FIGO proposed new criteria for corpus cancer staging in 1988. One hundred and seventy-one cases of endometrial cancer were classified by these new staging criteria, and differences between grouping and survival rates for the new and old criteria were compared. 1) The number of cases grouped in old stages I, II, III and IV were 67, 86, 8 and 10, whereas those for new criteria 94, 25, 35 and 10, respectively. In 7 cases new staging was not possible because of preoperative radiation therapy and inoperability. 2) Cases of old stage I were reclassified as new stage I 54(80.5%), stage II 2(3.0%) and stage III 8(12.0%), and 3 cases were not classified. Those of old stage II were 39(45.2%), 21(24.55%), 25(29.1%) and 1(1.2%) and old stage III were 1(12.5%), 2(25.0%), 2(25.0%) and 3(37.5%), respectively. 3) Survival rates for the cases with both stage I and II cancer were very good without any apparent difference between them. Those for new stage III were very poor in comparison with old stage III. 4) In cases of stage I according to the new criteria, the depth of myometrial invasion correlated well with the survival rate. The prognosis of G3 cases with myometrial invasion over 1/2 was the poorest. 5) Survival rates among subclassifications of stages II and III were not significantly different from each other.

Endocrine cell types in 12 argentaffin and six argyrophil carcinoids and in nonneoplastic epithelia of the appendix vermiformis were investigated histochemically, immunohistochemically, and ultrastructurally. The nonneoplastic epithelia contained serotonin (Ser), peptide YY (PYY), glicentin (Gli), neurotensin (Neu), and somatostatin (So) cells in decreasing frequency. Out of 30 nonneoplastic Ser cells examined ultrastructurally, 28 cells were EC1 cells and two were non-EC cells. Eleven of 12 argentaffin carcinoids could be immunostained with anti-Ser serum and all of those 11 were composed almost totally of Ser cells. One of the 11 contained a small number of Neu cells. Ultrastructurally, 11 argentaffin carcinoids were composed predominantly of EC1 and/or ECn cells, and one was composed primarily of non-EC cells. Out of the six argyrophil carcinoids, five were argyrophil, non-argentaffin carcinoids; three consisted almost totally of PYY cells; one consisted of 60% PYY cells, 40% So cells and a few Gli cells; and one consisted of Ser cells alone. Ultrastructurally, the first four of those tumors were composed of D1 and/or L cells and the latter tumor was composed of ECn cells. The remaining one argyrophil carcinoid contained a few Ser-positive argentaffin cells and consisted almost totally of ECn cells which were found in both parts, with and without argentaffinity. It is concluded that the appendiceal carcinoids comprise two distinct groups on the basis of the main constituting cell type: Ser-positive, argentaffin carcinoids, composed of EC cells and peptide (especially of PYY)-positive, and Ser-negative, argyrophil non-argentaffin carcinoids of D1 and/or L cells.
A female hospital pharmacist and a nurse developed occupational asthma due to the inhalation of isonicotinic acid hydrazide (INH) powder. The mechanism of sensitization was examined in this study. Furthermore, IgE antibodies specific to INH and isonicotinic acid (INA), the major metabolite of INH, were examined by means of enzyme-linked immunosorbent assay (ELISA). IgE anti-INH antibody activity was detected by ELISA in sera from 5 out of 8 hospital workers who inhaled INH powder during the handling of INH, and in sera from 10 out of 142 tuberculosis patients receiving oral INH. IgE anti-INA antibody activity was detected in sera from 3 of the 8 hospital workers, and from 7 of the 142 tuberculosis patients. Neither IgE anti-INH nor anti-INA antibody activity was detected in sera from 45 patients with asthma or from 42 healthy volunteers. The specificity of the IgE antibodies and the cross-antigenicity between INH and INA were confirmed by ELISA inhibition studies. Our results indicate that sensitization to INH can occur not only by inhalation but also by oral administration, and that the asthmatic symptoms of the patients were caused by an IgE antibody specific to INH as a hapten.
The patient was a girl, 10 years 10 months of age, who had received the orthodontic treatment of chin cap and quad helix appliance for anterior and posterior crossbite. An opening click with pain in the left temporomandibular joint (TMJ) and trismus occurred in the period of treatment. The pumping manipulation and an anterior repositioning splint therapy were performed for closed lock. After the 4 months of wearing, the clinical symptoms disappeared and the upper second and lower first premolars were extracted for fixed appliance therapy. This case was finished and debanded after 2 years 10 months of the initial examination. At the end of active treatment, a maxillary removable retainer and a mandibular fixed canine to canine retainer were placed. The patient's dentition has remained essentially stable with a normal function of TMJ.
Fiberoptic bronchoscopy is a well established methods as a useful tool in the diagnosis of pulmonary tuberculosis with smear negative cases. In order to get the early and definite diagnosis of pulmonary tuberculosis, we performed transbronchial aspiration and bronchial lavage by a fiberoptic bronchoscope in 97 patients. All patients (1) were clinically suspected of having active tuberculosis; (2) showed abnormal chest roentgenogram suggesting tuberculosis; (3) showed negative sputum smears of acid-fast bacilli, or had no sputum. The results of the study were summarized as follows: 1) Final diagnosis of study subjects were 90 patients of active pulmonary tuberculosis, and 7 patients of pulmonary atypical mycobacteriosis. 2) Sputum culture of acid-fast bacilli was positive in 22 out of 90 patients with active pulmonary tuberculosis. 3) Smear and culture examination of acid-fast bacilli of transbronchial aspirates were positive in 9 and 28, respectively out of 90 patients. 4) Smear and culture examination of acid-fast bacilli of bronchial lavage were positive in 12 and 39, respectively out of 90 patients. 5) A rapid and definite diagnosis was made in 16 out of 90 patients by transbronchial aspirates or bronchial lavage. 6) Atypical mycobacteria were detected in 7 out of 97 patients by transbronchial aspirates or bronchial lavage. 7) There were no serious complications such as pneumonia and exacerbation of pulmonary tuberculosis. These results suggested that transbronchial aspiration and bronchial lavage were useful procedures for rapid and definite diagnosis of pulmonary tuberculosis.
We had clinicopathologically studied early cancer-like advanced gastric cancer in relation to peptic ulcer (UI). Early cancer-like advanced cancers with the difficulty to distinguish from early gastric cancer were selected for materials. Result were as followed: 1) Early cancer-like advanced cancers were consisted of 133 lesions, of which 128 lesions (96.2%) had peptic ulcer in cancerous lesion (73 lesions were active stage and 55 lesions were scarring stage). 2) Early cancer-like advanced cancers were 17.0% in all advanced gastric cancers. Proper muscle cancer (in which depth of cancerous invasion is up to proper muscle coat) was more common in early cancer-like advanced cancers than in Borrmann type advanced cancers. 3) Early cancer-like advanced cancers with peptic ulcer showed wide intramucosal cancerous infiltration. Thus, average of maximum diameter was 51.8 mm. 4) Those ulcer were commonly reaching to proper muscle coat or subserosa. Submucosal fibrosis was prominent and scattered proliferation of tumor cells were often seen within fibrosis. 5) Extent of cancerous infiltration in mucosa was more wide than that in submucosa. The above findings have led us to consider that early cancer-like advanced cancers have grown because of high degree submucosal fibrosis by the deep ulcerations due to "malignant cycle".
The growth type of early colorectal carcinoma was classified into two types. The first type is intramucosal polypoid growth (PG-Ca) and the second type nonpolypoid growth (NPG-Ca) which shows mainly massive infiltration of tumor cells below the submucosal layer. The incidence of adenoma-carcinoma sequence was 72 of 75 lesions (96.0%) in pedunculated polypoid carcinoma, and 61 of 71 lesions (85.9%) in sessile and broad-based polypoid carcinomas. Their average sizes were 15.0 and 18.7 mm, respectively. Submucosal invasive carcinoma (SM-Ca) showed a low incidence. They were detected as microscopical or scattered lesions with a few lymphatic and venous permeation. The NPG-Ca contained 32 lesions. Intramucosal carcinoma without adenoma showing slight depression consisted of ten lesions of which the average size was 5.1 mm. The other 22 lesions showed massive submucosal invasion with marked lymphatic and venous permeation. The average size was 10.3 mm being smaller than PG-Ca. Histologically, NPG-Ca was not accompanied with adenoma. The NPG-Ca arose from de novo carcinoma less than 10 mm in diameter and invaded into the submucosal layer. In advanced carcinoma, the PG-Ca showed a low incidence (21.8%), and almost all cases were of the NPG type (78.2%). The NPG advanced carcinomas increased in those over the size of 20 mm. It is concluded that nonpolypoid early colorectal carcinomas easily progress to advanced carcinoma, and de novo carcinoma occupied about 80% of colorectal carcinoma.
The histogenesis of Ewing's sarcoma (EW) and extraskeletal Ewing's sarcoma (EEW) is still disputable. Their relationship to the so-called Askin's tumor, neuroectodermal tumor of bone, and peripheral neuroblastoma remains to be established. In an attempt to clarify these points, immunocytochemical and ultrastructural studies were done on tissues from 14 cases of EW, 4 cases of EEW, and 9 cases of primitive neuroectodermal tumor (PNET) and compared with neuroblastoma and olfactory neuroblastoma. Six tumors categorized initially as EW and EEW on biopsy, turned out to be PNET by extensive histologic and/or ultrastructural observations. Abundant glycogen was recognized not only in 16 of 18 cases of EW and EEW, but also in seven of nine cases of PNET. Fine fibrillar cell processes were seen between tumor cells, at least in limited areas even in cases of EW and EEW. Immunocytochemically, neuron-specific enolase (NSE), neuroblastoma cell surface antigen (NBCA), neuron cell surface antigen (NCSA), and neurofilament (NF) were demonstrated not only in neuroblastoma, but also frequently in cases of EW, EEW, and PNET. The results seem to suggest that EW and EEW represent the most immature forms of neuroectodermal tumor. Electron microscopic study showed predominantly primitive cells with occasional areas of cell processes, neurosecretory granules, and microtubules, suggesting a neuroectodermal origin.
The many different theories on the histogenesis of alveolar soft part sarcoma (ASPS) have caused great confusion. Owing to the recent rapid advance in immunohistochemical studies, two major hypotheses have been proposed. One group of researchers supports the idea that ASPS shows myogenic differentiation, while the other group opposes the idea. This confrontation is essentially one between a group that believes in the immunohistochemically demonstrated presence of desmin in ASPS and a group that denies it. In the present study we detected desmin in 6 of 10 formalin-fixed paraffin sections (although there were differences due to the use of five commercially available types of anti-desmin antibodies). When acetone-fixed paraffin sections and periodate-lysin-paraformaldehyde (PLP)-fixed frozen sections were used in one and three cases, respectively, they were found to be desmin positive, regardless of the type of antibody. The consistent positivity for all anti-desmin antibodies in the cases treated with acetone or PLP is very suggestive of a myogenous origin of ASPS. It is important to take into consideration the fact that formalin-fixed paraffin sections are not very suitable for immunohistochemical study of desmin.
Carcinoid tumor of the bile duct is extremely rare. Ten cases have been reported in the literature. This report describes the eleventh case. A 55-year-old woman was hospitalized with biliary stenosis. Sonography (US) and computed tomography (CT) demonstrated a tumor in the upper common hepatic duct (CHD). Percutaneous transhepatic cholangiography showed extraluminal growth of the tumor. The tumor was resected and histologic examination showed carcinoid tumor of the common hepatic duct.

An autopsy case report of leiomyosarcoma of the thoracic aorta is presented. The patient was a 75-year-old Japanese woman who complained of hypertension, melena and lumbago. The autopsy revealed severe occlusion of the thoracic aorta due to the tumor. Histologically the tumor consisted of fascicular proliferation of atypical fusiform cells. Metastases were seen in the bilateral adrenal glands, pancreatic tail and bones (vertebrae and femur). At the distal portion of the aortic tumor, there was a secondary dissecting aneurysm.
The tongue and lingual papillae of the Japanese Insectivora, the Shinto shrew (Sorex caecuiens saevus), the long-clawed shrew (S. unguiculatus), the dsinezumi shrew (Crocidura dsinezumi dsinezumi) and the Japanese water shrew (Chimarrogale himalyica platycephala), were observed by scanning electron microscope. The tongue of these animals had two vallate papillae. In two species of the Sorex a papilla in the vallate papilla was surrounded by two separated trenches, but in the other species it was surrounded by only a continuous trench and a clear vallum. The fungiform papillae in the Sorex were less developed than those of the other species. In the Sorex and Crocidura, there was no filiform papilla on the lingual apex. These genera, however, have papillary projections in the margin of the lingual apex. The results of this investigation suggest that the Sorex and Crocidura indicate an ancient form of the mammalian tongue. These characters, furthermore, were compared among seven species in six genera added three species observed by Kobayashi et al. (1983) to this study.
In this study I examined the macroscopic, radiological and histopathological changes on the contralateral temporomandibular joint following condylectomy on one of the bilateral temporomandibular joints on adult female Macaca fuscatas. I made a standardized radiographic apparatus for the temporomandibular joint and head for the Macaca fuscata on an experimental basis for radiological observation through the whole passage of time before making the experiment. The results were as follows: 1) In macroscopic observation, I noticed the entire mandible directed toward the operated side at opening the mandible and the anterior and molar (unoperated side) open bite after the operation. Besides the masticatory side altered and the degree of tooth attrition made progress with the passage of time. 2) According to radiographic investigation with an anteroposterior radiograph, complicated shift and rotation of the mandible were observed since the operation. According to the radiograph of condyle, distal displacement of the temporomandibular joint, widening of joint space and clockwise rotation of the mandible were confirmed. A shadow of bone was found in the articular surface of the mandibular head on the 123rd day. 3) The following findings were obtained in histopathology. (1) Heterotopic calcification appeared in an articular disk of one Macaca fuscata in control group and two in the 83rd-day-passaged Macaca fuscatas. (2) In the articular disk and the tissue around them, a tiny projection of connective tissue appeared in the posterior synovial portion of the superior articular cavity on the 10th day. And the intermediate zone of the articular disk showed itself to be fibrous small bundle daily as days passed on and cartilaginoid cells were found. (3) In the front edge of mandibular corpus and attachment region of the lateral pterygoid muscle, absorption of bone was observed by the 40th day from the operation, and additions of bone were observed during the period between the 40th and 83rd days from the operation. (4) The thickness of the cartilaginous lamina of mandibular head and fibrous velum varied much in different periods and areas. In the 83rd-day-passaged sample, osteosclerotic change and the irregularity on the articular surface were remarkably noticed. On the 123rd day the back of the mandibular head and the contralateral front wall of the back of the articular process showed absorption of the bone on a large part. (5) A large part of the articular surface in articular tubercle was irregular and some osteosclerotic change was observed. And on the 83rd day a good number of clones of cartilage cells were found.
The responsiveness and action mechanisms of steroid hormones and epidermal growth factor on human endometrial carcinoma cells are analyzed by using in vitro culture system. 1) The Ishikawa cells, derived from a well differentiated endometrial adenocarcinoma and possess ER and PR, are shown to respond to estrogens by increasing a variety of parameters, viz cell proliferation, PR levels, ALP and DNA polymerase activities. 2) ER and PR of those cells are localized in the nuclei by immunocytochemical staining using the monoclonal antibodies against to ER and PR, confirming the correctness of Gorski and Greene's one step theory involving the action mechanisms of steroid hormones. 3) Progestins reduced the ER level and stimulate E2DH activities and glycogen content, which are completely abolished by anti-progestin (RU486), suggesting that PR of those cells should be functional. 4) These responses to steroid hormones of Ishikawa cells are synergistically enhanced or appeared earlier by addition of EGF. 5) The main metabolite of E2 incubated with Ishikawa cells is E2-3-sulfate instead of E1, indicate that the higher estrogenic status may be persisted in endometrial cancer tissues.
