[Afterloading interstitial brachytherapy by Ir-192 seed assembly for malignant brain tumor].

We developed and applied interstitial after-loading endocrine therapy as a cancer treatment modality. Between Sept. 1982 and May 1983, 5 patients with malignant brain tumors were treated using the removable after-loading Iridium-192 seed assembly implant technique. 4 of the 5 patients experienced recurrent gliomas after the operation alone or in combination with external post-operative radiotherapy. In 3 patients, local control was achieved and has been maintained for more than 11 months. Our preliminary brachytherapy procedures for the treatment of malignant brain tumors are described.

[Chemotherapy after prostatectomy--effects of long term administration of Pivmecillinam (PMPC)].

The therapeutic efficacy of Pivmecillinam (PMPC) against urinary tract infections after prostatectomy was investigated. PMPC was orally administered at the dose of 300 mg/day for 2-14 weeks after initial intravenous postoperative chemotherapy with other antibiotics for 0-24 days (average 5.7 days). Thirty patients underwent retropubic prostatectomy and the other 59 patients received TUR-P. The chemotherapeutic efficacy for 19 patients who had over 10(3) CFU/ml of microorganisms was as follows: The effective rate was 48.4% after 2 weeks, 53.8% after 4 weeks, 66.7% after 5-6 weeks and 100% after 7-9 weeks. The prophylactic efficacy for 70 patients who had no or less than 10(3) CFU/ml of microorganisms was 64.3% after 2 weeks, 79.6% after 4 weeks, 86.7% after 5-6 weeks and 90.0% after 7-9 weeks. The overall clinical efficacy of PMPC for 89 patients was 60.7% after 2 weeks, 74.2% after 4 weeks, 83.3% after 5-6 weeks, 90.0% after 7-9 weeks and 100% after 10-14 weeks. No severe side effects were found in these 89 cases. In conclusion, PMPC is a useful drug for the long term treatment after prostatectomy.

[Assessment of cardiac function by left heart catheterization: an analysis of left ventricular pressure-volume (length) loops].

The mechanical property of the cardiac muscle has been classically analyzed in two ways; shortening of muscle fiber, and the development of tension within the muscle. In the ejecting ventricle, left ventricular (LV) function can be analyzed by the analogous two-dimensional framework of pressure-volume loops, which are provided by plotting the instantaneous volume against corresponding LV pressure. The integral pressure with respect to volume allows to assess a total external ventricular work during ejection. The diastolic pressure-volume relations reflect a chamber stiffness of the ventricle. Force-velocity relations also provide an useful conceptual framework for understanding how the ventricle contracts under given afterload, with modification of preload. In the presence of coronary artery disease, the regional nature of left ventricular contractile function should be defined as well as the global ventricular function as described above, because the latter is determined by the complex interaction of dysfunction of the ischemic myocardium and of compensatory augmentation of shortening of the normally perfused myocardium. We utilized a computer technique to analyze the local wall motion of the ischemic heart by cineventriculography. The boundaries of serial ventricular images are automatically traced and superimposed using the external reference system. Radial grids are drawn from the center of gravity of the end-diastolic image. Measurement of length of each radial grid throughout cardiac cycle provides the analysis of movement of the ventricle at a particular point on the circumference. Using phasic pressure obtained simultaneously with opacification as the common parameter, segmental pressure-length loops are constructed simultaneously at various segments. The loops are similar over the entire circumference in the normal heart, being rectangular in morphology and with synchronous behavior during contraction and relaxation. However, the marked distortion of pressure-length loops with clockwise rotation or figure of eight inscription is observed in the ischemic segments. Systolic work of the ischemic segment diminishes dramatically, and the loops exhibit varying degrees of inclination. The control segment loops also show an inclination to the opposite direction of the ischemic loops. These differences are presumably related to the local redistribution of the myocardial tension during systole in the ischemic ventricle. Thus, the method described should be of particular value in assessing the regional myocardial function in the ischemic ventricle and effects of various interventions which modify ischemia.

[Result of radiotherapy for esophageal cancer--radical radiotherapy combined with intracavitary radium irradiation].

Between 1974 and 1980, 61 primary esophageal squamous cell carcinoma were treated by external irradiation combined with additional intracavitary radium therapy. The esophageal primary control rate was 36% (22/61) and the 5-year survival rate was 24.7%. We believe that external radiation therapy followed by additional intracavitary radium irradiation produces good results.

[Radiotherapy treatment of esophageal carcinoma--analysis of 5-year survival].

Between 1966-1977, 250 patients with esophageal carcinoma were treated by radiotherapy alone. The 5-year survival rate was 10.4% (12 of 115) in the curative cases. 52 of 5-year survivors, including 40 cases from the literature, were analyzed according to age, sex and the extent, location and type of tumor. 37 patients (71.1%) were between 50 and 69 years of age. There were more female than male survivors. Patients whose esophagograms showed spiral-type tumor had a worse prognosis than other tumor types. 16 of the 52 (30.8%) patients had received intracavitary treatment. We posit that an additional boost dose by intracavitary techniques improves the local control of the primary lesion. Radiation-induced myelitis and carditis were noted in some patients.

[Adjuvant BCG immunotherapy for breast cancer (T1N0M0, T2N0M0)].

The present study was performed to evaluate the effect of BCG immunotherapy for breast cancer (T1n0M0, T2n0M0) in nonrandomized series. During a 5-year follow-up study, patients in the BCG-treated group suffered neither recurrence nor death, while the historical control group had 6 cases of recurrence and 5 cases of disease death. As seen in our series, we concluded that breast cancer patients should be advised to accept adjuvant BCG immunotherapy after mastectomy.

[Multiple primary cancer and radiation-induced cancer of the uterine].

This report is concerned with multiple primary cancers developing in invasive uterine cancer. Second primary tumors were recorded 27 women with a total of 30 non-uterine cancer (exception of radiation-induced cancer). 17 patients of radiation-induced neoplasm were observed (Rectal cancer 4, soft part sarcoma 4, cancer of urinary bladder 3, bone tumor 3, uterine cancer 2 and cancer of Vulva 1). One case is 4 lesions (corpus, sigma, thymoma and stomach), 2 cases are 3 lesions (uterine cervix, stomach and maxillary sinus: uterine cervix, thyroidal gland and radiation-induced soft part sarcoma). Only 5 of these 17 patients were known irradiated dose (50 Gy approximately 55 Gy), however others unknown. The mean latent periods of 17 cases of radiation induced neoplasms are 19.4 years. 16 patients of late second cancers of the cervix appearing from 11 to 36 years (average 19.5 years) after initial radiotherapy were recorded.

[Analysis of gastric biopsy specimens group III by a revised group classification].

A revised group classification of histology in gastric biopsy specimens was recently proposed by the Japanese Research Society For Gastric Cancer. The specimens judged as Group III by the revised classification contained; adenoma that used to be judged as Group III by the former classification, lesions difficult to distinguish between adenoma and carcinoma, lesions difficult to distinguish between atypical regenerative epithelium and carcinoma, and so on. Pathologists are requested to describe whether the lesion is adenoma or not and to consult with clinicians about the lesion in order to reduce the possibility that a case of well-differentiated adenocarcinoma is misdiagnosed as Group III by the revised classification.

[Study on radiation-induced malignant neoplasms].

Nineteen patients with newly developed malignant neoplasms following radiotherapy are presented. The primary lesions of 6 patients were tuberculous cervical lymphadenitis, the primary lesions of the other 13 patients were malignant neoplasms. The radiation-induced malignant neoplasms of 15 patients were epithelial, only 4 patients were non-epithelial. The prognosis was not good. However, we think that radiation-induced malignant neoplasms are not highly radioresistant and that the benefits of radiotherapy for malignant neoplasms outweigh the risk of radiation-induced malignant neoplasms.

[A case of hypospadias in a woman whose incontinence was repaired surgically].

On August 23, 1982 a 24-year-old woman presented with incontinence following her first delivery. From infancy she used to void urine frequently. When she was 4 years old a left nonfunctioning kidney was diagnosed and left nephrectomy was done. The external genitalia were remarkable in that there was a solitary orifice in the vestibule. The other items in the examination were normal. Vaginal examination revealed that the urethral meatus was on the anterior vaginal wall about 3 cm proximal to its orifice. Bladder neck and proximal urethral narrowing combined with suspension of bladder neck was performed on January 19, 1983. Postoperative course was uneventful and she was continent when the bladder was filled up to 200 ml while she stood erect. Our case might belong to group 3 according to Blum 's classification.

Characterization of a new fetal hemoglobin variant, Hb F Izumi A gamma 6Glu replaced by Gly, by molecular secondary ion mass spectrometry.

Molecular secondary ion mass spectrometry has characterized the structure of a new fetal hemoglobin variant, Hb F Izumi, without separation of peptides or amino acid analysis. First, the mass spectrum of a tryptic digest of the abnormal gamma globin revealed a decreased by 72 mass units in the molecular mass of peptide T-1,2, indicating the presence of a Glu leads to Gly substitution. Next, the analysis of the digest produced by the addition of staphylococcal protease, which specifically cleaves glutamyl peptide bonds, determined the site of substitution at 6th glutamic acid residue in peptide T-1,2 which contains two glutamic acid residues. Since this mass spectrometric approach provides digitalized data on peptide analysis, we call it 'digit printing'. The high sensitivity of this technique is especially promising for the analysis of molecular abnormality in various genetic disorders.

Structure of rice ferricytochrome c at 2.0 A resolution.

The crystal structure of ferricytochrome c from rice embryos has been solved by X-ray diffraction to a resolution of 2.0 A, applying a single isomorphous replacement method with anomalous scattering effects. The initial molecular model was built on a graphics display system and was refined by the Hendrickson and Konnert method. The R factor was reduced to 0.25. Rice cytochrome c consists of III amino acid residues. In comparison with animal cytochromes c, the peptide chain extends for eight residues at the N-terminal end, which is characteristic for plant cytochromes c. These additional residues display a collagen-like conformation and an irregular reverse turn, and are located around the C-terminal alpha-helix on the surface or the rear side of the molecule. Two hydrogen bonds between the carbonyl oxygen of the N-terminal acetyl group and O eta of Tyr65, and between the peptide carbonyl oxygen of Pro-1 and O epsilon 1 of Gln89, are involved in holding these eight residues on the molecular surface, where Tyr65 and Gln89 are invariant in plant cytochromes c. Except for the extra eight residues, the main-chain conformations of both rice and tuna cytochromes c are essentially identical, though small local conformational differences are found at residues 24, 25, 56 and 57.
