Effect of zonisamide (CI-912) on a synaptic system model.

The effect of the experimental antiepileptic drug zonisamide (1,2-benzisoxazole-3-methanesulfonamide, ZNS) on the trigeminal complex of cats was compared with the effect of established antiepileptic drugs. Intravenous administration of 10-40 mg/kg ZNS significantly depresses descending excitatory mechanisms, as well as segmental and descending inhibitory mechanisms, but has only a minor effect on segmental excitatory mechanisms. This spectrum of activity is similar to that of valproate, and suggests that ZNS should also be a broad-spectrum antiepileptic drug. In agreement with our experimental observations, it has been found that ZNS is effective against complex partial, generalized tonic clonic, and myoclonic seizures. The antiepileptic profile of ZNS in conventional screening tests resembles that of carbamazepine (CBZ) and phenytoin. However, CBZ exacerbates rather than prevents myoclonic seizures. Our experimental model thus provides a more accurate prediction of ZNS's clinical spectrum of activity. The relationship of these findings to the mechanism of action of antiepileptic drugs is discussed.

Inhibitory effects of calcium channel antagonists on motor dysfunction induced by intracerebroventricular administration of paraquat.

This study reports the effects of Ca2+ channel blockers (Ca antagonists) on intraneuronal Ca2+ ([Ca2+]i) movements and on the disturbance of rotarod performance produced in rats by intracerebroventricular administration of paraquat. Paraquat (50 nmol) produced a decrement in rotarod performance which was present at 30 min. and maximal at 60 min. and was not associated with overt behavioural changes; larger doses of paraquat (100-400 nmol intracerebroventricularly) produced paresis and convulsions which severely disrupted rotarod behaviour. The disruption of rotarod performance after paraquat (50 nmol intracerebroventricularly) was significantly reduced by giving Ca antagonists (flunarizine, verapamil and nicardipine) not only intraperitoneally 15 min. after paraquat but also intracerebroventricularly immediately before paraquat. The order of pharmacological potency was flunarizine > or = verapamil > nicardipine. In contrast, intracerebroventricular administration of Bay K 8644, a Ca agonist, enhanced the disruption of rotarod performance caused by paraquat (50 nmol). In in vitro studies, paraquat markedly potentiated the rapid increase in [Ca2+]i levels evoked by 50 mM KCl in rat brain synaptosomal fraction, although paraquat alone produced a small prolonged rise in [Ca2+]i levels which had a slow onset. The above results suggest that paraquat induced neurotoxicity is associated with increased [Ca2+]i levels in brain neuronal cells, and that paraquat might effect on membrane activity instability.

Changes in antioxidative mechanisms in elderly patients with non-insulin-dependent diabetes mellitus. Investigation of the redox dynamics of alpha-tocopherol in erythrocyte membranes.

BACKGROUND: Recently, it has been suggested that the onset and aggravation of diabetes are closely related to free radicals. Also, vitamin E is a lipophilic free radical scavenger that is localized mainly in biomembranes. OBJECTIVE: The purpose of this study was to clarify the defensive mechanisms against oxidative stress by investigating the differences in the redox dynamics of alpha-tocopherol in plasma and erythrocyte membranes between elderly patients with non-insulin-dependent diabetes mellitus (NIDDM) and healthy elderly subjects. METHODS: Total, alpha-, beta-, gamma- and delta-tocopherol and alpha-tocopherolquinone concentrations were determined by high-performance liquid chromatography with a redox detection mode using a series of four coulometric working electrodes. RESULTS: The alpha-tocopherolquinone/alpha-tocopherol ratio in plasma and erythrocyte membranes was not different between the two groups. Both the alpha-tocopherol concentrations in erythrocyte membranes and ratio of alpha-tocopherol in erythrocyte membranes to alpha- tocopherol in plasma was significantly lower in elderly NIDDM patients than in healthy subjects. CONCLUSION: These findings suggest that alpha-tocopherol is used normally in both plasma and erythrocyte membranes and alpha-tocopherol uptake in erythrocyte membranes is significantly decreased in elderly NIDDM patients. The functional disorder of the antioxidative activity of alpha-tocopherol in erythrocyte membranes due to impairment of this transfer mechanism may be associated with the pathogenesis of NIDDM.

A case of hybrid phenotypic chronic myelomonocytic leukemia transformed from aplastic anemia.

A case of hybrid phenotypic chronic myelomonocytic leukemia (CMMoL) transformed from a hematological remission state of severe aplastic anemia is reported. A 63-year-old woman was admitted to our hospital with complaints of easy fatigability and dizziness in December 1978. A diagnosis of aplastic anemia was made from findings of pancytopenia and hypoplastic marrow without dysplasia, and she was treated successfully with mepitiostane. Seven years later, anemia and thrombocytopenia reappeared with monocytosis. On the second admission, the peripheral blood showed a white blood cells count of 3.5 X 10(3)/microliters with 54% monocytes. The bone marrow was hypercellular with monocytosis, and dysplasia of three cell lineages was noted. A diagnosis of CMMoL was made. Cytogenetic analysis of bone marrow cells revealed a karyotype of 45 XX, -7, 11q-, and surface markers were polyphenotypic, including CD20+, CD14+ and CD13+. This is the first report of CMMoL with biphenotype which was transformed from aplastic anemia.

Milk of calcium renal stone in a patient with acute promyelocytic leukemia.

A case of milk of calcium renal stone is reported. This is a rare disease in which a suspension of calcium salts is formed within a renal cyst. The pathognomonic sign is a fluid level seen in a standing position and an oval density seen in a supine position. In the present case, the milk of calcium was found to develop in a hydronephrotic kidney during the course of acute promyelocytic leukemia and this condition was suggested to be a complication of infection.

Danazol treatment for paroxysmal nocturnal hemoglobinuria.

Danazol was administered to two patients with paroxysmal nocturnal hemoglobinuria (PNH) with a dramatic effect on the hematological findings. The patients, 31- and 41-year-old females, were initially diagnosed as having aplastic anemia, and were initially treated with anabolic steroid and immunosuppressive therapy, respectively. Sugar water and Ham tests turned positive at the start of danazol therapy in the former patient and after two months in the latter patient. This drug produced a dramatic improvement in the hemoglobin level and the platelet count and showed few side effects in the patients. A possible mechanism of action of danazol for PNH is briefly discussed.

Micronucleus tests on N-ethyl-N-nitrosourea with mouse peripheral blood reticulocytes using acridine orange-coated slides.

The micronucleus test with peripheral blood using acridine orange-coated slides was validated in male CD-1 mice treated once with N-ethyl-N-nitrosourea (ENU) at doses of 6.25, 12.5, 25.0, and 50.0 mg/kg body weight. Peripheral blood preparations were made 0, 24, 48, and 72 h after treatment. The frequencies of micronucleated peripheral reticulocytes in the ENU-treated groups increased dose-dependently, peaking at 48 h after treatment. The results indicate that the method used in the present study can be an alternative to the method using bone marrow polychromatic erythrocytes.

Improvement of pancreaticojejunostomy in pancreatoduodenectomy.

We compared two methods of pancreaticojejunostomy in 36 patients undergoing pancreatoduodenectomy. Pancreatoduodenectomy was performed in 17 men and 19 women aged 35-81 years, including 20 with pancreatic head cancer and 11 with papilla of Vater cancer. Group A (n = 19) underwent pancreaticojejunostomy with a stent tube that was exteriorized via the jejunum. In group B (n = 17), the stent tube ran transhepatically from the pancreatic duct before being exteriorized. The operating time, volume of pancreatic secretions, and time until stent removal were not significantly different between the two groups. However, there were 3 deaths related to early anastomotic leakage and 1 pancreatic fistula in group A, while group B had only 2 spontaneously healing pancreatic fistulas. Our results suggested that the pancreaticojejunostomy method used for group B is superior due to its simplicity, shorter operating time, and better outcome.

Disseminated intravascular coagulation in transfusion-associated graft-versus-host disease. Report of two cases.

In two cases of transfusion-associated graft-versus-host disease (TA-GVHD), severe bleeding tendencies were observed besides the characteristic features of the disease. An elevation of fibrin degradation products (FDP), D-dimer, thrombin-antithrombin III complex (TAT) and plasmin-alpha 2 plasmin inhibitor complex (PIC), was revealed. These findings were compatible with disseminated intravascular coagulation (DIC). To our knowledge, the association of TA-GVHD and DIC has not been described, and it is our speculation that allogeneic immune reactions may play some pathogenic role in developing DIC in TA-GVHD. The DIC may be responsible for the rapid progression of multiple organ failure (MOF) in TA-GVHD.

Neural responses of the glossopharyngeal nerve to several bitter stimuli in mice.

The neural reaction to bitter stimuli was tested in the glossopharyngeal (GL) and chorda tympani (CT) nerves of mice. Relative responses to several bitter stimuli (quinine, iso-alpha-acids, L-tryptophan and L-phenylalanine) were greater in the GL nerve than in the CT nerve. Strain variability for taste sensitivity to bitter stimuli existed between two strains (C57BL and BALB/C). The GL nerve response to caffeine, iso-alpha-acids and L-phenylalanine in the BALB/C strain was greater than the response in the C57BL strain.

Secretion of an anti-inflammatory, immunomodulatory factor by Schistosomulae of Schistosoma mansoni.

Penetration and migration of schistosomulae of Schistosoma mansoni through the skin of mice is associated with reduced inflammatory responses especially after a moderate infection. Previous studies to identify the mechanisms by which schistosomulae of S. mansoni suppress inflammatory responses in human skin showed that the excretory/secretory (ES) products of schistosomulae of s. mansoni contain activities that induce production of the antiinflammatory cytokine IL-1ra from human keratinocytes. In the present study we have characterized the ES products of the schistosomulae of S. mansoni to identify the IL-1ra inducing activity. We demonstrate that this anti-inflammatory activity is associated with a protein of molecular mass 16.8 kDa (Sm 16.8). Depletion studies confirm that Sm 16.8 is the major IL-1ra inducing activity in the ES products. A comparison of the proteins in the ES products of schistosomulae of S. mansoni with those of Trichobilharzia ocellata, a bird schistosome that induces an acute inflammation in human skin, shows that Sm 16.8 is absent in the ES products of T. ocellata. Interestingly, ES products of the schistosomulae of T. ocellata were potent inducers of IL-1 alpha from human keratinocytes, whereas ES products from schistosomulae of s. mansoni induce little or no IL-1 alpha secretion from keratinocytes. Functional studies show that Sm 16.8 suppresses antigen induced lymphoproliferative responses in vitro. Addition of Sm 16.8 to spleen or axillary lymph node cell cultures resulted in a significant reduction in antigen induced IL-2 secretion. These studies show that schistosomulae of S. mansoni elaborate an anti-inflammatory, immunomodulatory factor that may help the parasite to evade host immune responses in the skin. Given the capabilities of Sm 16.8 to induce IL-ira and suppress lymphoproliferation, this protein may also have a potential use as a therapeutic agent for inflammatory skin disorders.
