An appliance for management of TMJ pain as a complication of Parkinson's disease.

This is a case report of a 71-year-old female with pain in the right TMJ, who was also suffering from Parkinson's Disease and was treated using a new intraoral occlusal appliance. The new appliance was designed to limit excessive mandibular excursion of the right side by restricting the mediotrusive movement of the left coronoid process. The appliance significantly suppressed involuntary mandibular excursion.

Basaloid-squamous cell carcinoma of the oral mucosa: report of two cases and study of the proliferative activity.

Two cases of basaloid-squamous cell carcinoma (BSC) of the oral mucosa are described. The first case occurred at the floor of the mouth in a 58-year-old man, and the second case occurred at the mandibular gingiva in a 79-year-old woman. The laboratory data of the first case showed a positive response to hepatitis C virus antibody. In the first case, the tumor mass measured 4 x 4 cm in size, and was located at the lingual side of the median mandible beside the sublingual gland. In the second case, the tumor mass measured 25 x 15 mm in size, and was located in the alveolar mucosa of the right mandible. Histologically, both tumors showed a neoplastic epithelium arranged in a solid pattern with evidence of peripheral palisading, central necrosis, and some squamous differentiation. The proliferative activities of the BSC were compared with conventional squamous cell carcinomas (SCC) in the oral floor and gingiva, respectively, by employing a sensitive argyrophilic nuclear organizer region (AgNOR) staining method. The number of AgNOR per nucleus of the BSC was higher than that of any other SCC cases. The results support the opinion that BSC of the oral mucosa has a worse prognosis than conventional SCC.

Persistent vesicourethral dysfunction following radical surgery for rectal carcinoma: urodynamic features and potential abatement with modified sphincterotomy (radical transurethral resection of the prostate).

BACKGROUND: Vesicourethral function returns after radical rectal surgery during the first year but rarely progresses after 1 year. We examined the urodynamics of patients whose voiding dysfunction remained after 1 year, and treated several with a modified sphincterotomy procedure similar to radical transurethral resection of the prostate. METHODS: We analyzed the urodynamic features of vesicourethral dysfunction in 16 male patients with persistent voiding dysfunction for more than 1 year following radical surgery for rectal carcinoma. Seven patients elected to undergo radical transurethral resection of prostate (radical TUR-P) for the relief of their persistent voiding dysfunction. RESULTS: The mean bladder volume at the first desire to void was 210 mL, the mean maximal bladder capacity was 343 mL, and the mean vesical compliance (Cves) was 27.1 mL/cm H2O. All patients demonstrated either vesical denervation supersensitivity (Vds) or uninhibited contraction. The mean maximal urethral closure pressure was 43.9 cm H2O, and urethral denervation supersensitivity was found in 77.8% (7/9), and sphincter dyssynergia in 66.7% (6/9) of patients. After radical TUR-P, 5 patients became free from the use of self-catheterization, 1 patient had a reduced residual urine rate, and 1 patient was unchanged, but no patient noted a change in urinary control. CONCLUSION: Urethral dysfunction after radical rectal surgery was caused by failure of the bladder to empty along with an underactive detrusor. Radical TUR-P was effective in restoring voiding function in a selected group of these patients.

Portal vein anomaly associated with deviation of the ligamentum teres to the right and malposition of the gallbladder.

PURPOSE: To develop a complete clinical picture of a portal vein anomaly associated with a rightward-deviated ligamentum teres. MATERIALS AND METHODS: Three hundred twenty-seven consecutive living donor candidates for partial liver transplantation were evaluated for hepatic vascular abnormalities with computed tomography supplemented with intraoperative examinations. Special attention was given to the branching pattern of the portal system and the relative positions of the umbilical portion of the portal vein, the ligamentum teres, and the gallbladder. RESULTS: Four donor candidates (1.2%) had a portal vein anomaly associated with a rightward-deviated ligamentum teres. In all four, the umbilical portion of the portal vein (and hence the ligamentum teres) was abnormal, lying above the gallbladder bed and giving off major branches to the right anterior segment. If unrecognized, these might have been ligated, leading to major complications in the donors. CONCLUSION: A portal vein anomaly associated with a rightward-deviated ligamentum teres is an important clinical entity that is not as rare as indicated in the literature. This anomaly should be kept in mind during preoperative examination of the liver.

Beta-adrenergic receptor number in human lymphocytes is inversely correlated with aerobic capacity.

In the present study, the relationships between beta-adrenergic receptor (beta-AR) expression and aerobic capacity evaluated by maximal oxygen consumption (.VO2max) and oxygen consumption level at ventilatory threshold (.VO2@VT) were investigated. Seventeen physically untrained and 25 trained men participated in the study. After supine resting, the peripheral blood was sampled for preparation of lymphocytes, the model cell used to analyze the beta-AR state. The total number of beta-AR in lymphocytes (beta-ARtotal) was inversely correlated with the VO2 max (r = -0. 368; P < 0.05) and the VO2@VT (r = -0.359; P < 0.05). Similar relationships were also observed between the number of beta-AR in cell surface and both VO2 max (r = -0.491; P < 0.05) and VO2@VT (r = -0.498; P < 0.05). However, no correlation was obtained between the number of beta-AR in intracellular compartments and either VO2 max or VO2@VT. The beta2-AR mRNA level quantified by the use of competitive reverse transcription-polymerase chain reaction was inversely correlated with VO2@VT (r = -0.567; P < 0.05) and positively correlated with beta-ARtotal (r = 0.521; P < 0.05). These findings suggest that the beta-AR number in lymphocytes is inversely correlated with aerobic capacity. This relationship may be explained by downregulation of beta-AR, including internalization with subsequent degradation of the receptors and inhibition of the beta-AR biosynthesis.

Clinical features of vesicoureteral reflux in infants and outcome of conservative therapeutic approach.

OBJECTIVES: We evaluated the clinical features of vesicoureteral reflux (VUR) detected in infants and the outcome of a conservative therapeutic approach. METHODS: Consecutively 67 infants with VUR (55 boys and 12 girls) were enrolled in this study. Of the 67 patients, 34 had primary and 33 had secondary VUR. Underlying abnormalities in secondary VUR were: posterior urethral valve (PUV) in 7; bulbar urethral stenosis (Cobb's collar) in 16; neurovesical dysfunction (NVD) in 8, and others in 2. Transurethral incision was performed in patients with PUV or Cobb's collar. NVD was managed with intermittent catheterization. All patients were followed with antibiotic prophylaxis. RESULTS: No significant difference was found in VUR grades between primary and secondary VUR. Spontaneous resolution of VUR was noted in 31% of primary and 54% of secondary VUR (p<0.02). VUR downgrading including VUR resolution was also noted more often in secondary than in primary VUR (80 vs. 48%; p<0.01). CONCLUSIONS: The distribution of primary and secondary VUR in infants was almost equal in our study. Resolution of reflux is seen more often in secondary than in primary cases. Thus, early detection and proper management of underlying lower urinary tract abnormalities, either structural or functional, are crucial in the treatment of VUR in infants.

Left coronary artery-left ventricle fistula with right coronary artery spasm.

A 72-year-old woman was admitted to our hospital for evaluation of chest pain. Coronary angiography showed a left coronary artery-left ventricle fistula. An acetylcholine provocation test induced vasoconstriction of the right but not the left coronary artery. Her chest pain was not relieved by combined therapy with isosorbide dinitrate, diltiazem and nicorandil. Because of the coronary spasm, beta-blockers could not be used. However, her chest pain was relieved after the administration of a minor tranquilizer. Thus, the patient's chest pain was unlikely to be associated with either the fistula or the coronary spasm.

Residual deformity in congenital radial club hands after previous centralisation of the wrist. Ulnar lengthening and correction by the Ilizarov method.

We used the Ilizarov method in seven patients with severe congenital radial club hands who had had previous wrist surgery, to correct residual shortening and bowing of the ulna together with recurrent wrist deformity. The mean age at operation was 6.5 years. The mean ulnar shortening was 5.3 cm and the mean angular deformity 42 degrees. The mean length gained was 51% of the original ulna. The mean healing index was 46.9 days (29.8 to 64.0). The ratio of the length of the lengthened ulna to the normal side improved on average from 64% to 95%. The angular deformity was initially completely corrected in six out of seven patients. The length ratio, however, decreased to 83% at the final follow-up. In four patients, the angular deformity partially recurred. We recommend correction of congenital radial club hand by staged procedures. The first is centralisation and stabilisation of the wrist and the second lengthening of the ulna and correction of the angular deformity using the Ilizarov method.

Morphodifferentiation of skeletal muscle fiber ends at the myotendinous junction in the postnatal Chinese hamster: a scanning electron microscopic study.

The postnatal morphodifferentiation of muscle fiber ends at the myotendinous junction was examined by scanning electron microscopy in gastrocnemius muscles of the Chinese hamster. Muscle fiber ends during the first week were simply conical or slightly complex, having a few pit-like invaginations and longitudinal and narrow clefts. By the second week, fiber ends gradually became complicated with an increased number of clefts and finger-like processes. Pits and short clefts linearly arranged towards the fiber tip were visible until the second week. This possibly indicates the linear elongation of the clefts by the fusion of adjacent pits to one another and to preexisting clefts. By the fourth week, the fiber ends had almost fully matured and displayed numerous cytoplasmic processes and lateral grooves.

Hemispheric laterality in contextual recognition.

Hemispheric dominance in contextual recognition is now under discussion. Auditory event-related potentials (ERPs) with a contextual decision task were recorded to study hemispheric dominance by using the wave energy and amplitudes of N400 for eight native volunteers. Two types of 80 sentences with four clauses were used as the structure: [subject] + [object or complement] + [object] + [two-syllable verb]. One type was congruent and the other deviated at the terminal clause as an incongruent sentence. Twenty-one channel ERPs were triggered at the verbs and recorded during -200 and 1,000 ms, with a noncephalic reference from 0.05 to 30 Hz. The wave energy was calculated as mean square potentials and was normalized with the background activity (pretrigger 200 ms). The amplitudes of N400 showed significant differences between the incongruent and congruent sentences, but the hemispheric dominance for the amplitudes of N400 was not significant between the incongruent and congruent sentences. Reaction times with the index finger to respond to the deviated verbs were 470.0 (mean) +/- 42.5 (SD) ms. The maximum normalized wave energy within the mean reaction time showed significant differences between the incongruent and congruent sentences, and the left hemispheric energy was shown to be significantly higher than the right one.

Differences in auditory scalp potentials evoked by meaningful words and nonlinguistic stimuli.

Differences in auditory evoked potentials with meaningful words and their reverse playback may reflect different neural processes in recognizing auditory stimuli. Auditory evoked potentials with the words and their reverse playback were analyzed for eight volunteers, and a maximal difference was observed at a latency around 400 msec. over the left middle temporal central area.

Reversible and irreversible tumor progression of a weakly malignant rat mammary carcinoma cell line by in vitro exposure to epidermal growth factor.

We examined the effects of epidermal growth factor (EGF) on tumor progression of a weakly malignant rat mammary carcinoma cell line, ER-1. In vitro treatment with EGF enhanced tumorigenicity, metastatic capacity and in vitro invasive capacity of ER-1 cells. The increased malignancy of ER-1 cells was reversible, when the cells were pretreated with EGF for 24 h, whereas it was irreversible when pretreated with EGF for 1 month. EGF treatment elevated the intracellular peroxide level in ER-1 cells. When ER-1 cells were treated with EGF in the presence of N-acetylcysteine, a chemical antioxidant, the reversible or irreversible EGF-induced progression was inhibited. These results suggest that the reversible or irreversible tumor progression in ER-1 cells occur in accordance with the duration of exposure to EGF, and that reactive oxygen species may be involved in the progression.

[Giant retroperitoneal hematoma in a patient with von Recklinghausen's disease].

A 51-year-old male of von Recklinghausen's disease presented with huge retroperitoneal hematoma caused by spontaneous arterial rupture. He died of uncontrollable hemorrhage during the operation. Gross appearance of aorta, renal artery, celiac artery, pulmonary artery and lumbar artery were extremely fragile, and there was a complete rupture between aorta and lumbar artery. On the microscopic examination, significant medial dysplasia were demonstrated in these arteries. Arterial stenosis is well-known vascular complication in von Recklinghausen's disease. Spontaneous rupture of major artery, however, has been extremely uncommon. This is the 26th case reported in the world literature, and 21th case in the Japanese literature.

[Determination of tocopherols in oils of mixed tocopherol as food additive using reverse-phase high-performance liquid chromatography].

Simultaneous determination of four tocopherols was developed using reverse-phase high-performance chromatography with a mixture of methanol and water (88:12) as a mobile phase. The alpha-, beta-, gamma- and delta-tocopherols in oils of mixed tocopherol as food additive were determined. It is clarified that the proposed method is useful for the quality control of food additive.

[Estimated production by the official inspection of coal-tar dyes (including dye aluminum lakes) in 1997].

The number of official inspection of coal-tar dyes and their lakes from April in 1997 till March in 1998 were 571 in total. The quantity which passed inspection amounted to 160.3 ton in Japan. The production of color in each month was summarised in Table 1, and by each producing company in Table 2. The food coal-tar dye produced in the largest quantity was Food Yellow No.4, occupying 39.8% in this period.

[Intracellular signaling pathways of angiotensin II receptor type 1 involved in the development of cardiovascular diseases].

Angiotensin II (AII) receptor type 1 (AT1), a G-protein-coupled receptor, is involved in the development of cardiovascular diseases such as hypertensin, cardiac hypertrophy, and atherosclerosis. Recent reports indicate that tyrosine phosphorylation of multiple intracellular molecules is responsible for most of these AII actions mediated by AT1, similar to receptor tyrosine kinase signaling pathways. AII activates MAPK by tyrosine phosphorylating the EGF receptor by the mechanism called transactivation with subsequent Ras activation in vascular smooth muscle and cardiac fibroblast cells. In contrast, AT1 leads to MAPK activation through PKC in cardiac myocytes. In addition to these signals, JAK/STAT pathways, which mediate cytokine actions, are also important for several AII functions through AT1.
