[Quality control in aspiration biopsy cytology].

Comparison and quality control of our experience of thyroid cytology (784 samples) and breast cytology (1,202 samples) were reported. False positive rate and other parameters were superior in breast cytology than thyroid. It may be due to the fact that cytological diagnostic criteria for follicular thyroid carcinoma from follicular adenoma is not established yet. More suspicious diagnosis in thyroid than breast also give low incidence of malignancy in thyroid suspicious diagnosis. As recommended by a guideline for both thyroid and breast cytology from the Papanicolaou society et al, conservative handling and protective diagnosis may help to reduce false positive and false negative rates.

[Breast cancer with liver metastasis responsive to docetaxel: case report].

A 59-year-old female underwent mastectomy for right breast cancer in November 1992. She received tamoxifen and anthracycline-containing chemotherapy as adjuvant therapy. In and after September 1994, she developed loco-regional recurrences five times in total, each of which was treated with surgery and conventional combination chemotherapy. In April 1997, she developed liver metastasis, which was refractory to biochemical modulation therapy (low-dose cisplatin + 5-FU). We, therefore, treated her six times with docetaxel 80 mg, which resulted in partial response of the liver metastasis and brought about a marked decrease in serum CA15-3 levels. Adverse effects of docetaxel were grade 3 alopecia and leucocytopenia. She has been well without re-growth of the liver metastasis for over five months.

[High-speed recording of fluorescein fundus angiography using an argon laser as the illumination source].

We constructed a new fluorescein fundus angiography system using a speedy and sensitive fundus camera and an argon laser apparatus as the illumination source. The system consists of a fluorescein fundus camera, a high-speed camera, a digital recorder, an argon laser apparatus, and a cathode ray tube(CRT)monitor. The frame speed was 200 frames/s. It was possible to record high speed fluorescein angiography with lower illumination by using an argon laser as the illumination source. This system allowed us to measure the rate of blood flow in the retinal vessels because it easily detected the dye front in the retinal vessels.

Effect of running training on brown adipose tissue activity in rats: a reevaluation.

The effect was investigated of running training on the thermogenic activity of brown adipose tissue (BAT) in rats. The exercised rats were trained on a rodent treadmill for 5 days per week and a total of 9 weeks. After the training, a significantly lower rate of increase in body weight was found, suggesting some training effect, whereas the training failed to induce a decrease in BAT mass. As previously reported (Yamashita, Yamamoto et al., 1993), there was also a markedly lower expression of uncoupling protein (UCP) mRNA in BAT from trained rats; nevertheless, no definite effect of the running training was noted on either UCP content or guanosine 5'-diphosphate binding in the mitochondria recovered from BAT. The results obtained suggest that running training has no overt effect on the thermogenic activity of BAT in rats.

[Long-term clinical course and prognosis of patients with Crohn's disease].

The aim of the present study is to clarify long-term clinical course, prognosis and factors influencing prognosis of patients with Crohn's disease suffering for more than 10 years. Sixty-eight patients (47 males, 21 females) with Crohn's disease in whom the clinical course could observed well at our hospital for more than 10 years were studied. We classified these patients into three groups (good, fair, bad) according to their changes of IOIBD assessment score. And we compaired some clinical factors in these groups to clarify factors influencing prognosis. The results were as follows: 1) Twelve patients (18%) were in good prognosis group, and 24 patients (35%) in fair one, and 32 patients (47%) in bad one. 2) IOIBD assessment score at the time of diagnosis, site of leision, and radiographic findings at the time of diagnosis showed significant difference between bad prognosis group and the other two groups. Although there have been many reports that Crohn's disease is generally progressive, our results indicated that more than half of the patients showed favorable prognosis during the follow-up period for more than ten years.

Decrease in prevalence of Buerger's disease in Japan.

BACKGROUND: Buerger's disease is a peripheral arterial occlusive disease that is becoming rare in Western countries but is more common in Asia. Whether it is a specific disease entity remains controversial. This study was undertaken to investigate changes in the prevalence and characteristics of Buerger's disease at a major institution in Japan. METHODS: Patients with Buerger's disease admitted to Nagoya University Hospital between January 1985 and December 1996 were studied retrospectively. Buerger's disease was diagnosed on admission according to Shionoya's clinical criteria. RESULTS: A total of 105 patients with Buerger's disease were evaluated on 126 admissions; 58 were new patients who were admitted for initial treatment, and 47 patients were experiencing a worsening of Buerger's disease and had a history of prior treatment. Forty-six new patients were admitted between 1985 and 1989, but only 12 new patients were admitted between 1990 and 1996 (9 +/ 3/ yr vs 2+/ 2/ yr, p = 0.0003). Between 1985 and 1989, 44 patients were admitted because of disease exacerbation, whereas only 24 such admissions occurred between 1990 and 1996 (9+/ 3/ yr vs 3 +/ yr, p = 0.0137). The number of admissions for atherosclerotic peripheral vascular disease did not change significantly in that period. Of the 105 patients, the majority (96%) were men; mean age at the time of disease onset was 36 +/ 8 years. The chief complaint on admission was gangrene/ulcer in 64%, rest pain in 13%, foot claudication in 6%, calf claudication in 6%, and other in 10%. CONCLUSION: The prevalence of Buerger's disease appears to be decreasing at our institution in Japan. Its clinical characteristics have not changed. A similar decrease in prevalence appears to have occurred in Western countries.

[The comparative study of LUMIWARD immunoassay system and CAP RAST system for determination of specific IgE antibody against mite in infantile atopic dermatitis].

We determined the levels of specific IgE antibody against mite in 91 infants with atopic dermatitis by CAP RAST system and LUMIWARD immunoassay system, and observed them for about two years. The correlation between CAP RAST and LUMIWARD was weak. The cut off point was set up at 0.06 IU/ml, most of the patients who had asthma attack or whose atopic dermatitis were poorly controlled during two years judged to be positive. In conclusion, the determination of low levels of specific IgE antibody against mite in infants with atopic dermatitis was reliable to prediction of allergy.

Iontophoretic application of prostaglandin E1 for improvement in peripheral microcirculation.

OBJECTIVE: The pharmacological effects of iontophoretically applied prostaglandin E1 (PGE1) on the peripheral microcirculation were investigated. MATERIAL AND METHOD: PGE1 concentrations permeated through hairless mouse skin iontophoretically were measured by HPLC at hourly intervals. 19 patients with arteriosclerosis obliterans were given PGE1 (20 microg) iontophoretically on the dorsum of the foot. As a control, normal saline was applied iontophoretically on another day. The increase rate in cutaneous blood flow as measured by laser Doppler flux (LDF) level was investigated. 5 of these 19 patients received PGE1 (20 microg) solution intravenously on another day. The increase rate in cutaneous blood flow as measured by laser Doppler flux (LDF) level was compared between iontophoresis and intravenous drip infusion. RESULT: PGE1 was transported across hairless mouse skin with iontophoresis in a time-dependent manner, while it did not permeate by passive transport. The percent increase in LDF level was significantly higher in the PGE1 group than in the control group(32.7+/-20.2% vs. 10.3+/-15.5%, p.001). The percentual increase in LDF level was significantly higher with iontophoresis than with intravenous drip infusion (43.7+/-19.5% vs. 11.7+/-28.3%, p < 0.05). CONCLUSION: The results indicated that iontophoretically applied PGE1 was effective in improving the impaired microcirculation due to peripheral arterial occlusive disease. This technique may be useful in managing severely ischemic legs.

[Recurrent squamous-cell lung cancer treated with bronchial-arterial infusion of docetaxel--case report].

A 67-year-old man developed a recurrence in the trachea and truncus intermedius three years after the operation of squamous-cell lung cancer. After no regression of the recurrent foci with bronchial-arterial infusion (BAI) using cisplatin, the recurrent tumor in the truncus intermedius disappeared completely with an intravenous infusion of docetaxel 80 mg (48 mg/m2). However, a recurrence in the trachea did not disappear even after the second intravenous infusion of docetaxel. Therefore, we treated the patient with BAI using a ten-minute administration of docetaxel (40 mg) to the right bronchial artery and the right 3rd intercostal artery, respectively. Adverse effects of the BAI with docetaxel were grade 1 nausea and slight pain in the right anterior chest. Although the therapy brought about no tumor regression in the trachea, the clinical outcome in this case suggested that BAI with docetaxel is a safe therapy.

Isolation and characterization of nitric oxide reductase from Paracoccus halodenitrificans.

Nitric oxide reductase was isolated from the membrane fraction of a denitrifying bacterium, Paracoccus halodenitrificans, in the presence of n-dodecyl beta-D-maltoside. A relatively simple and effective procedure to purify NO reductase using DEAE-Toyopearl and hydroxyapatite (ceramic) chromatographies has been developed. The enzyme consisted of two subunits with molecular masses of 20 and 42 kDa associated with the c-type heme and two b-type hemes, respectively. The optical and magnetic circular dichroism (MCD) spectra of the oxidized (as isolated) and reduced enzymes indicated that the heme c is in the low-spin state and the hemes b are in the high- and low-spin states. The EPR spectrum also showed the presence of the split high-spin component (g perpendicular = 6.6, 6.0) and two low spin components (gz,y,x = 2.96, 2.26, 1.46, gz = 3.59). Although the presence of an extra iron was suggested from atomic absorption spectroscopy, a non-heme iron could not be detected by colorimetric titrations using ferene and 2-(5-nitro-2-pyridylazo)- 5-(N-propyl-N-sulfopropylamino)phenolate (PAPS). One of the extra signals at g = 4.3 and 2.00 might come from a non-heme iron, while they may originate from an adventitious iron and a certain nonmetallic radical, respectively. When CO acted on the reduced enzyme, both of the low-spin hemes were not affected, and when NO acted on the reduced enzyme, the optical and MCD spectra were of a mixture of the oxidized and reduced enzymes. Consequently, the reduction of NO was supposed to take place at the high-spin heme b. The heme c and the low-spin heme b centers were considered to function as electron mediators during the intermolecular and intramolecular processes.

Up-regulation of integrin beta 3 expression by cyclic stretch in human umbilical endothelial cells.

The effect of uni-axial cyclic mechanical stretch on the expression of the adhesion protein integrin was investigated. Human umbilical endothelial cells (HUVECs) cultured on fibronectin coated silicon membranes were subjected to uni-axial cyclic stretch. The level of expression of integrin beta 3 mRNA was found to be increased and peaked at 4 hours in response to cyclic stretch using a semiquantitative RT-PCR method. The increased level of the integrin mRNA from stretched HUVECs remained higher than that from non-stretched controls. The amount of integrin beta 3 also increased and peaked at 12 hr. Immuno-fluorescent microscopy revealed that the amount of integrin beta 3 adhesions increased in stretched HUVECs compared with that in non-stretched HUVECs. These results suggest that uni-axial cyclic stretch up-regulates the expression of integrin beta 3. This increase in integrin beta 3 may enhance the adhesiveness to the substratum and contribute to the protection of HUVECs against being peeled off from the vessel wall.

Study of in vitro cytotoxicity of a water soluble organic arsenic compound, arsenosugar, in seaweed.

In the present study, we demonstrated the cytotoxic effect of a dimethylarsenic compound in seaweed, (R)-(2',3'-dihydroxypropyl) 5- deoxy-5-dimethylarsinoyl-beta-D-riboside, namely arsenosugar (AsSug), on mammalian cells, murine macrophages, in comparison with that of an inorganic arsenical, arsenite, in vitro. More than 99.5% pure AsSug was synthesized. Arsenite was strongly and equally toxic to both peritoneal macrophages (PMs) and alveolar macrophages (AMs), and the concentration of arsenite that inhibited the viability of cells by 50% compared to the viability of control cells (50% inhibitory concentration; IC50) was 5 microM. In contrast, AsSug showed no cytotoxicity to both PMs and AMs at the microM concentration level; however, it induced different and interesting cellular responses in both macrophages at high concentrations, 1-10 mM. AsSug enhanced the viability of PMs at an optimal dose of 5 mM; conversely, it showed weak but significant cytotoxicity to AMs (IC50 = 8 mM).

Endothelium-dependent frequency modulation of Ca2+ signalling in individual vascular smooth muscle cells of the rat.

1. We visualized intracellular Ca2+ concentration ([Ca2+]i) changes, using fluo-3 as an indicator, of individual vascular smooth muscle cells and endothelial cells within intact rat tail arteries by confocal microscopy. 2. Using a piezo-driven objective, we focused on endothelial and smooth muscle cell layers alternately to obtain Ca2+ images of their cells. In the presence of 1 microM acetylcholine (ACh), individual endothelial cells responded with intermittent increases in the [Ca2+]i (Ca2+ oscillations). At the same time, the frequency of Ca2+ oscillations in smooth muscle cells induced by electrical stimulation of the perivascular sympathetic nerve was greatly decreased. 3. A [Ca2+]i rise during the oscillations in the endothelial cells propagated in the form of a wave along the long axis of the cells. 4. In the presence of a NO synthase inhibitor, no significant inhibitory effect of ACh on the Ca2+ signalling in the vascular smooth muscle cells was detected, although the Ca2+ oscillations in the endothelial cells persisted. 5. The inhibitory effect of ACh on the frequency of Ca2+ oscillations in the vascular smooth muscle cells was mimicked by 1 microM sodium nitroprusside, a NO donor. 6. These results indicate that Ca2+ waves and oscillations in vascular endothelial cells regulate NO production, which modulates vascular tone by decreasing the frequency of Ca2+ oscillations in smooth muscle cells activated by sympathetic agonists.

Subcutaneous tissue distribution of vancomycin from a fibrin glue/Dacron graft carrier.

We investigated the tissue distribution of vancomycin (VCM) incorporated in fibrin glue (FG) in a rat model. One VCM-loaded FG Dacron graft (VCM-FG, VCM 0.6 mg/ graft) was implanted in the subcutaneous tissue of the anterior abdominal wall of each rat. VCM was injected intravenously at an equal dose (0.6 mg/rat) after implantation of one control graft (without VCM-FG). After the implantation and the iv injection of an equal dose of VCM (0.6 mg/rat), the tissue distribution of VCM for up to 24 h was determined through analysis of the implanted VCM-FG grafts, which released VCM over a 24 h period. The area under the VCM concentration-time curve (AUC) of the tissue was 89.58 micrograms.h/g after the implantation of the VCM-FG graft, and 7.40 micrograms.h/g after the iv injection of VCM, respectively. The targeting index of the tissue, defined as the ratio of AUC after the implantation of the VCM-FG graft to that after VCM iv injection, was 12.11. None of the six VCM-FG Dacron grafts after implantation became infected following inoculation with S. aureus ATCC 25923 (0.1 mL 10(8) CFU/mL). These results suggest that this VCM-FG Dacron graft delivery may be useful in preventing local infection by enhancing the delivery of VCM to the local areas of the implanted site in rats.

Different role of serum components and cytokines on alveolar macrophage activation by soluble fungal (1-->3)-beta-D-glucan.

In this study, we investigated the mechanism of alveolar macrophage activation by systemic administration of SSG, a soluble highly branched (1-->3)-beta-D-glucan obtained from a fungus Sclerotinia sclerotiorum IFO 9395. Multiple i.v. administration (10 mg/kg; once daily for 10 consecutive days) of SSG enhanced some functions of alveolar macrophages, such as lysosomal enzyme activity and nitric oxide secretion, on day 1 after the last administration, and it also elevated the concentrations of serum protein, interferon gamma and SSG in bronchoalveolar lavage fluid on the same day. On the in vitro assay system, stimulation by SSG alone (500 microg/ml) slightly augmented the lysosomal enzyme activity of alveolar macrophages, but it had no effect on nitric oxide production of cells. Stimulation by serum (1 or 10% mouse serum) or serum components, such as fibronectin (25 microg/ml) and albumin (500 microg/ml), alone strongly augmented only the lysosomal enzyme activity of alveolar macrophages, but it had no effect on nitric oxide secretion from cells, and no synergism or additive-like effect was observed between serum components and SSG. In contrast, stimulation by crude lymphokine (5%) or recombinant murine interferon gamma (100 U/ml) alone did not induce augmentation of lysosomal enzyme activity and nitric oxide production of alveolar macrophages in vitro, but when cells were incubated together with crude lymphokine or recombinant murine interferon gamma and SSG (500 microg/ml), a significant combined effect was observed on both functions of alveolar macrophages. In addition, pretreatment of crude lymphokine or recombinant murine interferon gamma enhanced the expression of beta-D-glucan specific binding sites on the alveolar macrophage surface in vitro though pretreatment by serum components had no effect. Based on these findings, the enhancement of alveolar macrophage functions by systemic administration of SSG appears to be mediated, at least in part, by both the simple effect of serum components including fibronectin and albumin leaked from pulmonary peripheral blood into the alveoli and the synergistic effect between lymphokines released from activated pulmonary T cells and SSG itself entering the alveoli after SSG injection via the priming effect of lymphokines which enhances the expression of beta-D-glucan specific binding sites on the alveolar macrophage surface.

A novel aortic smooth muscle cell line obtained from p53 knock out mice expresses several differentiation characteristics.

Here we report that we could obtain a highly differentiated smooth muscle cell line by screening the expression of a-smooth muscle actin from p53 knook out mice aorta. This cell revealed extended bipolar shape and expressed h-caldesmon and calponin as well as a-smooth muscle actin as protein markers of differentiated smooth muscle. Further intracellular calcium increase was induced by application of noradrenaline in a dose dependent manner and calcium oscillation was also observed in a higher dose (100 microM). Appropriate application of 5-azacytidine enhanced these tendencies and induced slow contraction by endothelin-1 and phenylephrine.

Role of endothelin-1 in astrocyte responses after acute brain damage.

We examined the possibility of the involvement of endothelin (ET)-1, a potent vasoactive peptide, in the process of astrocyte proliferation after brain injury. Acute brain damage in rats was induced by cold-injury. Astrocytes changed from a differentiated state to an immature, RC-1-positive state immediately after the injury. In the injured site, the level of ET-1-like immunoreactivity in the tissue was significantly increased on the first postoperative day and was sustained at a high level for 5 days. ET(B) receptor mRNA was markedly but transiently down-regulated only on the first day after the injury. Brain extracts (BE) were prepared from the injured tissues, and their effects on the proliferative characteristics of astrocytes were examined in primary culture of astrocytes. The flat morphology, which was observed in association with cell proliferation, and DNA synthesis of astrocytes were enhanced by treatment with each of the BE from 1 (D1-BE), 3 and 5 days after the injury. A monoclonal antibody that recognizes the C-terminus of rat ET-1 and ET-3 inhibited the DNA synthesis of astrocytes induced by D1-BE. These results provide experimental evidence that ET-1 may participate in the initiation of gliosis in the acute phase of brain damage.
