Immunohistochemical examination of rat skeletal muscle during graft-versus-host disease (GVHD), a systemic immune reaction, was performed to investigate specific immune reactivities focusing on major histocompatibility complex (MHC) expression and inflammatory cell infiltration of skeletal muscle during a systemic immune reaction. MHC class II expression and inflammatory cell infiltration did not increase. MHC class I was expressed along the contour of muscle fibres, and most strongly expressed by the cells which were distributed throughout the endomysium and perimysium. Seventy-six percent of these MHC class I+ cells carried endothelial cell-markers, while 24% of them did not. The latter cells were revealed not to be inflammatory cells such as lymphocytes, granulocytes or macrophages when examined by immunostaining using several exudate-cell markers. Neither were they myosatellite cells because they were located outside the basement membrane. These results may be useful for considering animal models of inflammatory myopathies such as polymyositis and dermatomyositis.

A-68-year-old man had hepatocellular carcinoma (HCC) in the area of S6 segment which was resected surgically. Three months after surgery, multiple recurrent lesions were found in both lobes of the liver. A transcatheter arterial embolization (TAE) with 5-FU and zinostatin stimalamer (SMANCS) was done. After TAE, the tumor mass disappeared in the right lobe and reduced to 10% in the left lobe. Although mass lesions remained unchanged for 3 months, the increased of AFP (143.9 ng/ml) was noticed after 4 months. Ten months after the second TAE with 5-FU and SMANCS, the disappearance of tumor masses was confirmed by diagnostic images.

We describe a new totally extracorporeal laparoscopy-assisted technique without insufflation of the peritoneum in the treatment of benign and malignant ascending and sigmoid colon lesions. Colon resection is performed using specially designed instruments that allow the surgeon to avoid insufflation of the peritoneum and, in case of cancer, allow for conventional lymph-adenectomy. With this new technique, the restrictions associated with the laparoscopic technique are nullified, and the surgeon can directly examine intra-abdominal contents. Our preliminary results are encouraging and demonstrate the feasibility of this technique as well as its positive results in terms of curability in selected cases of colon cancer. Additional experience will delineate the real clinical value of this technique in the treatment of malignant disease.
An 8 year old boy with adrenoleukodystrophy and his asymptomatic brother, both with abnormal electro-encephalography as the initial sign of adrenoleukodystrophy, are reported. After suffering from aseptic meningitis caused by Echo virus 30, the first case was suspected of having a complication of meningitis because of persistent abnormal electro-encephalography (i.e. slow wave bursts in the occipital region). This patient was finally diagnosed as having adrenoleukodystrophy because of the increased level of very long chain saturated fatty acids, and typical occipital lesions demonstrated by magnetic resonance imaging and computed tomography. Dietary therapy was tried, but the course was progressive. The brother's electro-encephalography was also abnormal and so was diagnosed as having asymptomatic adrenoleukodystrophy because of an increased level of very long chain saturated fatty acids, although brain magnetic resonance imaging was normal. Abnormal electro-encephalography can therefore be an initial sign of adrenoleukodystrophy even if magnetic resonance imaging shows no abnormality in the brain.

Ten cases of newly diagnosed pediatric B cell non-Hodgkin's lymphoma or acute lymphoblastic leukemia (B-NHL, stage I & II 6 cases, stage III & IV 2 cases/ALL 2 cases) experienced during the last 7 years (1987-1994) were treated by BLK88 protocol, which consisted of HD-CPM (1,200 mg/m2), and HD-MTX (1,000 mg/m2) with VCR, ADR, and/or AraC combination, and CNS prophylaxis by triple intrathecal injection. The therapy duration was 24 weeks for B-NHL (36 weeks for B-ALL). The results showed that while one of the six cases in stage I & II relapsed, and other 4 cases of stage III & IV B-NHL/ALL remained in complete remission. On the other hand, all of the four cases in stage III & IV in historical controls had relapsed. Neutropenia and liver dysfunction were observed during therapy, but they were tolerable. We conclude that BLK88 is a very useful protocol for B-NHL/ALL in childhood.
Periventricular leukomalacia (PVL) is one of the neuropathological varieties of the neonatal hypoxic-ischemic encephalopathy. Recently we established a new canine model of PVL, by ligation of bilateral common, external and internal carotid arteries. In this paper we describe the relationship between neuropathological and neurological/behavioral findings of the PVL dogs.

Familial aggregation of atherosclerotic risk factors is well known, however inter-spouse relation of the risk factors has remained controversial. As spouses are not blood-related, any risk factor relation probably does not reflect genetic factors, but the environmental factors to which the couple is exposed. Moreover, the effect of assortative marriage should be considered. The present study enrolled 97 married couples who attended a general health screening in 1993 in a mountain village of Japan. The screening was conducted between April and November at a single outpatient clinic in the village. Data on weight, height, blood pressure (BP), serum risk factors for atherosclerosis and marriage year were obtained as part of the screening programme. They were divided into three groups according to marital duration. The simple correlation coefficients for systolic/diastolic BP between the spouses were 0.22/0.19. The coefficients declined to 0.05/0.08 after controlling. The correlation coefficients for serum lipoproteins were low and between -0.06 and 0.11 after controlling. The correlation coefficients for blood sugar were -0.05 and -0.13 after controlling. In subgroup analyses stratified by marital duration, the correlation coefficients for BP, body mass index (BMI) and blood sugar were increased in both the short and long marital duration groups. The correlation coefficients for high-density lipoprotein (HDL) cholesterol and triglycerides increased with longer marital duration. Although a larger population study will be needed to confirm these findings, these results suggest the presence of interspouse correlation and reflect the influence of assortative marriage and environmental factors on spouse concordance.
The prognosis of pancreatic cancer is extremely poor in digestive cancers even with systemic therapy including surgery. Thus development of a new therapeutic approach is awaited. Recently, new trials of cancer treatments using monoclonal antibodies conjugated with anti-cancer drugs or labeled with isotopes have been reported. Here, we discuss the possible therapeutic application for pancreatic cancer of adriamycin-conjugated and 131I-labeled Nd2, which is produced against pancreatic cancer mucin.
The reproducibility of the measurement (accuracy of repeated measurement) of intraocular lens (IOL) location in situ was studied using an anterior eye segment analysis system (NIDEK EAS-1000). The measurements were performed as follows: (1) errors at photography: 1) errors by the individual photographer 2) errors among three photographers. (2) errors at analysis: 1) errors by individual analysists 2) errors among six analysists. The greatest standard deviation was the accuracy of repeated measurement. Accuracy of repeated measurement (tilt and decentration) was (1) 1) 1.104 degrees, 0.083 mm, (1) 2) 0.866 degrees, 0.072 mm, (2) 1) 0.597 degrees, 0.079 mm, (2) 2) 0.562 degrees, 0.079 mm. From the data obtained in this study, the reproducibility of the EAS-1000 was satisfactory for measurement of IOL location.
Prophylaxis against human T cell lymphotropic virus type I (HTLV-I) is of primary importance for the eradication of adult T cell leukemia and other diseases associated with this virus. Hyperimmune globulin (H-IgG) prepared from healthy blood donors with high antibody titers for HTLV-I was evaluated for its prophylactic effect against HTLV-I in Japanese macaques (Macaca fuscata). Normal IgG (N-IgG) prepared from seronegative healthy blood donors was used as control. Both preparations contained 50 mg/ml IgG and H-IgG had a neutralizing antibody titer of 1:7100 by vesicular stomatitis virus (HTLV-I) pseudotype neutralization assay. Two macaques were infused with 2 ml/kg N-IgG and three macaques were immunized with 2-0.5 ml/kg H-IgG. They were immediately challenged by inoculation of 8 x 10(6)/kg cells from an HTLV-I-producing rabbit lymphoid cell line (Ra-1). Another macaque was immunized with 1 ml/kg H-IgG 24h after inoculation of 8 x 10(6)/kg Ra-1 cells. HTLV-I infection, as determined by seroconversion and verified by polymerase chain reaction, occurred in both of the N-IgG-injected macaques but in none of the four H-IgG-injected macaques. These results demonstrate the protective efficacy of H-IgG against HTLV-I infection in a primate model and provide an experimental basis for passive immunization trials in humans.
Mesangial expansion is one of the histological abnormalities observed among the various glomerular diseases. It is due largely to the accumulation of extracellular matrix (ECM) proteins, such as type IV collagen, laminin and fibronectin. In order to examine the abilities of mesangial cells to produce type IV collagen, we used enzyme-linked immunosorbent assay to measure type IV collagen secreted from cultured rat mesangial cells. After exposing confluent cells to endothelin-1 (ET-1, 0.01 microM), insulin-like growth factor-1 (100 ng/ml) angiotensin-II (1 microM) and transforming growth factor-beta (1 ng/ml) for 24 hours, type IV collagen production increased 1.5 approximately 2.5 times. ET-1 stimulated type IV collagen production in a dose-dependent fashion, and this effect was blocked by the protein kinase C inhibitor, staurosporine. Atrial natriuretic peptide (ANP) inhibited the increase in type IV collagen production caused by ET-1 in a dose-dependent fashion. Addition of 8-bromo-cGMP similarly suppressed the stimulation of type IV collagen production caused by ET-1. These findings indicate that ET-1 stimulates type IV collagen synthesis in cultured rat mesangial cells by a mechanism probably involving activation of PKC, and that ANP inhibits ET-1-induced production of type IV collagen through a cGMP-dependent process.
We described a 60-year-old man with 5-year history of insulin dependent diabetes mellitus who developed continuous rigidity of truncal muscle and painless, rhythmic muscular spasm of trunk and proximal lower and upper extremities. The rigidity continued even in sleep. The painless muscle spasm was often precipitated by volitional movement and emotional stimuli. Intravenous administration of diazepam strongly attenuated the muscle spasm as well as truncal rigidity. Surface electromyography showed the continuous contraction of abdominal and paraspinal muscles. The rhythmic, clonic spasm of shoulder, triceps brachii, intercostal, abdominal, paraspinal and quadriceps femoris muscle induced by voluntary neck flexion was not compatible with typical stiff-man syndrome. Antibody against glutamic acid decarboxylase (GAD) was detected in the serum and cerebrospinal fluid of this patient. His condition was getting well with oral intake of sodium valproate. While painless, rhythmic spasm and persistent rigidity during sleep ruled out the patient from typical stiff-man syndrome, he was supposed to have the same pathophysiological mechanism as the anti-GAD autoantibody positive stiff-man syndrome.
