Values for urinary beta 2-microglobulin and N-acetyl-beta-D-glucosaminidase in normal healthy infants.

BACKGROUND: Measuring urinary beta 2 microglobin (B2M) and N-acetyl-beta-D-glucosaminidase (NAG) excretion is widely used as a valuable clinical tool in assessing renal tubular lesions. However, few data are available on normal values for urinary excretion of B2M and NAG in infancy. METHODS: Urinary B2M and NAG were measured in healthy infants. The logarithmic values of urinary B2M, NAG, B2M/creatinine ratio and NAG/creatinine ratio were distributed almost normally and reference ranges were calculated from the logarithms of the observed values. RESULTS: The levels of urinary B2M and B2M/creatinine ratio were highest in the 1-month-old group, followed by a decrease during the first 3 months. Urinary B2M excretions in the 3-month-old group showed rather lower levels than those of the 12-month-old and 36-month-old groups. Although urinary NAG excretions were almost constant throughout all groups, urinary NAG/creatinine ratio decreased gradually until 3 years of age. CONCLUSIONS: We suggest that these reference ranges are of importance in evaluating tubular damage due to a variety of renal diseases in infancy.

Multiple early gastric stump carcinomas after gastrectomy for peptic ulcer.

The remnant stomach after partial gastrectomy is considered to have a predilection for the development of primary gastric carcinoma. However, early gastric stump carcinomas are uncommon because the diagnosis of gastric stump carcinoma is more difficult than that of carcinoma in the intact stomach. Triple early gastric stump carcinomas, as in the present case, are exceedingly rare and may provide some clues for further investigation of carcinogenesis in the gastric stump. We studied about the histological appearance, genetic alterations (P-53 gene, c-erbB-2 gene and K-ras gene), and expression of tumor-associated antigens (carcinoembryonic antigen, carbohydrate antigen 19-9, and sialyl-Tn) in this rare case. The three carcinomas differed from each other histologically. With respect to genetic alterations, c-erbB-2 was amplified in one lesion, but no mutations of K-ras and P-53 gene were detected. The three carcinomas also differed from each other on the expression of tumor-associated antigens. In noncancerous mucosal epithelium at the anastomosis showing hyperplasia and cystic formation of glandular epithelial cells, no genetic alterations were detected, but sialyl-Tn and carbohydrate antigen 19-9 were expressed. These results suggest that there may be different processes of carcinogenesis of the three carcinomas even though they occurred under identical environmental conditions to those that have increased cancer risk.

Basement membrane-like structures occurring on the surface of dental papilla mesenchymal cells during odontogenesis in the monkey Macaca fuscata.

The aim of this study was to examine the nature of a basement membrane-like structure occurring along the surface of dental papilla cells during odontogenesis. The tooth germs of Japanese macaques (Macaca fuscata) were prepared for both ultrastructural and immunohistochemical studies. Characteristic discrete structures which resembled basement membranes were observed along the surface of the dental papilla cells, preodontoblasts, and also of the odontoblasts. Immunoperoxidase staining showed that these structures contained major basement membrane components including laminin, heparan sulfate proteoglycan, type IV collagen, as well as fibronectin. They tended to be less prominent along dental pulp fibroblasts and fully developed odontoblasts in comparison with those along the papilla cells localized close to the basement membrane of the inner enamel epithelium. Therefore, possible roles of these basement membrane-like structures would be: 1) to support the cells, 2) to momentarily secure the appropriate positions of differentiating mesenchymal cells, and 3) to regulate epithelial-mesenchymal interactions for odontoblast differentiation.

Presymptomatic brain lesions on MRI in a patient with intravascular malignant lymphomatosis.

A 58-year-old man with a intravascular malignant lymphomatosis initially developed myeloradiculopathy without cerebral symptoms. His MRI, however, demonstrated solid, wedge-shaped, and well-demarcated lesions in the deep white matter and a string-shaped lesion along with nerve fibers in the splenium of corpus callosum. A variety of cerebral symptoms manifested a month afterward. The possibility of this disease should be considered in cases of undiagnosed myeloradiculopathy with such silent brain lesions.

Isolation of acriflavine resistant Erysipelothrix rhusiopathiae from slaughter pigs in Japan.

Erysipelothrix rhusiopathiae is the causative agent of swine erysipelas. Although an attenuated vaccine is used in Japan, recent increases in disease occurrence have cast doubts on its efficacy. We investigated the similarity between the vaccine strain and E. rhusiopathiae field isolates by the analysis of acriflavine resistance (the vaccine strain marker), serotype, DNA fingerprinting and pathogenicity to mice. Although 7 acriflavine resistant E. rhusiopathiae isolates were separated from arthritic lesions of slaughter pigs, we were unable to prove that they were identical to the vaccine strain.

A progesterone antagonist cannot prevent fetal survival if the uterine horn is incised.

The fetuses released into the abdominal cavity by uterine incision escape from most physical influences of the uterus. This study examined whether these fetuses require progesterone actions for survival during late pregnancy in rats. A longitudinal incision in one uterine horn (with the other horn intact) together with bilateral ovariectomy (OVX), removal of the main progesterone-production sites, or sham OVX, were performed on day 18 of pregnancy. Thereafter the rats were given daily subcutaneous injections of anti-progesterone RU 486 (10 mg/kg), or vehicle alone, and the fetal survival rate in each uterine horn was examined on day 21. In those controls which received sham OVX plus injections of vehicle, fetal survival rates were more than 80% in both uterine horns. In the other groups, which received sham OVX plus injections of RU 486, or OVX plus injections of vehicle, or OVX plus injections of RU 486, the fetal survival rates in the intact uterine horns were 4%, 0% and 0%, respectively. In the incised uterine horns of these groups, however, the fetal survival rates were 59%, 67% and 56%, respectively. The results suggest that progesterone, which is required for maintaining pregnancy, may not be essential for survival of fetuses released into the abdominal cavity. Progesterone actions unrelated to uterine physical environment are likely to be dispensable for fetal survival during late pregnancy in rats.

Differences in clinical features and computed tomographic findings between embolic and non-embolic acute ischemic stroke: a quantitative differential diagnosis.

A diagnosis based on the presumed mechanism of stroke onset is useful for management strategies in acute ischemic stroke. Ninety-two patients with embolic (cardiac or artery-to-artery) and 107 with non-embolic (thrombotic or hemodynamic) stroke were diagnosed on strict cerebral angiographic criteria alone. To clearly discriminate between these two groups, the neurological and computed tomographic (CT) findings were then compared. Rapidity of onset, vomiting, urinary incontinence, level of consciousness, cervical bruit, anisocoria, tongue deviation, sensory disturbance, and CT findings (location of hypodense area, findings of brain edema and hemorrhagic transformation) were discriminatory factors between the two groups (p<0.01). According to these 11 items, we prepared a numerical table for quantitative differential diagnosis. A diagnostic accuracy of 98.9% for embolic and 87.9% for non-embolic stroke in internal verification, and 90.0% and 82.9%, respectively, in external verification was observed. The differences in clinical features and CT findings between embolic and non-embolic stroke may reflect the pathophysiological mechanisms of the occlusive process of cerebral artery as well as the extent and severity of ischemia.

Total body bone development during early childhood in very low birth weight infants without cerebral palsy and mental retardation.

Total body bone mineral density was measured by dual energy X-ray absorptiometry in 52 children who were very low birth weight (VLBW) infants without cerebral palsy and mental retardation (postconceptional age, from 10 mo to 6 y and 6 mo). VLBW infants in this study seemed to show compensatory acceleration of total body bone development, catching up with the control group during early childhood. However, in VLBW infants with at least one of the three factors such as total parenteral nutrition for 1 week or more, assisted ventilation for 1 week or more, or oxygen therapy for 28 d or more in their early stage after birth, adequate mineral supplementation might be especially important for long-term bone development.

TGF-beta1 produced by gastric cancer cells affects mesothelial cell morphology in peritoneal dissemination.

In vitro morphologic change of mesothelial cells was observed following the addition of serum-free conditioned medium (SF-CM) from peritoneal dissemination cell line OCUM-2MD3. The same morphologic change of mesothelial cells was observed following the addition of 10 ng/ml TGF-beta1, but not following the addition of b-FGF, IGF-I, VEGF or PDGF-AA. In the in vivo study, mesothelial cells of mice treated with SF-CM from OCUM-2MD3 and TGF- beta1 were separated from one another, resulting in exposure of the submesothelial connective tissue. The molecular size of the mesothelial morphology changing activity was estimated by running the SF-CM from OCUM-2MD3 through a gel filtration column TSK-gel G2000SW. The mesothelial morphology changing activity was recognized at positions equivalent of Mr 6, 500-30,000. 25 kDa TGF-beta1 was detected in the active fraction from the TSK-gel G2000SW column and the SF-CM of OCUM-2MD3 by Western blotting using a monoclonal antibody against TGF-beta1. These findings suggest that TGF-beta1 produced by gastric cancer cells changes the morphology of mesothelial cells and may thus be closely associated with peritoneal dissemination.

Tranilast and cisplatin as an experimental combination therapy for scirrhous gastric cancer.

We examined the effect of Tranilast on the reduction of the administered dose of cisplatin using a scirrhous gastric cancer model. Scirrhous gastric cancer cell line, OCUM-2M, and gastric fibroblasts, NF-10, were used. The IC50 values of CDDP to OCUM-2M cells were decreased by Tranilast in vitro. The combination treatment with Tranilast and CDDP decreased the xenografted tumor size. The combination therapy decreased fibrosis and mitosis, and increased apoptosis. These findings suggest that Tranilast increased the CDDP response on scirrhous gastric cancer. The combination treatment with Tranilast and CDDP may be useful as a new therapy for scirrhous gastric carcinoma.

[Basic calponin expressing human neuroblastoma cell line of KP-N-YS].

Recent studies have demonstrated that human neuroblastoma (NB) cell lines have at least two morphological appearance of neuroblastic (N-type) and substrate-adhesive (S-type) cells. Our previous study revealed that S-type cells expressed alpha-smooth muscle actin and/or desmin, suggesting the smooth muscle cell characteristics of S-type cells. In the present study, a new human NB cell line, KP-N-YS, was established from bone marrow metastasis of a four-year-old boy with advanced NB, originating from the left adrenal gland. Subsequently N-type cell clone (YS1n) and S-type cell clone (YS2s) was isolated from the parent cell line. Parent and clones cell lines were identified as NB by surface membrane antigen and cytoskeletal protein analysis, and these cell lines were demonstrated as common progenitors by chromosomal analysis. Furthermore the presence of basic calponin were determined by indirect immunofluorescence, Western blot, as well as by RT-PCR (reverse transcriptase-polymerase chain reaction). Our demonstration of basic calponin not in N-type cells, but in S-type cells supports the plausible smooth muscle cell characteristics of this NB cell line.

Classification of Listeria monocytogenes by PCR-restriction enzyme analysis in the two genes of hlyA and iap.

PCR-restriction enzyme analysis in the two virulence-associated genes was performed. The hlyA gene cording for listeriolysin O and the iap gene cording for an invasion associated factor were amplified with primers SH2 or SI3. The PCR products obtained were cleaved with 32 restriction enzymes, and restriction profiles from 12 strains, 6 each of serotypes 1/2a and 4b, were compared. We obtained two profiles for the hlyA using 4 restriction enzymes and eight profiles for the iap by using AluI, and the results of the profiles did not correlate with the serotypes. The polymorphism in the iap region was of a higher degree than that in the hlyA region, and the PCR-restriction enzyme analysis of the iap gene with primers SI3 and AluI was confirmed as one of the useful epidemiological analysis methods for listerosis outbreaks.

Subtraction CT angiography with controlled-orbit helical scanning for detection of intracranial aneurysms.

PURPOSE: Our goal was to evaluate the utility of subtraction three-dimensional CT angiography for the detection of intracranial aneurysms. METHODS: Thirty-six patients with intracranial aneurysms were examined using newly devised controlled-orbit helical scanning and conventional angiography. Three-dimensional CT angiograms and subtraction 3-D CT angiograms were compared with conventional angiograms for their characterization of intracranial aneurysms. RESULTS: Fifty aneurysms were depicted on conventional angiograms, of which 48 (96%) were seen on the 3-D CT angiograms. Three-dimensional CT angiography was superior or equivalent to conventional angiography for depicting the shape, direction, and location of 33 (66%) of 50 aneurysms; however, it was often less useful than conventional angiography in delineating intracranial aneurysms adjacent to bone. Subtraction 3-D CT angiograms were obtained in 32 patients with a total of 46 aneurysms (in four cases, aneurysms were not depicted owing to excessive motion artifacts), and were superior or equivalent to conventional angiograms in all 46 cases. CONCLUSIONS: Subtraction 3-D CT angiography with the use of controlled-orbit helical scanning is effective in the detection of intracranial aneurysms.

Expression of intercellular adhesion molecule-1 in invasive breast cancer reflects low growth potential, negative lymph node involvement, and good prognosis.

To understand the role of intercellular adhesion molecule-1 (ICAM-1) in tumor progression in the host, we examined ICAM-1 expression in breast cancer by immunohistochemistry. This study included 274 female patients with invasive breast cancer, with a median follow-up of 98 months. The molecule was identified in formalin-fixed, paraffin-embedded primary tumors, and the relationship to clinicopathological factors and prognosis was analyzed. ICAM-1 expression occurred in 50.3% of patients. ICAM-1 expression had negative correlation to tumor size (P = 0.003), lymph node metastasis (P < 0.0001), tumor infiltration (P = 0.003), nuclear pleomorphism (P = 0.004), and nuclear grade (P = 0.042). Patients with ICAM-1-positive tumors had better relapse-free and overall survival than those with negative tumors (P < 0.0001 and P = 0.0003, respectively). These results suggest that expression of ICAM-1 on cancer cells might have a role as a suppressor of tumor progression under the host immune surveillance system.

Expression of proliferating cell nuclear antigen in bronchial epithelium after lung transplantation in the rat.

BACKGROUND: The normal, mature airway epithelium in experimental animals has a very slow cell turnover and minimal proliferation. The aim of this study was to investigate the expression of proliferating cell nuclear antigen (PCNA) as an index of bronchial cell proliferation in the Brown Norway to Lewis rat pulmonary allograft model with or without immunosuppression. METHODS: Brown Norway left lungs were transplanted into Lewis recipients. Some recipients were treated with a high dose of cyclosporine and FK506. Lewis-to-Lewis donor-recipient combination was performed as a control. Lungs were excised on postoperative days 3 and 5. Routinely processed, paraffin-embedded sections were prepared and stained by PCNA. Counts of PCNA-positive cells in the perivascular cellular infiltrate and bronchial surface epithelium were compared with the histologic grade of rejection. RESULTS: The PCNA index (percent of nuclei immunostaining for PCNA) in bronchial surface epithelium was significantly higher in allografts (21.0% +/- 3.1% at 3 days, 31.4% +/- 9.8 % at 5 days, p < 0.05) than in isografts (5.4% +/- 3.0% at 3 days, 4.7% +/- 4.6% at 5 days). The PCNA index was also greater in the perivascular infiltrates of rejecting lungs (23.9% +/- 3.7% at 3 days, 29.1% +/- 6.6% at 5 days). However, in the cyclosporine- and FK506-treated groups, the PCNA index in bronchial surface epithelium was suppressed to less than 5% at 3 and 5 days. Even at 50 days after transplantation, PCNA-positive cells were rare in bronchial epithelium of FK506-treated grafts. CONCLUSIONS: Bronchial epithelium in isografts has a relatively low rate of proliferation. In rejection, allografts have a very rapid cell turnover and proliferation. Proliferating epithelium may be a consequence of immune events or it may contribute to the pathogenesis of those events.

[A case of non-photosensitive, self-induced epileptic seizures with pacygyria].

We report an 11-year-old boy with a non-photosensitive epileptic self-induced seizures, pacygyria and familial ataxia. His grandmother and aunts had dysarthria, and his mother had developed progressive ataxia and myoclonus since 40 years old. His older sister had ataxia, mental retardation and epilepsy. As for the boy, motor developmental delay with muscle hypertonicity of left extremities was recognized at the age of 5 months. Mental retardation and ataxia was recognized at the age of 3 years and slight mental regression is recognized at the age of 11 years. No special findings were detected in an examination of his blood and cerebrospinal fluid, including amino acids, lysosomal enzymes activity and genetic analysis for dentatorubralpallidoluysian atrophy. Brain magnetic resonance imaging revealed pachygyria of the right cerebral cortecies. At the age of two, he began to induce seizures with impairment of consciousness in himself by waving his right hand over his face which was directed toward a source of bright light. At the age of seven, he developed spontaneous seizures with impairment of consciousness. An EEG showed frequent spikes in the occipital areas, on the right and left sides occurring either independently or synchronously. Intermittent photic stimulation and pattern stimulation did not induce a paroxysmal discharge in EEG. Ictal EEG suggested that the origin of the seizures was the occipital lobe. Treatment with valporate and zonisamide was effective in reducing the seizures. The findings of our case imply the pathogenesis of self-induced seizures and the relationship between PME and neuronal migration disorders.

In vivo determination of ultratrace amounts of prostaglandin in plasma by high-performance liquid chromatography/laser-induced fluorometry/ultrasensitive laser spectrometry under severe conditions.

We succeeded in determining ultratrace prostaglandin amounts in plasma, at the femtomolar level, using laser-induced fluorometry through a complete redesign of the analytical procedures. Practical samples, especially plasma, contain large amounts of admixtures, and prostaglandin in plasma (pg/mL) has been considered to be difficult to detect because the samples and reagents supplied by conventional procedures are neither pure nor stable enough to get good results by ultrasensitive laser spectrometry. We completely redesigned the analytical procedures after careful investigations of the reagent purification and the column separation conditions based on a newly found behavior of the reagent and derivatized prostaglandin in a small quantity of ethanol in the mobile phase. A lower determination limit of 23 pg/mL (65 fmol) was achieved, the variance was 12% at 25 pg/mL, and the recovery rate was 88-89%. This method was applied to in vivo analysis of the concentration of prostaglandin E1 administered as a prodrug of prostaglandin E1 (delta(8)-9-O-butyryl prostaglandin F1 butyl ester, AS-013) by intravenous infusion to beagle dogs. A clear correlation between the change of blood pressure and the prostaglandin E1 concentration was confirmed.
