Effects of ryo-kan-kyomi-sin-ge-nin-to extract on degranulation of and histamine release from rat mast cells.

The effects of Ryo-kan-kyomi-sin-ge-nin-to (RKSG)) extract, a medicinal agent traditionally used in China and Japan for treatment of asthma, on the degranulation of and histamine release from rat mast cells were studied. At a concentration of 5 mg/ml RKSG, degranulation of mast cells stimulated either by antigen (DNP-Ascaris) or compound 48/80 was markedly suppressed. At a concentration of 1-5 mg/ml RKSG, histamine release from mast cells due to application of either antigen or compound 48/80 was inhibited in a dose-dependent fashion. These results suggest that RKSG may be useful for the treatment of type I allergy-related diseases.

Endometrial cysts: diagnosis with MR imaging.

The value of 1.5-T magnetic resonance (MR) imaging in diagnosing endometrial cysts and differentiating them from other gynecologic masses was prospectively evaluated in 374 female patients with clinically suspected adnexal masses. A suggestive diagnosis of endometrial cyst was made when a cyst that was hyperintense on T1-weighted images exhibited homogeneous hyperintensity on T2-weighted images. A definitive diagnosis was made when a cyst that was hyperintense on T1-weighted images exhibited hypointense signal on T2-weighted images (shading) or when the lesion consisted of multiple hyperintense cysts on T1-weighted images (multiplicity) regardless of the signal intensity on T2-weighted images. Surgery was performed in 293 patients, and confirmation was obtained in 354 lesions. MR imaging enabled accurate diagnosis of 77 of 86 endometrial cysts and exclusion of the diagnosis of endometrial cyst in 263 of 268 other gynecologic masses with or without internal hemorrhage. The overall diagnostic sensitivity, specificity, and accuracy were 90%, 98%, 96%, respectively. MR imaging seems to be an acceptable diagnostic test on which clinical decisions can be based in selecting treatment.

Positive inotropic action of veratridine in rat atria: possible involvement of prostanoids.

Veratridine caused a positive inotropic action in the electrically driven left atria of rats. Quinacrine (a phospholipase A2 inhibitor), indomethacin (a cyclooxygenase inhibitor) and aspirin (a cyclooxygenase inhibitor), but not nordihydroguaiaretic acid (a lipoxygenase inhibitor), inhibited the response to veratridine. Verapamil and nifedipine also inhibited the response to veratridine. The positive inotropic effect of arachidonic acid was abolished by aspirin and indomethacin. However, the positive inotropic effect of PGF2 alpha was not affected by indomethacin, quinacrine or aspirin. PGE2, but not STA2 and PGI2, also caused the positive inotropic effect. However, the negative inotropic effect was observed in the presence of PGE1 and PGD2. Veratridine shifted the concentration-response curve of Ca2+ to the left in a Ca(2+)-free medium. Indomethacin only inhibited the veratridine-induced potentiation of Ca2+ responses. Veratridine increased the level of PGF2 alpha in the left atria and this action was completely inhibited by indomethacin, aspirin and quinacrine. Veratridine also increased the level of PGE2. These results imply that the positive inotropic action of veratridine is partly due to stimulation of the release of arachidonic acid leading to the increase in prostaglandins in rat atria.

Hyperimmunoglobulinemia D following cancer chemotherapy.

Five classes of serum immunoglobulin levels were investigated in 107 children with malignant or benign tumors. Hyperimmunoglobulinemia D (hyper-IgD) was observed in 31 of 82 children who were in complete remission off chemotherapy with a median follow-up of 4.5 years after cessation of chemotherapy. On the other hand, hyper-IgD was not found among 12 children with malignant or benign tumors treated with chemotherapy and a low incidence of hyper-IgD was observed during chemotherapy (1 of 13 cases). The result indicates that hyper-IgD is not uncommon in children off chemotherapy, suggesting that dysregulation of IgD synthesis persists long after cessation of antineoplastic drugs.

Effect of altered activation sequence on epicardial QRST area and refractory period in dogs.

BACKGROUND: We investigated the effects of activation sequence on cardiac surface QRST areas and refractory periods in experiments on dogs. METHODS AND RESULTS: Right and left ventricular pacings were performed, and the pacing site was altered every 6 minutes. After 4 minutes of a given pacing, 54 unipolar electrograms distributed over the entire cardiac surface were recorded. Next, refractory periods at electrode sites near pacing electrodes were measured. Paired right ventricular/left ventricular (RV/LV) pacing data were obtained six or seven times in each sample. Although the QRST isoarea maps during the two activation orders were qualitatively similar, it was recognized consistently from the right ventricle-left ventricle difference map that leads around the RV free wall had positive values and that leads around the LV free wall and apex had negative values. Compared with the same leads at RV and LV pacing, QRST areas were larger when pacing sites were near the leads. The local QRST areas of individual leads at which we measured local refractory period were consistently larger during drive from proximal pacing sites than during drive from distant pacing sites. Refractory periods were consistently longer during proximal pacing than during distal pacing, and there was a positive correlation between change in local QRST area and change in refractory period (r = 0.64) during altered activation sequence, whereas there was an inverse correlation between change in QRST area and change in refractory period (r = -0.91) during localized myocardial warming. CONCLUSIONS: Both local QRST areas and local refractory periods were dependent on the activation sequence, and there was a positive correlation between QRST areas and refractory periods during various activation sequences compared with localized myocardial warming.

Signal-averaged body surface mapping for the assessment of low-amplitude potentials. Relation between ventricular depolarization and repolarization in normal subjects.

To examine the relation between ventricular depolarization and repolarization, body surface isopotential maps at the end of the QRS complex were studied in 32 normal subjects using a signal-averaged body surface mapping system. The number of beats averaged was 96-154 (mean 126.2). In this study, there were 8 types of isopotential map patterns at the end of the QRS complex. Mean +/- SD of QRS duration, appearance time of repolarization, and disappearance time of depolarization were 82.0 +/- 8.7 msec, 71.8 +/- 10.5 msec, and 79.7 +/- 9.4 msec, respectively. Time duration of overlapping depolarization and repolarization was 8.6 +/- 6.4 msec. The early repolarization was widely distributed on the left anterior chest and the upper sternal region. These results demonstrated the difference between the appearance time of repolarization and the disappearance time of depolarization for each lead. We concluded that it is difficult to evaluate ECG waves in the terminal portion of the QRS complex with the dipolar theory only.

Relation between the incidence of arrhythmias and ischemic ST-segment depression during dipyridamole electrocardiography test in patients with coronary artery disease.

To examine the incidence of arrhythmias in dipyridamole infusion and the relation between dipyridamole-induced arrhythmias and ST-segment depression, dipyridamole electrocardiography tests were performed on 100 patients with coronary artery disease. Dipyridamole was infused at a rate of 0.568 mg/kg for 4 min, and 87-lead body surface mapping was performed to determine ischemic ST-segment depression. Positive ischemic response was defined as greater than or equal to 0.10 mV horizontal or downsloping ST-segment depression below the baseline, lasting 80 msec after the J point. Arrhythmias were observed by continuous electrocardiographic monitoring using a CM-5 lead electrocardiography. With respect to ventricular premature contractions (VPC), a group of patients with previous myocardial infarction (MI group) had a significantly higher incidence than a group of patients without previous myocardial infarction (non-MI group) before (16.7% vs. 1.7%, p less than 0.01) and after (38.1% vs. 3.4%, p less than 0.005) the dipyridamole infusion. The incidence of supraventricular premature contractions (SVPC), however, was not significantly different between the MI and non-MI groups. A group of patients with positive ischemic response had a significantly higher incidence of SVPC after the dipyridamole infusion than a group of patients with negative ischemic response (p less than 0.005). However, there was no significant difference in the incidence of VPC between the negative and positive ischemic response groups. These results suggest that dipyridamole-induced VPC is not always associated with ischemic ST-segment depression, but dipyridamole-induced SVPC is associated with dipyridamole-induced ischemic ST-segment depression in patients with coronary artery disease.

[Hemispheric differences in visual recognition of two Kanji characters].

The purpose of the present study was to examine hemispheric differences in visual recognition of two kanji letters. Kanji phrases and nonsense kanji letters were presented unilaterally to the left or right visual hemifield in thirty normal right-handed men by a tachistoscope. The experiments were separated into three conditions; the mixing tasks (ten phrases and ten sets of nonsense letters are presented by mixture), the phase task (twenty phrases are presented), and the nonsense letters task (twenty sets of nonsense letters are presented). Under all conditions, significant right visual hemifield superiorities for the accuracy of recognition of phrase and nonsense letters were observed.

Actual characteristics of the glands distributed in the lamina propria mucosae of the fowl esophagus.

We performed histological and histochemical investigations on the glands distributed in the lamina propria mucosae of the fowl esophagus and demonstrated their actual characteristics. 1. Glandular cells of the compound tubular glands located in the lamina propria mucosae contained a number of fine pepsinogen granules. 2. Reactions to neutral, weak and strong acid mucopolysaccharides, neutral mucus type II and III and sialomucin were evidently positive in these cells. 3. Based on the facts in 1) and 2), we consider that the glands located in the fowl esophagus are undifferentiated gastric glands. 4. The same glands possessed no parietal cells. 5. We demonstrated that the esophageal cardiac glands in the lamina propria mucosae of human esophagus were undifferentiated gastric glands, and they possessed parietal cells. These glands were confirmed in humans alone among the mammalia. 6. The significance of the existence of the same kind of gland in human and fowl esophagus is extremely important. 7. PAS-positive substance in the above-mentioned glands in the fowl esophagus contains sulfuric, acid, neutral mucopolysaccaride and neutral mucus of type II and III but no glycogen. The compound tubular glands distributed in the lamina propria mucosae of the fowl esophagus have been described as mucous glands. We performed histochemical investigation and demonstrated that these glands were undifferentiated gastric glands.

Proventricular glands in fowl.

Some researchers have already described the fowl proventriculus. However, we believed there was a need for detailed carbohydrate histochemical investigations on the same glands. Moreover, some researchers had erred about the lamina muscularis mucosae. The results of these investigations are as follows. 1. The proventricular glands consist of both superficial and profound gastric glands. 2. The superficial glands are distributed in the lamina propria mucosae while the profound glands exist in the tela submucosa. 3. The superficial glands are simple, branched tubular glands. The columnar glandular cells are arranged in a simple layer and react strongly to PAS, AB (pH 2.5 and 0.5). These appear to be dark purple when they are stained with PAS-AB (pH 2.5). Some other methods have also been tried. 4. Judging from the data 3), the superficial gastric glands contain neutral, weak and strong acids, sulfuric and acid mucopolysaccharides, sialomucin, and II and III neutral mucus type. 5. Glandular cells in the body and basal portions of the superficial gastric glands contain a large number of fine pepsinogen granules. 6. Judging from the data of 3)-5), we believe that the superficial gastric glands are undifferentiated gastric glands and that they are same kinds of glands that are found in mammals. 7. A large number of profound gastric glands fill the tela submucosa. They are compound tubular glands, and are composed of many glandular alveoli. Their columnar glandular cells are arranged in a simple layer. 8. These glandular cells react moderately to PAS, negatively to AB (pH 2.5 and 0.5) and PAS-AB (pH 2.5). Moreover, we observed some other reactions.(ABSTRACT TRUNCATED AT 250 WORDS)

Pepsinogen granules in the esophageal epithelium of the rock snake.

We demonstrated a large number of pepsinogen granules in the esophageal pseudostratified ciliated epithelium of the rock snake. 1. The snake esophagus is covered with a pseudostratified ciliated epithelium. 2. This epithelium develop mostly in the upper portion of the esophagus. 3. Long supranuclear portions of the same cells are strongly PAS-positive and contain pepsinogen granules. Those cells possess cilia. 4. The strongly PAS-positive cells and pepsinogen granules decrease or are lost in the middle and lower portions of the esophagus. 5. Glands are distributed in the lamina propria mucosae of the esophagus of the Japanese lizard and gecko. Those in humans and the bird are compound tubular glands and those of the Japanese lizard and gecko are bottle-shaped. The pepsinogen granules of these glands are secreted into the excretory ducts and then discharged into the esophageal lumen. 6. However, the same granules of the snake are contained in the supranuclear portion of the epithelium and are secreted directly into the esophageal lumen. 7. The mode of pepsinogen granule secretion of the esophagus is most simple in the snake.

Glands distributed in the lamina propria mucosae of the esophagus in the gecko and Japanese lizard.

In an earlier study, we found compound tubular glands distributed in the lamina propria mucosae of human and fowl esophagus. Subsequently, we discovered bottle-shaped glands in the Japanese lizard and gecko esophagus in the same lamina as that of the human and fowl. Moreover those glands produced equivalent pepsinogen granules. We provide below, a detailed description on the results. 1. Bottle-shaped glands were distributed in the lamina propria mucosae of the Japanese lizard and gecko esophagus. 2. A large number of those glands were distributed in the lower region of the esophagus, but did not exist in the upper and middle regions of the esophagus. 3. The esophageal mucous membrane of the gecko and Japanese lizard were covered with a simple columnar ciliated epithelium, and the same epithelium reacted strongly to PAS and AB (pH 2.5), moderately to AB (pH 0.5) or negatively. 4. PAS-AB (pH 2.5) stain presented a dark blue color or a deep red color or a deep red and dark blue mixed color in one section. 5. The above-mentioned glands contained pepsinogen granules. 6. Those glands do not possess parietal cells.
