[Study of the innervation of the spinal ligaments at the lumbar level].

The authors studied the innervation of the human lumbar spinal ligaments on cadaver or surgical specimens. In the ligaments annexed to the neural arch and in the posterior longitudinal ligament were found free-ending fibers and amyelinic perivascular fibers. In the anterior longitudinal ligament, coexisting with perivascular fibers were found encapsulated corpuscular formations on the ventrolateral aspect of the junction between the intervertebral disc and vertebral body. These findings comfort the role of the anterior longitudinal ligament in proprioception essential both in static and dynamic function of the spine.

[Anatomic and physiologic comments concerning the innervation and function of the omohyoid muscle in man].

Electrophysiological studies of the rather strange, enigmatic, likely vestigial omohyoid muscle led the authors to point out the dual nature of its two bellies. Electromyographically, the inferior belly with a nearly constant activity may be opposed to the superior belly with intermittent activity. Motor nerve stimulation combined with electromyographic study demonstrated the superior belly to be supplied by the ansa cervicalis while the inferior belly is supplied by the accessory nerve. Based on these findings, anatomical studies conducted with histological examination were consistent with this double innervation.

[Anatomic study of a omphalosite monster].

The authors report the main anatomical features of a twin holoacardius amorphus monster. Diagnosis of the abnormality was assured by echography at the end of a "normal" gemellary pregnancy. Echograms were consistent with the diagnosis of "amorphus monster" and this was supported by dissection. The caudal extremity of the fetus was clearly identified by the presence of recognizable lower limbs in spite of the several abnormalities of the pelvic area. The cephalic extremity was characteristic of the malformation. Examinations of visceres allowed to precisely identify digestive structures such as intestinal loops (with disseminated atresias). It could not be find any kidney structure. Medially, a small rounded mass was observed and could be identified as hypoplasic cardiac structure. Other main anatomic features consisted of an enormous liquid-filled cavity largely communicating with both the lower part of the neural tube and the basis of the cranium. The placenta was unique and two umbilical with several and large anastomotic by-passes were increased from it.

Abnormalities, Multiple↗

[Unusual late disclosure of left congenital diaphragmatic hernia].

A posterolateral hernia through Bochdalek foramina was discovered in a 27 months old child under unusual circumstances. During lifting an heavy weight, migration occurred of the abdominal content through the foramina into the left thorax. The size of the pleuroperitoneal communication was well demonstrated by peritoneography. Possible rupture of a diaphragmatic relaxation was eliminated on findings at surgery. Reconstructive surgery led to an uneventful recovery which is the rule in this type of late-discovered hernia.

[Cystic abdomino-mediastinal lymphangioma. Apropos of a case. Review of the literature].

A further case of cystic lymphangioma is reported, characteristic features being the multiple abdominal and thoracic localizations of the lesions. Apart form splenic sites, it is rare to detect lesions in the mesogastric and mediastinal regions, particularly as propagation of abdominal lymphangioma towards the mediastinum is an exceptional occurrence. A literature review provides data on the various regions in which cysts have been reported in the body, and the diagnostic problems they raise.

[Outcome of nonsurgically treated pseudoappendicular syndromes in children. Retrospective study of 479 cases].

A retrospective study was conducted to assess outcome in 479 children admitted to a surgical pediatric department with pseudo-appendicular syndromes untreated by surgery. A precise diagnosis had been made during the first admission in 186 cases. At a later date, each child had been examined clinically and a detailed history taken, or a questionnaire was sent to the families or treating physician. Replies were received concerning 263 children, including 205 who had had no further episode of abdominal pain, 24 who had had similar atypical bouts of abdominal pain and 34 who had been operated upon at a later date (representing 12.9% of replies). These findings would appear to justify avoidance of surgery, after admission for observation, whenever the atypical symptomatology, negative results of paraclinical investigations and the progression towards total remission of the affection is observed.

[Talo-calcaneal continuity. Myth or reality?].

On the basis of two separate biomechanical studies of talus and calcaneus, the authors show: the absolute continuity of the trabecular structure of these bones, the mechanical behaviour characterised as "fragile" for the talus and "intermediate" for the calcaneus, superficial strains underload: by photoelastic studies, they are show to be in reversed phases both on their lateral and medial sides. Authors conclude that these two bones act together as sectional across which collect dispach and absorb transmitted forces during gate.

[Anatomical study of a human case of total celosomia].

The authors studied a human specimen delivered at 36 gestational weeks and presenting with a caelosomia. The various abnormalities could be classified as: superior caelosomia: ectocardia, diaphragmatic hernia; middle caelosomia: liver outside the abdominal cavity, mesenterium commune O, meso-hepato-cave; inferior caelosomia: cloacal extrophia. In addition to the forementioned abnormalities, a lumbosacral meningocele and multicystic kidneys were found. All these abnormalities are exceptionally found all together in one human specimen and this monster might be classified in the total caelosomia group.

[Anatomic and histologic study of the human ileocecal valve. Developmental aspects as a function of age].

Based on gross anatomy and histological studies of thirty specimens ranging from nineteen weeks of gestational age to eighty-two years old, the authors present an analytical study of the ileocecal valve. The results confirm the evolutive aspect of the ileocecal valve with age, but, however seems to demonstrate a functional regression correlated with the frequency of its incontinence in adulthood.

[Prophylaxis of infectious complications in maxillo-facial surgery. Value of cefazolin. (A review of 400 cases)].

The value of short-term prophylaxis with antibiotics in maxillo-facial surgery and plastic facial surgery is studied. 200 patients were included in the study and compared to 200 controls who were given the usual systematic antibiotic therapy, with a different antibiotic, for more than 6 days. 400 case-reports were thus retrospectively analyzed. The results show that when the surgical procedure lasts for less than three hours, short-term prophylaxis with antibiotics is more effective than the usual systematic antibiotic therapy given for more than 6 days.

[Study of the talus and calcaneus using microdurometry, penetrometry and compression].

The authors give the results of various methods such as hardness micrometry, indentation tests and the radiographic study of fracture patterns, which they have applied to the talus and to the calcaneus. It appeared to them that: --these techniques outline the phenomenon of rupture behaviour of both talus and calcaneus; --the talus behaves as a fragile type of material whereas the calcaneus is of the intermediate type. The authors therefore think that these methods can be of great interest in a better understanding of the biomechanical properties of bone.

Biomechanical Phenomena↗

[Gastric tumor in children. Apropos of 3 cases].

Gastric tumours are exceptional in children and are usually discovered due to digestive tract bleeding or after palpation of a mass in the abdomen. Appropriate exploratory techniques are need to correctly localize the lesions in the stomach. Nevertheless, the absence of any particular signs can mask the polymorphic nature of the lesions that can only be determined by pathologic examinations. The extent of the surgical exeresis and prognosis depend on the nature of the pathological process.
