[Antihypertensive and diuretic effects of indapamide in normotensive and hypertensive rats (author's transl)].

Antihypertensive and diuretic actions of indapamide (SE-1520) were investigated in rats and compared with those of trichlormethiazide (TCMT). In normotensive rats, indapamide in a dose of 100 mg/kg p.o. did not show a significant effect on the blood pressure. In DOCA-saline and uni-nephrectomized DOCA-saline hypertensive rats, indapamide above 1 mg/kg and TCMT above 3 mg/kg with single oral or repetitive administration for 2 weeks reduced the blood pressure level. In spontaneously hypertensive rats (SHR), both indapamide and TCMT lowered the blood pressure with single doses above 10 mg/kg or repetitive doses above 3 and 10 mg/kg, respectively. In the diuretic test using normal rats, indapamide in doses ranging from 0.1 to 30 mg/kg increased urine volume and urinary electrolyte excretion. TCMT showed a more potent diuretic action at a lower dose level. In SHR, indapamide and TCMT produced a greater urine volume and electrolytes excretion. Indapamide inhibited the carbonic anhydrase activity and the potency was about 1/25 of that of acetazolamide in vitro. The antihypertensive activity of indapamide was more potent than that of TCMT and the reverse order to that of their diuretic potencies. It is suggested that the mechanism of antihypertensive effect of indapamide is different from that of TCMT.

[The propensity to malignant changes in heterozygous moles].

There are at least two mechanisms of origin for complete mole, each with different genetic implications; 1) the fertilization of an empty egg by a haploid sperm and its subsequent duplication (homozygous mole), 2) the fertilization of such an egg by two haploid sperms, which produces an heterozygous constitution of allelic genes (heterozygous mole). The present study was undertaken to investigate whether the difference of origin would determine whether the moles underwent malignant transformation or not. By using of chromosomal, enzymatic and HLA polymorphisms, 35 moles were confirmed to be androgenetic in origin. Among them fifteen homozygous and 4 heterozygous moles were identified. The persistency of high HCG titration of the urine in the mother was observed in all 4 cases from the latter (4/4-100%), though the percentage of the development of sequelae in the former was low (1/15-6.7%). The diagnosis of destructive mole was established in the uterine lesion of 2 heterozygous moles by pathological examination. These two patients had lung metastases. A high incidence of sequelae after the expulsion of heterozygous moles suggests that the heterozygous constitution of allelic genes plays an important role in the process of malignant transformation of trophoblasts.

Nucleotide sequences of small ribosomal RNA and adjacent transfer RNA genes in rat mitochondrial DNA.

The nucleotide sequences of genes for small 12 S ribosomal RNA and two transfer RNAs (tRNAPhe and tRNAVal) and their common upstream region in cloned rat mitochondrial DNA were determined. This DNA sequence of 1709 bp extends between the HindII and HindIII cleavage sites in the EcoRI-A and EcoRI-D fragments, respectively. There is no long reading frame for protein synthesis starting from the HindIII site and extending all the way to the HindII site, or vice versa. The heavy strand was found to be the template for these RNA transcripts. The 5' and 3' ends of the 12 S ribosomal RNA gene are contiguous with the tRNAPhe and tRNAVal genes, respectively. The structural features of the two tRNA genes are significantly different from the standard prokaryotic tRNA pattern. Since similar contiguous natures of these genes in human (Crews and Attardi, Cell 19 (1980) 775; Eperon et al., Nature 286 (1980) 460) and mouse (Van Etten et al., Cell 22 (1980) 157) mitochondrial DNA have been reported, comparative studies on the structures of these genes were performed by computer analysis and the characteristic features are discussed.

The risk of nonfebrile seizures in children who have experienced febrile convulsions.

The frequency of development of nonfebrile seizures in 116 children who had experienced at least one febrile convulsion and were followed for more than five to eight years was 4.3% (5 cases). Of these, three cases had prolonged generalized convulsions of the clonid type. (2) The risk factors identified as nonfebrile seizures after febrile convulsions, were the preexisting neurological abnormality or developmental retardation, focal features and more than a 10-minute duration of the first febrile convulsions, and abnormal paroxysmal discharges at the initial interietal EEG recordings.

The diagnostic accuracy of computed tomography in obstructive biliary disease: a comparative evaluation with direct cholangiography.

Computed tomography (CT) and direct cholangiography were performed in 75 patients for the evaluation of obstructive biliary disease. The two modalities correlated well in cases of moderately or severly dilated intrahepatic bile ducts that showed tortuosity and/or engorgement, but mild dilatation with stretching of theperipheral intrahepatic bile ducts that was seen with direct cholangiography was detected with CT in only one of nine patients. Consequently, the CT diagnosis was correct in 41 of 51 patients (80.4%) with obstructive biliary disease; however, CT was correct in only 15 of the 25 patients (60%) with nontumorous obstructive biliary disease. It is concluded that, in cases of clinically suspected nonneoplastic obstructive disease, percutaneous transhepatic cholangiography or endoscopic retograde cholangiopancreatography should be performed even when dilated ducts are not demonstrated by CT.
