[Clinical significance of argyrophilic nucleolar organizer regions (AgNOR) and microvessel quantitation by staining for factor VIII-related antigen as prognostic indicators in stage I breast cancer].

The clinical significance of AgNOR and microvessel quantitation as prognostic indicators in Stage I breast cancer were examined. This study included 99 patients with Stage I breast cancer, with a median follow-up period of 119 months. AgNOR was stained by Crocker's method and microvessels were identified by immunohistochemical staining for Factor VIII-related antigen in formalin-fixed, paraffin-embedded primary tumors. AgNOR score related with tumor size. Microvessel count was related to estrogen receptor status. When patients were classified into two groups by the mean AgNOR score, higher scores had a tendency to be correlated with shorter overall survival. And when classified by microvessel count, higher counts were significantly associated with shorter overall, survival (p < 0.01). The microvessel count might be a useful predictor and the AgNOR score could be used to corroborate predictions for identifying patients at high risk for relapse and death in Stage I breast cancer.

Natural and subclinical Corynebacterium kutscheri infection in rats.

Distribution of Corynebacterium kutscheri was determined in 41 rats housed in a conventionally managed colony that were infected naturally and subclinically. At 2, 5, 10, 20 and 25 months after initial isolation of C. kutscheri, attempts were made to isolate C. kutscheri from 17 sites, with a new selective medium, FNC agar. In total, the prevalence (97.6%) of C. kutscheri isolation was significantly (P < 0.001) higher than the frequency (70.7%) of antibody detection. None of the rats manifested any distinct clinical signs of disease and macroscopic lesions caused by C. kutscheri were not detected. In 40 rats with subclinical infection, the organisms were most frequently isolated from the oral cavity, esophagus, cecal contents, and colon and rectum (> 95.0%). The isolation rate was next highest in the trachea, submaxillary lymph nodes, and nasal cavity (47.5 to 52.5%). The organisms hardly colonized the lung, liver, and kidney. Mean numbers of organisms found in the esophagus, cecal contents, and colon and rectum ranged from 10(3.9) to 10(4.2) CFU/g, and were significantly (P < 0.05, P < 0.01) high in comparison with those in the lung. These results indicated that many healthy rats in the naturally infected colony harbored C. kutscheri, and the organisms colonized the oral cavity, esophagus, cecal contents, and colon and rectum most frequently.

Natural habitats of Corynebacterium kutscheri in subclinically infected ICGN and DBA/2 strains of mice.

Subclinically infected mice of ICGN and DBA/2 strains housed in a conventionally managed colony were examined to determine natural habitats of Corynebacterium kutscheri. At 5, 7, 9, 12 and 13 months after initial isolation of the organism from oral cavity and cecal contents of five ICGN mice, attempts were made to isolate C. kutscheri from 19 sites using a new selective medium, furazolidone-nalidixic acid-colimycin agar. From the initial survey to 13 months, C. kutscheri was isolated from 27 of 29 ICGN mice (93.1%) and 9 of 10 DBA/2 mice (90%). In contrast, antibody against C. kutscheri was detected in only 3 of 29 ICGN mice (10.3%). None of the mice manifested distinct clinical signs of infection, and only 1 ICGN mouse had macroscopic lesions such as hepatic abscess and large spleen. In 21 ICGN and 9 DBA/2 mice that harbored the organism without macroscopic lesions, the organisms were most frequently isolated from the oral cavity (ICGN:100%, DBA/2:66.7%), cecum (ICGN:95.2%, DBA/2:100%), and colon and rectum (ICGN:95.2%, DBA/2:100%). Remarkable differences between the two mouse strains were observed in colonization of the nasal cavity (ICGN:85.7%, DBA/2:0%) and trachea (ICGN:71.4%, DBA/2:33.3%). In mice of both strains, the organisms rarely colonized the lung, liver, and kidney. Mean numbers of organisms in the cecum, and colon and rectum ranged from 10(4.1) to 10(4.6) colony-forming units/g and were significantly higher in comparison with those in the small intestine (P < 0.01, P < 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

[Immunohistochemical study of proliferation index of astrocytomas classified by new histopathological grading].

A new grading method of astrocytoma, proposed by Daumas-Duport et al, was based on four criteria:nuclear atypia, mitosis, necrosis and endothelial proliferation (0 criteria = Grade 1, 1 criteria = Grade 2, 2 criteria = Grade 3, 3 or 4 criteria = Grade 4). To elucidate the correlation of the classification based on four criteria, cellularity and proliferative activities, we studied proliferating index of 67 astrocytomas, using immunohistochemical staining for Ki-67 antibodies (MIB-1). A new grading method was related to proliferating index in this study, however, cellularity of tumor cells was not related to proliferating index. In this study, endothelial proliferation was invariably seen in high grade(Grade 3 and 4) astrocytomas. These tumor vessels were associated with dysfunction of blood-brain barrier and some growth factors accelerated to tumor cell proliferation. Relation between histopathological grade of astrocytoma and proliferation index was supposed, and new grading system was thought to be useful for routine pathological examination.

[Neuroblastoma].

Neuroblastoma is the most common and highly malignant tumor. The 2-year survival rate for NB patients for 1970s was 32% in US and 29% in Japan. But, improvement of prognosis was observed by recent advances in surgery, chemotherapy and numerous other supportive therapies. We introduce the some treatment regimens to patients with neuroblastoma which should be selected by the age and the stage at diagnosis and other prognostic factors such as N-myc amplification, trk overexpression, chromosome anomalies (lp-. double minutes, homogeneous staining region) of neuroblastoma cells and histological pathology. As a general rules, patients under 1 year of age without unfavorable prognostic factors should be treated less intensive regimen, even their tumors are progressive stages. Conversely, patients with progressive stages over 1 year of age without unfavorable factors, it is necessary to treat with intensive protocol. Furthermore, to patients of all age group with unfavorable factors, they are given a very strong intensive treatment through advances in supportive therapies such as the new antiemetics, G-CSF, antibiotics, or IVH etc.. Recent treatment regimens to the patients with neuroblastoma are presented.

[Endoscopic treatment of colorectal cancer].

Endoscopic polypectomy should be applied only for early colorectal carcinomas. Intramucosal carcinoma do not have a risk of lymph node metastases. However, there is an about 10% risk of lymph node metastases among carcinomas showing submucosal invasion (sm carcinoma). When risk factors revealed to be positive after polypectomy, subsequent surgical resection of the large bowel with lymph nodes dissection is needed, because these sm carcinomas are considered to have a high risk of lymph node metastases. Therefore, accurate diagnosis of depth of invasion is essential to prevent subsequent surgical resection following endoscopic polypectomy. Endoscopy, barium enema and endoscopic ultrasonography (EUS) are all considered to be effective for an accurate diagnosis of depth of invasion. Endoscopic polypectomy includes hot biopsy, snare polypectomy and endoscopic mucosal resection (EMR). Appropriate maneuver must be chosen, considering the characteristics of the lesion. Major complications after endoscopic polypectomy are bleeding and perforation of the large bowel. Including an establishment of a new risk factors, further efforts must be made to prevent unnecessary additional surgical resection of the large bowel following endoscopic polypectomy.

[A case report: postoperative recurrence of peritoneal dissemination of gastric cancer responding to sequential methotrexate and 5-FU (5-fluorouracil)].

In a 61-year-old female patient, the recurrence of peritoneal dissemination after total gastrectomy due to gastric cancer responded well to chemotherapy of sequential methotrexate and 5-FU. A total of 10 courses of this chemotherapy diminished ascites, normalized the value of CA 19-9, and re-opened the left obstructed ureter. During this therapy, the patient's condition was good, with no experience of nausea or leukopenia.

Successful allogeneic bone marrow transplantation in a case with myelodysplastic syndrome which developed following Fanconi anemia.

We report the case of a 14-year-old boy with myelodysplastic syndrome (MDS/RAEB) which developed following Fanconi anemia. The patient received BMT from an HLA-identical sister. Based on the in vitro CY-sensitivity test, 100 mg/kg of CY was administered for conditioning combined with 6 Gy TBI. Mucosal symptoms such as stomatitis, diarrhea and hematuria were severe, but manageable, and engraftment was successful. The patient has maintained normal trilineage hematopoiesis with > 90% Karnofsky score for 30 months with disappearance of a clonal chromosomal abnormality (47,XY, +i(lq)) which was detected before BMT.

Detection and partial characterization of Pasteurella multocida found in feline skin lesions.

Pasteurella multocida was isolated from 21 of 105 purulent skin lesions in household cats. The bacterium was in pure culture in nine specimens and predominant in six specimens. Its viable counts were 10(2) to 10(7) colony forming units/ml. Of 21 isolates of P. multocida, seventeen were considered to be capsular type A. The predominant capsular and somatic type was the serotype A:3,4. Inoculation of the filter-sterilized supernatants of the isolates induced an erythematous response in guinea pig skins. These findings suggest that P. multocida is a candidate as a pathogen of feline skin lesions and the erythema-inducing activity of the bacterium may participate in the formation of skin lesions in household cats.

Incidence and prevalence of inflammatory bowel disease in Japan: nationwide epidemiological survey during the year 1991.

The aim of this nationwide study was to determine the recent incidence and prevalence of inflammatory bowel disease, i.e., Crohn's disease (CD) and ulcerative colitis (UC), in Japan. We mailed out a preliminary examination sheet with diagnostic criteria, asking about the presence of patients with inflammatory bowel disease, to all hospitals in Japan that have more than 200 beds for general use. The rate of reply was 60.93%. A total of 4243 patients with CD were reported. The incidence per 100 000 population per annum was 0.51 (0.71 in males, 0.32 in females). The prevalence per 100 000 population per annum was 5.85 (7.94 in males, 3.83 in females). Peak age at onset was 20-24 years in males and 15-19 years in females. A total of 12559 cases of UC were reported. The incidence per 100 000 population per annum was 1.95 (2.23 in males, 1.68 in females). The prevalence per 100 000 population per annum was 18.12 (18.70 in males, 18.17 in females). Peak age at onset was 20-24 years in males and 25-29 years in females.

Total colectomy and ileorectal anastomosis in ulcerative colitis.

In an attempt to determine the best indications for the classically adopted ileo-rectal anastomosis (IRA) and the new techniques of restorative proctocolectomy, namely, ileal J-pouch-anal anastomosis (IAA) ilea J-pouch-anal canal anastomosis (IACA), we retrospectively studied 72 surgically treated patients with ulcerative colitis (UC) followed in our surgical department in the period between 1963 and 1994. Compared to these new techniques, IRA had a lower incidence of postoperative fecal incontinence, and was one-stepped in the majority of the patients. No significant difference regarding postoperative bowel function, operation time, volume of bleeding, hospital stay, and the need for postoperative prednisolone was observed. We concluded that IRA is a good procedure that is indicated for patients receiving high-dose prednisolone, for those who need a quick return to social activity, and for those with poor anal function. IACA is a good indication for those patients with good anal function assessed preoperatively, who agree to receive a multi-step operation. For those patients with cancer or dysplasia, IAA should be the operation of first choice.

Results of cancer surveillance in ulcerative colitis.

A total of 144 patients with total or left-sided colitis of 7 years or more duration were enrolled in a surveillance program to screen for dysplasia at Tokyo University Hospital between 1979 and 1994. The program consisted of an annual colonoscopy, with multiple biopsies being performed at 10-cm intervals in the colonic flat mucosa; additional biopsies were taken from elevated lesions. A higher proportion of patients who had had four surveillance colonoscopies or more up to the detection of carcinoma had an earlier stage of carcinoma compared with patients who had had less than three colonoscopies. In addition, the incidence and stage of carcinoma were higher in the patients with an initially higher grade of dysplasia and in those patients with elevated lesions compared with patients with flat mucosal lesions with the same grade of dysplasia. Microspectrophotometric DNA analysis was performed for 66 consecutive patients who had been under surveillance between 1987 and 1990. Dysplasia and carcinoma occurred at a higher rate in patients who had an initial abnormal DNA content compared with patients who showed diploidy. These findings indicate that surveillance colonoscopy with DNA analysis could be useful in the early detection of colonic carcinoma in long-standing ulcerative colitis.

Platelet-activating factor induces cell growth through tyrosine phosphorylation pathway in cultured rat mesangial cells.

Accumulating evidence suggests that platelet-activating factor (PAF) may play a role in renal pathophysiology. Therefore, in order to investigate this notion further, the effects of PAF on cell growth and tyrosine phosphorylation were analyzed in cultured rat mesangial cells. PAF was found to enhance a time and concentration-dependent increase in phosphotyrosine in several proteins and stimulate 3H-thymidine incorporation. Tyrosine phosphorylation was also enhanced by PAF in protein kinase C (PKC) depleted cells, whereas a tyrosine kinase inhibitor, genistein, inhibited tyrosine phosphorylation of these proteins at the concentration of 1 microgram/ml. PAF stimulated 3H-thymidine incorporation at concentrations below 10(-6) M, but exerted progressive inhibition at concentrations above 10(-6) M. Pre-treatment with phorbol 12-myristate 13-acetate (PMA) did not affect PAF-enhanced incorporation at lower concentrations of PAF, and reversed the inhibitory effects of PAF at higher concentrations. Finally, genistein pre-treatment completely inhibited PAF-induced cell growth at the concentration of 1 microgram/ml. Both tyrosine phosphorylation and 3H-thymidine incorporation induced by PAF were completely inhibited by pre-treatment with the PAF-receptor antagonist, CV-6209, at the concentration of 10(-5)M. These results suggest that PAF enhancement of tyrosine phosphorylation occurred in a PKC-independent manner and that a tyrosine kinase was associated with PAF-induced tyrosine phosphorylation. Moreover, they indicate that the phosphoinositide hydrolysis-PKC pathway is not essential for PAF-induced cell proliferation, and that PKC activation may play an inhibitory rather than a stimulatory role in mitogenesis in response to PAF. Our results indicate that the tyrosine phosphorylation pathway induced by PAF may participate critically in downstream mitogenic signaling through the PAF receptor.

Increased expression of sialyl Lewis A and sialyl Lewis X in liver metastases of human colorectal carcinoma.

Sialyl Lewis A (SLA) and sialyl Lewis X (SLX) have been shown to be specific ligands for endothelial leukocyte adhesion molecule-1 (ELAM-1), and may be involved in the process of adhesion between cancer cells and endothelium. We used immunohistochemical methods to study the expression of SLA, SLX and CEA in both primary tumors and matched metastatic liver lesions of colorectal carcinomas. Specimens from primary tumors and matched liver metastases from 24 patients with colorectal carcinomas were studied immunohistochemically. The degree of expression of CEA in liver metastases was similar to that in primary tumors, but SLA and SLX were expressed on a larger proportion of tumor cells in liver metastases than in primary tumors. Our findings suggest that colorectal carcinoma cells expressing SLA and/or SLX form metastatic liver tumors. They also suggest that expression of SLA and SLX in primary of colorectal carcinoma can be used as a prognostic indicator of metastasis.
