[The effects of lowering of blood pressure on pain sensitivity in spontaneously hypertensive rats].

It has been reported that elevation of blood pressure produces a reduction in pain sensitivity. This study was designed to clarify the correlation in spontaneously hypertensive rats (SHR) between fall in blood pressure and pain sensitivity. In seven-weeks-old male SHR, the angiotensin converting inhibitor delapril (10 mg/kg/day) or calcium antagonist nifedipine (3 mg/kg/day) was administered orally every day for 8 weeks. Systolic blood pressure (SBP) pretreatment was significantly higher in the SHR than in normotensive Wistar-Kyoto (WKY) rats and pain sensitivity measured with the hot plate method was significantly lower in the SHR than in the WKY rats. Administration of both drugs produced a significant suppression of elevation of SBP, and produced a significant elevation of pain sensitivity. Furthermore, at 8 weeks after drug administration, urinary norepinephrine (UNE) significantly decreased and plasma beta-endorphin (beta-end) significantly increased. A significant correlation was noted between pain sensitivity and SBP and also between pain sensitivity and UNE. Of these, pain sensitivity was the more closely correlated to degree of change in UNE than to degree of change in SBP. It appears that elevation of pain sensitivity is due to suppression of the sympathetic nervous system by antihypertensive drugs, but not to elevation of beta-end levels. These data suggest that a fall in blood pressure through administration of delapril or nifedipine reverses decrease in pain sensitivity in SHR and that decrease in sympathetic tone plays an important role in the restoration of levels of sensitivity to pain.

Enhancement of potassium ion activity in cat hippocampus during REM sleep.

During slow-wave sleep, extracellular potassium ion activity in the dorsal hippocampus of the cat was comparable to that during quiet wakefulness. During rapid eye movement (REM) sleep, it showed a gradual increase by 0.2-0.3 mM in the CA1 pyramidal layer and its close vicinity. No phasic increase was observed when REM bursts occurred. The enhanced K+ activity during REM sleep is considered to reflect the overall activity of the neurons in the pyramidal layer.

Activation of hyaluronidase by 8-methoxypsoralen-polyamine photoproducts.

Ultraviolet radiation of 8-methoxypsoralen (8-MOP) in the presence of various polyamines resulted in stable photoproducts that were very soluble in water and showed hyaluronidase-activating properties. Among them, the photoproducts obtained from the reaction systems of 8-MOP-spermine and 8-MOP-spermidine markedly activated hyaluronidase. The enzyme activity was not affected by 8-MOP alone and the photoproduct of 8-MOP (8-MOP-P). From these facts, it was suggested that the photoproducts with hyaluronidase-activating properties might play an important role in the onset of 8-MOP-induced photosensitivity.

[Plasma alpha-human atrial natriuretic polypeptide levels during surgery for pheochromocytoma].

We measured plasma alpha-human atrial natriuretic polypeptide (alpha-hANP) and catecholamine concentrations during anesthesia and surgery for pheochromocytoma in five patients ranging in ages from 19 to 69. Plasma catecholamine concentrations and systemic blood pressure increased extremely during surgical manipulation of the tumor, while plasma alpha-hANP levels did not change even during surgical manipulation or after removal of the tumor as compared with preanesthetic values. Our findings suggest that plasma alpha-hANP levels were unchanged during anesthesia and surgery for human pheochromocytoma.

[Vena caval flow patterns in patients with constrictive pericarditis: analysis by catheter-tip Doppler flowmetry].

Changes in superior and inferior vena caval flow patterns were analyzed in 5 patients with constrictive pericarditis and were compared with those of 10 normal control subjects. Caval flows were measured using catheter-tip Doppler flowmeters. The normal controls showed biphasic M-shaped flow patterns; the peaks of the first forward flow (S wave) and of the second forward flow (D wave) appeared coincident with mid-systole and mid-diastole, respectively. Reverse flows fell during the atrial contraction period (A wave) and late systole (V wave). In the normal controls, the ratios of the S wave to the D wave (S/D ratio) and the A wave to the S wave (A/S ratio) were 2.15 +/- 0.41 and 0.18 +/- 0.10, respectively, and there was a disproportionate respiratory variation in the S and D waves in the normal controls. In constrictive pericarditis, superior and inferior vena caval flow velocities were lower than those in the normal controls. The S/D and A/S ratios were 1.46 +/- 0.27 (p < 0.05 vs control) and 0.66 +/- 0.15 (p < 0.01 vs control), respectively, with the A wave increasing in proportion to the severity of constrictive pericarditis. In addition, there was only a minimal respiratory variation in constrictive pericarditis. In conclusion, recognition of the patterns of the superior and inferior vena caval flow velocities may be useful for diagnosing constrictive pericarditis.

Enzymatic hydrolysis of indometacin farnesil, a prodrug of indomethacin, by carboxylesterase in cultured synovial cells.

The hydrolysis of indometacin farnesil (IMF) in the synovial cells of rat and human and the subcellular fractions of rat liver were investigated in relation to the inhibition of prostaglandin E2 (PGE2) production in the synovial cells. The inhibition of PGE2 production in cultured human synovial cells by anti-inflammatory drugs was potent in the order of IND, IMF and acetyl salicylic acid. However, when the cells were pretreated with IMF, the inhibitory activity of IMF was retained even after the compound was washed out from the medium. No duration of the inhibition was seen in the pretreatment of the cells with IND or acetyl salicylic acid. These results suggest that IMF incorporated into the synovial cells was hydrolyzed gradually to IND. In fact, IMF was taken up by rat synovial cells in culture and considerable amount of IND, which increased with culture period, was found out in the cells. Furthermore, the IMF hydrolase activity was found in microsomal and lysosomal fractions of rat liver, and the hydrolase was identified as carboxylesterase by using bis-(p-nitrophenyl) phosphate, a specific inhibitor of carboxylesterase.

[An autopsy case of primary lung leiomyosarcoma].

We report autopsy findings for a case of leiomyosarcoma originating in the right lung in an 86-year-old man. We surveyed current literatures and compared the immunohistochemical results in the present case with those reported previously in Japan. The solid tumor measured 6 x 7 x 7 cm sized and was located in the right lung. Its cut-surface appeared grayish to yellowish and contained areas of hemorrhage and necrosis. The lungs contained scattered nodular lesions less than 2 cm in diameter. Metastatic lesions were also confirmed in other tissues such as the heart, pericardium, liver, spleen, left adrenal gland and thyroid. Microscopically, 14 mitoses were detected in every 10 high-power fields. An immunohistochemical study revealed positivity for smooth muscle myosin and actin, and negativity for CEA, and appeared to be important in differential diagnosis and determination of histogenetic type.

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--9. Control of intraoperative hypertension with diltiazem].

Intraoperative hypertension over 160 mmHg systolic and sinus tachycardia over 100 bpm are often observed during total intravenous anesthesia with droperidol, fentanyl and ketamine. Fifty-seven surgical patients who developed hypertension over 160 mmHg systolic during various operative procedures under this type of anesthesia were given diltiazem intravenously to overcome the situation. Their blood pressure and heart rate decreased soon after the administration of diltiazem. The rate pressure product was reduced significantly. Neither preoperative hypertension nor difference of doses between 5 mg and 10 mg of diltiazem had any significant relationship with hypotensive effect of intravenous diltiazem. But the higher the systolic-pressure was just before the administration of diltiazem, the more effective diltiazem was. No adverse effects with this drug was observed. We can conclude that intravenous diltiazem in a dose of 5 mg or 10 mg may be repeatedly given to overcome hypertension or sinus tachycardia during this type of anesthesia without any adverse effects.

[Mobile neurinoma of the cauda equina; a case report].

A case of mobile tumor of the cauda equina was presented with a brief review of factors relating to the mobility. A 66-year-old man was admitted to our hospital complaining of left lumbago aggravated by coughing or assuming the supine position. However, neurological examination failed to reveal any abnormalities. The initial myelography showed complete obstruction above the superior border of the L1 vertebra, while the second myelography showed the same obstruction, but it had moved upwards to the superior border of T12. Surgery revealed an elastic, soft tumor and cystic dilatation of the subarachnoid space above and below the tumor. The tumor was mobile but originated from a single elongated and tortuous nerve of the cauda equina. Pathologic examination revealed neurinoma.

[Infection prophylaxis in patients with hematological malignancies (II)--Successful prophylaxis of tuberculosis with isoniazid].

In a retrospective study to evaluate the efficacy of isoniazid (INH) for the prevention of tuberculosis, we studied 1760 patients with hematological malignancies over a twenty-year period (1970-1989). 759 patients received oral INH, most of all received 400 mg per day. Only one (0.1%) of the patients receiving INH developed tuberculosis, whereas nine (0.9%) of the 1001 patients who did not receive INH developed tuberculosis (p less than 0.05). We found that INH was very effective in the prevention of tuberculosis in patients with hematological malignancies, and was well tolerated.

[A simple method of retrograde intubation].

A safe and simple method to facilitate retrograde intubation was presented. An epidural catheter was introduced into the oropharynx through a 16-G extracath sheath inserted through the cricothyroid membrane into the trachea. After passing the catheter beyond the distal end of the endotracheal tube, a knot was made. Intubation was done by pulling the catheter, which can be cut apart and pulled out afterwards. This method would make the damage or contamination of the cricothyroid membrane minimum and the trouble of losing the catheter could be abolished. We believe this way of intubation can be applied universally because it needs no special tool or device.

Regulation of ovarian carbonyl reductase mediated by estrogen receptor in immature rats.

In the present study, the enhancing effect of synthetic estrogen on ovarian carbonyl reductase, a new prostaglandin (PG)-metabolizing enzyme, was investigated, and the antagonistic effect of antiestrogen on this enhancement was examined in immature rats. Ovarian carbonyl reductase activity towards 13,14-dihydro-15-keto-PGF2 alpha (15KD-PGF2 alpha), 4-benzoylpyridine (4BP) and menadione was determined photometrically and radiochemically, and quantitation of ovarian carbonyl reductase content was performed by Western blot analysis. Diethylstilbestrol (DES) and hexestrol (HX) enhanced the increasing effect of human chorionic gonadotropin (hCG) on ovarian carbonyl reductase activity and content when these synthetic estrogens (0.2 mg/kg) were administered for 3 days from 26 days of age, before hCG treatment. On the other hand, tamoxifen, which inhibits the binding of estradiol to the estrogen receptor, significantly prevented estradiol (E2) from enhancing the effect of hCG on ovarian carbonyl reductase. Furthermore, the ovarian carbonyl reductase activities towards the three substrates correlated well with the ovarian carbonyl reductase content. These results indicate that ovarian carbonyl reductase in immature rats may be regulated by a specific increase in the ovarian response to luteinizing hormone mediated by estrogen receptor.

The ram: a novel low molecular weight GTP-binding protein cDNA from a rat megakaryocyte library.

A novel low Mr GTP-binding protein cDNA was isolated from a rat megakaryocyte cDNA library with a synthetic oligonucleotide probe corresponding to an 8-amino acid sequence specific for c25KG, a GTP-binding protein previously isolated from human platelet cytosol fraction [(1989) J. Biol. Chem. 264, 17000-17005]. The cDNA has an open reading frame encoding a protein of 221 amino acids with a calculated Mr of 25068. The protein is designated as ram (ras-related gene from megakaryocyte) protein (ram p25). The amino acid sequence deduced from the ram cDNA contains the consensus sequences for GTP-binding and GTPase domains. ram p25 shares about 23%, 39% and 80% amino acid homology with the H-ras, smg25A and c25KG proteins, respectively. The 3.5-kb ram mRNA was detected abundantly in spleen cells.

Carbonyl reductases from rat testis and vas deferens. Purification, properties and localization.

Three enzyme forms (T1, T2, T3) from rat testis and two from rat vas deferens (V1, V2) of carbonyl reductase have been highly purified to apparent homogeneity. These carbonyl reductases from rat reproductive organs have several similarities in terms of molecular mass (32-33 kDa), isoelectric point (pI 5.9-6.4), immunochemical properties, cofactor requirement (NADPH dependency) and sensitivity to sulfhydryl reagents. The isoenzymes from the vas deferens (V1, V2) have similar catalytic activities, whereas those from the testis (T1, T2, T3) showed different catalytic activities from each other. All enzymes, however, reduced quinones, aromatic aldehydes and ketones, while T3, V1 and V2 were characterized as possessing high affinity towards prostaglandins. An immunoinhibition study using a specific antibody indicated that these enzymes were solely responsible for the overall catalytic activities of 13, 14-dihydro-15-oxo-prostaglandin F2 alpha, 4-benzoylpyridine, and 4-nitroacetophenone reduction and prostaglandin F2 alpha oxidation in both testis and vas deferens cytosol. The immunohistochemical staining revealed a positive immunoreactivity to antibody only in the Leydig cells of the testis, but neither the germ cells nor Sertoli cells in the seminiferous tubule. The staining also showed that the enzymes in the vas deferens were primarily localized in mucosal epithelium cells.

S. cerevisiae genes IRA1 and IRA2 encode proteins that may be functionally equivalent to mammalian ras GTPase activating protein.

The IRA1 and IRA2 genes of S. cerevisiae encode closely related proteins that also share homology with mammalian GAP (ras GTPase activating protein). The RAS1 and RAS2 proteins overexpressed in ira mutants accumulated in the GTP-bound form, whereas in the wild-type strain the proteins were found mostly in the GDP-bound form, indicating that IRA1 and IRA2 negatively regulate the level of RAS-GTP. In contrast, the RAS2Val-19 or RAS2Thr-66 mutant protein was bound to GTP in high amounts irrespective of the IRA genotype. Overexpression of bovine GAP suppressed the phenotypes of ira mutants by reducing the level of RAS-GTP, suggesting that IRA proteins may be functionally analogous to mammalian GAP.
