Synergistic induction of anchorage-independent growth of NIH3T3 mouse fibroblasts by cysteine proteinase inhibitors and a tumor promoter.

We have previously reported that Ras protein is a potent cysteine proteinase inhibitor. In order to examine whether the cysteine proteinase-inhibitory activity of Ras is involved in carcinogenesis, the effects of the following probes were investigated. Cystatin alpha is a cysteine proteinase-specific inhibitor and has some amino acid sequence homology with Ras. Ras has a CAAX motif (C, cysteine; A, aliphatic amino acid; X, any amino acid) at the carboxyl terminus, which is indispensable for the biological activity. Thus, cystatin alpha carrying a CAAX motif (cystatin alpha-CVLS) was examined. A v-Ha-Ras deletion mutant, Ras delta 42-49, has undetectable GTP binding activity, yet it retains a similar protease inhibitory activity to that of wild-type v-Ras. These genes were inserted into a eukaryotic inducible expression vector and transfected into NIH3T3 cells. The expression was effectively induced by treatment with a glucocorticoid hormone, dexamethasone. The expression of cystatin alpha-CVLS or Ras delta 42-49 alone induced neither transformation nor morphological changes. However, when their expression was induced in the presence of a tumor-promoting phorbol ester, a remarkable increase in the anchorage-independent growth was observed in cystatin alpha-CVLS- and Ras delta 42-49-transfected clones. These results suggest that cysteine proteinase inhibitors and a tumor promoter synergistically transformed NIH3T3 cells. It is thus possible that the cysteine proteinase-inhibitory activity of Ras might play a key role in the early stage of carcinogenesis.

Increasing gonadotropin-releasing hormone release by perifused hypothalamus from early to late anestrus in the beagle bitch.

An in vitro perifusion system was used to investigate pulsatile gonadotropin-releasing hormone (GnRH) release from hypothalamic fragments derived from beagle bitches at different stages of the estrous cycle. The spontaneous GnRH release from the excised tissue fragments that include the "mediobasal hypothalamic-preoptic area-suprachiasmatic nucleus units' was episodic throughout all stages of the estrous cycle with a significantly high release rate during late anestrus and late proestrus. The GnRH release rate and plasma levels of luteinizing hormone were positively correlated (r = 0.94, P < 0.01). These results suggest that during the course of anestrus in the bitch the GnRH release rate increases while the pituitary responds accordingly.

Prognostic value of vascular endothelial growth factor expression in gastric carcinoma.

BACKGROUND: Many studies have shown that angiogenesis plays an important role in the growth, progression, and metastasis of solid tumors. Recently, several angiogenic factors have been identified. Vascular endothelial growth factor (VEGF) is thought to be one such angiogenic factor and is also thought to be a selective mitogen for endothelial cells. We investigated the correlation between the expression of VEGF and the progression of gastric carcinoma. METHODS: One hundred twenty-nine specimens resected from patients with gastric carcinoma were investigated by staining with a polyclonal antibody against VEGF. Correlations between the expression of VEGF, microvessel density, and various clincopathologic factors were studied. RESULTS: Microvessel density, determined by immunostaining for Factor VIII related antigen, was significantly higher in VEGF-positive tumors than in VEGF-negative tumors. VEGF positivity was correlated with vessel involvement, lymph node metastasis, and liver metastasis. Moreover, patients with VEGF-positive tumors had a significantly poorer prognosis than those with VEGF-negative tumors. Multivariate analysis indicated that the expression of VEGF is an independent prognostic factor in patients with gastric cancer. According to the mode of recurrence, the frequency of hepatic metastases was significantly increased among patients with VEGF-positive tumors. CONCLUSIONS: The expression of VEGF may be a good prognostic indicator for patients with gastric carcinoma and may also be useful as a predictor of the mode of recurrence in patients with gastric carcinoma.

Flat adenoma as a precursor of colorectal carcinoma in hereditary nonpolyposis colorectal carcinoma.

BACKGROUND: Because flat adenoma shows a higher malignancy rate compared with other types of polyps, it is considered to play an important role in the carcinogenesis of colorectal carcinoma. In the present study, we examined flat adenomas in hereditary nonpolyposis colorectal carcinoma (HNPCC) patients. METHODS: Nine HNPCC patients who presented with flat adenomas were examined. All patients underwent either surgery or endoscopic polypectomy for colorectal carcinoma and/or adenoma. In all patients, annual colonoscopy had been performed once a year throughout the follow-up period after the initial treatment. When colorectal polyps were detected during follow-up colonoscopy, all lesions were endoscopically excised. Clinicopathologic features and microsatellite instability of both malignant lesions and adenomas were examined. RESULTS: Thirteen malignant lesions were detected: seven advanced carcinomas and six early carcinomas. Among 4 early carcinomas with submucosal invasion, 3 lesions (75%) were categorized as superficial type, with a configuration similar to flat adenoma. The frequency of flat adenoma was strikingly high in HNPCC patients in the present study. Among 73 polyps detected, 37 (50.7%) were flat adenomas. Both malignant lesions and flat adenomas had proximal predominance, 61.5% and 59.5%, respectively. Eleven of 15 lesions (73.3%) showed replication error. CONCLUSIONS: These results suggest the importance of flat adenoma as a precursor of colorectal carcinoma in some groups of HNPCC patients. Further study is essential to elucidate the natural history of flat adenomas in HNPCC patients.

HTLV-I-seronegative, genome-positive adult T-cell leukemia: report of a case.

An HTLV-I-seronegative case of adult T-cell leukemia (ATL) carrying the HTLV-I genome is reported. Screening serological tests were negative and Western blot analysis revealed only a faint band for HTLV-I p24. Polymerase chain reaction (PCR) disclosed the presence of HTLV-I gag, pol, env, pX, and LTR sequences in the lymph node and peripheral blood. Southern blot analysis revealed a monoclonal integration of HTLV-I in the lymph node and peripheral blood. The tumor cells expressed viral antigens after short-term culture. The clinical course was consistent with ATL in that the patient exhibited hypercalcemia and abnormal lymphocytosis as well as hepatosplenomegaly and lymphadenopathy. We recommend that PCR analysis for HTLV-I be performed even in seronegative cases when ATL is clinically suspected.

Ultrastructural characterization of internal basement membrane of junctional epithelium at dentogingival border.

BACKGROUND: Although a number of electron microscopic observations of the dentogingival border have been reported, observation of the detailed ultrastructure of the internal basement membrane of the junctional epithelium at the border has not yet been done. METHODS: The internal basement membrane of a rhesus monkey (Macaca mulatta) was ultrastructurally characterized with high-resolution electron microscopy. The head and neck region of a 5-year-old animal was perfused with a fixative containing 1.25% glutaraldehyde, 0.75% formaldehyde, and 0.04% CaCl2 in 0.1 M phosphate buffer, pH 7.4. Isolated mandibular segments were further fixed with 2.5% glutaraldehyde, 2% formaldehyde, and CaCl2 (300 mg/l) in 0.1 M sodium cacodylate buffer, pH 7.4, for 24 hr at 4 degrees C. The segments were then demineralized and cut into small pieces that were postfixed with 1% osmium tetroxide and further processed for electron microscopy. RESULTS: The internal basement membrane takes the form of both a common basement membrane and, in places, a multilayered basement membrane. This basement membrane was found to have fundamental ultrastructural features common to other basement membranes. That is, the bulk of the basement membrane is composed of a network of irregular anastomosing strands referred to as "cords." Heparan sulfate proteoglycan-containing ribbonlike 4.5-5-nm-wide "double tracks" were also present in this basement membrane. However, the lamina densa of this basement membrane was unusually thick (160 nm) compared with the thickness (30-60 nm) in other basement membranes, and the cords were also much thicker (8.3 +/- 1.9 nm vs. the usual 3-5 nm). CONCLUSIONS: From these results, it is concluded that this particular basement membrane is specialized for mechanical strength. This specialization is likely to be suitable for the purpose of the tight sealing of the periodontal tissues from the oral environment.

Analysis of fetal rat liver using monoclonal antibodies.

BACKGROUND: In previous studies, we developed several monoclonal antibodies (mAbs) against fetal and adult rat liver to analyze the hemopoietic microenvironment of the fetal liver during the gestational period. In this study, we have developed two new mAbs against fetal rat liver cells and have examined the characteristics at various gestational ages of fetal liver and of adult liver. METHODS: The characteristics of these monoclonal antibodies were demonstrated by examining several tissues using immunohistochemical staining and flow cytofluorometry. RESULTS: Monoclonal antibodies HAM10 and HAM11 were developed against fetal rat liver cells. These reacted with the cytoplasm of fetal and adult hepatocytes. HAM10 antigen expression was strong at approximately day 18 of gestation in the active period of hemopoiesis in fetal rat liver but was much lower in adult liver. HAM10 antigen expression also increased in liver after partial hepatectomy and was reduced abruptly to a normal level thereafter. HAM11 antigen expression in fetal liver was weaker than that of HAM10 antigen expression. The degree of HAM11 antigen expression increased as gestation proceeded, reaching a maximum in adult liver. CONCLUSIONS: Both HAM10 and HAM11 antigens may play a role in the morphogenesis of hepatocytes and in the hemopoietic microenvironment for hemopoietic cells. Moreover, HAM10 antigen is may play a role in hepatocyte proliferation in the fetal liver, whereas HAM11 antigen may contribute to the maturation of fetal- to the adult-type hepatocytes.

Interleukin-6 and stress hormone responses after uncomplicated gasless laparoscopic-assisted and open sigmoid colectomy.

PURPOSE: Laparoscopic colectomy has increasingly been advocated as an option for treatment of colonic disease. The purpose of this study was to compare effects of laparoscopic-assisted sigmoid colectomy (LAS) and conventional open colectomy (OPEN) on postoperative cytokine and stress hormone responses. METHODS: Fourteen patients with sigmoid colon cancer, apparently free of preoperative complications, were analyzed. Patients in both groups underwent sigmoid colectomy with lymphadenectomy. LAS was performed by the gasless abdominal wall-lifting method. A 5 cm incision was placed at the beginning of the operation. Blood samples were taken preoperatively and postoperatively for measurement of interleukin-6, glucagon and C-reactive protein. Urinary catecholamine excretions were also determined postoperatively. RESULTS: The two groups of patients were similar with respect to age (61 +/- 7 for LAS vs. 64 +/- 9 for OPEN) and sex. Intraoperative blood loss did not differ significantly between groups (112 +/- 97 ml for LAS vs. 366 +/- 380 ml for OPEN). Operative times for LAS tended to be longer than those for OPEN (231 +/- 67 vs. 169 +/- 45 minutes; P = 0.08). Similar time courses of postoperative interleukin-6, C-reactive protein, and stress hormone responses were observed in both groups. No significant differences were observed in the magnitude of changes except that the serum interleukin-6 level on day of surgery (postoperative day 0) was significantly higher in LAS patients than in those receiving OPEN. In addition, interleukin-6 levels showed a significant positive correlation with operative duration (r = 0.582; P < 0.05). CONCLUSIONS: Data suggest that stress responses after sigmoid colectomy, in patients undergoing LAS, are comparable with those of patients receiving OPEN and that the early interleukin-6 response after surgery appears to be associated with operative time.

Overexpression of p53 protein and histologic grades of dysplasia in colorectal adenomas.

PURPOSE: To clarify the relation between tumor-suppressor gene p53 expression and histologic grades of dysplasia in colorectal adenomas, we performed immunohistochemical analysis in a series of 59 colorectal polyps and 40 advanced carcinomas. METHODS: Adenomatous polyps were stained by hematoxylin and eosin and classified into mild, moderate, and severe dysplasia (intramucosal carcinoma), according to the World Health Organization's classification. RESULTS: p53 was positive in 7.1 percent (2/28) of mild, 29.4 percent (5/17) of moderate, and 62.5 percent (5/8) of severe dysplasia. In submucosal and advanced carcinomas, positivity rates were 75 percent (3/4) and 47.5 percent (19/40), respectively. Different staining patterns were found, according to grades of dysplasia. In the adenomas with mild or moderate dysplasia, a few focal crypts showed localized p53-positive staining. Adenomas with severe dysplasia had two different staining types. One was a focal staining type as shown in mild or moderate dysplasia; the other was a diffuse staining type, in which glands with mild or moderate dysplasia, surrounding severe dysplasia area, were also stained. Submucosal and advanced carcinomas showed a strong positive staining in cancer cells only. CONCLUSIONS: Overexpression of p53 protein in adenomas with mild or moderate dysplasia and existence of two types of expression in adenomas with severe dysplasia were observed. These facts suggested the possible existence of different pathways in the adenoma to carcinoma progression.

Reversible suppression of pituitary-testicular function by a sustained-release formulation of a GnRH agonist (leuprolide acetate) in dogs.

Pituitary-testicular function was studied in 15 dogs following treatment with a sustained-release formulation of a GnRH agonist, leuprolide acetate (LA). Adult male dogs were treated with a single subcutaneous injection of microencapsulated LA (0.1 or 1 mg/kg). Treatment with LA at a dose of 1 mg/kg resulted in decreased (P<0.001) ejaculatory volume and disappearance of morphologically normal spermatozoa within 8 wk and the effect persisted for 6 wk, while the 0.1 mg/kg dose was not adequate to effect suppression of spermatogenesis. The larger dose treatment (1 mg/kg) caused a transient rise in plasma levels of LH and testosterone followed by a marked decline to below the normal level by 2 wk, the low levels being maintained for at least 5 wk, indicating a prolonged effect of LA treatment on pituitary-gonadal axis. Twenty weeks after treatment with LA, a complete return to normal spermatogenesis was observed. The full reversibility of spermatogenesis in the dog after LA treatment suggests that this peptide could be used as a reversible method of male contraception.

Lenńox-Gastaut syndrome associated with leukoencephalopathy.

We report a 13-year-old boy with Lennox-Gastaut syndrome (LGS) associated with leukoencephalopathy. He was diagnosed with unclassified acute lymphocytic leukemia at the age of 3 years. After initial chemotherapy, he received intravenous methotrexate (total dosage 1,035 mg), intrathecal methotrexate (total dosage 221 mg), and whole brain irradiation (2,400 cGy). From about the age of 8 years, he developed slurred speech, hyperactivity, and mental deterioration. Cranial CT revealed calcification of the subcortical white matter. At age 9 years, he exhibited tonic seizures and atonic seizures. EEG showed diffuse slow spike-wave discharges, which are characteristic of LGS. Although multiple antiepileptic drugs have been prescribed, the frequency of seizures remains unchanged and his mental state is becoming progressively worse.

Seizure susceptibility during recovery from hypercapnia in neonatal dogs.

Seizure susceptibility during recovery from hypercapnia was investigated in seven anesthetized neonatal dogs; 13, 20, or 30% CO2 gas was administered for 30 min through a ventilator to result in three levels of hypercapnia in which measured PaCO2 values were approximately 70, 100, and 140 mm Hg. Thereafter, the animals were allowed to recover for 45 min; during this recovery phase, electrocorticography was performed. In five of seven dogs, approximately 1.5 Hz slow irregular spike and wave bursts appeared at 6 min after abrupt withdrawal from hypercapnia and lasted several minutes. This seizure activity was followed by a brief period of electrical suppression. This phenomenon was most often observed during the recovery from moderate hypercapnia and between the PaCO2 values of 100 and 50 mm Hg. When seizure activities appeared in the electrocorticogram, arterial blood pressure increased -40 mm Hg from the preseizure level. These results suggest that neonatal seizures may occur during recovery from hypercapnia.

Loss of Raf-1-binding activity of v-Ha-Ras by the deletion of amino acid residues 64-72 and 143-151.

In order to elucidate the molecular events in signal transduction, examination of the interaction between Ras and Raf-1 seems crucial. Many Raf-1 mutants have been investigated in terms of their binding activities to Ras, where only a few Ras mutants have been examined thus far. We have investigated the Raf-1-binding activities of v-Ha-Ras and 21 insertion/deletion mutants of this protein. The results show that the mutants have varying levels of Raf-1-binding activity that are related neither to their transforming activity nor to their guanine nucleotide-binding activity. Deletion in the effector domain of Ras did not completely abolish Raf-1-binding, whereas the deletion in amino acid residues 64-72 or 143-151 resulted in complete loss of Raf-1-binding activity.

Postoperative decentration of three-piece silicone intraocular lenses.

PURPOSE: To compare the postoperative decentration of three-piece silicone intraocular lenses (IOLs) with polymide haptics and with polypropylene (PP) haptics and to determine whether the difference was caused by the haptic material. SETTING: Kimura Eye and Internal Medicine Hospital and Hitani Eye Clinic, Hiroshima, Japan. METHODS: Using a previously described photographic method, we calculated the postoperative decentration in randomly selected patients who had phacoemulsification and capsular bag implantation of three-piece silicone IOLs. In 64 patients, the IOL had polyimide haptics and in 57, PP haptics. One surgeon performed all the surgery using the same technique. RESULTS: Mean decentration of IOLs with polyimide haptics was 0.34 mm +/- 0.17 (SD) and of those with PP haptics, 0.48 +/- 0.30 mm. Although follow-up of the polyimide group was longer than that of the PP group (12.8 +/- 5.7 months and 8.7 +/- 4.0 months, respectively), decentration in the polyimide group was significantly smaller (P < .01). CONCLUSION: The lower rate of postoperative decentration in the IOLs with polyimide haptics was related to the haptic material.

Relationship between gelation rate of controlled-release acetaminophen tablets containing polyethylene oxide and colonic drug release in dogs.

PURPOSE: We hypothesized that sufficient gelation of orally administered hydrophilic matrix tablets before they reach the colon could, as a result of continuous erosion of the gelated matrix, prevent the decrease in colonic drug release which normally occurs here. The purpose of this study was to elucidate the effect of gelation of hydrophilic matrices containing polyethylene oxide on colonic drug release in dogs using controlled-release (CR) acetaminophen tablets. METHODS: Two types of CR tablets were prepared, a slow gelling tablet (SG) and a rapid gelling tablet (RG) containing an extra highly water soluble filler. In vitro and in vivo performance were examined. RESULTS: SG and RG showed similar drug release behavior in vitro. In oral administration to dogs, the two formulations showed similar gastrointestinal transit, reaching the colon within 2-4 h after oral dosing. Further, they showed similar maximum plasma levels (Cmax) and time to Cmax (Tmax). In contrast, however, the two tablets produced different plasma levels from 2 h post-dosing, with plasma levels of RG higher than those of SG and with smaller individual variation. Directly observed colonic drug release behavior of RG was similar to in vitro drug release, whereas that from SG was suppressed. CONCLUSIONS: Colonic drug release is closely related to the gelation of hydrophilic matrix, and rapid gelation provides continuous in vivo drug release.

Thymidine phosphorylase/platelet-derived endothelial cell growth factor expression associated with hepatic metastasis in gastric carcinoma.

It is known that angiogenesis plays an important role in the growth and metastasis of solid tumours. Several angiogenic factors have been identified and platelet-derived endothelial cell growth factor (PD-ECGF) is thought to be one such factor. Recently, it was reported that thymidine phosphorylase (dThdPase) is identical to PD-ECGF. Using immunohistochemical staining with an anti-dThdPase antibody, we investigated the correlation between dThdPase expression and the microvessel density in 120 gastric carcinomas. The microvessel density, determined by immunostaining for factor VIII-related antigen, was significantly higher in dThdPase-positive tumours than in dThdPase-negative tumors. There was a significant correlation between dThdPase expression and the increment of microvessel density. Moreover, regarding distant organ metastasis, the frequency of hepatic metastasis was significantly higher (P < 0.01) in patients with dThdPase-positive tumours than in those with dThdPase-negative tumors. In summary, it was suggested that dThdPase expression is closely associated with the promotion of angiogenesis and hepatic metastasis in gastric carcinoma.

Clinical significance of serum CYFRA 21-1 in gastric cancer.

We studied the clinical significance of the soluble cytokeratin 19 fragment detected with monoclonal antibody CYFRA 21-1 in the sera of patients with histologically proven gastric cancer. Sera of 110 patients with gastric cancer were analysed for CYFRA 21-1 levels by a two-step sandwich enzyme immunoassay. There were no significant differences between CYFRA 21-1 levels and the histotype, depth of invasion or vessel invasion. However, CYFRA 21-1 was significantly higher in the presence of peritoneal metastases, liver metastases and extensive nodal involvement. When the positive cut-off value was defined as 5 ng ml-1, the CYFRA 21-1 in the stage IV and recurrent cases was 55.6% and 66.7%, respectively, which was as high as carcinoembryonic antigen (CEA) and greater than carbohydrate antigen 19-9 (CA 19-9). The positivities in stage I/II and III were zero and 5.9%, respectively, and false-positive rate in 76 patients with benign gastrointestinal disorders was 2.6%. There appeared to be no correlation between CYFRA 21-1 and CEA or CA 19-9. The patients with above 5 n ml-1 of CYFRA 21-1 had a significantly poorer prognosis. Multivariate analysis indicated that CYFRA 21-1 was an independent prognostic factor, while CEA and CA 19-9 failed to be of prognostic value. In conclusion, CYFRA 21-1 is a reliable tumour marker for gastic cancer in predicting very advanced cases, recurrence of the disease and overall poor prognosis.
