Preserved activation of thyrotropin receptor antibody to stimulate thyroid function despite long-term treatment in euthyroid patients with Graves' disease.

Clinical evaluation was conducted to ascertain whether thyrotropin receptor antibody (TRAb) in the normal range may still be involved in the regulation of thyroid function after prolonged treatment for Graves' disease. All patients (n = 33) were treated with antithyroid drugs for an average of 10.6 years and were under euthyroid conditions in which normal blood levels of tri-iodothyronine (T3) were significantly correlated with blood thyrotropin (TSH) levels, but not with titers of TRAb. A significant correlation was observed between TRAb titer and thyroid-stimulating antibody (TSAb) activity. In contrast, this correlation was not found in normal subjects. After administration of T3 (75 microg daily for 8 days), the patients showed increased levels of T3 with concomitant suppression of TSH levels. Under these conditions, linear regression analysis showed significant correlations of TRAb titer and TSAb activity with 24-h thyroid radioiodine uptake (r = 0.641 and 0.621 respectively, P < 0.01), in contrast to declining blood thyroxine levels. Moreover, the immunoglobulin G (IgG) of the patients precipitated to a greater extent than IgG from normal subjects a peptide consisting of the amino acid sequence near the terminus of the human TSH receptor. These findings indicated that TRAb at normal levels possessed significant unremitting activities on thyroid function despite long-term treatment in euthyroid patients with Graves' disease.

Primary modulation by oestradiol of the production of an oviduct-specific glycoprotein by the epithelial cells in the oviduct of newborn golden hamsters.

The effects of steroid hormones (oestradiol and progesterone) on the appearance of a golden hamster oviduct-specific glycoprotein (GHOGP) in the epithelium of the oviduct of the newborn golden hamster were investigated by immunoblotting and immunohistochemical staining with a GHOGP-specific monoclonal antibody. Newborn golden hamsters (1.5 days old) were injected daily with oestradiol (1 microgram) or progesterone (10 microgram). An oviductal extract of oestradiol-treated golden hamsters for 4 days apparently immunoreacted with the monoclonal antibody on a broad band with a molecular mass of more than 200 kDa by immunoblotting under reducing conditions. This broad band was consistent with the migration of GHOGP in an extract of adult oviducts. Consecutive daily injections of oestradiol induced the appearance of GHOGP in undifferentiated epithelial cells of the oviduct of neonates. In oviducts of oestradiol-injected animals, GHOGP was first detected in the Golgi region and then increased in amount to fill the supranuclear cytoplasm of the epithelial cells. The inductive effect of oestradiol was dose-dependent. In contrast, consecutive daily injections of progesterone had no effect on the appearance of GHOGP in the oviductal epithelium. The effects of oestradiol and progesterone in organ culture of oviducts were examined in vitro, by culturing oviductal organs from 1.5-day-old newborn golden hamsters in chemically defined medium supplemented with oestradiol or progesterone for 2 days and then subjected to immunohistochemical staining. The immunoreaction was detected only in the epithelial cells of oestradiol-treated oviducts at concentrations of > 0.01 ng ml-1, but not in the cells of untreated and progesterone-treated oviducts. These results indicate that the production of GHOGP in the epithelial cells of the oviduct of newborn golden hamsters is induced by oestradiol both in vivo and in vitro. It is suggested that oestradiol may be involved in the synthesis of GHOGP in the oviduct during postnatal development of golden hamsters.

Effect of rolling bed on decubitus in bedridden nursing home patients.

Decubitus is one of the most difficult management problems encountered in bedridden elderly patients. Relief of pressure over decubitus is the most important principle of the management. We developed a rolling air cushion bed which turns the patient to a 15-degree inclined lateral position with an inflating ripple mattress, a longitudinally aligned air inflatable tube. The position of the patients was changed between right and left laterals and to supine every 15 minutes automatically. Nineteen bedridden patients with decubitus used the rolling air cushion bed for 3 months and 12 bedridden patients with decubitus used a conventional bed and had their position changes every 2 hours by care givers. Severity of decubitus was divided into 4 grades and the decubitus significantly improved from 2.8 (S.E. 0.2) to 2.0 (S.E. 0.3) after 3 months in patients using the rolling air cushion bed, while in patients with conventional beds it changed from 3.0 (S.E. 0.2) to 3.2 (S.E. 0.2) (not statistically significant). We suggest that the rolling air cushion bed would be beneficial to decubitus relief in bedridden elderly and may relieve labor by care givers.

Peripheral blood stem cell transplantation for hepatoblastoma with microscopical residue: a therapeutic approach for incompletely resected tumor.

We report a nine-month-old boy with stage III B hepatoblastoma of caudate lobe origin. Surgical resection was attempted following six courses of chemotherapy, but viable tumor cells remained microscopically at resection margins. Subsequently, he received peripheral blood stem cell transplantation (PBSCT), whose preparative regimen being consisted of carboplatin, etoposide, tetrahydropyranyl adriamycin, and melphalan. Since then, the patient shows no relevance of local relapse or distant metastasis without any chemotherapy. PBSCT for patients with post-operative residue may improve the outcome of advanced hepatoblastoma and worth of a further clinical investigation.

Multinucleated variant endothelial cells (MVECs) of human aorta: expression of tumor suppressor gene p53 and relationship to atherosclerosis and aging.

Multinucleated variant endothelial cells (MVECs) generally exist in atherosclerotic human aorta and even in nonatherosclerotic aorta. Because the number of nuclei is increased in every MVEC, and because DNA instability was suspected, a series of oncogene expressions was conducted to clarify the nature of nuclear abnormality. The tumor suppressor gene p53 was found to be specifically expressed in the multinuclei of MVECs, while double nuclei were sometimes positive, and mononuclear typical endothelial cells were always negative for p53. Polymerase chain reaction-single-strand conformation polymorphism (PCR-SSCP) revealed extra bands in exons 5 and 7 of the p53 gene, but no additional band in exons 6 and 8. In a BCL family, BCL-2 was coexpressed in one or two nuclei in the perinuclear space of the multinuclei of MVECs, whereas MCL-1, BCL-XS/L, and BAX were all negative, indicating that the BCL-2 coding gene is expressed only in the corresponding one or two nuclei of the multinuclei. Another oncogene, c-MET (hepatocyte growth factor receptor), was universally expressed in either type of endothelial cells, but other oncogenes, k-RAS and c-ERBB2, were not expressed in either type. MVECs were derived from human aorta and therefore non-tumorous somatic cells. No morphologic evidence of apoptosis was found. Although it is unclear that the extra bands came from the MVECs or just from ECs associated with atherosclerosis, combined immunocytological studies and PCR analysis suggest that MVECs express mutant type p53.

[Method of quantitative measurement of human platelet aggregation in clinical practice].

We examined the quantitative measurement of platelet aggregation in 20 volunteers and 89 patients with ischemic heart disease (IHD). Subjects in whom platelet aggregation was induced by an adenosine diphosphate (ADP and an epinephrine solution were clearly divided into two groups: "normal" and "accentuated". We chose the maximum aggregation time for the ADP solution and the maximum transmission rate for the epinephrine solution for the quantitative measurement of platelet aggregation. The results were as follows. The maximum aggregation time with a 1.0 microM ADP solution in the normal group of 13 volunteers was 0.71 +/- 0.12 min, (mean +/- SD), in 50 IHD patients it was 0.91 +/- 0.49 min, in the "accentuated" group of 7 volunteers it was 5.34 +/- 1.18 min, in 39 IHD patients it was 6.01 +/- 1.22 min. The maximum light transmission rate with a 0.1 microM epinephrine solution in the normal group of 14 volunteers was 8.04 +/- 4.14%, in 52 IHD patients it was 13.74 +/- 5.75%, in the "accentuated" group of 6 volunteers it was 76.33 +/- 9.91%, and in 37 IHD patients it was 77.26 +/- 13.39%. The difference between the "normal" and "accentuated" groups was statistically significant (p < 0.001), with the ADP and with the epinephrine solution. Next, we examine the effect of antiplatelet therapy (dipyridamole and aspirin) on 15 patients selected from among those with IHD who had accentuated platelet aggregation when the ADP solution was added. Their maximum aggregation time was 5.98 +/- 1.24 min, and their maximum platelet aggregation rate when the epinephrine solution was added was 78.90 +/- 11.60% (10 patients), 20.00 +/- 3.24% (5 patients). After the therapies, the maximum aggregation time in 10 patients was 0.94 +/- 0.21 min (return to normal condition), and in 5 patients it was 5.90 +/- 1.09 min (not effective). The maximum platelet aggregation rate in 10 patients was 14.11 +/- 5.88% (normal condition), and in 5 patients it was 75.00 +/- 9.51% (not effective). Finally, we examined the long term effects of various antiplatelet drugs. Most of the patients in whom platelet aggregation was accentuated got well.

[Significance of MIB-1 labeling index and TUNEL labeling index in papillary carcinoma].

Papillary carcinomas, which comprise more than 80% of all thyroid gland cancers, are generally regarded as slow-growing tumors with relatively good prognosis. Histologically and pathologically, there is a close relationship in papillary carcinoma between differentiation and the clinical picture, particularly progress and prognosis, However, tumors with the same degree of differentiation may have very different growth rates. Therefore, it is important that we have some clinical indicator, in addition to differentiation, for projecting clinical prognosis, including the likelihood of postoperative recurrence, and for planning follow-up care. In this study, we used paraffin embedded specimens obtained from 40 cases of papillary carcinoma and assessed the proliferative ability of the tumor with MIB-1 staining. Moreover, in 28 of the 40 cases we used TUNEL staining to study apoptosis in cancer cells, then correlated these two indicators with known information about the clinical pathology and prognosis. The degree of differentiation and intensity of the MIB-1 labeling index correlated almost exactly. Some well-differentiated papillary carcinomas stained heavily with MIB-1. In fact, in all cases of postoperative recurrence the carcinomas were well differentiated, and over half of those showed strong MIB-1 staining. The difference between the MIB-1 labeling index for recurring cancers and non-recurring cancers was statistically significant. In contrast, there was no correlation between the TUNEL labeling index and clinical pathology or postoperative recurrence. However, in cases of well-differentiated papillary carcinoma with anaplastic reversion in the recurrence, the TUNEL labeling index was particularly low at the time of the first operation. This indicates that for the purpose of patient follow-up, an extremely low TUNEL labeling index may be an important piece of information. We also believe that the MIB-1 labeling index is another indicator of potential postoperative recurrence.

[Mutation of p53 gene and genomic instability in testicular tumors].

BACKGROUND: During the past decade, studies of human cancer have begun to yield molecular information on the identify of the multiple genetic changes in the development and progression of tumorigenesis. We investigated alterations of p53 and genomic instability in testicular tumors. MATERIALS AND METHODS: Polymerase chain reaction (PCR) single-strand conformation polymorphism was performed for analysis from exons 5 to 8 of p53 gene in 22 cases and PCR-microsatellite instability analysis using 8 microsatellite markers were conducted in 19 cases of testicular tumor. RESULTS: No mutations were noted for exons 5 to 8 of the p53 gene. Differences in unrelated microsatellites for tumor and corresponding normal DNA were detected in 5 of 19 (26.3%) cases examined. Alterations noted in more than 2 microsatellites were observed in 3 of 19 (15.8%) and categorized as replication error (RER) phenotype. Two of 7 (28.6%) seminomatous and 1 of 12 (8.3%) non-seminomatous testicular tumors patients showed RER. Two of 16 (12.5%) stage T1-3N0M0 and 1 of 3 (33.3%) stage T1-3N1-3M0-1 showed RER. CONCLUSIONS: Alterations in microsatellite instability may be involved in the development of testicular tumor.

[Drug delivery system for the treatment of osteoporosis].

The number of agents for the treatment of osteoporosis has increased over the past few years allowing more and more differentiated therapy. Calcitonin, vitamin D3, bisphosphonate, estrogen, vitamin K2 and calcium are administered for the treatment of osteoporosis in Japan. The purpose of drug delivery system (DDS) is to provide a practical approach to increasing efficacy and minimizing side effects of the drugs. In fact by the development of DDS, topical administration, prodrug and targeting therapy are used in the treatment for osteoporosis. It is well known that calcitonin in susceptible to proteolysis in the intestine. So, calcitonin must be given intramuscular or subcutaneous injection for therapy. DDS of calcitonins is studying in many routes of administration, such as nasal, transdermal, ocular, oral, bronchial, rectal and vaginal.

Experimental infection in SPF kittens with a particular calicivirus strain originally isolated from lion.

Two SPF kittens were experimentally infected via the intranasal route with a strain of calicivirus originally isolated from sick lion suffering from vesicular disease. Fever, 40.3 degrees C and 40.7 degrees C, and vesicular formation in tongue and snout were reproduced in both kittens. The infected virus was recovered from nasal, oral and rectal swabs. A longer duration of virus recovery was proved with oral swab samples taken from 1 to 10 and 12 days post infection. This suggests that tongue and oral tissues are the main tissues for virus multiplication.

[Microwave hepatic tumor coagulation therapy during stoppage of hepatic blood flow using balloon catheters].

The usefulness of microwave hepatic tumor coagulation therapy (MTC) with stoppage of hepatic blood flow using a balloon catheter was assessed. By placing a 5-French balloon catheter in a hepatic artery and 6-French balloon catheter in a hepatic vein, hepatic arterial and portal venous flow of the liver segment including the tumor was interrupted. The effects of balloon occlusion were evaluated by CT during arterial portography. MTC with stoppage of hepatic flow was performed for two patients with metastatic liver tumor 2 cm in diameter. Enhanced CT obtained after MTC showed no enhancement of the tumor, indicating complete necrosis. There were no complications and the two patients were discharged within 4 days after MTC. MTC with stoppage of hepatic blood flow is effective for treatment of hepatic metastatic tumor.

[Effectiveness of continuous intravenous prostaglandin E 1 against pulmonary hypertension].

Pulmonary hypertension is a progressive disease for which no effective therapy has been found. Short-team treatment with alprostadil alfadex (PGE 1) can improve hemodynamics but the effects of long-term treatment have not been reported. We administered intravenous PGE 1 to 16 patients with pulmonary hypertension for 4 weeks in addition to conventional therapy. Hemodynamic and symptomatic effects were evaluated. The infusion was interrupted in 7 patients (44%) because of fever and an increase in levels of C-reactive protein in serum. In the remaining 9 patients, the drug lowered pulmonary artery pressure and resistance, and imareased cardiac output significantly in 7 patients. PGE 1 can improve hemodynamics and relieve symptoms in patients with pulmonary hypertension, barring severe side effects.

[A case of renal oncocytoma associated with acquired cystic disease of the kidney].

We report a case of renal oncocytoma associated with acquired cystic disease of kidney (ACDK). A right renal mass was incidentally found on an annual ultrasonography of the kidneys in a 56-year-old female patient who had been on maintenance hemodialysis for 6 years. Computerized tomography (CT) showed a right hypervascular renal mass, suggesting a renal cell carcinoma associated with ACDK. Right radical nephrectomy was performed. The post-operative pathological diagnosis was renal oncocytoma. Renal oncocytoma in ACDK is very rare, and the pathological characteristics of oncocytoma are discussed.

[Anesthesia for a patient with von Willebrand disease and asthma].

An emergency appendectomy was performed in a patient with von Willebrand disease (vWD) and asthma. vWD is a hemostatic disorder in which the patient lacks von Willebrand factor, playing a critical role in the mediation of platelet adhesion. A 12-year-old boy with vWD underwent an appendectomy under general anesthesia. The patient had not previously received any medication for the treatment of vWD. His bleeding time was over 18 minutes. Confact F injection improved it to 6 minutes. The patient was given Confact F during the perioperative period and no abnormal hemorrhage occurred. Heat-treated factor VIII concentrates can compensate for the low concentration of von Willebrand factor and they have been designed by several different companies and individual researchers with slight variations in composition among the various products. This indicates that some of these products can not be effective in compensating for vWD. In this particular case, Confact F was suitable for this patient with vWD (II A type). In addition, the patient was suffering from asthma. To avoid introducing any superfluous irritants that might induce an asthma attack, we removed the endotracheal tube under deep anesthesia and maintained the airway with a mask until the patient became conscious.

Impaired regional fatty acid uptake and systolic dysfunction in hypertrophied right ventricle.

UNLABELLED: Little information is available regarding the determinants of systolic contractile function of the hypertrophied right ventricle (RV). The purpose of this study was to clarify the relationship between myocardial metabolism and contractile function in the hypertrophied RV due to pulmonary hypertension (PH). METHODS: Iodine-123-labeled 15-(p-iodophenyl)-3-(R,S)-methylpentadecanoic acid (BMIPP) and 99mTc-sestamibi (MIBI) SPECT were performed to calculate the RV-to-left ventricle (LV) tracer uptake ratio (RV/LV) in 21 patients with PH (6 with primary PH and 15 with chronic thromboembolic PH). The patients also underwent electron-beam CT to assess RV ejection function (RVEF) and percentage systolic wall thickening (%SWT) and right heart catheterization to measure mean pulmonary arterial pressure (mPAP). RESULTS: There were significant positive correlations between mPAP and MIBI-RV/LV (r = 0.89, p < 0.001) and between mPAP and BMIPP-RV/LV (r = 0.86, p < 0.001). However, 8 patients showed lower BMIPP-RV/LV than MIBI-RV/LV, indicating the impairment of myocardial fatty acid uptake in the RV. These patients had lower RVEF and %SWT compared to those with normal myocardial fatty acid uptake (RVEF = 28% +/- 10% compared to 40% +/- 9% and %SWT = 33% +/- 27% compared to 74% +/- 30%, respectively; p < 0.05 for both comparisons). Although mPAP did not differ between the groups, the RVEF-mPAP and %SWT-mPAP regression lines drawn from the patients with impaired myocardial fatty acid uptake were located below the lines from the patients with normal myocardial fatty acid uptake, suggesting disproportionately decreased RV myocardial contractility for a given mPAP in patients with impaired myocardial fatty acid uptake. The patients with the impaired fatty acid uptake in the RV had a significantly higher death rate (log-rank test, p < 0.05). CONCLUSION: The results from this preliminary study suggest that myocardial fatty acid uptake is impaired in the failing hypertrophied RV due to PH.

Fluorine-18-fluorodeoxyglucose PET identification of cardiac metastasis arising from uterine cervical carcinoma.

Cardiac metastasis of uterine cervical carcinoma is rare. We describe a patient with a past history of uterine cervical carcinoma who presented with metastasis to the heart, lungs and distant lymph nodes 3 yr after surgery and chemotherapy. Since the patient complained of chest pain and demonstrated electrocardiogram abnormalities, we performed echocardiography, electron beam CT and MRI, which revealed a tumor in the right ventricular wall. The tumor was assessed by 67Ga scintigraphy and 18F-fluorodeoxyglucose (FDG) PET scanning. The mean differential 18F-FDG uptake ratio of the tumor was 7.9, suggesting malignancy, which was later confirmed by myocardial biopsy. Information about the extent of the tumor and partial necrosis within it was provided by 18F-FDG PET. Although both radionuclide imaging techniques also detected metastatic lesions in the lungs and lymph nodes, 18F-FDG PET scanning detected small lesions more sensitively than 67Ga scintigraphy.
