[A case control study of breast cancer, mammary cyst and dietary, drinking or smoking habit in Japan].

A matched pair analysis was done among patients with breast cancer, those with mammary cyst and those without any abnormality in mammary gland by ultrasonic echogram (U.S.) to investigate the relationship between dietary or smoking habit and mammary disease. Breast cancer was diagnosed pathologically, and mammary cyst by U.S.. 49 cancer-normal pairs and 36 cyst-normal pairs were obtained. Breast cancer had significant relationships with a large intake of bread (risk ratio (RR) = 1.60, 95% confidence interval (CI) = 1.03-2.49), margarine (RR = 2.17, CI = 1.20-3.49), cakes (RR = 1.83, CI = 1.18-2.84), soft drinks with sugar (RR = 8.00, CI = 1.33-48.29), meat (RR = 1.48, CI = 1.04-2.10) and a small intake of alcohol (RR = 0.20, CI = 0.06-0.63). Mammary cyst was significantly related with a large intake of fat (RR = 1.56, CI = 1.02-2.38). The result indicates that in Japan women with westernized dietary habit have a larger risk of breast cancer.

Regulation of rat liver maturation in vitro by glucocorticoids.

The biochemistry of liver maturation was studied by using the RLA209-15 fetal rat hepatocyte line that is temperature sensitive for maintenance of the differentiated fetal liver phenotype. At 33 degrees C these cells were dedifferentiated; but at 40 degrees C they were phenotypically differentiated and, like normal fetal hepatocytes, synthesized moderate levels of albumin and transferrin, high levels of authentic (69,000 and 73,000 molecular weight) rat fetal alpha-fetoprotein (AFP), and low levels of a 65,000-molecular-weight variant AFP. Our results indicated that administration of glucocorticoid hormones to RLA209-15 cells at 40 degrees C induced a series of events associated with normal hepatocyte maturation; synthesis of fetal AFP was inhibited, whereas the synthesis of variant AFP, albumin, transferrin, tyrosine aminotransferase, and alpha 1-acid glycoprotein was induced. The variant AFP was produced by RLA209-15 cells at both temperatures and was encoded by an mRNA of 1.7 kilobases (kb). The fetal AFP was encoded by an mRNA of 2.2 kb. Normal adult rat liver contained three AFP mRNAs of 2.2 (minor), 1.7, and 1.5 kb. The 1.7-kb adult liver AFP mRNA comigrated with the RNA found in RLA209-15 cells, and both directed the synthesis of a 50,000-molecular-weight precursor polypeptide of the variant AFP. Administration of glucocorticoids to RLA209-15 cells grown at 33 degrees C stimulated synthesis of both the fetal and variant AFPs, but the levels of the 2.2-kb AFP mRNA were preferentially increased. RLA209-15 cells contained two glucocorticoid receptor mRNAs of 6.8 and 4.5 kb. The glucocorticoid-mediated maturation described above was blocked by the antiglucocorticoid RU486.

[Hydronephrosis in pregnancy and its etiology].

Using ultrasonography, the cause and incidence of hydronephrosis were investigated in humans and rabbits and the following results were obtained. The degree of hydronephrosis was classified into grade I (calyceal diameter; up to 5 mm) to grade V (calyceal diameter; 21 mm or more). In normal non-pregnant women, the calyceal diameter was within grade I. The hydronephrosis was found in 45.5-87.0% of pregnant women in the right kidney and in 29.1-51.0% in the left. Throughout pregnancy, the incidence was significantly higher in the right than in the left. On the 6th puerperal day, the incidence of hydronephrosis was lowered, but the diameter did not return to the non-pregnant level. Comparison of women who practised swimming during pregnancy and those who did not reveal the incidence of hydronephrosis was lower in the swimming group than in the non-swimming. In rabbits, calyceal dilatation increased as gestation advanced and hydronephrosis was found in progesterone-treated ovariectomized rabbits. It was concluded that the hydronephrosis in the 1st trimester was due to increased steroid hormones, but also that, in the 2nd trimester, the combined effect of hormones and mechanical obstruction was responsible for its occurrence.

I-cell disease: clinical studies of 21 Japanese cases.

Clinical pictures of 21 cases with I-cell disease patients, 12 males and 9 females, were analyzed. Characteristic coarse facial features and shortness of stature were observed in all cases. In general, the motor development was found to be more severely retarded than the mental development of the patients. Rather little involvement of the nervous system seemed to cause somewhat acceptable mental development in some cases, and also cause the absence of epileptic seizures in all cases. Involvement of the cardiovascular system, especially progressive hypertrophic cardiomyopathy, could be highly responsible for frequent sudden death of I-cell disease patients.

Ontogeny of substance P-containing structures in the ocular tissue of the rat: an immunohistochemical analysis.

The ontogeny of the substance P-like immunoreactivity (SPI) containing system in the ocular tissue of the rat was examined by means of the indirect immunofluorescence method. SPI-containing amacrine cells first appeared at postnatal day 4 and displaced SPI-containing amacrine cells at postnatal day 5. After this time, they developed markedly reaching their maximum content and distribution at postnatal day 14. On the other hand, SPI-containing cells in the trigeminal ganglion were first seen at gestational day 17 and reached their maximum content at birth. SPI-containing fibers in the cornea and uvea were first observed at gestational days 17-19. The SPI-containing fibers in the iris reached their maximum content at birth, while those in the cornea, choroid and ciliary body were fully developed at postnatal day 3. In the adult rats, numerous SPI structures were still seen in the ocular tissue, retina, cornea and uvea. These findings suggest that SPI might play some role in the developing ocular tissue in addition to its neurotransmitter or neuromodulator role in the adult, because SPI structures appear in the retina before establishment of synaptogenesis and in the cornea and uvea during the fetal stage.

A successful trial of enzyme replacement therapy in a case of argininemia.

A 5-year-old boy with severe mental retardation and spastic quadriplegia accompanied by tonic seizures and hyperammonemia was diagnosed as having argininemia due to an arginase deficiency in his erythrocytes. His motor and mental abilities began to deteriorate at the age of 3 years. Thereafter, he lost his ability to stand alone, to sit and even to crawl by himself. After he was diagnosed as argininemia , a protein restricted diet was given as therapy, which was accompanied with a supplement of essential amino acids. However, his clinical condition had not improved very much. The erythrocytes in a normal person was found to have the ability to decrease the patient's elevated plasma arginine level to normal when they are mixed in vitro. First we tried replacing his red cells by a blood transfusion. Then we replaced them with the aid of an IBM 2997 blood cell separator. Following this his clinical and biochemical condition improved, and as a result so did his sitting and crawling abilities. It appears that the replacement of red blood cells improves not only the clinical and biochemical conditions, but the general condition of the patient as well.

Overall distribution of substance P nerves in the rat cornea and their three-dimensional profiles.

The overall distribution of substance P-like immunoreactive (SPI) fibers, examined by using whole-mounts of the rat cornea, was investigated by means of indirect immunofluorescence. SPI fibers entered the cornea from two levels; one from the middle layer of the sclera and the other from the episclera. From the sclera, a thick SPI fiber trunk, extending to the central part, subdivided into smaller SPI fiber bundles and approached the epithelium. The SPI fiber bundles from the episclera were smaller than those from the sclera. However, both fiber bundles formed a dense fiber network in the uppermost part of the stroma. This fiber plexus dissociated into SPI fibers extending to the superficial part of the epithelium where they formed an abundant arborization of fine SPI fibers. The results suggest that these fibers originate from SPI neurons in the trigeminal ganglion.

A mouse model for Niemann-Pick disease: phospholipid class and fatty acid composition of various tissues.

Recently, a strain of mice bearing an autosomal recessive gene, spm, has been described. On the basis of clinical and pathological findings these mice have been suggested as a useful model of human Niemann-Pick disease. Phospholipids and their fatty acid compositions were analyzed for liver, spleen, whole brain, erythrocytes, and blood plasma from "Niemann-Pick"animals (spm/spm) and heterozygous controls (spm/+). Sphingomyelin and bis(monoacylglycero)phosphate accumulated in the liver and spleen of the affected mice, whereas no significant proportional change of phospholipids was observed in the whole brain. The phospholipid composition in erythrocytes and blood plasma of the homozygous mice was not different from that of the heterozygous controls. The fatty acyl chain profile of accumulated bis(monoacylglycero)phosphate was characterized by the high content (more than 80%) of unsaturated fatty acids; the main components were oleic acid, linoleic acid, and docosahexaenoic acid. A high unsaturation index of the fatty acyl chain was found in sphingomyelin accumulated in organs and in almost all phospholipids of brain, erythrocytes, and blood plasma of "Niemann-Pick" mice. It is conceivable that desaturation of fatty acids is enhanced in the "Niemann-Pick" mice.

[The cervical blood flow using hydrogen gas clearance method].

Employing a hydrogen gas clearance method, cervical blood flow abbre . CxBF hereafter, was measured in non-pregnant and various pregnant stages and the following results were obtained. CxBF in 19 non-pregnant cases was 54.1 +/- 22.0 ml/min/100 gr. The measurement was 63.1 +/- 38 in the early, 54.5 in the late proliferative, 65.2 +/- 17.1 in the early and 25.3 +/- 8.9 in the late secretory stage, suggesting that the measurement of CxBF was effected by hormonal conditions. No relationship was found between uterine weight and CxBF . CxBF was 96.8 +/- 11.3 in 6-7, 48.6 +/- 26.4 in 8, 55.5 +/- 9.8 in 9-13 gestational weeks, that is it was highest in the early stages and gradually decreased with the advance of pregnancy. CxBF was considered to be affected by the physical distance between the cervix and the site of the placenta of the chorion frondosum . CxBF was 22.5 +/- 10.8 in and after the 37th gestational week and the value significantly decreased in the terminal stage of pregnancy (r = -0.758, p less than 0.01). The decrease in the terminal stage is supposed to reflect the circulatory insufficiency, that is, cervical ripening. CxBF in molar pregnancy was higher than in normal pregnancy.

Bone marrow transplantation for Niemann-Pick mice.

The Niemann-Pick mice which received bone marrow transplants showed a decreased accumulation of sphingomyelin and cholesterol quantitatively in their spleen. The sphingomyelin deposit in the bone marrow was also reduced histochemically. However, the neurological manifestations were not improved by the bone marrow graft.

Leucine-enkephalin-like immunoreactivity in the chicken retina with a special reference to its fine structures.

Leucine-enkephalin-like immunoreactivity (LEI) in the chicken retina was investigated using light and electron microscopic immunohistochemistry. A flat-mount technique for light microscopy clearly demonstrated that LEI cells were distributed widely throughout the entire retina, without any differences between the central and peripheral retinal regions. Frozen sections for light microscopy confirmed the previous findings in pigeons that LEI is localized in amacrine cells. The present electron microscopic observations confirmed the findings of light microscopy and revealed that LEI terminals make synaptic contract mainly with vesicle-containing processes that seemed to belong to the amacrine cells. The present study further suggests that LEI terminals are both pre- and postsynaptic elements onto vesicle-containing processes mentioned above.
