[Surgical treatment of postinfarction ventricular septal perforation--the difference between a cardiogenic shock group and a congestive heart failure group].

Twelve patients with postinfarction ventricular septal perforation (VSP) were divided into 2 groups based upon the preoperative status and the time interval between the operation and the occurrence of VSP after acute myocardial infarction (AMI). Group I were in cardiogenic shock unresponsive to either pharmacologic supports or IABP, and needed an emergency repair of VSP. The other group (group II) were in congestive heart failure responding to some extent to pharmacologic supports and IABP, and VSP of this group was repaired on the elective or semiemergency basis. Group I comprised of 7 patients, 5 males and 2 females, with a mean age of 65.9 +/- 12.6 years, and group II included 5 patients, 2 males and 3 females with an averaged age of 72.6 +/- 3.4 years. The mean time duration between AMI and the operation, and between the occurrence of VSP and the operation were 3.1 and 1.6 days in group I and 13.4 and 8.0 days in group II. The operative mortalities were 57% in group I and 0% in group II, a remarkable difference. The reasons why group I had a poor prognosis were analysed and were found as follows: (1) group I sustained a larger AMI of anteroseptal area together with the lateral and/or inferior infarctions more often than group II. (2) Group I had frequently multiple organ failure (MOF) even prior to operation due to cardiogenic shock. (3) Group I had severer right ventricular failure than group II, in which the right atrial pressure was markedly elevated. In group I, the right heart failure remained and was prolonged even after surgery reflected by the RAP/LAP ratio over 1 and finally resulting in MOF. To improve surgical results in group I, the operation should be undertaken on the emergency basis before MOF is completed, and patch reconstruction of the left ventricular free wall is recommended in patients with a wide AMI and a high positioned anterior septal perforation. When RV failure is dominant, not only a LV assist device but also a RV assist device may also improve the results.

[Ontogeny of pancreatic gastrin cells in neonatal rat].

It is well known that gastrin immunoreactive cells are observed in the fetal and postnatal rat pancreas. The role of the gastrin cells is unknown, however, it has been suspected that pancreatic gastrin may influence the development of pancreas and the gastrointestinal tract. The present study shows the localization of pancreatic gastrin cells and the ability of auto-proliferation of them in development. Gastrin cells were seen in 0-d to 2-wk-old rats in islet and in 0-d to 4-wk-old rats among exocrine cells. The ratio of gastrin cells to islet cells decreased in neonatal development and in rats older than 14-d gastrin cells were never observed in islet. Labeling indices of pancreatic gastrin cells after one injection of tritiated thymidine were 2.6% to 4.6% for 10 days after birth. It was suggested that gastrin cells have the ability of autoproliferation for 10 days after birth.

Percutaneous absorption enhancer applied to membrane permeation-controlled transdermal delivery of nicardipine hydrochloride.

In the design of nicardipine hydrochloride-transdermal delivery systems (NC-TDS), the enhancing and regulating effects of penetration-enhancers and permeation controlling membranes were evaluated. Laurocapram (Azone) was selected as a model enhancer. Since its enhancing effect is considered to occur in the stratum corneum, its release from ethylene-vinyl acetate copolymers (EVAc), ethylene-vinyl alcohol copolymers (EVAl) or poly(2-hydroxyethylmethacrylate) (pHEMA) membranes was used as a criterion for membrane selection. The release rate was highest from pHEMA. Accordingly, a TDS consisting of a NC reservoir containing 1 w/v% Azone and a pHEMA membrane was prepared, and the effect of skin stripping on the plasma concentration of NC after NC-TDS administration was evaluated in rats. The increased ratio in drug plasma concentration caused by skin stripping was lower when the NC-TDS treatment was compared with a non-regulating NC-gel treatment. Our results suggest that the membrane permeation-controlled TDS of NC may be useful for long-term constant drug delivery with minimum dependence on skin conditions.

[Changes in anterior pituitary function during ACTH therapy of patients with infantile spasms].

Serum cortisol, prolactin (PRL), TSH, GH, LH and FSH levels were measured before and immediately after daily ACTH-Z therapy (0.01 mg/kg/day, 1-2 weeks) for 5 patients with infantile spasms and one patient with myoclonus epilepsy. Total number of ACTH-Z therapy were 8 times, and all patients became seizure free after ACTH-Z therapy. In 6 occasions, TRH, LH-RH and insulin tolerance tests were performed before and after daily ACTH-Z therapy. Serum cortisol levels were significantly increased after daily ACTH-Z therapy but all other hormone levels were significantly decreased. In TRH and LH-RH tolerance tests, peak levels and increments of PRL, LH and FSH were significantly decreased after daily ACTH-Z therapy and those of TSH were mildly decreased. In one case insulin tolerance test revealed an adequate decrease of blood glucose before and after ACTH-Z therapy, and there was a poor GH response after ACTH-Z therapy. Daily ACTH-Z therapy was thought to suppress secretion of anterior pituitary hormones.

[Efficacy of etoposide in bone metastasis of breast cancer].

We have used ADM, MMC, CDDP and other drugs for a case of bone metastasis of breast cancer, but the bone destruction was advanced and she could not walk. We have also used etoposide, a new chemotherapeutic drug, for the same case. Two months later bone sclerosis was seen by X-ray film and pain disappeared. Bone sclerosis then advanced after 6 months, she has begun to stand, and after 8 months she has been able to walk with a cane. There was no severe side effect. Etoposide was very effective for bone metastasis of the breast cancer.

Composition and distribution of retinal and 3-hydroxyretinal in the compound eye of the dragonfly.

Retinoids in the compound eyes of nymphs and adult dragonflies in 11 families of the 3 suborders were extracted by the oxime method, and analysed by high performance liquid chromatography. Almost all of the species examined contained both retinal and 3-hydroxyretinal in the compound eye. The ratio of 11-cis 3-hydroxyretinal to 11-cis retinal (3-OH ratio) was calculated as an index of the retinoid composition. The 3-OH ratios of the whole eye of nymphs in all the suborders and of adults of the suborder Zygoptera were very high, 2.2 at the minimum, but in Anisozygoptera and Anisoptera most of the ratios were distributed between 1 and 2.7. In the family Gomphidae, exceptionally low 3-OH ratios, less than 1, were observed in several species. The regional distributions of the retinals in the adult compound eyes were also examined. In the Zygopteran compound eye, both retinals were distributed evenly all over the eye, while in the compound eye of the other two suborders, the 3-OH ratios in the dorsal area of the eye were extremely low. In several species of Gomphidae and Libellulidae the ratios in the dorsal areas were zero. From the correspondence of these results and the compartment of the compound eye, it appeared that the large ommatidia in the dorsal area contained only retinal. This was confirmed when the large facet region in the dorsal part of the compound eye of an Anax was excised and examined, and only retinal was detected. However, the ventral area of the true dragonflies' compound eye which did not include the large ommatidia contained both retinals, and the 3-OH ratio was more than ten. The biological significance of using both retinals as chromophores of visual pigments in the dragonfly eye is discussed in relation to the structure of the ommatidia and to the vision of dragonflies.

Determination of salsolinol by ion-exchange chromatography with glycylglycine as the post-derivatizing agent.

The determination of salsolinol in human urine was carried out by ion-exchange chromatography on two coupled columns of a weakly acidic ion exchanger with a hydrophilic matrix (Asahipak ES-502C). Salsolinol was first isolated from urine by adsorption on Amberlite CG-50. It was eluted together with catecholamines by 2/3 M boric acid solution. The amines were then separated by isocratic elution from the first column of Asahipak with 0.05 M sodium succinate buffer (pH 5.5) containing 0.015 M borate and 0.5 mM ethylenediaminetetraacetate. Epinephrine, norepinephrine, dopamine and salsolinol were eluted in that order. The salsolinol-containing fraction was then transferred, by column switching, to a second Asahipak column and eluted with the same mobile phase. Salsolinol was determined fluorimetrically by reaction with glycylglycine in the presence of hexacyanoferrate(III) at pH 7.5-8 and 65 degrees C. Samples could be analysed every 47 min. The detection limit for salsolinol was 2 pmol/ml.

Formation of visual pigment chromophores during the development of Xenopus laevis.

Retinoids in the eyes of Xenopus laevis at several developmental stages, were analyzed by high-performance liquid chromatography (HPLC). At stage 37/38, larval eyes contained mainly all-trans isomers of retinal, 3-dehydroretinal, retinyl ester and 3-dehydroretinyl ester. Ratios of all-trans 3-dehydroretinal to retinal and of all-trans 3-dehydroretinyl ester to retinyl ester were almost 1 at the stage. With the advance of development, the amounts of all-trans retinal and 3-dehydroretinal decreased; however, those of all-trans retinyl ester and 3-dehydroretinyl ester increased. The chromophores of visual pigments, 11-cis retinal and 3-dehydroretinal, were detected at stage 40 (total; 0.2 pmol/eye) and their amounts increased after that stage. The ratio of 11-cis 3-dehydroretinal to retinal was almost 1 at stages 40-42. The ratio became larger after stage 43 and was almost 19 at stage 46. The ratio of all-trans 3-dehydroretinyl ester to retinyl ester, also, increased after stage 42 and reached 11 at stage 46. The mechanism of 11-cis formation during development is discussed in relation to retinoid conversions in the eyes.

Fluorescence of hematoporphyrin in living cells and in solution.

The fluorescence properties of hematoporphyrin (Hp) and its derivative (HpD) were investigated in leukemia cells and in normal lymphocytes under a microscope, and the results were compared with those in solution. The spectra and the time behaviour of Hp (or HpD) fluorescence in living cells were found to be almost the same as those in Hp solution of very high concentration. This implies that Hp is much more concentrated in the cells than in the medium. It was also found that irradiation with intense light easily gives rise to a photoproduct which gives an additional peak in the fluorescence spectrum. Possible methods to increase the sensitivity of cancer detection and localization are discussed.

Intermediate peptides of insulin degradation in liver and cultured hepatocytes of rats.

1. Bestatin, a microbial aminopeptidase inhibitor, induced accumulation of low-molecular weight intermediate peptides of insulin degradation in liver of rats in vivo and in primary cultured rat hepatocytes. However, bestatin did not affect the association and internalization of the hormone into hepatic cells. 2. Results of the HPLC analyses showed that the intermediate peptides of insulin degradation are small ones and specifically accumulate only in the presence of bestatin. 3. The above results, together with those employing other protease inhibitors, show that cytosolic bestatin-sensitive protease(s), trypsin-like protease(s) and thiol protease(s) play an important role in the intracellular degradation process of insulin.

Nucleotide sequence of the Bacillus subtilis phoR gene.

The nucleotide sequence of phoR, the positive and negative regulatory gene for alkaline phosphatase and phosphodiesterase formation in Bacillus subtilis, was determined. The sequence data predicted an open reading frame of 1,740 base pairs (579 amino acids) which overlaps the 5 base pairs of the preceding phoP coding sequence. The deduced amino acid sequence was significantly homologous with that of the Escherichia coli phoR gene product, which is the sensory element for the pho regulon.

Treatment of brain tumors with iridium-192 seeds.

Interstitial radiation therapy of brain tumors requires positioning of the radioactive sources with good precision in order to achieve a sufficient dose in the tumor without too much damage to surrounding tissues. A method for application of 192Ir seeds in brain tumors is described and preliminary results in seven patients reported. The method, which uses CT-guided stereotactic localization and computerized dose planning, is at present fairly complicated. The preliminary results are promising and no serious complications have so far been observed.
