Substance P analogues containing D-histidine antagonize the behavioural effects of intrathecally co-administered substance P in mice.

The antagonistic effect of newly synthesized substance P (SP) analogues containing D-histidine was examined on behavioural responses induced in mice by SP, neurokinin (NK) A, physalaemin, eledoisin, somatostatin and bombesin. [D-Pro2,D-Trp7,9]SP (DPDT-SP) and [D-Arg1,D-Trp7,9,Leu11]SP (spantide) were used as references for comparison. When co-administered with SP intrathecally, all the SP analogues used decreased the SP-induced response which consists of scratching, biting and licking. DPDT-SP and spantide attenuated non-specifically the SP-like behavioural responses induced by physalaemin, eledoisin, NK A and somatostatin. In general, the introduction of D-histidine in position 9 of the SP molecule resulted in potent antagonistic activity of the SP derivative on the behavioural responses to SP. Of these SP analogues, [D-Arg1,D-Pro2,4,D-Phe7,D-His9]SP attenuated selectively the behavioural responses produced by NK-1 receptor agonists such as SP and physalaemin. Simultaneous injection of [D-Phe7,D-His9]SP-(6-11) selectively inhibited the SP-induced behavioural response without affecting the other peptide-induced behavioral response. The results suggest that the behavioural antagonism induced by [D-Arg1,D-Pro2,4,D-Phe7,D-His9]SP and [D-Phe7,D-His9]SP-(6-11) is probably due to the specific blockade of spinal NK-1 receptors.

A case of refetoff syndrome: selective venous sampling for TSH is useful in differentiating thyroid hormone resistance from TSH secreting tumor.

A 22-year old man with a goiter and clinical manifestations of mild thyrotoxicosis (finger tremor, palpitation, tachycardia) was diagnosed as a syndrome of inappropriate secretion of TSH. Serum concentrations of T4, free T4, T3 and TSH were 24.1 micrograms/100 ml, 4.07 ng/100 ml, 261 ng/100 ml and 1.72 microU/ml, respectively. Thyroidal 131I uptake at 24 hr was 80%. The BMR was within the normal range. He had a normal TSH response to TRH (500 micrograms) with a peak level of 23.8 microU/ml. The basal level of alpha-subunit of TSH was not elevated (0.35 ng/ml). Oral 1-T3 administration (75 and 150 micrograms daily) raised serum T3 concentration, reduced basal TSH and blunted TSH response to TRH. The diurnal variation of TSH was maintained. There was no evidence of abnormalities in the secretion of other pituitary hormones. These findings were compatible with thyroid hormone resistance. However, the presence of a microadenoma in the pituitary gland was suspected with CT scan. Bilateral and simultaneous venous sampling for TSH from inferior petrosal sinus showed no gradient in TSH concentration indicating that a TSH secreting pituitary tumor was unlikely. These data suggest that inappropriate TSH secretion in the present patient is resulted from resistance to thyroid hormone. In the present study selective venous sampling is useful to differentiate the thyroid hormone resistance from a TSH secreting tumor.

The circadian variation of blood pressure and heart rate in patients with hyperthyroidism.

The circadian variation of blood pressure (BP) and heart rate (HR) was examined in 18 normal subjects, 15 patients with essential hypertension treated with beta-blockers and 21 patients with hyperthyroidism. Most of the patients with hyperthyroidism were also treated with beta-blockers. The 24 hr BP was measured with an ambulatory blood pressure monitoring device (UBP-100) every 5 min. A nocturnal fall in BP and HR was observed in the patients with essential hypertension treated with beta-blockers and the patients with mild to moderate hyperthyroidism as well as normal subjects. On the other hand, in patients with severe hyperthyroidism the nocturnal fall was observed in HR alone, and the fall was small in its amplitude. There was a significant negative correlation between triiodothyronine (T3) level and percentage amplitude of the nocturnal fall in systolic BP (n = 21, r = -0.5, p less than 0.01). However, this relation was not significant in diastolic BP and HR. These results indicate that excess thyroid hormone may modulate the circadian variation of BP and HR.

Relationship between serum free thyroid hormone concentrations and target organ responsiveness in thyroid disease patients before and after treatment.

Relationship between serum free thyroid hormone concentrations and several markers of peripheral tissue response to thyroid status was studied in 24 patients with hyperthyroidism, 17 with euthyroidism with goiter and 10 with primary hypothyroidism before and after the initiation of standard forms of treatment. Before treatment, serum free T4 correlated significantly, either positively or negatively, with the basal metabolic rate (BMR), duration of the achilles tendon reflex (ATR), resting pulse rate (RPR), serum total cholesterol (T-cholesterol), ratio of the pre-ejection period to the left ventricular ejection time (PEP/LVET), serum high density lipoprotein (HDL) and creatine phosphokinase (CPK) in the descending order. Serum free T3 also correlated significantly all the these parameters. After treatment, changes in both serum free T4 and free T3 correlated well with those of the BMR and ATR in the patients with hyperthyroidism, and with those of the ATR, serum CPK and T-cholesterol in the patients with hypothyroidism. These results indicate that these peripheral parameters correlated well with serum free thyroid hormone concentrations in thyroid disease patients. Therefore, these parameters can be used to assess thyroid functions in patients with thyroid hormone resistance.

Modulation of pain in fibromyalgia (fibrositis syndrome): cerebrospinal fluid (CSF) investigation of pain related neuropeptides with special reference to calcitonin gene related peptide (CGRP).

The cerebrospinal fluid (CSF) levels of calcitonin gene related peptide (CGRP) were 0.94 +/- 0.06 fmol/ml (mean +/- SEM), of substance P, 35.1 +/- 3.2 fmol/ml and of substance P (1-7), 10.8 +/- 1.2 fmol/ml, as measured by radioimmunoassay in 26 female patients with fibromyalgia. No correlation was found between the levels of CGRP and the substance P and substance P (1-7) levels (r = 0.316, p = 0.14). Our results show that the anatomical coexistence of pain related neuropeptides in neurons is not necessarily reflected by the levels of these peptides measured in the CSF. The presence of CGRP in the CSF could be important since it can enhance the nociceptive activity of tachykinins. This may be of importance in the pathogenesis of pain in fibromyalgia.

[Transseptal suture fixation in the management of prosthetic valve dehiscence].

Valve dehiscence resulted from prosthetic valve endocarditis and aortitis is the most common cause of the reoperation after prosthetic valve replacement. In our experience, the results of reoperation has been far from satisfactory. For the successful management of prosthetic dehiscence, we have adopted the transseptal suture technique of accurately securing the prosthetic valve. Five patients with prosthetic valve dehiscence and one patient with native valve endocarditis were operated upon using the transseptal techniques. Left ventricular-aortic discontinuity was repaired using composite valve-tube graft together with this technique in two patients. All but one patient who succumbed to multiple organ failure induced by preoperative hemodynamic deterioration have survived the operation without recurrent infective endocarditis or valve dehiscence.

Substance P(1-7) antagonizes substance P-induced aversive behaviour in mice.

Substance P (SP) and its fragments were administered intrathecally into awake mice. SP and C-terminal fragments caused dose-dependent reciprocal hindlimb scratching responses. SP(5-11) was more potent than SP not only in inducing scratching response but also in inducing aversive behaviour including licking and biting. SP(1-7) induced no behavioural reactions. However, when low doses of SP(1-7) (1.0-4.0 pmol) were injected simultaneously with SP or SP(5-11) (0.1 nmol), aversive behaviours induced by SP or SP(5-11) were significantly reduced. These results indicate that SP(1-7) formed endogenously could modulate the actions of SP or SP(5-11) in the spinal cord.

Adenosine, deoxyadenosine, and deoxyguanosine induce DNA cleavage in mouse thymocytes.

When thymocytes were cultured with adenosine, deoxyadenosine, or deoxyguanosine at 1 mM for 24 h, DNA cleavage at internucleosomal sites with multiples of approximately 180 bp was induced, followed by lactate dehydrogenase release into the medium. In the presence of coformycin, an adenosine deaminase inhibitor, or formycin B, a purine nucleoside phosphorylase inhibitor, DNA cleavage was induced by these nucleosides at concentrations of less than 50 microM. Other purine and pyrimidine ribo- and deoxyribonucleosides did not induce DNA cleavage or LDH release. Because thymocyte nuclei contain a Ca2+,Mg2+-dependent endonuclease, which preferentially cuts DNA in its linker regions, DNA fragmentation induced by the three purine nucleosides was suggested to occur through increased activity of the endonuclease. The DNA cleavage induced by the nucleosides required protein phosphorylation and synthesis, inasmuch as it was inhibited by an inhibitor of protein kinases, H-7, and by an inhibitor of protein synthesis, cycloheximide. The inhibition of DNA cleavage was accompanied by a reduction in lactate dehydrogenase release, suggesting a causal relationship between DNA cleavage and cell death. The DNA cleavage and subsequent cell lysis might be related to the selective thymocyte deletion observed in patients with adenosine deaminase or purine nucleoside phosphorylase deficiency.

Enkephalins interact with substance P-induced aversive behaviour in mice.

The effect of the endogenous opioid peptides, methionine-enkephalin (Met-ENK), beta-endorphin (beta-END) and dynorphin-(1-17) (DYN) on the aversive behavior produced by intrathecal (i.t.) administration of substance P (SP) was studied in mice. A low dose of i.t. administered Met-ENK gave a marked reduction of the SP-induced response. In the tail-flick assay, such doses of Met-ENK were ineffective in producing antinociception. At much higher doses, however, Met-ENK obtained antinociceptive activity. In contrast, beta-END and DYN had about the same potency in inhibiting the SP-induced behavioural response and in the tail-flick test, respectively. These results suggest that opioid peptides, particularly enkephalin neurons in the spinal cord influence SP-induced aversive behaviour.

Comparison of the antinociceptive effects of new [D-Arg2]-dermorphin tetrapeptide analogs and morphine in mice.

The antinociceptive effects of synthetic dermorphin tetrapeptide analogs containing D-Arg in position 2, H-Tyr-D-Arg-Phe-Gly-NH2 and H-Tyr-D-Arg-Phe-beta-Ala-OH, were measured in mice by the tail-pressure test. The antinociceptive effect produced by intracerebroventricular (ICV), intrathecal (IT) and subcutaneous (SC) administration of either peptide was greater than that produced by morphine. Oral (PO) administration of the peptides showed approximately the same antinociceptive potency as morphine. In addition, the antinociceptive effect produced by SC and PO administration of either peptide was of longer duration than morphine. Pretreatment with naloxone resulted in nearly complete antagonism of the antinociceptive effects produced by ICV and IT administration of either peptide or morphine. Dose ratios (ICV/IT) of H-Tyr-D-Arg-Phe-Gly-NH2 and H-Tyr-D-Arg-Phe-beta-Ala-OH, which were calculated from the AD50 (Antinociceptive Dose = 50% MPE) values, were 5.8 and 6.2, respectively, whereas that of morphine was only 1.46. These results suggest that the mechanisms of the antinociceptive effects of [D-Arg2]-dermorphin tetrapeptide analogs differ from morphine, and that these peptides may possess higher affinities than does morphine for opioid receptors in the spinal cord.

Clinical investigation of olfactory and auditory function in type I pseudohypoparathyroidism: participation of adenylate cyclase system.

Olfactory and auditory function was examined in five patients with type I pseudohypoparathyroidism; in two patients the activity of stimulatory guanine nucleotide-binding protein was examined and found to have low values. The olfactory tests were performed by T & T olfactometer and intravenous injection of thiamine propyl disulphide. The four patients receiving olfactory test showed a disturbance in recognization of the odorants. All five patients had normal hearing at frequencies with the normal range. An adenylate cyclase system is thought to play an important role in olfactory transduction, whereas cochlear function may be unaffected by this system in the normal state.

Antinociception and physical dependence produced by [D-Arg2] dermorphin tetrapeptide analogues and morphine in rats.

1. The antinociceptive effects of [D-Arg2] dermorphin tetrapeptide analogues, H-Tyr-D-Arg-Phe-Gly-NH2 and H-Tyr-D-Arg-Phe-beta-Ala-OH when administered subcutaneously (s.c.) in rats were measured by the tail-flick test. In addition, the appearance of typical withdrawal signs upon cessation of administration or on subsequent treatment with naloxone were measured after chronic administration of either peptide or morphine. 2. The dose of peptides and of morphine in the physical dependence test was determined from the AD50 to inhibit the tail-flick test in rats. Doses from 4 to 64 times the AD50 doses were employed in the s.c. administration schedules. 3. The intensity of the antinociception induced by either peptide was greater than that produced by morphine. Moreover, the antinociception induced by the peptides was of much longer duration than that produced by morphine. 4. Abrupt withdrawal after chronic administration of either peptide produced only slight loss of body weight. In contrast, morphine withdrawal produced sharp loss of body weight. 5. Naloxone precipitated withdrawal signs after chronic administration of either peptide were less intense than those after chronic morphine. 6. These results suggest that the antinociception produced by these peptides is more intense and of longer duration than that produced by morphine. It is also interesting to note that the physical dependence produced by these peptides is less marked than that produced by morphine.

Measurement of thyroid stimulating hormone (TSH) in human urine.

Using a highly sensitive and specific immunoradiometric assay kit for human TSH, we measured TSH concentrations in unprocessed urines in normal subjects, in patients with primary hypothyroidism, and patients with renal disease. In five of ten normal subjects TSH was detectable in urine samples (less than 20-69 microU/day). In five patients with hypothyroidism, the urinary TSH excretion was increased. In seven out of ten patients with nephrotic syndrome, eight out of nine patients with chronic renal failure and two patients with tubular dysfunction, the urinary TSH excretion was increased. The urinary TSH excretion correlated significantly with both urinary protein excretion and urinary beta 2-microglobulin excretion. These results suggest that the renal handling of TSH involves both glomerular filtration and tubular re-absorption, and that urinary TSH excretion is increased when serum TSH is increased and either glomerular or tubular function is impaired.

Recurrence of subacute thyroiditis over 10 years after the first attack in three cases.

We saw a total of 4 episodes of the recurrence of subacute thyroiditis in 3 patients out of 222. The recurrent episodes were similar to the first episodes of subacute thyroiditis. The titers of various viral antibodies were not increased significantly during the clinical course of the recurrence. Regarding the HLA typing, A26, B35 and C3 were positive in all 3 patients. The association between the occurrence of subacute thyroiditis and the presence of HLA-B35 and C3 has hitherto been reported, although the association of HLA-A26 with recurrent type of subacute thyroiditis was observed and described for the first time in this report. It is suggested that HLA-A26 may somehow be related to the predisposition to the recurrence of subacute thyroiditis which developed after more than 10 years.

Properties of thyrotropin (TSH) binding to plasma membranes prepared from rat fat tissues.

Rat fat membranes were prepared from male Sprague-Dawley rats. At 37 degrees C, TSH binding to rat fat membranes was rapid and unstable, although the binding reached a steady state after 2 hr and unchanged up to 24 hr at 4 degrees C. The binding to rat fat membranes was significantly inhibited by 50 mM NaCl and almost completely inhibited by 150 mM NaCl. TSH binding to rat fat membranes was not affected by 10 mM dithiothreitol (DTT) or 1 mM diamide although the binding to human thyroid membranes was inhibited by 10 mM DTT significantly. Conventional Scatchard analysis revealed a single class of binding site which had lower Ka value (2.6 X 10(8) M-1) than that of high affinity binding site of human thyroid membranes (9.3 X 10(8) M-1). Immunoglobulin G (IgG) from patients with Graves' disease inhibited the binding of TSH to rat fat membranes. A significant correlation was observed between the inhibiting activity of Graves' IgG measured with rat fat and human thyroid membranes (r = 0.82, p less than 0.01). In conclusion, TSH receptors on rat fat membranes were not identical to those on human thyroid membranes, but TSH receptor antibodies crossreacted with TSH receptors in rat fat tissue.
