Bacithrocins A, B and C, novel thrombin inhibitors.

Novel thrombin inhibitors, bacithrocins A, B and C, have been isolated from the culture broth of Bacillus laterosporus Laubach NR 2988. The structures of these inhibitors have been determined to be N-acyl-L-phenylalanyl-DL-arginal by the 2D-NMR experiments on their oxidation products and by amino acid analysis. Bacithrocin A inhibits thrombin, factor Xa and trypsin with IC50s of 48, 13 and 0.65 microM, respectively, which are similar to those of bacithrocins B and C. Bacithrocins prolong the clotting time induced by thrombin and factor Xa.

Pyogenic psoas muscle abscess: report of three cases.

We report three cases of primary psoas muscle abscess. Two of these cases were treated by open drainage through a lumbar incision and the other was by ultrasonography (USG)-guided percutaneous aspiration of the abscess. Antibiotic therapy was also instituted in all cases. Post-operatively, the patients recovered well with no sign of recurrence. USG-guided percutaneous drainage combined with appropriate antibiotic therapy appears to be the first choice procedure for treatment of the primary psoas abscess if correct diagnosis is promptly made and the procedure can be performed.

[G-CSF and G-CSF receptor].

Relationship between leucocytosis and G-CSF blood concentration were examined. Increase in blood concentration of G-CSF of surgical patient and dialysis patient with leucocytosis were noticed. Determination of G-CSF receptor on leucocyte was performed by the flow cytometry using biotinylated G-CSF. Modulation of G-CSF receptor on leucocyte at surgery and dialysis was identified in vivo. Studies of G-CSF blood concentration and modulation of G-CSF receptor are useful in analysis of dynamics of leucocyte and analysis of infectious disease.

Prognosis after pacemaker implantation in cardiac sarcoidosis in Japan. Clinical evaluation of corticosteroid therapy.

Corticosteroids (CS) are useful drugs for the treatment of cardiac sarcoidosis with severe conducting defects due to sarcoid granuloma. Despite the continuous administration of CS, many patients with severe cardiac involvement may eventually die of congestive heart failure. The purpose of this study was to evaluate the efficacy of CS in patients who had a pacemaker implanted. Questionnaires were obtained from 29 institutes, and 34 cardiac sarcoidosis patients (8 males and 26 females) with pacemaker implantation were enrolled in this survey. We analyzed the survival period in these patients by the Kaplan-Meier method. There was no statistically significant difference in the survival of these patients in terms of their age, sex or disease duration (time from the onset of sarcoidosis to cardiac involvement). However, their survival was affected by the grade of dyspnea, the presence of heart failure, and certain abnormal findings on a myocardial scintigram and echocardiogram. In order to evaluate the effect of CS on the prolongation of survival, we measured the survival of the patients treated with CS and those not treated with CS. However, because of the small number of patients not treated with CS, we were unable to detect any statistically significant difference in survival. Therefore, we analyzed 104 cases in order to evaluate CS therapy: the 34 cases from the questionnaires and 70 cases reported in the literature over the last 10 years.(ABSTRACT TRUNCATED AT 250 WORDS)

Interleukin-5 mRNA levels in blood and bone marrow mononuclear cells from patients with the idiopathic hypereosinophilic syndrome.

High blood eosinophil counts in humans are usually due to parasitic infections, allergic processes or malignant diseases. Interleukin-5 (IL-5) is thought to be the principal eosinopoietic stimulus in most of these patients. As the causes of persistent eosinophilia in patients with the idiopathic hypereosinophilic syndrome (HES) are (by definition) unknown, a semi-quantitative assay for IL-5 mRNA in eosinophils and mononuclear cells was carried out using samples from 11 patients with HES. In three patients, unstimulated peripheral blood mononuclear cells (PBMC) and bone marrow mononuclear cells (BMMC) had markedly raised levels of IL-5 mRNA. In four of five patients studied, phorbol myristate acetate (PMA) stimulation of PBMC induced them to produce more IL-5 mRNA than four healthy subjects. In five patients, bone marrow IL-5 mRNA levels were related to blood eosinophil counts. Blood eosinophils from all six patients tested, and bone marrow granulocytes from four patients had undetectable levels of IL-5 mRNA. It was concluded that HES can be separated into two groups. One has high levels of IL-5 mRNA and/or an enhanced IL-5 mRNA response to stimulation. They may well respond to treatments which inhibit the effects of this cytokine on eosinophil progenitor cells. The second group appears to have a disease which is IL-5 independent.

Expression of lacto-N-fucopentaose III (CD15)- and sialyl-Lewis X-bearing molecules and their functional properties in eosinophils from patients with the idiopathic hypereosinophilic syndrome.

As the carbohydrate lacto-N-fucopentaose III (CD15 antigen or X-determinant) and its sialylated derivative sialyl-Lewis X are involved in the adhesion of cells rolling along the surface of endothelial cells, experiments were done to study the presence of these molecules on human eosinophils from patients with the idiopathic hypereosinophilic syndrome. Normal-density eosinophils from some patients showed higher levels of expression for lacto-N-fucopentaose III than light-density eosinophils. In contrast, sialyl-Lewis X was highly expressed by light-density eosinophils. Activation of normal-density eosinophils with calcium ionophore A23187 resulted in increased expression of these molecules for a short time. Monoclonal antibodies to these carbohydrates stimulated eosinophils to secrete eosinophil cationic protein, but not eosinophil peroxidase, and acted as costimulatory signals for C3b-induced degranulation of eosinophil cationic protein. It was suggested that CD15 and sialyl-Lewis X might contribute to eosinophil-mediated tissue injury in patients with the idiopathic hypereosinophilic syndrome.

[Hemodynamic effects of Amrinone in patients following open heart surgery].

The hemodynamic effects of amrinone, a phosphodiesterase III inhibitor, were studied early after cardiac surgery in 10 patients. CI, SVI and SWI increased significantly. SAP, mPCWP, TSVR and PVR decreased significantly. DO2 and VO2 increased significantly. HR and DP were not affected. These data demonstrate that amrinone is a valuable inotropic agent with vasodilating effect for the early post-operative management of cardiac surgical patients, since it augments left ventricular performance without increase of myocardial oxygen consumption and induction of arrhythmia.

[Effect of nicardipine cardioplegia on A-V conduction system in coronary bypass surgery].

The effect of diltiazem and nicardipine cardioplegia on A-V conduction system was analyzed in 75 patients undergoing coronary bypass operation. The dose of these drugs into the crystalloid cardioplegia was 10 micrograms/kg in nicardipine group (n = 29) and 100 micrograms/kg in diltiazem group (n = 46). Among these patients, there was no operative death and no patient required IABP after surgery and there was no difference on the post operative CK-MB level, on the cardiac index and on the pulmonary pressure between two groups. The number of patients who had complete A-V block after release of aortic cross clamping was significantly smaller in nicardipine group than in diltiazem group: 45.7% in diltiazem group vs. 6.9% in nicardipine group at 30 minutes and 21.7% vs. 6.9% at 60 minutes. We conclude that the suppressive effect on A-V conduction system was significantly lower in nicardipine cardioplegia than in diltiazem cardioplegia in the patients undergoing coronary bypass operation.

Size-regulation and biochemical activities of the multicellular spheroid composed of rat liver cells.

It was reported that cell-sheets composed of rat liver cells could be obtained by using a collagen-conjugated thermo-responsive polymer, poly-N-isopropyl acrylamide, as a cell substratum and they were then able to transform into multicellular spheroids in the hydrophobic dish. In this study, we succeeded in easily obtaining the cell-sheets with the intended size by using the etched substratum. Biochemical analyses of spheroids derived from cell-sheets with areas of 0.2, 0.5 and 2.5 cm2 were carried out. Each cell-sheet rapidly aggregated at the same rate and finally turned into a multicellular spheroid within 5 days after the detachment. However, both decreases of DNA and LDH contents in cell-sheets with an area of 2.5 cm2 within 24 hr after the detachment were more obvious than in the others, respectively. It is tempting to speculate from these data that the vital spheroids with the desired size can be easily obtained by using our new method.

Maturation of human megakaryocytic cell line (CMK-7) on the human umbilical vein endothelial cell (EC) monolayer.

Human umbilical vein endothelial cells (EC) were studied to determine whether they were capable of stimulating the maturation of a human megakaryocytic cell line CMK-7. CMK-7 overlaid on EC monolayers in direct contact conditions (CMK-7EC1) and in non-contact conditions (CMK-7EC2), were compared with unstimulated CMK-7 (CMK-7US) and CMK-7 treated with 10(-8) M phorbol ester (CMK-7TPA). The increases in cell-size, CD41b and CD42 positivity, adenosine triphosphate (ATP) and serotonin (5-HT), platelet factor 4 (PF4), beta-thromboglobulin (beta-TG), and DNA contents, indicated both stimulated endomitosis and cytoplasmic maturation in the CMK-7TPA. In contrast, CMK-7EC1 increased in size, in ATP and 5-HT contents and in PF4 and beta-TG contents, without an increase in DNA content, indicating that only cytoplasmic maturation occurred. These findings showed that EC was able to stimulate the cytoplasmic maturation of CMK-7 only under direct contact conditions. Furthermore, the CMK-7EC1 on the EC monolayer developed long cytoplasmic processes that resembled those of megakaryocytes in the terminal stage.

Development-related changes in matrix metalloproteinase expression in human aortic smooth muscle cells.

BACKGROUND: It is known that extracellular matrix-degrading enzymes play an important role in tissue remodeling and that large amounts of structural proteins including types I, III, IV, and V collagens and elastin are produced by smooth muscle cells (SMC) in the arterial wall. We have recently shown that matrix metalloproteinases (MMPs) produced by human aortic medial smooth muscle cells are closely related to the proliferation of the cells, leading to the formation of atherosclerotic plaques, characteristic of intimal remodeling. For a better understanding of the mechanism of atherogenesis, therefore, it is important to clarify the relationship between the production of matrix-degrading enzymes and artery development. EXPERIMENTAL DESIGN: In vivo or in vitro synthesis of MMPs by SMC was analyzed by immunohistochemistry and immunoblotting. Elastase activity in the culture medium was also estimated. RESULTS: Production of proMMP 1, 2, and 3 was detected in cultured SMC isolated from the aortas of both neonates and fetuses; in medial SMC cultured from young individuals, production of proMMP-1 and -3 was extremely decreased, but was apparent in intimal SMC. Immunohistochemical observation indicated that in the media of fetal or neonatal aorta, SMC synthesized large amounts of the three proMMPs; in aortas from older individuals, proMMP-2, but not proMMP-1 and -3, was detected in the media, and relatively large amounts of proMMP-1, -2, and -3 were produced by SMC in the slightly thickened intima. Assay of elastase activity in the culture medium gave results similar to those for MMPs. CONCLUSIONS: We conclude that the production of proMMP-1 and -3 is associated with phenotypic modulation of SMC to a "synthetic" state, and that the ability of SMC to produce MMPs plays an important role in the development and/or aging of the human aorta through remodeling of the extracellular matrix; furthermore elastase is also involved in these processes in the arterial wall.

[Vitamin and mineral status in chronic renal failure--the effect of protein restriction].

Essential amino acid (EAA), vitamin and mineral abnormalities are found in patients with chronic renal failure (CRF). We considered altered dietary restriction to be one of many factors that can promote these abnormalities. We determined the EAA, vitamin and mineral content of low-protein diets and vitamins and minerals in the blood of outpatient populations with various degrees of CRF (serum creatinine 3.6-6.8mg/dl). The results obtained were as follows: 1. 20g-protein/day diet can provide an adequate amount of EAA. 2. Low protein diets (30g protein/day diets) were inadequate in vitamins A, B1, B2 and B6. Vitamins A, B1, B2 and B6 in these diets were 70-80%, 40-50%, 30-35% and 20-25% of the minimum daily requirements, respectively. 3. Low protein diets (30g protein/day diets) were inadequate in Ca, Fe and Zn. The Ca and Fe contents of these diets were 30% and 50% of the minimum daily requirements, respectively. The Zn content of a diet rich in vegetable protein, although low in protein, overall (vegetable: animal = 2:1), was about 55% of the minimum daily requirement. 4. About 21.4% of the CRF patients had low levels of plasma vitamins B1. About 28.6% and 33.3% of the CRF patients had low levels of plasma vitamins B2 and B6, respectively. Plasma folic acid levels were not inadequate in these patients. 5. About 28.6%, 50% and 35.7% of the CRF patients had low levels of plasma Ca, Fe and Zn, respectively. In conclusion, it seems that supplementation of vitamins B1, B2, B6, Ca, Fe and Zn should be considered for protein-restricted CRF patients.

[Simultaneous bilateral testicular tumors: a case report].

We report a case of bilateral simultaneously-occurring testicular tumors. A 43-year-old man was admitted to our clinic with a complaint of right inguinal pain in May 1992. Under the diagnosis of bilateral testicular neoplasms, bilateral high orchiectomy was performed. Histological examination revealed typical seminoma of bilateral testes. The patient was diagnosed with stage IIIO seminoma, and he was treated with combination chemotherapy (PVB). No evidence of disease has been seen after chemotherapy.

Blood levels of leukotrienes (LTC4, D4, E4, B4) in asthmatic patients during attack and remission.

To assess the contribution of the leukotrienes, LTC4, D4, E4 and B4 during bronchial asthma attacks, simultaneous determination was made of their levels in venous blood. 25 patients with bronchial asthma (15 atopic types, 10 non-atopic types) participated in this study and 4 normal controls were used. Samples were obtained using heparinized syringe from the patients before treatment. A radioimmunoassay was conducted to measure LTs after purification with a Sep-pak column and separation by HPLC. In normal subjects, the levels were less than the minimal detectable amounts. LTC4, D4, E4 and B4 during asthmatic attacks were 100 +/- 179, 88 +/- 116, 479 +/- 291, and 55 +/- 73 (Mean +/- SD) pg/ml respectively (n = 27). Peptide LTs in remission were below minimal detectable levels. LTD4 in patients with moderate attacks was significantly (p < 0.05) higher than in those with mild attacks. Peptide LTs in moderate attack exceeded those in mild attacks, although not to a statistically significant degree. No significant differences in LT during attacks could be detected in atopic or non-atopic type patients. LTs would thus appear importantly involved in asthmatic attacks in atopic and non-atopic type patients, although other chemical mediators may give rise to airway inflammation.

[The clinical effect of factor XIII on drug-induced hemorrhagic cystitis].

The therapeutic effects of Factor XIII (F XIII) concentrate against drug-induced hemorrhagic cystitis (HC) was investigated. HC occurred in 4 children with malignant disease during anti-cancer chemotherapy. Two (CML and T-ALL) of 4 patients developed HC after the administration of high dose cyclophosphamide as conditioning for allo bone marrow transplantation or peripheral blood stem cell autografts, and the other 2 patients (rhabdomyosarcoma, Wilm's tumor) developed HC after the administration of ifosfamide for relapse. When F XIII concentrate at a dose of 20 to 230 U/kg was administrated immediately after the onset of HC, the symptoms, i.e., bladder irritability and macrohematuria disappeared within a few days. The F XIII serum levels of those patients were low (27-57%), and the levels increased (63-230%) after administration of F XIII concentrate. The two patients with relapsed solid tumor showed no symptoms of HC during subsequent ifosfamide treatment when F XIII concentrate was administrated to maintain a normal F XIII range. These results suggest that the administration of F XIII concentrate may be useful for the prophylaxis and treatment of drug-induced HC in patients with a low F XIII level.

[Molecular cloning and posttranslational processing of human preprothyrotropin-releasing hormone].

Human PreproTRH gene and its hypothalamic cDNA were cloned. The human preproTRH protein contains six copies of TRH progenitor sequence flanked by paired basic amino acids. Radioimmunoassays with specific antibodies raised against five human preproTRH cryptic peptides and chromatographic analyses identified that the posttranslational processing of preproTRH in hypothalami and placentae could be different. Moreover, transfection assays using human preproTRH promoter-luciferase chimeric-plasmids demonstrated that human preproTRH promoter activities were regulated negatively by T3 in GH3 cells. Cloning of the human preproTRH gene would facilitate the elucidation of the molecular mechanism of actions of TRH and TRH cryptic peptides in human hypothalamo-pituitary-thyroid axis and central nervous system.
