Ultrasonography of the sublingual space.

Fifty-three patients were evaluated by ultrasonographic visualization of the sublingual space as part of routine neck sonography. Delineation of the sublingual gland was good or fair in 93% of the cases. The normal gland was 32.06 mm long in the sagittal plane. The submandibular duct was identified in 53% of the cases and measured less than 2.5 mm in diameter. The sublingual vessels were detected in 93% of the cases and had almost the same mean diameter as the duct. Four cases of frequent pathologies in this region were also examined. Ultrasonography is the examination to use for diseases of the sublingual space.

[Leukocyte activity and occurrence of tissue injury by G-CSF].

G-CSF (granulocyte-colony stimulating factor) is one of the cytokines which increase and activate neutrophils. These effects have been confirmed by a lot of clinical studies. Although only few side effects have been reported so far, we saw a patient with hypertension, facial edema, and severe headache after G-CSF administration. We have measured the sticking strength of leukocytes to the walls of narrow pores in modified Nuclepore filtration method of Kikuchi, in various clinical situations, and we have denoted the pore filtration time as the rheological activity of leukocytes (RAL). In this study, we tried to measure RALs of patients after G-CSF administration (60-100 micrograms) and to compare them with those of non-treated normal individuals. We also checked serum levels of granulocyte elastase in the patients. In the static state (in which neutrophils were not stimulated), RALs of the patients were 8.0 +/- 1.5sec. [N = 8], and those of the controls were 3.8 +/- 1.6sec. [N = 21] (p < 0.001). In the activated state (in which neutrophils were stimulated by formyl-methionyl-leucyl-phenylalanine), PALs of patients were quite high, i.e. those of the patients and controls were 592 +/- 322sec. and 69 +/- 62sec., respectively (p < 0.001). In all cases given G-CSF, the serum levels of granulocyte elastase were above the normal range. In view of our results, we it is possible that highly activated leukocytes would injure tissue and then induce organ failure and several symptoms, as in this study.

[General anesthesia conducted twice in myotonic dystrophy].

A 42-year-old woman with myotonic dystrophy underwent general anesthesia twice. In the first operation, muscle rigidity was recognized when the patient was placed on lithotomy position under anesthesia with thiopental and nitrous oxide. In the second operation, rigidity was not observed under vecuronium, sevoflurane-nitrous oxide anesthesia. Neuromuscular monitoring of this patient did not show any difference of neuromuscular blockade with vecuronium compared with other 9 normal patients. We conclude that vecuronium and sevoflurane can be used safely in a patient with myotonic dystrophy under neuromuscular monitoring.

[Interaction between vecuronium and pancuronium].

Interaction between vecuronium bromide (VB) and pancuronium bromide (PB) with regard to a change in duration of action was investigated in 32 elective surgical patients divided into four groups. Initial and supplemental drugs were as follow; group I: VB-->VB, group II: PB-->VB, group III: VB-->PB, group IV: PB-->PB. The muscle response was quantified electromyographically. Anesthesia was induced with thiopental. VB or PB 0.08 mg.kg-1 (initial dose) was given to facilitate endotracheal intubation. Anesthesia was maintained with 66% nitrous oxide and 2% sevoflurane in oxygen. Supplemental dose (0.015 mg.kg-1) of the muscle relaxants was administered at 10% recovery of twitch height. Duration of action was defined as the interval between administration and 10% recovery. Duration of action of supplemental doses of VB was significantly longer in group II than in group I. That of PB was significantly shorter in group III than in group IV. Therefore, it should be noticed that duration of action of supplemental relaxant is largely modified by the initial one.

[A case of yellow nail syndrome presenting as recurrent pneumonia].

We report a case of yellow nail syndrome (YNS) associated with recurrent episodes of pneumonia. The patient, a 31-year-old female, presented with yellow nails, chronic bronchitis and pleural effusion. The etiology of YNS remains unknown although it has been postulated to be associated with lymphatic abnormalities. YNS is a rare disease with only 22 cases reported in Japan. However, YNS is often associated with respiratory infections, and should thus be borne in mind.

An experimental model of acute liver injury using multicellular spheroids composed of rat parenchymal and non-parenchymal liver cells.

Massive hepatic cell necrosis can be induced by Corynebacterium parvum and lipopolysaccharide (LPS) in rats. In this model, serum LDH, GOT and GPT activities are significantly increased in vivo within several hours after LPS injection. An in vitro experimental acute liver injury animal model was produced by using multicellular spheroids composed of rat parenchymal and non-parenchymal liver cells. These multicellular spheroids were prepared by detaching the confluent monolayer on the collagen-conjugated thermo-responsive polymer coated culture dish at a temperature below the lower critical solution temperature and culturing it on the non-adhesive substratum. LPS caused clear elevations of GOT, GPT and LDH activities from these spheroids into the medium. However, the increase of LDH activity was only observed in the monolayer culture system. These results suggest that the multicellular spheroids of liver cells are useful models as an alternative to animal tests for hepatotoxicity.

Clinical study of 47 patients with reoperation after open mitral commissurotomy.

Mitral lesion in 47 patients who received reoperation after an initial open mitral commissurotomy (OMC) for mitral stenosis (MS) was studied. The diagnosis before OMC consisted of pure MS in 38 patients, with mild mitral regurgitation (MR) in 8 and with moderate MR in 1. At OMC the diameter of the mitral orifice was a mean of 1.3 +/- 0.5 cm and a mobile cusp was found in 12 and a thickened cusp in 35 patients. Subvalvular fusion was seen in 15 patients, calcification of cusp or subvalvular region in 10 and left atrial thrombus in 6. After OMC, the diameter of the mitral orifice was opened to a mean of 3.2 +/- 0.3 cm. The interval between the initial operation and the time of reoperation was a mean of 124 months. The mitral lesion necessitating reoperation was MS in 18, moderate to severe MR in 10, and MSR in 19. At reoperation the mitral orifice ranged in diameter from 0.8 to 4.2 cm (mean, 2.3 +/- 0.8 cm). There was no correlation between the occurrence of MR or restenosis and the subvalvular division at initial OMC. Left atrial thrombus was identified in four and calcification in 17 patients. At reoperation 46 patients received mitral valve replacement, while only 1 received a second OMC. The removed valve showed no pathological findings of active rheumatic lesion but more severe calcification and hyalinization of the valve than those at initial operation. There were three early deaths (6.4%) after reoperation. The recurring mitral lesion seemed to consist of deteriorated pathological changes of the valve and MR and thus mitral valve replacement was recommended to provide a favorable operative result for non-flexible lesion after OMC.

[Pathogenesis of development of lung cancer in idiopathic interstitial pneumonia--growth factors in bronchoalveolar lavage fluid].

It has been generally accepted that lung fibrosis as in idiopathic interstitial pneumonia (IIP) is frequently associated with the development of lung cancer. This observation implies that the mechanism involved in carcinogenesis and/or enhanced proliferation of cancer cells is common to the fibrosing process. However, there are few studies reported on the pathogenesis of associated lung cancer except for several studies assessed from the point of view of surgical pathology. This study was undertaken to learn whether BAL fluid, which reflects the local milieu of the fibrosing process, enhances the proliferation of human lung cancer cell line Lu-99, Lu-65 and rat lung fibroblasts as assessed by 3H-thymidine incorporation. BAL fluid was obtained from patients with IIP (n = 8) and normal volunteers (n = 8). BAL fluids from patients with IIP enhanced the mean incorporation of 3H-thymidine of Lu-99 up to 3.6 times (p < 0.01) compared to that of normal volunteers. Furthermore, the mean incorporation of Lu-65 was increased up to 1.8 times (p < 0.05) by BAL fluids from patients with IIP. In contrast, BAL fluids from patients caused no significant increase of the mean incorporation of rat lung fibroblasts as compared to normal BAL fluids. The enhancing activities on the growth of cancer cell line Lu-99, Lu-65 were eluted in several fractions by high performance liquid chromatography using Superose 12. These observations indicate that the BAL fluid of IIP patients contains factors enhancing the growth of cancer cells.

[Resuscitation from experimentally induced hemorrhagic shock with hypertonic saline (effects on thoracic duct lymph flow and tissue oxygen tension)].

We compared the effect of hypertonic salt solution (7.2%, HS) with that of normal saline (NS) and lactated Ringer's solution (LR) for the treatment of hemorrhagic shock. We monitored hemodynamic parameters, thoracic duct lymph flow, and tissue oxygen tension over 3 hours after hemorrhage. Twenty-seven anesthetized mongrel dogs (0.5% halothane) were bled to an aortic pressure of 60 mmHg for 90 min following 40 mmHg for 30 min and then they were resuscitated with each solution. In NS group, the volume transfused was twice the bled volume, and in other two groups, each solution was transfused providing equal amounts of sodium as NS group. We found that hemodynamics were restored in HS group as well as in other two groups. On the other hand, thoracic duct lymph flow and tissue oxygen tension of renal cortex and liver increased significantly over other two groups. We conclude that small volume resuscitation with 7.2% NaCl may be effective in the initial treatment of hemorrhagic shock from the view of tissue circulation in vital organs.

Differentiation of thermophilic species of Campylobacter, in particular C. coli and C. jejuni, with atypical characteristics, by analysis of protein-banding profiles on non-denaturing polyacrylamide gels.

The protein-banding patterns after electrophoresis of 23 strains of thermophilic Campylobacter, including 17 strains of C. coli and C. jejuni which had atypical characteristics with respect to the hydrolysis of hippurate and susceptibility to nalidixic acid, were characterized. Of the atypical strains of C. coli and C. jejuni 16 out of 17 gave protein-banding patterns which were essentially identical to those of typical strains, and distinct patterns were obtained from strains of two other species. The banding patterns of soluble proteins on non-denaturing polyacrylamide gels after electrophoresis appear to be useful for the differentiation of atypical strains of C. coli and C. jejuni. Strain 11791 which was originally identified as C. coli was identified as C. lari from its protein-banding profile.

[Evaluation of renal graft function with radionuclide dynamic images--application of modified Rutland's analytical method].

An uptake coefficient (UC) is estimated with applying the method proposed by Rutland to the renal dynamic images with 99mTc-diethyltriamine pentaacetate (DTPA). UC estimated with this method is proportional to glomerular filtration rate in the original Rutland's method the cardiac and renal time-activity curves (TAC) were used for input and output functions, respectively. In this paper, our method of estimating an input and output function by applying factor analysis method to renal dynamic images and it's clinical application are presented. In 11 patients, U"s estimated by this modified method revealed good correlations between the UCs and CCr (r = 0.75), between UCs and effective renal plasma flow (r = 0.67), and inverse correlations between UCs and serum creatinine values (r = 0.65). The results showed that UC in this method can be a useful parameter for grafted renal function. This method is simple in technique and applicable for the evaluation of split renal function in renal transplant donors.

[Toxic effects of ifosfamide in the treatment of bone and soft tissue sarcomas].

A total of 64 courses of ifosfamide (IFM) treatments for sarcoma patients were evaluated for toxic effects. A dose of 100 ml of 1/2 diluted Maalox by normal saline was instilled into urinary bladder with clump of catheter for 15 min for uroprotection instead of Mesna that was not available in Japan at that time. Forty-six courses of ifosfamide treatment in 12-16 g/m2 doses with daily Maalox instillation resulted only one hemorrhagic cystitis, while 18 courses at a dose of 6-10 g/m2 of IFM without Maalox eventuated in 5 cases of hemorrhagic cystitis. Forty-six courses of ifosfamide treatment at a dose of 12-16 g/m2 (mean dose of 14.4 g/m2) for 5 to 6 day continuous infusion were evaluated also for myelosuppression, nephrotoxicity, neurotoxicity, and other toxicities. Myelosuppression was acceptable, although the absolute neutrophil count of the 80% course was below 500. No patient showed neurotoxicity characterized by confusion or somnolence. No course resulted in abnormal serum creatinine elevation, although two of 46 courses caused an abnormal decrease of creatinine clearance. One patient had arrhythmia that required medical treatment after 5 courses of IFM treatment.

[Immunocytochemical investigations of occluded saphenous vein grafts].

Occluded saphenous vein aorto-coronary bypass grafts were investigated using two types of monoclonal antibodies, specific to either the muscle actin (HHF 35) or the macrophage (HAM 56). The early post operative occlusions of saphenous vein grafts were associated with the deposition of mural thrombi with little intimal proliferation. The late occlusions of saphenous vein grafts were associated with intimal fibrous proliferation without mural thrombus formation. Proliferated smooth muscle cells were a major constituent of intimal fibrous proliferation in both early and late occluded grafts. Macrophages were not detected in both groups. The proliferation of smooth muscle cells in the intima may play a role in the development of late occlusion, rather than the organizational process of accumulated mural thrombi.

[Fat-soluble vitamins: biotransformation].

Fat-soluble vitamins are essential for growth and normal functions of human body. These vitamins are generally absorbed from the gastrointestinal tract only when bile is present. Recently, it is recognized that fat-soluble vitamins have hormone-like functions in the human body. The carotene (provitamin A) is a remarkably potent source of vitamin A, and beta-carotene is the prominent type of carotene in nature. It plays an essential role in the function of the retina. With regard to the biotransformation, more than 90% of the intake of performed vitamin A is in the form of retinol esters, usually as retinyl palmitate. Vitamin A is readily absorbed from the normal gastrointestinal tract, and the absorption of the vitamin is related to that of lipid. Prior to entering the circulation, the hepatic retinyl ester is hydrolyzed and retinol itself is released from the liver. In the blood, vitamin A is present mainly as retinol associated as a protein complex. Normal calcium and phosphate metabolism is dependent on proper levels of vitamin D, and blood levels of these ions are influenced by gastrointestinal absorption, metabolism, and renal excretion of vitamin D. Both vitamin D2 and vitamin D3 are absorbed without esterification from the small intestine. Most of the vitamin appears first in lymph and primarily in the chylomicron fraction as a lipoprotein complex. Bile is essential for adequate intestinal absorption and deoxycholic acid is the most important constituent of bile.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of discrete type of subpulmonary stenosis].

We reported a 8-year-old girl having discrete type of subpulmonary stenosis. This patient was associated with atrial septal defect (ASD), valvular pulmonary stenosis, and persistent left superior vena cava (PLSVC). The incidence of this type of subpulmonary stenosis is less frequent than that of subaortic lesion. ASD was closed by the autologous pericardium and a transannular patch was used for enlargement of the right ventricular outflow tract followed by commissurotomy of the pulmonary valve and resection of subpulmonary discrete lesion. She was uneventful and postoperative pulsed doppler echocardiogram showed good results.
