[Surgical indications for thoracotomy in cases of small advanced lung cancer by use of DNA content analysis and mediastinoscopy].

Twenty-seven patients with small advanced lung cancer were treated at Kyoto Katsura Hospital. Of 27 patients, 19 patients underwent thoracotomy. The mean survival time of the 19 resected patients was 49 months, while that of the 8 unresected patients was 36 months. There was no statistically significant difference between these mean survival times. Of the 19 resected patients, 6 patients underwent a relative curative operation (RCO), 4 patients underwent a relative noncurative operation, and 9 patients underwent an absolute noncurative operation. The respective mean survival times were 79 months, 30, and 33. We compared the mean survival time of RCO cases with that of the unresected cases, and there was a significant difference in favor of the RCO cases. So we recommend thoracotomy for patients with small advanced lung cancer if RCO is possible, and that requires of a histological classification of less than N2. We performed mediastinoscopy for 18 of the 27 patients to check for mediastinal lymph node metastasis, and 16 of those patients were confirmed to have metastasis pathologically. Mediastinoscopy is useful and reliable for confirming lymph node metastasis, because it gives us a pathological diagnosis. We also performed DNA content analysis on the 19 resected cases. There were 7 DNA diploidy cases, 11 DNA aneuploidy cases, and 1 case couldn't be determined. There was no significant difference between the mean survival times of DNA diploidy cases and the DNA aneuploidy cases.

[Gene analysis of pulmonary lymphoproliferative disorders].

The authors performed gene analysis of pulmonary lymphoproliferative disorders. Cell suspensions were obtained from tissues of malignant lymphoma or pseudolymphoma in Cases 1 to 3. High-molecular-weight DNA was extracted from these specimens, digested with restriction endonucleases, size-fractionated by agarose-gel electrophoresis and transferred by the Southern procedure to nitrocellulose. Hybridization to nick-translated 32P DNA probes of the immunoglobulin JH, C kappa, C lambda, regions, and T cell receptor beta 1 region. In case 1 and 2, which were diagnosed as B cell lymphoma, cells from tumor had rearranged heavy chain genes, clearly establishing the clonal nature. In Case 3, which was diagnosed as pseudolymphoma, the tumor contained clonal immunoglobulin gene rearrangements as detected with both the JH heavy chain and C kappa light chain gene probes. It was concluded that gene analysis is an effective procedure for establishing a diagnosis of lymphoma in neoplastic disorders of uncertain cell type and for detecting clonal T cell or B cell populations with atypical lymphofollicular hyperplasia.

[Usefulness of transesophageal echocardiography in thoracic aortic disease: comparison with computed tomography and angiography].

To evaluate the usefulness of transesophageal echocardiography in thoracic aortic disease, transesophageal echocardiography was performed in 11 patients with thoracic aortic disease (true aortic aneurysm in two patients, aortic dissection in nine patients). Findings obtained by transesophageal echocardiography were compared with those obtained by computed tomography (nine patients) and angiography (six patients). Results were as follows: 1. True aortic aneurysm: Although transesophageal echocardiography and computed tomography could evaluate the size of the aorta and detect mural thrombus, angiography underestimated degree of aortic dilatation because of the presence of mural thrombus. 2. Aortic dissection: Although computed tomography could not differentiate thrombus in the false lumen from mural thrombus in two patients, transesophageal echocardiography could discriminate those two easily in one patient from properties and motion of the intimal flap, and flow information in the false lumen. Angiography could not evaluate the range of dissection in any patients with thrombus in the false lumen. With transesophageal echocardiography we could detect the entry in all six patients with aortic dissection of DeBakey type III except 1 patient with aortic dissection of DeBakey type I. Transesophageal echocardiography could obtain real-time image of most of the aorta except for a part of the ascending aorta. Therefore, transesophageal echocardiography was useful for diagnosing thoracic aortic disease and distinguishing between true aortic aneurysm and aortic dissection.

[Clinical investigation on the patients with tachyarrhythmic syncope with special reference to comparative study between long QT syndrome and ventricular tachycardia without long QT interval].

The long QT syndrome (LQTS) is one of the important diseases that may lead to sudden death mainly in childhood, however etiology and pathogenesis are still poorly understood. The group studied consisted of 6 patients with a history of ventricular tachyarrhythmic syncope, 3 with long QT syndrome (LQTS) and 3 without long QT interval, and of 4 patients with ventricular tachycardia without syncopal episode. Their ages ranged from 5 years to 17 years. Histopathology of endomyocardial biopsy was nonspecific and mild in two cases but in one patient with LQTS, who had several episodes of syncope and refractory ventricular arrhythmia, remarkable subendocardial fibrosis, interstitial fibrosis and hypertrophy of myocytes were demonstrated. As far as ventricular tachycardia without long QT interval was concerned, in the patients with VT with syncope, histopathological abnormalities were more remarkable than in those without syncope. Electrophysiological findings in the patients with LQTS showed no characteristic findings, but only mild abnormalities with functional atrioventricular conduction disturbance on programmed atrial pacing. No inducible VT was demonstrated. Although electrophysiologic study and endomyocardial biopsy are of limited value, such studies are considered to be worthwhile for treating ventricular arrhythmias, and making a prognosis of the patients with tachyarrhythmic syncope and LQTS.

Characterization of cross-reactive polysaccharide antigen with serotype a and d strains from Streptococcus sobrinus 6715.

The polysaccharide antigen (designated SI) from Streptococcus sobrinus 6715 (serotype g) which cross-reacts with serotype a and d strains was purified by a specific anti-cross-reactive g-a antibody-Sepharose 4B affinity column. By a double immunodiffusion analysis, the SI antigen was found to lack the serotype-specific g site, but contained the cross-reactive sites g-a, g-d and g-(a-d) on a single molecule. Polysaccharide SI was composed of galactose, glucose and rhamnose in a molar ratio of 4.79:1.52:1. The results of the test on the inhibition of the precipitin reaction and methylation analysis suggested that the cross-reactive site g-a of the SI antigen appeared to have two regions, one containing galactose residues and the other, beta-linked glucose residues.

Follow-up in mitral valve prolapse by phonocardiography, M-mode and two-dimensional echocardiography and Doppler echocardiography.

To assess the serial phonocardiographic and echocardiographic change in patients with mitral valve prolapse (MVP), phonocardiograms and echocardiograms were reviewed retrospectively in 116 patients (48 men and 68 women, mean age 27 years) who had been determined to have MVP and were reexamined 4.3 years (range 1 to 14) later by phonocardiography and echocardiography between 1971 and 1988. Follow-up phonocardiograms showed periods when 5 of 18 patients with silent MVP developed mid- or late systolic clicks. Of 57 patients with mid- or late systolic clicks, 15 had silent MVP, 6 developed a late systolic murmur with or without systolic clicks and 1 developed a pansystolic murmur. Two of 9 patients with an isolated late systolic murmur developed a pansystolic murmur. M-mode echocardiograms showed that left atrial and left ventricular dimensions at end-diastole and end-systole increased in patients with systolic murmur (33 +/- 10 vs 35 +/- 11, 46 +/- 6 vs 50 +/- 7 and 29 +/- 4 vs 31 +/- 5 mm, respectively, all p less than 0.001) and no statistically significant changes in any of these dimensions were found in patients without a systolic murmur. The degree of MVP evaluated by the anteroposterior mitral leaflet angle on the 2-dimensional echocardiogram was more severe in patients with a systolic murmur than in patients without systolic murmur (157 +/- 12 vs 131 +/- 16 degrees, p less than 0.001). The degree of prolapse did not change during the follow-up periods. The number of patients with mitral regurgitation detected by pulsed Doppler echocardiography increased from 21 of 72 (29%) to 31 of 72 (43%).(ABSTRACT TRUNCATED AT 250 WORDS)

Characterization of glucan involved in the reduction of dental caries in rats.

Culture supernatant fluids (SF) of mutant 108 obtained from Streptococcus sobrinus 6715, when incubated with sucrose, produce large amounts of water-soluble glucans. Mainly, two water-soluble glucan synthesizing glucosyltransferases (GTases) were found in culture SF by isoelectric focusing. These two GTases had higher activities than those from the parent strain. The GTase which synthesizes water-insoluble glucan totally lacked the mutant. When the glucan preparation synthesized by the SF was subjected to gel filtration, a major large molecular weight fraction (designated glucan A) and a small molecular weight fraction (designated glucan B) were obtained. Glucan A yielded a smaller molecular weight glucan after treatment with dextranase. Glucan B was not hydrolyzed by the same enzyme.

Utilization for protein synthesis of leucine and valine compared with their keto analogues.

Fasting rats were given [3H]leucine plus [14C]2-ketoisocaproate or [3H]valine plus [14C]2-ketoisovalerate, plus 33 micromol of each compound. The ratio of 14C to 3H (R) in protein 6 h after injection of these isotopes is a measure of the extent to which extracellular keto acid, as compared with extracellular amino acid, serves as the source of the intracellular amino acid used for protein synthesis. R for 2-ketoisocaproate (KIC) vs leucine was 0.45 +/- 0.03 for whole body protein after oral injection and 0.83 +/- 0.02 after iv injection. R values for 2-ketoisovalerate (KIV) vs valine were similar. R, measured in the protein of various organs and in albumin, fibrin, and globin, varied more than twofold. We conclude that at least half of KIC and KIV given orally in this dosage is oxidized in splanchnic organs during first pass but that, nevertheless, these keto acids given orally serve as significant sources of the intracellular amino acids used for protein synthesis in most organs, particularly brain and heart.

Bromodeoxyuridine labeling studies on the proliferation of intestinal mucosal mast cells in normal and athymic rats.

The proliferation of mucosal-type mast cells (MMC) in rat small intestine was studied using a bromodeoxyuridine (BrdU)-labeling method. After 24-h cumulative injections of BrdU into adult SD rats, 3.5% of MMC were labeled, while only 0.3% and 0.1% of mast cells were labeled in back skin and ear respectively. From the results, it was concluded that MMC division occurred more than 10 times as frequently as the division of skin mast cells. Similar results were obtained in athymic adult rats (F344/N Jcl-rnu) in which the number of MMC was similar to that in heterologous animals. Thus, thymic factor(s) or T cells may not have an important role in MMC division in normal states. When SD rats were infected with Nippostrongylus brasiliensis, vigorous proliferation of MMC was brought about 13 to 15 days after infection. At that period, 40% of MMC were labeled by a single injection of BrdU, and 85% of MMC were labeled by 9-h cumulative injections of BrdU, with the result that most MMC rapidly proliferated in the intestinal mucosa during this period. Mitotic figures of MMC were sometimes observed. On the contrary, hyperplasia of MMC was not observed in athymic rats infected with nematodes. Therefore, MMC hyperplasia after nematode infection is dependent on thymic factor or T cells, and its mechanism is different from that of MMC division in normal states, in which thymic factor(s) or T cells are not essential.

Characterization of monoclonal antibodies against glucosyltransferase synthesizing water-insoluble glucan from Streptococcus sobrinus B13.

Four hybrid cell lines secreting monoclonal antibodies (McAbs) against glucosyltransferase synthesizing water-insoluble glucan (GTase-I) were generated by fusion of myeloma cells (P3U1) with splenocytes from mice immunized with GTase-I from Streptococcus sobrinus B13-N. Cell lines 29E7, 21GC7, and 42HB8 were found to secrete an IgG2a-type immunoglobulin, and 29EG, an IgM-type immunoglobulin. These two isotypes of McAbs were used for the determination of the binding sites on the GTase molecule, with use of an enzyme-linked immunosorbent assay. The binding site for McAb 29EG was different from the site that bound other McAbs. McAb 29EG was found to inhibit water-insoluble glucan synthesis by GTase-I by 50%, and 29E7 was found to inhibit it weakly. However, other McAbs did not show any inhibitory effect in spite of binding to GTase-I. McAb 42HB8 strongly inhibited GTase-I-mediated adherence of heat-killed cells of S. sobrinus B-13N to glass surfaces. When the McAbs were tested for their cross-reactivity among GTase preparations from different mutans streptococci, McAb 29EG reacted with S. cricetus and S. sobrinus, but not with other mutans streptococci. Three other McAbs, 21GC7, 29E7, and 42HB8, were found to react only with the enzyme from S. sobrinus. These findings indicated that the specificity of the McAbs studied varied with respect to the antigenic sites on the GTase-I molecule, and that some of the sites differed in their functions.

Cardiac magnetic resonance imaging in evaluation of anatomical structure and function of the ventricles.

Cardiac magnetic resonance imaging (MRI) is being widely employed for evaluation of cardiovascular anatomies and functions. However, the indications for cardiac MRI to obtain information which cannot be obtained using other conventional methods have not yet been determined. To demonstrate of usefulness of MRI in delineating the apex of the left ventricle and free wall of the right ventricle, end-diastolic short axis MRI images were obtained in 20 patients with apical hypertrophy and in 9 normal volunteers. To compare the accuracy of estimations of left ventricular volumes obtained using the modified Simpson's method of MRI with that using the MRI area length method, 19 patients, in whom left ventriculography had been performed, were studied. The apex of the left ventricle was evaluated circumferentially and distribution of hypertrophied muscles was defined. Sixty-five percent of the length of the right ventricular free wall was clearly delineated. Correlation coefficients of the ejection fraction between MRI and angiography were 0.85 with the modified Simpson's method of MRI, and 0.62 with the area length method of MRI. Three themes were chosen to demonstrate good clinical indications for cardiac MRI.

[Paradoxical left ventricular blood flow during the isovolumic relaxation period in non-obstructive hypertrophic cardiomyopathy: Doppler and M-mode echocardiographic study].

The left ventricular blood flow pattern during the isovolumic relaxation period was investigated in 17 patients with apical hypertrophy (mean 54 +/- 8 years) and 30 with asymmetric septal hypertrophy without left ventricular outflow obstruction (mean 54 +/- 14 years). Thirty-five age-matched normal subjects served as controls (mean 51 +/- 10 years). All cases were examined by pulsed Doppler echocardiography simultaneously with phonocardiography. In all the control subjects, an apically-directed flow was observed, which occurred simultaneously with the onset of the second heart sound. In contrast, a basally-directed flow was detected during the isovolumic relaxation period in 13 patients (76%) with apical hypertrophy and in 28 (93%) with asymmetric septal hypertrophy. In these cases, an apically-directed flow was often observed before the second heart sound. Standard M-mode echocardiography revealed a significant increase in the left ventricular short-axis dimension at the base during the isovolumic period in patients with hypertrophic cardiomyopathy, while the normal controls showed no significant change. Moreover, peak velocity of the basally-directed flow correlated significantly with the rapidity of dilatation of the basal dimension (r = 0.61, p less than 0.01). Thus, this basally-directed blood flow in the left ventricle during the isovolumic relaxation period may be caused by the asynchronous left ventricular dilatation during the isovolumic period.

Development of a new surgical procedure for repairing tracheobronchomalacia.

We have developed a new surgical method for repairing tracheobronchomalacia. In experiments on dogs we tried external fixation of Marlex mesh (Bard Cardiosurgery Division, Bellerica, Mass.) on the trachea. We first made models of tracheomalacia by making fractures or resections in intrathoracic tracheal cartilages and then made an external fixation of Marlex mesh on the malacic segments of the trachea. In 11 dogs Marlex mesh was sutured onto the trachea with absorbable thread. The trachea was firmly supported after 2 to 6 months, compared with three controls in which no external fixation was made. However, mucosal defects associated with ischemia caused by the suture developed in four of the 11. In 13 more dogs Marlex mesh was bonded to the trachea with fibrin glue. After 3 to 8 months the supporting strength of the trachea increased up to the level of the normal trachea. There was no evidence of inflammation or of mucosal defects. Therefore Marlex mesh was applied to a 44-year-old-man who had experienced frequent attacks of cough syncope. After the operation the attacks of cough syncope and collapsing of his airway disappeared completely.

[A case of giant cell tumor originating in the rib].

A rare case of giant cell tumor originating in the rib is reported. A 23-year-old female was admitted because of a mass in the right chest wall, right back pain and bloody pleural effusion. Exploration revealed tumor originating from the 2nd or 3rd rib and invading the intercostal muscles and the right lung. We completely resected the tumor with chest wall and performed right pneumonectomy. The defect of the chest wall was reconstructed using polyester mesh covered with a Gore-tex soft tissue patch. The pathological diagnosis was a malignant giant cell tumor of the rib. She is now doing well and has had no recurrence for 18 months since the operation.

[Flow cytometric analysis of cellular DNA content of lung cancer with reference to survival].

The cellular DNA content of lung cancer were measured by flow cytometry on 223 paraffin-embedded specimens prepared from resected lung carcinomas. According to the histological type of lung cancer, the mean values for the DNA Index were 1.41 in adenocarcinoma, 1.39 in squamous cell carcinoma, 1.33 in large cell carcinoma and 1.84 in small cell carcinoma. The DNA Index of small cell carcinoma was thus slightly higher than that of the other histological types, without statistically significant difference. Of 223 lung carcinoma cases, 131 (59.1%) were DNA aneuploidy and 92 (40.9%) were DNA diploidy. DNA aneuploidy was found in 56.1% of adenocarcinomas, 59.5% of squamous cell carcinomas, 53.3% of large cell carcinomas and 100% of small cell carcinomas. According to the staging of the lung cancer, the mean values of the DNA Index were 1.40 in stage I, 1.46 in stage II, 1.36 in stage IIIA, 1.48 in stage IIIB and 1.48 in stage IV. No statistically significant differences were found among these stages. DNA aneuploidy was found in 57.1% of stage I, 57.9% of stage II, 54.7% of stage IIIA, 60.0% of stage IIIB and 75.0% of stage IV. The correlation of DNA content with survival were investigated in 94 cases with stage I non-small cell carcinoma which underwent absolute curative resection. Of 94 cases, the 5-year survival rate of 40 cases with DNA diploidy was 81.1% and a mean survival time 111 months, while this one of the remaining 54 with DNA aneuploidy was 58.4% and a mean survival time 80 months.(ABSTRACT TRUNCATED AT 250 WORDS)

Pneumocystis carinii infection in corticosteroid-treated cats.

Corticosteroids were administered to produce Pneumocystis carinii infection in cats. Six of 10 cats, injected intramuscularly for 97-141 days with 2 mg/cat twice weekly of betamethasone sodium phosphate, developed a light infection with P. carinii. Six of 7 cats, injected intramuscularly for 11-168 days with 10-25 mg/cat weekly of prednisolone acetate, also developed a light infection with P. carinii. There was no significant difference in the infection rate between the sexes and ages of the cats. Using Giemsa staining and Gomori's methenamine silver nitrate stain, P. carinii organisms were indistinguishable morphologically from human and rat P. carinii. The cysts and trophozoites were usually present singly or in small groups, and they always were adhering to the periphery of alveoli. The inflammatory changes were inconspicuous except for the fact that alveolar macrophages often were seen. Corticosteroid-treated cats should be useful in the study of experimental P. carinii infection. This is the first reported case of experimentally induced P. carinii infection in cats.
