Endothelin ETA and ETB receptors mediate endothelin-1-induced apamin-sensitive relaxation in the guinea pig ileum.

Endothelin (ET) receptors involved in ET-1-induced responses of the longitudinal muscle of the isolated guinea pig ileum were studied. ET-1 caused concentration-dependent contractions, while ET-3 and selective ETB-receptor agonists, IRL1620 and sarafotoxin 6c (S6c), showed little or no effect. The ET-1-induced contractions were antagonized by BQ-123, an ETA-receptor antagonist, or PD142893, an ETA/ETB-receptor antagonist, indicating that the contraction is mediated by the ETA receptor. In preparations precontracted with carbachol, ET-1 elicited relaxations at lower concentrations and contractions at higher concentrations. ET-3, IRL1620 and S6c caused relaxations. These relaxations were little affected by BQ-123, but greatly antagonized by PD142893. The ET-1-induced relaxations were slightly affected by BQ-788, an ETB-receptor antagonist, but were markedly inhibited by the combination of BQ-788 and BQ-123. In ETB receptor-desensitized preparations, ET-1-induced relaxations were antagonized by BQ-123, whereas ET-3, S6c and IRL1620 showed no response. All these relaxations were abolished by apamin. These results indicate that ETA and ETB receptors mediate relaxation of the ileal smooth muscle through activation of apamin-sensitive K+ channels.

Different expression of immediate-early genes in the rat paraventricular nucleus induced by stress: relation to corticotropin-releasing factor gene transcription.

Stressful stimuli induce the transcriptional activation of the corticotropin-releasing factor (CRF) gene as well as that of various immediate-early genes (IEGs). Among them, members of the fos/jun families play an important role in the regulation of gene expression. The possible relation between stress-induced IEG expression and CRF gene transcription was investigated by analysis of stress-induced changes in the abundance of c-fos, c-jun, jun-B, jun-D, and NGFI-B mRNA as well as CRF heteronuclear RNA (hnRNA), which reflects gene transcription, in the rat paraventricular nucleus (PVN) by in situ hybridization. Restraint stress induced rapid (within 5 min) and transient increases in the level of c-fos mRNA and CRF hnRNA in the PVN, with peak expression apparent 30 min after the onset of stress. The induction of jun-B and NGFI-B gene expression was also rapid and transient, but was delayed relative to that of c-fos mRNA and the CRF hnRNA. In contrast, both jun-D and c-jun were constitutively expressed in the PVN, and the amount of the corresponding transcripts increased only slightly in response to stress. These results reveal differential patterns of expression of IEGs in the PVN in response to stress. The observation that the level of mRNAs corresponding to these IEGs did not increase before that of CRF hnRNA may suggest that the products of these genes do not seem to directly mediate the stress-induced increase in CRF gene transcription in the PVN.

[Long-term renal prognosis in patients with lupus nephritis treated with pulse methylprednisolone].

To elucidate the long-term renal prognosis in patients with lupus nephritis treated with pulse methylprednisolone, the renal outcome and the risk factors associated with poor renal prognosis were studied in 14 female lupus nephritis patients treated with pulse methylprednisolone. The mean age when the pulse methylprednisolone was started was 28.1 years old and their renal pathology were 3 of WHO III, 10 of WHO IV and 1 of WHO V. Four of the patients fell into renal insufficiency at 5.7 (mean, 1.3 approximately 10) years after pulse methylprednisolone, however residual 10 patients kept their renal function for 11.5 (mean, 3.5 approximately 19.6) years observation. The deterioration of renal function significantly (p = 0.033) correlated with glomerular sclerosing index before pulse methylprednisolone, but did not correlate with sustained immunological activity, hypoproteinemia or hypertension. Because glomerular sclerosing index was thought to be one of the risk factors for poor renal prognosis, the indication for pulse methylprednisolone in lupus nephritis was the patients with low glomerular sclerosing index.

Independent thallium-201 accumulation and fluorine-18-fluorodeoxyglucose metabolism in glioma.

UNLABELLED: SPECT with 201TI is an effective procedure for evaluating the malignancy of glioma. Our goal was to investigate the diagnostic relevance of both 201TI SPECT and [18F]fluorodeoxyglucose (FDG) PET and the relation between 201TI uptake and glucose metabolism in glioma using comparative SPECT and PET studies. METHODS: Thallium-201 SPECT and FDG dynamic PET studies were performed in 20 patients with untreated glioma (5 with glioblastoma, 5 with anaplastic glioma, 10 with low-grade glioma). Thallium-201 uptake in the tumor was estimated using the 201TI index, defined as the ratio of 201TI uptake in the tumor to that in the contralateral normal brain on SPECT images obtained 15 min after intravenous injection. We measured regional glucose metabolic parameters, including rate constants and regional cerebral metabolic rate of glucose utilization (rCMRgl), in the tumor. We then compared the regional 201TI index and glucose metabolic parameters with the histologic characteristics, malignancy and computed tomographic/ magnetic resonance imaging findings. In addition, we investigated the correlation between the 201TI index and glucose metabolic parameters. RESULTS: Thallium-201 SPECT showed abnormal 201TI uptake in all patients with glioblastoma and anaplastic glioma. Thallium-201 indices of glioblastoma (202.6 +/- 22.1%) and anaplastic glioma (176.6% +/- 26.6%) were significantly higher than that for low-grade glioma (106.7% +/- 13.8%). The rCMRgl value of glioblastoma (17.6 +/- 3.5 mumole/100 g/min) was also significantly higher than that for low-grade glioma (10.8 +/- 4.5 mumole/100 g/min), although rCMRgl showed a large variability in both high- and low-grade glioma. Rate constants of FDG kinetics had no correlation with histological grade of glioma. Some patients with high-grade glioma, however, showed false-negative results with FDG-PET because of high normal brain uptake of FDG. Conversely, most low-grade glioma could not be localized by 201TI SPECT. There was no correlation between the 201TI index and glucose metabolic parameters. CONCLUSION: Thallium-201 indices and rCMRgl values for glioblastoma were higher than those for low-grade glioma. Thallium-201 uptake in the tumor may be independent of increased glucose transport or metabolism. Thallium-201 SPECT and FDG-PET are complementary in the diagnosis of glioma, although 201TI SPECT is more significantly correlated with the malignancy of glioma.

[Effect of hypercholesteremia on vascular endothelial function and albumin excretion rate in patients with diabetes mellitus].

Tissue-type plasminogen activator (tPA) is an endothelium-derived vasoactive substance which is released to the blood stream by exercise, blood occlusion, and desmopressin (DDAVP). The increased capacity of the plasma tPA level raised by these factors is thought to reflect in vivo endothelial function. On the other hand, endothelial dysfunction has been reported in patients with hypercholesteremia as well as in those with diabetes mellitus. Therefore, diabetic patients with hypercholesteremia were administered 5 mg of simvastatin daily for one month and plasma tPA responses evoked by DDAVP were examined before and after treatment for hypercholesteremia. While the treatment of simvastatin for one month significantly reduced serum cholesterol levels from 257 +/- 12 mg to 206 +/- 10 mg (no change in HbA1c was observed during the study), plasma tPA levels and % delta vWF (von Willebrand factor) following DDAVP infusion significantly increased from 11.4 +/- 1.2 ng/ml to 13.4 +/- 1.4 ng/ml and from 69.3 +/- 23.4% to 126.5 +/- 47.4%, respectively. However, neither increase in plasma levels of guanosine 3', 5'-cyclic monophosphate (cGMP) nor change in the depressive response of blood pressure was observed following DDAVP infusion after the treatment of simvastatin. In addition, no change in urinary albumin excretion rate was observed with the treatment of hypercholesteremia. Therefore, it was suggested that improvement in hypercholesteremia may ameliorate vascular endothelial dysfunction in diabetic patients with hypercholesteremia and that hypercholesteremia may enhance endothelial dysfunction in these patients.

Intracerebroventricular administration of neuropeptide Y inhibits release of noradrenaline in the hypothalamic paraventricular nucleus caused by manual restraint in the rat through an opioid system.

Intracerebroventricular injection of 1.5 micrograms neuropeptide Y (NPY) had no effect on basal release of noradrenaline (NA) in the hypothalamic paraventricular nucleus (PVN), measured by intracerebral microdialysis in the rat. However, it blocked the increase in NA release caused by manual restraint but not that by tail-pinch, and the effect was blocked by naloxone (1.0 mg/kg body weight). Thus, NPY attenuates NA release in the PVN by a painless stressor, such as manual restraint, through an opioid system.

Induction of stress proteins in mouse peritoneal macrophages by the anti-rheumatic agents gold sodium thiomalate and auranofin.

Gold sodium thiomalate and auranofin, anti-rheumatic gold-containing compounds, induced some stress proteins in cultured mouse peritoneal macrophages. The enhanced synthesis of two proteins, heme oxygenase (a 34-kDa protein) and a 23-kDa protein, was particularly prominent. The 23-kDa protein induced by the gold compounds was identical to that found in macrophages exposed to oxidative stress and was suggested to have antioxidant activity. Intraperitoneal injection of gold sodium thiomalate and oral administration of auranofin to mice induced enhanced synthesis of these proteins in peritoneal macrophages analyzed ex vivo. These data suggest that increased synthesis of these proteins may have a role in mediating the pharmacologic effect of these agents.

The response pattern of noradrenaline release to repeated stress in the hypothalamic paraventricular nucleus differs according to the form of stress in rats.

The effect of two repeated forms of stress, manual restraint and tail-pinch, on noradrenaline (NA) release in the hypothalamic paraventricular nucleus of the rat was examined by intracerebral microdialysis. Manual restraint significantly increased NA release, but the stimulatory effect gradually declined when the stress was repeated at intervals of 120 min. High K+ induced a great increase in NA release even when manual restraint produced no significant effect on NA release. In contrast, tail-pinch significantly increased NA release to a greater extent than manual restraint, and the increase in NA release did not change when the stress was repeated three times at intervals of 120 min. These results suggest that desensitization of NA neurons ending in the hypothalamic paraventricular nucleus occurs in response to painless stress, such as manual restraint, whereas no attenuation of NA release is caused by repeated stress accompanied by pain, such as tail-pinch.

Calcified astrocytoma of the amygdalo-hippocampal region in children.

Three pediatric cases of temporal lobe seizure due to calcified glioma of amygdalo-hippocampal region are described. Computed tomography and magnetic resonance imaging showed dense calcification with no postcontrast enhancement in the amygdalo-hippocampal region. Positron emission tomography showed low oxygen metabolism, low glucose metabolism, hypermetabolism of amino acids, and low regional cerebral blood flow in the tumors. Single photon emission computed tomography showed a high accumulation of 201Tl chloride and 123I-isopropyl iodoamphetamine in one tumor, but otherwise low radioisotope uptake. These studies indicated low-grade malignancies. The patients were treated by partial tumor removal and radiotherapy. Histological examination of the tumor specimens showed astrocytoma with interstitial calcification. One patient died due to tumor recurrence, while the others are doing well with minimal seizure. We recommended temporal lobectomy in similar cases to achieve complete remission.

Effect of triethylenepentaminehexaacetic acid on the renal damage in cadmium-treated Syrian hamsters.

Cadmium (Cd)-induced nephropathy was treated by triethylenepentaminehexaacetic acid (TTHA) in male Syrian hamsters. Hamsters injected three times a week with 3 mg/kg body wt CdCl2 showed proteinuria, urinary N-acetyl-beta-D-inglucosaminidase (NAG), and fractional excretion of sodium (FENa) when compared to saline-injected control. Cd-treated hamsters injected ip with TTHA 10 mg/kg body wt five times a week showed reduction of renal damage, including reductions in urinary protein (from 6.7 +/- 2.2 to 4.3 +/- 0.5 mg/d) and NAG (0.17 +/- 0.06 to 0.04 +/- 0.02 U/d). Urinary excretion of Cd was significantly increased (from 87 +/- 51.3 to 3052 +/- 1485 mg/L) by TTHA administration. Cd concentration in renal cortical tissue was slightly reduced (26.4 +/- 3.0 to 21.8 +/- 2.7 mg/g. protein). Excretion of malondialdehyde (MDA) was increased only in Cd-injected hamsters (to 2.1 +/- 1.6 nM/L), and elevated MDA in renal cortical tissue was not reduced by the administration of TTHA (1041 +/- 105 vs 1104 +/- 358 nM/g protein). Glutathione (GSH) concentration in the renal cortex was significantly elevated after Cd administration and further increased after TTHA administration (5.5 +/- 2.1 to 9.8 +/- 2.0 micrograms/50 mg protein). There were no marked effects on creatinine clearance (Ccr) and hematocrit. Moreover, renal morphological changes were improved significantly by treatment with TTHA. We demonstrated the efficacy of TTHA in the treatment of Cd-induced nephropathy in hamsters. Although the precise mechanism of the TTHA effects on Cd-induced nephropathy has not been elucidated, it might involve GSH reducing the elevated MDA concentration in renal tissue.

Positron emission tomography with 4-[18F]fluoro-L-m-tyrosine in MPTP-induced hemiparkinsonian monkeys.

PET imaging studies with 4-[18F]fluoro-L-m-tyrosine (FMT) in normal macaca monkeys showed selective accumulations of radioactivity in the striatum with time. In monkeys rendered hemiparkinsonian by intracarotid infusion of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP), FMT uptake was eliminated in the lesioned striatum. FMT-PET studies were able to detect dopaminergic terminals in both normal and hemiparkinsonian monkeys, and clearly showed a reduction in aromatic L-amino acid decarboxylase (AAAD) activities in the MPTP-lesioned striatum. These results show that FMT is promising as a PET tracer for the evaluation of central dopaminergic systems in parkinsonism.

Chlordiazepoxide attenuates stress-induced activation of neurons, corticotropin-releasing factor (CRF) gene transcription and CRF biosynthesis in the paraventricular nucleus (PVN).

Corticotropin-releasing factor (CRF) plays a role in coordinating endocrine, autonomic, and behavioral responses to stressful stimuli. Benzodiazepines exert many effects which oppose those of CRF, including anxiolysis and suppression of the pituitary-adrenal axis. In the present study, we employed in situ analysis of CRF heteronucleous RNA (hnRNA) and c-fos mRNA to assess stimulus-induced CRF gene transcription rate following stress and its modulation by chlordiazepoxide (CDP). Male albino rats were exposed to restraint stress for 30 min and sacrificed 30 and 120 min after the onset of stress. Either CDP or vehicle was given intraperitoneally 60 min before stress. To determine plasma ACTH levels by immunoradiometric assay, another group of rats was decapitated 10 min after the onset of restraint stress. Restraint stress induced rapidly and significantly c-fos mRNA and CRF hnRNA expression in the PVN at the 30 min time point. Increases in both RNA copies were significantly inhibited by administration of CDP at doses of 5 and 10 mg/kg. CRF mRNA concentrations were increased significantly in the PVN 120 min after stress and again, CDP attenuated significantly these increases in the PVN. The plasma ACTH increase in response to stress was inhibited significantly by CDP administration at every dose tested. CDP did not change CRF mRNA levels in the non-stressed animal.(ABSTRACT TRUNCATED AT 250 WORDS)

Intracerebroventricular administration of corticotropin-releasing factor antagonist attenuates c-fos mRNA expression in the paraventricular nucleus after stress.

Corticotropin-releasing factor (CRF) plays a role in coordinating endocrine, physiological and behavioral responses to stressful stimuli. We have previously reported that centrally administered CRF rapidly induces c-fos mRNA expression in most areas that express c-fos following stress: the limbic structures including the hippocampus, amygdala, septal nucleus and hypothalamic nuclei such as the paraventricular nucleus (PVN), and brainstem nuclei such as Barrington's nucleus and locus ceruleus (LC). These results suggest several candidate structures through which CRF could exert its effects on the central nervous system. The aim of the present study was to explore the role of endogenous CRF in activating these brain regions after stress using a specific CRF receptor antagonist, alpha-helical CRF(9-41). One hundred micrograms of alpha-helical CRF or vehicle alone was injected into the right lateral ventricle of rats. Thirty minutes later, they were exposed to restraint stress for 30 min, and the rat brain was then perfused transcardially with 4% paraformaldehyde. Another control group of rats was sacrificed 60 min after intracerebroventricular (ICV) injection of vehicle without restraint. In situ hybridization was performed by hybridizing sections with 35S-labeled c-fos cRNA probes. ICV injection of vehicle alone induced a weak c-fos mRNA expression in the lateral septal nucleus (LSV) and PVN in the rats without restraint, probably due to the mild stress of ICV injection. Restraint stress produced strong c-fos mRNA induction in the PVN, LSV, LC and Barrington's nucleus. However, ICV injection of 100 micrograms alpha-helical CRF significantly decreased c-fos mRNA expression in the PVN without affecting c-fos mRNA induction in the LSV, LC, or Barrington's nucleus.(ABSTRACT TRUNCATED AT 250 WORDS)

Stress-induced activation of neuronal activity and corticotropin-releasing factor gene expression in the paraventricular nucleus is modulated by glucocorticoids in rats.

Intronic in situ hybridization methodology provides a means of determining the rate of gene transcription under basal and stimulated conditions. In the present study, we have used intronic in situ hybridization to the corticotropin-releasing factor (CRF) gene to measure hypothalamic CRF gene transcription after stress as well as its modulation by glucocorticoids. Using this and conventional exonic in situ hybridization we examined the time course of changes in c-fos mRNA, and CRF heteronuclear RNA (hnRNA) and mRNA concentrations in the paraventricular nucleus (PVN) of male Wistar rats after restraint stress. In addition, we determined the effects of adrenalectomy and dexamethasone administration on c-fos and CRF gene expression in the PVN. Restraint stress induced a rapid induction (within 5 min) of c-fos mRNA and CRF hnRNA expression in the PVN. Both RNA concentrations peaked at 30 min then decreased and were undetectable 2 h after stress onset. In contrast, the concentration of CRF mRNA increased gradually and a significant elevation was first detected 60 min after the beginning of stress. Adrenalectomy augmented and dexamethasone pretreatment inhibited c-fos mRNA, CRF hnRNA, and mRNA induction after stress. The data suggest that stress-induced activation of neurons, CRF gene transcription, and CRF synthesis in the PVN are modulated by glucocorticoids.

Dopaminergic regulation of aldosterone secretion: its pathophysiologic significance in subsets of primary aldosteronism.

Although aldosterone (Aldo.) secretion is regulated by various humoral factors, evidence has accumulated to support an involvement of dopaminergic system in its regulation. The pathophysiological significance of the dopaminergic system in primary aldosteronism (PA) however remains unknown. In the present study, we examined the effects of metoclopramide (MCP) on Aldo. secretion in normal subjects (n = 11) and patients with essential hypertension (EH, n = 8), aldosterone-producing adenoma (APA, n = 10), and idiopathic hyperaldosteronism (IHA, n = 6). Plasma Aldo., prolactin (PRL), renin, cortisol, serum sodium, and serum potassium levels were determined before and 30 min after i.v. bolus injection of 10 mg MCP at 9 a.m. Plasma Aldo. showed a significant increase after MCP in normal subjects, EH, and APA, but not in IHA. The incremental response of plasma Aldo. was largest in APA and smallest in IHA. The percentage increase in plasma Aldo. from the basal level was significantly attenuated in IHA, while no significant difference was seen among other groups. Although plasma PRL showed a significant increase in response to MCP, no difference of the change was seen among the groups. There was no significant change in plasma cortisol, renin, serum sodium, and serum potassium levels in response to MCP. In addition, the response of Aldo. to MCP was normalized in APA after unilateral adrenalectomy, while that of PRL did not change. These results indicate that the adrenal dopaminergic activity is enhanced in APA and attenuated in IHA and suggest an involvement of the dopaminergic system in the pathogenesis of IHA.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical aspects of drug-induced tubulo-interstitial nephropathy].

It is well known that many drugs can induce tubulointerstitial nephropathy (DTIN) in the kidney. There are two kinds of onset mechanisms such as dose-dependent and allergic nephropathy. Risk factors inducing DTIN are known as aging, dehydration, preexisting renal damage, hypertension and serum electrolyte abnormalities. Treatment of DTIN is firstly to cease suggestive drugs of inducing nephropathy and then followed by diuresis, correcting serum electrolyte levels and sometimes hemodialysis or hemoadsorption to reduce the suggested drug levels in the circulation. In allergic nephropathy, high-dose glucocorticoid therapy is available for improving renal function rapidly. The prevention of DTIN is to know the patients background and monitor serum drug levels.
