Ethyl eicosapentaenoate restored the immunosuppression in mice fed fat-free diet.

The effects of eicosapentaenoic acid (EPA) on the humoral immune response in fat free diet-fed (FF) mice were studied. The lowered anti-SRBC PFC activity of ICR male FF mice was restored in a dose-dependent manner when EPA was administered orally at doses of 60-360 mg/kg/day for 20 days. In in vitro experiments, EPA similarly enhanced anti-SRBC PFC activity but did not affect the response against lipopolysaccharide. Furthermore, EPA did not cause any substantial effect on T suppressor cell activity induced by Concanavalin A in vitro. On the other hand, T helper cell activity induced by keyhole limpet hemocyanin was augmented. From these results, it is suggestive that EPA caused immunopotentiation to FF mice at least partially by an enhancement of T helper cell activities.

[Determination of various tumor markers, with special reference to sialyl SSEA-1 antigen in lung cancer].

Tumor markers including sialyl SSEA-1 antigen (SLX) were measured in 77 patients with lung cancer and 116 patients with benign respiratory diseases. Increased levels of serum SLX were observed in 57.6% (19/33) of adenocarcinoma of the lung, and an overall positive rate was 42.9% (33/77) in lung cancer cases. The false positive incidence of SLX in the sera from the patients with non-malignant diseases was as low as 8.6%. Among other tumor markers determined, serum CA 19-9, CEA had a relatively high positive rate, but the false positive incidence were not low. In cases of poor prognostic cases of with adenocarcinoma of the lung, SLX levels became levels higher than the previous ones, but not in cases of other cell types were not. Not only in the patients with lung cancer but also in those with benign respiratory diseases such as bronchiectasis and lung fibrosis, high levels of SLX seemed to suggest poor prognosis. These results indicate that SLX may contribute for monitoring and prognosis of the cases of lung cancer and other respiratory diseases.

Antihypertensive efficacy and safety of urapidil, alone or in combination with beta-blockers, in patients with phaeochromocytoma.

The clinical efficacy and safety of urapidil, used alone or in conjunction with beta-blockers, were evaluated in 14 patients with phaeochromocytoma. Following a 1-week placebo run-in period, the patients were treated with sustained-release capsules of urapidil, initially 30 mg twice a day, followed by dose adjustment within 30-270 mg (mean +/- s.d. 144 +/- 73) per day for 7-29 days (21 +/- 8 days). In six patients, beta-blockers were added to control associated tachycardia. Blood pressure and pulse rate were successfully controlled in 11/14 patients (78.6%) during the therapy. Both the frequency and the severity of hypertensive paroxysms were clearly reduced in 7/8 patients, who showed frequent paroxysms of hypertension during placebo treatment. A variety of subjective symptoms observed in 13 patients during placebo treatment improved during drug therapy in nine patients (69.2%). Side effects occurred in five patients but were minor and well tolerated except in one patient, who was withdrawn from urapidil monotherapy due to facial oedema and finger stiffness which persisted even after reducing the daily dose from 306 to 270 mg. Overall, in terms of antihypertensive effectiveness, improvement in subjective symptoms and the safety profile, urapidil was considered very useful in four patients (28.6%), useful in six (42.9%), slightly useful in three and useless in one. Urapidil therefore appears to be a worthwhile agent in the treatment of patients with phaeochromocytoma.

[A case of polycystic kidney with bilateral nephrectomy].

A 56-year-old woman in chronic hemodialysis had been suffering from uncontrollable fever for the past 7 months. Her original disease was diagnosed as familial polycystic kidney and three of her five brothers were found to have the same disease. Her chromosome was 46,XX,21P+ and laboratory examination revealed severe anemia, malnutrition, liver dysfunction, pyuria and candidiasis of urine. Abdominal echogram and CT scan revealed polycystic kidneys and multiple liver cysts. She was admitted to our hospital and was diagnosed as having pyelonephritis of the right kidney. As her condition was not improved by conservative therapy right nephrectomy was performed. One month later, spiking fever and left tenderness reappeared. Those symptoms could not be controlled by conservative therapy and left nephrectomy was performed again. Pathological examination on nephrectomized kidneys showed interstitial nephritis, hyaline degeneration and proliferative change of glomeruli, microabscess, colloid of tubules and calcification of parts of Henle's loops. Nephrectomy has been performed in 1.6 to 10.0% of polycystic kidneys due to references since 1952. Eight of the 22 polycystic kidneys (36.3%) seen at our hospital during the past 10 years have been removed.

Hematological response to the new synthetic muramyl dipeptide derivative muroctasin after chemotherapy in patients with malignant lymphoma. A randomized crossover trial.

A randomized crossover trial was performed in 20 patients receiving chemotherapy for malignant lymphoma. N2-[(N-acetylmuramoyl)-L-alanyl-D-isoglutaminyl]-N6-stearoyl-L-lysine (MDP-Lys(L18), muroctasin) at a dose of 200 micrograms was subcutaneously administered after one of two cycles of the same protocol. The administration was started on day 4 after the start of chemotherapy and continued for 10 days. The mean length from the start of chemotherapy to nadir of white blood cell (WBC) was not significantly different between the control and muroctasin cycles. The mean WBC and neutrophil counts at nadir of the control cycle were significantly lower than those at the same point of muroctasin cycle, respectively. A positive effect of muroctasin cycle, defined as WBC count at nadir being increased by 1,000/mm3, WBC count being increased by 1,000/mm3 on two points examined after nadir, or faster reach to nadir and faster recovery to normal range of WBC than in the control cycle, was observed in 7 (35%) of 20 patients. On the other hand, only one (5%) patient showed a superiority of the control cycle over the muroctasin cycle. Toxic effects of muroctasin were observed in 34.8% of patients, but were tolerable for most patients. These results show that muroctasin has a clinical efficacy in the restoration of leukopenia after chemotherapy.

[Characterization of helical coil microwave antenna for interstitial hyperthermia].

Implantable microwave antennas for interstitial hyperthermia have been constructed by using a fine wire helical coil extension of the coax feedline inner conductor. The effects of the variable coil length the winding density, and the antenna insertion depth in the tissue on the heating patterns from a single antenna were characterized in phantom models. Results indicated that the helical coil microwave antenna provided localized heat at depth near the antenna tip, and the heating patterns were easily modified by minor changes in the coil length, winding density, and altering the separation gap. The characteristics of the helical coil microwave antenna may increase its potential application to cases of interstitial hyperthermia.

Glutathione depletion by 2-nitroimidazole-1-acetohydroxamic acid as a radiosensitizer in hypoxic rat hepatocytes.

The effects of nitroimidazoles as radiosensitizers on intracellular glutathione (GSH) level were investigated in rat isolated hepatocytes. Dinitroimidazoles have lowered almost completely GSH level during the incubation for 30 min under oxic (95% O2+5% CO2) condition, while mononitroimidazoles had scarcely affected. In the case of hypoxic (95% N2+5% CO2) condition, however, 2-nitroimidazoles, not 4-nitroimidazoles, as well as 2,4- and 4,5-dinitroimidazoles have caused the significant depletion of GSH. This suggests that nitro group in the 2-position of imidazoles may be responsible for the GSH depletion under hypoxia. Especially, 2-nitroimidazole-1-acetohydroxamic acid (KIH-801) was found to be the most potent GSH depletor only under hypoxic, not oxic conditions, and might be useful for the new hypoxic cell radiosensitizer instead of misonidazole.

Pharmacological studies of lappaconitine. Analgesic activities.

The analgesic activity of lappaconitine, which is contained in the root of Aconitum sinomantanum Nakai, was examined after oral and subcutaneous administration to mice or rats by using methods for screening of analgesics, i.e., hot plate, tail immersion, tail pinch, tail pressure, acetic acid-induced writhing, bradykinin-induced flexor reflex of hind limb and Randall-Selitto methods. The results were compared with those for morphine, indometacin and acetylsalicylic acid (ASA). Analgesic activities of lappaconitine were greater than those of indometacin and ASA, but generally about 2 to 5 times less than those of morphine. However, in the rat tail immersion test, orally administered lappaconitine exhibited more potent analgesic activity than morphine; in this test, lappaconitine was almost equipotent when given orally and subcutaneously, whereas the potency of orally administered morphine was only one-twentieth of that of subcutaneously administered morphine. Like morphine, lappaconitine increased the pain threshold of the normal paw as well as that of the inflamed paw when tested by the Randall-Selitto method. The results show that lappaconitine has strong analgesic activity, and further suggest that the central nervous system may be involved in the action on the pain threshold.

Effects of hypophysectomy and pituitary hormones on hepatic microsomal carboxylesterase isozymes in male rats.

Hepatic microsomal carboxylesterase (EC 3.1.1.1) from rat liver microsomes showed a different capacity for the hydrolysis of various substrates. In hypophysectomized male rats, hepatic carboxylesterase activities showed substrate-dependent changes, i.e., an decrease in malathion and p-nitrophenylacetate hydrolases and increase in isocarboxazid hydrolase. When human growth hormone is administered to hypophysectomized male rats, carboxylesterase activities were decreased. Conversely, subcutaneous injection of ovine prolactin to hypophysectomized rats, carboxylesterase activities showed substrate-dependent changes. On the other hand, in hypophysectomized male rats, the amount of carboxylesterase isozymes showed different changes, i.e., an increase in the amount of RL2 contents and decrease in RL1 contents, and the amount of RH1 was not changed. These results suggested that hepatic microsomal carboxylesterases are regulated, at least in part, by some pituitary hormones, and that these hormones have selective effects on the different isozymes of carboxylesterase.

Studies on the assessment of neurotoxicity in children with acute lymphoblastic leukemia.

Central nervous system (CNS) prophylaxis caused a remarkable reduction in the incidence of CNS disease, however there has evolved a growing concern regarding the immediate or late toxicities to the developing CNS. Twenty-eight children with acute lymphoblastic leukemia who survived for more than 2 years were examined for the assessment of neurotoxicity induced by CNS prophylaxis and its treatment. The patients were stratified into three groups: Stratum I, prophylaxis with methotrexate; Stratum II, prophylaxis with cranial irradiation with methotrexate; and Stratum III, with CNS leukemia. Once CNS disease developed the sequelae were frequent and severe, due to the elevated methotrexate levels in the cerebrospinal fluid. CNS prophylaxis with intermediate-dose methotrexate was less toxic to the developing CNS than prophylactic cranial irradiation, especially in children under 5 years of age. Electroencephalograms and evoked potentials are likely to find increasing application in defining the CNS sequelae of acute lymphoblastic leukemia in children and its treatment. Although the sample size was small, the findings delineate specific areas of neurotoxicity.
