Serum immunosuppressive acidic protein in haemodialysis patients with and without acquired cystic disease of the kidney.

Serum levels of immunosuppressive acidic protein (IAP) were measured in 20 haemodialysis patients with acquired cystic disease of the kidney (ACDK) and in 44 without it. Serum IAP levels in haemodialysis patients with and without ACDK were significantly elevated compared to that in controls. However, there was no considerable difference between haemodialysis patients with and without ACDK. A significant reduction in this marker was not found after dialysis treatment. This indicates that elevation of IAP may be related to an increased incidence of renal cell carcinoma in haemodialysis patients.

Effect of recombinant human erythropoietin on synthesis of methylguanidine in uraemic patients on haemodialysis or continuous ambulatory peritoneal dialysis.

The effect of recombinant human erythropoietin (rHuEPO) on synthesis of methylguanidine was studied in 6 uraemic patients on haemodialysis and 5 uraemic patients on continuous ambulatory peritoneal dialysis (CAPD). The two groups of patients were started on a 24-week course of thrice weekly 1500 IU of rHuEPO by the intravenous route. Serum methylguanidine level and methylguanidine/creatinine ratio were comparable in these groups. In the two groups no significant differences were observed in these measurements comparing the pretreatment values with those 4, 8, 12 or 24 weeks after starting rHuEPO administration. During rHuEPO therapy, serum methylguanidine levels and methylguanidine/creatinine ratio showed no considerable difference between the two groups. These findings suggest that administration of rHuEPO does not alter methylguanidine synthesis in uraemic patients on haemodialysis and CAPD.

Change in haemoglobin concentration, haematocrit and vasoactive hormones in haemodialysis patients with erythropoietin-associated hypertension.

The changes in haemoglobin concentration, haematocrit, plasma renin activity (PRA) and atrial natriuretic peptide (ANP) were studied in 10 haemodialysis patients with erythropoietin-associated hypertension. All patients received intravenously 1500 IU of recombinant human erythropoietin (rHuEPO) thrice weekly for 24 weeks. Treatment with rHuEPO induced significant rises in haemoglobin concentration (p < 0.001) and haematocrit (p < 0.01). However, the difference between post- and pretreatment levels of haemoglobin (delta Hb) was not correlated with that between post- and pre-treatment mean blood pressure (delta MBP). No correlation was found between delta Ht (difference between post- and pre-treatment values of haematocrit) and delta MBP. These results indicate that elevation of the haematocrit and haemoglobin concentration of haemodialysis patients does not necessarily lead to an increase in blood pressure. In these patients, no significant differences were observed in PRA and ANP, comparing pre-treatment values with those measured 4, 8, 12 or 24 weeks after commencing rHuEPO. This suggests that neither PRA nor ANP play a central role in the pathogenesis of rHuEPO-induced hypertension.

Serum level of immunosuppressive acidic protein in haemodialysis patients with or without renal cell carcinoma.

OBJECTIVE: To assess the significance of immunosuppressive acidic protein (IAP) for detection of renal cell carcinoma in patients undergoing haemodialysis. PATIENTS AND METHODS: The serum concentration of both IAP and creatinine was measured in 74 patients undergoing haemodialysis, 10 of whom had renal cell carcinoma and 64 of whom had no renal cell carcinoma. Control data was obtained from 13 healthy blood donors. RESULTS: The serum IAP level of patients undergoing haemodialysis was significantly elevated compared with that of the controls. The serum level of IAP in patients with renal cell carcinoma who were undergoing haemodialysis was also considerably higher than that of patients with no renal cell carcinoma. The serum creatinine levels were not significantly different between patients with and without renal cell carcinoma but were significantly elevated when compared with the controls. CONCLUSION: The measurement of serum IAP concentration is of value in the detection of renal cell carcinoma in patients undergoing haemodialysis.

Pericentric inversion of chromosome 9 associated with chordee without hypospadia.

We report a case of pericentric inversion of chromosome 9 associated with chordee without hypospadia. Plasma levels of LH and testosterone were within normal ranges, but the plasma FSH level was slightly elevated. The LH response to LH-RH stimulation and the response of plasma testosterone to HCG were normal. However, the administration of LH-RH induced an abnormally low response of plasma FSH.

[Successful surgical correction of endocardial cushion defect in a 64-year-old female patient].

We have experienced a surgical correction of ECD of a 64-year-old female patient. The preoperative left ventriculogram showed typical goose-neck deformity of the left ventricle and mild mitral regurgitation. The correction consisted of repair of the mitral cleft, and patch closure of the ostium primum. Postoperative course was uneventful. The operation of ECD should be recommended even in an older patient.

[A case of paragonimiasis with a tumor of the intrathoracic chest wall].

A case of paragonimiasis with a tumor of intrathoracic chest wall was reported. The patient was a 46-year-old male who complained of cough and showed abnormal findings on chest X-ray. After performing various examinations, the patient was diagnosis as having a tumor of the intrathoracic chest wall. On surgery, the tumor was enveloped in a thick capsule and its lumen was filled with yellow-white atheromatous material. Calcification was not found. Pathologically, an egg-like structure of a parasite was confirmed inside the sequestrum attached to the lining of the inner surface of the capsule. Past history of pleuritis and ingestion of freshwater crabs confirmed the diagnosis of paragonimiasis.

[A case of cardiac angiosarcoma with a pseudoaneurysm formed in the right coronary artery].

A case of superficially infiltrating cardiac angiosarcoma damaging the right coronary artery and forming a pseudoaneurysm was studied. The patient was a 25-year-old male who had undergone cardiac tamponade twice in the past six months. Since heart murmur was audible in auscultation, thorough examination was performed. Cardioechography and cardiac angiography revealed a pseudoaneurysm of hen egg size involving the proximal right coronary artery. Although the pseudoaneurysm and right atrium ruptured during operation, a coronary bypass was placed and the right atrium was repaired with an equine pericardium. Angiosarcoma was identified by pathological examination of the excised specimen. The patient died two months after operation due to lung metastasis.

[Surgical removal of right atrial myxoma in the elderly].

We have experienced a surgical treatment of right atrial myxoma in a 76-year-old woman. The patient was admitted with complaints of palpitation and low grade fever. Echocardiography revealed the atrial tumor, which prolapsed into the right ventricle in diastole. The tumor was removed under cardiopulmonary bypass including the atrial septum where it was attached. The tumor weighed 68 g and showed typical pathological findings of myxoma. The patient is doing well 6 years after surgery.

Distinct subdomains of human endothelin receptors determine their selectivity to endothelinA-selective antagonist and endothelinB-selective agonists.

The endothelin (ET) family of peptides acts via two subtypes of G-protein-coupled heptahelical receptors termed ETA and ETB, which have distinct rank orders of affinity to endothelin receptor agonists and antagonists. To delineate which portions of the receptor molecules determine ligand selectivity, we have constructed a series of chimeras between human ETA and ETB receptors and characterized the chimeric receptors expressed in heterologous cell lines by competitive radioligand binding analysis and by measuring agonist-induced transients of intracellular Ca2+. We demonstrate that the binding determinant for the ETB-selective agonists ET-3, BQ3020, and IRL1620 residues within the region spanning the putative transmembrane helices IV-VI and the adjacent loop regions. In contrast, the transmembrane helices I, II, III, and VII plus the intervening loop regions specify the selectivity for BQ123, an ETA-selective antagonist. BQ123 exhibited no detectable agonistic activity in all wild-type and chimeric receptors tested. A chimeric receptor that has the transmembrane helices IV-VI (and adjacent loops) from the ETB receptor inserted into the remaining regions from the ETA receptor binds both the ETA- and ETB-selective ligands with high affinities. Moreover, BQ123 competitively inhibits the binding of the amino-terminally truncated ETB agonists, 125I-BQ3020 and 125I-IRL1620, to this chimeric receptor, suggesting that BQ123 is a mimic of the carboxyl-terminal linear portion of endothelins. These findings indicate that there are at least two separable ligand interaction subdomains within the endothelin receptors.

Characterization of phosphoramidon-sensitive metalloproteinases with endothelin-converting enzyme activity in porcine lung membrane.

Endothelin-1 (ET-1), a 21 amino-acid potent vasoconstrictor peptide, is produced from the biologically inactive intermediate big ET-1 via an endoproteolytic cleavage between Trp-21 and Val-22 by endothelin converting enzyme (ECE). cDNA sequence analysis predicts that the two other members of the endothelin family, ET-2 and ET-3, are also generated from the corresponding intermediates called big ET-2 and big ET-3, respectively. The metalloproteinase inhibitor phosphoramidon inhibited the conversion of big ET-1 into mature ET-1 both in vivo and in cultured endothelial cells, suggesting that ECE may be a neutral metalloproteinase. In this study, we solubilized and partially purified ECE from the membrane fraction of porcine lung. Using gel filtration chromatography, we separated two distinct ECE activities, designated M1 (apparent molecular mass approx. 300 kDa) and M2 (approx. 65 kDa). Optimum pH for the cleavage of big ET-1 by M1 and M2 was 7.0 and 7.5, respectively. M1 efficiently converted human big ET-1(1-38) to ET-1, but not human big ET-2(1-37) or human big ET-3(1-41)-amide. In contrast, M2 converted both big ET-1 and big ET-2, but not big ET-3. M1 was inhibited by phosphoramidon (IC50 approx. 1 microM) but not by thiorphan or bacitracin. In contrast, M2 was inhibited by much lower concentrations of phosphoramidon (IC50 approx. 0.3 nM), as well as by thiorphan and bacitracin. ECE activity in M1 was able to bind to a concanavalin A-agarose column and was eluted by alpha-methyl-D-glucoside, indicating that the ECE is glycosylated. From these results, M1 and M2 from the porcine lung membrane are similar to the candidate of ECE in endothelial cells and neutral endopeptidase in kidney (EC 3.4.24.11), respectively. Taken in conjunction with the previous finding that neither thiorphan nor bacitracin affected the conversion of endogenously synthesized big ET-1 in cultured endothelial cells, we conclude that physiologically relevant ECE found in the endothelial cells is more similar to M1 than to M2.

Anorchia associated with gonadotropin deficiency.

We report an anorchic patient with normal male phenotype and 46,XY karyotype. Plasma levels of gonadotropins and testosterone were abnormally low. Administration of human chorionic gonadotropin caused no increase in plasma testosterone, and injection of luteinizing hormone-releasing hormone resulted in a lack of plasma gonadotropin response. Sex-determining region Y was detected by DNA studies of peripheral white blood cells. Laparoscopy revealed absence of testis, Wolffian and Müllerian derivatives. We discuss the embryology and hormone condition in an anorchic patient.

[A case report of aortic valve and VSD Dacron patch infective endocarditis after VSD patch closure 15 years ago].

A 39-year-old man who had undergone the patch closure of the VSD 15 years ago, was admitted with a diagnosis of infective endocarditis due to Alcaligenes Xylosoxidans. Echocardiography revealed vegetation of the aortic valve and a high echoic lesion on the ventricular septum. Surgical findings showed vegetation of the aortic valve and a subannular type mycotic aneurysm. In the aneurysm, the infected pledget used in a previous surgery was found. After debridement, direct closure of the aneurysm, aortic valve replacement (AVR) using a #25 SJM prosthetic valve, and mitral annuloplasty were performed. Two months later, fever developed. The patient was diagnosed with prosthetic valve endocarditis and a second surgery was performed. The prosthetic valve was clear, but an infected Dacron patch used for VSD closure 15 years earlier was found. Debridement, patch closure of the ventricular septum, and re-AVR were performed. The post-operative course was uneventful. This is thought to be a rare case, because infection extended from the aortic valve to the VSD type II Dacron patch, and remained to the VSD type II Dacron patch.

Nerve conduction velocities and hair concentrations of trace elements in haemodialysis patients.

Nerve conduction velocities and hair concentrations of trace elements were studied in 19 male patients with chronic renal failure undergoing haemodialysis. Both motor and sensory nerve conduction velocities were significantly lower in haemodialysis patients as compared to controls (p < 0.001). Calcium and aluminium concentrations were significantly higher in patients (p < 0.01), however, vanadium and arsenic levels were significantly lower in patients (p < 0.01). In concentrations of copper and zinc there was no significant difference between patients and controls. There were no significant correlations between hair concentrations of trace elements and nerve conduction velocities except between calcium concentration and sensory nerve conduction velocity. These facts suggest that nerve conduction velocities are not influenced by changes of trace element concentrations in hair in patients with chronic renal failure undergoing haemodialysis.

[Pheochromocytoma--basic and clinical analyses].

A variety of vasoactive substances including biogenic amines, neuropeptide Y, somatostatin, enkephalin, ACTH, corticotropin-releasing hormone, growth hormone releasing hormone, vasoactive intestinal peptide, calcitonin, and atrial natriuretic factor have been extracted from intra-adrenal and extra-adrenal pheochromocytomas in men. Some of them appear to play an important role for the development of hypertension or clinical serious symptoms. However, informations on the molecular forms of other substances in pheochromocytomas are still limited, and precise amount of the peptides or hormones in the tumors has not yet been quantitated. Numerous in vitro or in vivo studies of this documented neoplasm over the years have been reviewed in this manuscript. Clinical analyses of early diagnosis, localization diagnosis, treatment of multiple endocrine neoplasia, preoperative and operative treatments are also evaluated in this paper. These informations will probably provide additional evidence for the multi-secretory APUD cells of neural crest origin and will contribute the therapy in patients with pheochromocytoma.
