceptible to DMBA (L-E; 55%, 25.0 weeks, S-D; 53%, 23.9 weeks). Ovariectomized S-D (47%, 24.9 weeks) and orchiectomized S-D (30%, 24.8 weeks) rats, which were gonadectomized at 30 days of age, respectively, were also much less susceptible. A variety of histologies, mostly malignant epithelial, mesenchymal or mixed tumors, were noted in each group. The carcinomatous response in the mammary tissue was much higher in female S-D (96%) than in female L-E (50%) rats, and very low in male and gonadectomized rats (10-20%). In contrast, the sarcomatous response in the mammary tissue was moderate in female and male L-E and male S-D (43-50%) rats, and low in the other groups (15-29%).
We examined the presence and characteristics of epidermal growth factor (EGF) receptors in hCG producing tumors (CC-2-JCK) transplanted in female nude mice. We also examined the in vivo effects of EGF on tumor growth. Specific receptors with apparent dissociation constants of 3.89 x 10(-10) and 1.0 x 10(-9) M and binding capacities of 5.96 x 10(-10) and 1.52 x 10(-9) M/mg protein for EGF have been identified in the hCG-producing tumor. [125I]EGF binding to the tumor tissues was time, temperature, and tissue weight dependent and specific. EGF and transforming growth factor-alpha (TGF alpha) competed for [125I]EGF binding, with 50% of the bound [125I]EGF displaced by approximately 0.52 nM EGF and 3.10 nM TGF alpha. TGF beta competed for [125I]EGF binding slightly. Five micrograms of EGF caused an increase in the rate of tumor growth, while 50 micrograms EGF strongly inhibited tumor growth. The concentration of [125I]EGF binding in the tumor treated with low doses of EGF was high, and that in the tumor treated with high doses of EGF was low. These changes in EGF binding were attributable to the changes in the number of high affinity EGF receptors with no significant alteration in binding affinity. In conclusion, the existence of high concentrations of EGF receptors with high affinity and specificity to EGF was demonstrated in an hCG-producing tumor transplanted in nude mice and appeared to be correlated with tumor growth.
Our purpose was to localize the intrarenal vascular sites of action of adenosine and glucagon. Renal blood flow (RBF) and glomerular filtration rate (GFR) were measured in anesthetized dogs, and renal perfusion pressure (RPP) was varied by an adjustable aortic clamp. At normal RPP, RBF was increased by all agents. In contrast, GFR was increased by glucagon, decreased by adenosine and unchanged by acetylcholine (ACh) or adenosine plus glucagon. The increases in RBF by glucagon occurred only at RPPs within the autoregulatory pressure range, and renal autoregulatory capability was attenuated during the infusion of glucagon. In contrast, adenosine increased RBF at RPPs both within and below the autoregulatory pressure range, and the autoregulatory capability was not perceptibly impaired. Superimposition of glucagon to adenosine caused further vasodilation, and the autoregulatory efficiency was completely attenuated. There was no difference between the RPP-RBF or RPP-GFR relations obtained during infusion of adenosine plus glucagon and ACh, which dilates both the afferent and efferent arterioles. It is generally accepted that afferent arteriolar resistance attains a minimum value at RPP near the lower limit of the autoregulatory range. Thus, our data indicate that glucagon and adenosine preferentially dilate the afferent arteriole and the efferent arteriole, respectively.
In this study we examined the expression of EGF receptor mRNA after EGF administration in hCG producing tumor (choriocarcinoma). We transplanted the tissue of choriocarcinoma into female nude mice and investigated the effects of EGF on the growth of tumors, the binding activity of EGF receptor and the expression of EGF receptor mRNA in the tumor tissues. Two doses of EGF 5.0 micrograms, 50 micrograms and phosphate buffered saline as a control were injected subcutaneously every day for four weeks. Removed tumors were used for immunocytochemical studies and EGF receptor mRNA investigations. HCG and EGF receptors were detected immunocytochemically in the tumor. The low dose EGF employed stimulated the tumor growth while the high dose EGF inhibited the tumor growth compared with that of the control group. The binding activity of EGF receptor and the expression of EGF receptor mRNA also changed in accordance with the stimulation or inhibition of tumor growth. The growth of hCG producing tumor by EGF administration appeared to be dependent upon the binding activity of EGF receptor and the expression of EGF receptor mRNA.
We performed coronary artery bypass grafting with the left internal mammary artery, right gastroepiploic artery, and inferior epigastric artery on a 60-year-old male. The inferior epigastric artery used as a free graft was placed between the in situ left internal mammary graft proximally and the obtuse marginal branch distally. Both the left internal mammary graft to the left anterior descending artery and the right gastroepiploic artery to the right coronary artery were used as an in situ graft. All grafts were patent two weeks after the operation and the patient was free from angina at three months follow-up period.

To investigate genetic and sex factors in the local tumorigenesis by 7,12-dimethylbenz(alpha)anthracene (DMBA) in the mammary gland, both males and females of two different inbred strains of rats, Wistar/Furth (WF) and Copenhagen (COP), were subjected to dusting with approximately 1 mg of DMBA powder directly onto the exposed inguinal mammary tissue at 30 days of age. Locally growing tumors to 2 cm in mean diameter were harvested during 28 weeks after the carcinogen application. The incidence of macroscopic tumors of mammary origin was 100% in 15 WF females, 19% in 16 WF males, 85% in 20 COP females, and 74% in 19 COP males. Histologic pictures indicated the carcinomatous pattern composing mainly of differentiated adenocarcinoma of ductular cells in 12 tumors (80%) from WF females but not in any tumors from the other groups. On the other hand, they showed the sarcomatous pattern characterized by undifferentiated sarcoma of stromal cells in 2 tumors (13%) from WF females, 3 tumors (19%) from WF males, 16 tumors (80%) from COP females, and 14 tumors (74%) from COP males. The other 2 tumors from 1 WF and 1 COP females revealed the carcinosarcomatous pattern. Therefore, mammary ductular cells of WF are highly susceptible to DMBA and may be modified by sex factors in their carcinogenesis. Mammary stromal cells of COP are extremely susceptible to DMBA independently of sex factors.
A 64-year-old male was admitted to our hospital suffering from dysphagia and dyspnea. The chest X-ray films and chest CT films revealed huge tumor in his upper-anterior mediastinum. The filling defect at lower rim of the left brachiocephalic vein on venography indicated malignant potential of the tumor. There was no abnormal value in blood and urine examinations except CA 19-9 which was 31.8 U/ml. The tumor was resected with bilateral pleura, pericardium and left brachiocephalic vein. Macroscopically the specimen accompanied with a lot of bleeding spots was soft and yellow and has no capsule. The size of tumor was 15 X 13 X 8 cm and its weight was 865 gm. No tumor recurrence was found until present time: two years after surgical therapy followed by adjuvant radiation therapy (60 Gray). Though histological findings of anterior part of the tumor were consisted of "Zellballen" cells and bleedings, the posterior part had cell atypia and invasion into vessels. These cells were slightly positive on Keratin stain and on NSE stain. Typical findings of paraganglioma were also indicated by electron microscopy: those findings were more chief cells, fewer sustentacular cells, abundant mitochondria and few dense-cored granules which are thought to secrete hormonal substances. Judging from these clinical and pathological results, this tumor was diagnosed nonfunctional aortico-pulmonary paraganglioma. Mediastinal paraganglioma is a very rare tumor. There is no report of malignant aortico-pulmonary paraganglioma in Japan after Glenner and Grimelys' study concerning extra-adrenal paraganglion system.
