Removal of mesenteric gastrinoma: a case report.

A 37-year-old man with a primary mesenteric gastrinoma is reported. A parathyroid adenoma had been removed 13 years ago. Six years earlier the patient underwent an emergency distal gastrectomy because of massive bleeding from a duodenal ulcer. Two months later stomal ulcers developed associated with hypergastrinemia. No gastrinoma was detected and total gastrectomy was performed. The fasting plasma level of gastrin was stable in the range from 580 to 920 pg/ml for the following 5 years. However, the level was found to abruptly increase to 4125 pg/ml. The level increased progressively to 11383 pg/ml within 1 year. A gastrinoma was identified in the jejunal mesenterium, and it was completely removed. After surgery the plasma level of gastrin decreased below the limit of the assay, and the paradoxical response to secretin was no longer observed.

Diffuse hypoplasia of the aorta as a possible cause of cardiac hypertrophy.

Left ventricular hypertrophy is caused by a wide variety of cardiovascular conditions, including systemic hypertension, aortic valvular disease, and hypertrophic cardiomyopathy. This paper describes a case of diffuse hypoplasia of the aorta in left ventricular hypertrophy. This condition has not previously been well documented. We emphasize the need for evaluation of aortic dimension when systolic hypertension and vascular bruits are observed in association with myocardial hypertrophy.

The effects of chin cup therapy on the mandible: a longitudinal study.

This study was conducted to evaluate the effects of chin cup therapy on the mandible and its dentition in persons with skeletal Class III malocclusion. The patients selected for this study were Japanese females treated with the extraoral chin cup appliance. Both the control and treatment samples were obtained from Japanese Universities where the longitudinal data were gathered. Lateral cephalometric radiographs were taken on the average of every 6 months for the treatment group and every year for the control group. Ten treated patients and seven control patients were studied. The duration of chin cup therapy was variable but averaged 3 years 1 month. The cephalograms were digitized on an electronic screen, and a cephalometric analysis was recorded from a computer program. The rate change values were then compared with those of the control group to yield a comparison of changes in mandibular growth rate, direction, and pattern in the treatment group. Active and posttreatment effects were evaluated. All measurements for the rate of change of absolute mandibular length were reduced by 60% to 68% from the control rate of growth during therapy. The mandible exhibited less downward displacement relative to cranial base during treatment. The mandibular plane angle and the gonial angle closed with growth in the Class III control sample but were variable in the treatment group. The skeletal profile was improved with treatment. This study indicates that the chin cup may be a viable mode of treatment for the preadolescent and adolescent mandibular prognathism patients.

Spontaneous transection of Raimondi peritoneal catheter.

Three cases of spontaneous transection of a Raimondi peritoneal catheter are reported. Two cases were in infants aged 14 and 10 months, respectively, with communicating hydrocephalus, and the other was in a 10-year-old boy with noncommunicating hydrocephalus associated with a pinealoma. The mechanism of this complication is discussed.

Regulation of gastric acid secretion by secretin and serotonin.

We have tested the interaction between serotonin and secretin in pentagastrin-induced gastric acid secretion in six dogs prepared with Thomas gastric and duodenal cannulas. We further examined the effect of methysergide, a serotonin antagonist, on gastric acid secretion. The intravenous administration of serotonin or secretin alone significantly inhibited pentagastrin-induced gastric acid secretion. The combined administration of serotonin and secretin inhibited gastric acid secretion to a greater magnitude than either secretagogue alone. The administration of methysergide diminished basal concentrations of secretin on gastric acid secretion. Methysergide also enhanced the stimulatory activity of pentagastrin-induced gastric acid secretion. The inhibitory action of intraduodenal acidification of pentagastrin-induced gastric acid secretion was reversed by methysergide.

Functional latissimus dorsi island pedicle musculocutaneous flap to restore finger function.

Functional latissimus dorsi island pedicle musculocutaneous flaps were used to restore flexion or extension of the wrist and digits in four clinical cases. By retaining the fascial origin of the latissimus dorsi from the posterior crest of the ilium, the entire muscle was transferred without dividing its neurovascular pedicle and microneurovascular anastomoses. Its facial origin successfully reached the finger flexor or extensor muscles of the forearm. The latissimus dorsi muscle was sutured to the digital flexor tendons in three patients and connected to the extensor tendons in one patient. All transfers restored active finger flexion or extension.

Cytoprotective effect of pentagastrin and epidermal growth factor on stress ulcer formation. Possible role of somatostatin.

This study was designed to test the effects of pentagastrin and epidermal growth factor (EGF) on stress-induced ulceration and on the antral content of gastrin and somatostatin (SLI) in rats. Four groups of 14 to 15 rats had been prepared for 7 days by one of the following methods: saline injection (control); injection of pentagastrin (250 micrograms/kg, 3 times/day); injection of EGF (10 micrograms/kg, 3 times/day); or injection of EGF plus pentagastrin. At the end of the treatment period, half of each group of rats were sacrificed (nonstress group). There were no ulcers in the nonstress control groups of rats. Stress was applied by water immersion in the remaining half of the rats. The injections of pentagastrin and/or EGF resulted in substantial increase in antral content of SLI. After 20 hours of stress, the ulcer index was 40.5 +/- 3.3 in the controls, compared to 6.4 +/- 1.2 and 16.2 +/- 2.3 in rats that received pentagastrin or EGF, respectively. Injections of both pentagastrin and EGF resulted in an ulcer index of 26.2 +/- 2.0, which was significantly lower than that in controls, but higher than that in rats treated with either peptide alone. The stress resulted in significant decrease in antral SLI in all groups of rats, whereas SLI content in rats treated with pentagastrin and/or EGF remained significantly higher than that of controls. Antral content of gastrin did not differ significantly in the four groups tested. The ulcer index was inversely correlated with antral SLI content. We confirm and extend previous observations that pentagastrin and EGF prevent stress ulcer formation, and suggest that endogenous SLI may account, at least in part, for their antiulcer activity.

Experimental evidence for a vagally mediated and cholecystokinin-independent enteropancreatic reflex.

Truncal vagotomy results in diminished pancreatic protein secretion in response to intraduodenal fat. This diminished secretion may be due, at least in part, to interruption of the vagal reflexes between the intestine and the pancreas that work independently of cholecystokinin (CCK). In five dogs with chronic pancreatic fistulas, plasma CCK concentrations and pancreatic protein secretion in response to an intestinal stimulant (intraduodenal oleate) and to two exogenous peptides (bombesin and CCK-33) were compared before and after bilateral truncal vagotomy. Vagotomy decreased integrated protein secretion by about 50% in response to intraduodenal oleate. In contrast, protein output in response to parenteral stimuli increased after vagotomy. Integrated output of CCK in response to intraduodenal oleate or to exogenous bombesin or CCK was not significantly affected by vagotomy, but release of pancreatic polypeptide was decreased significantly in response to all stimuli after truncal vagotomy. These data provide evidence that truncal vagotomy decreases pancreatic protein secretion in response to intestinal stimulants by interrupting enteropancreatic reflexes mediated by the vagus, while maintaining normal (or supranormal) sensitivity of the pancreas to endogenous and exogenous CCK.

Structures of deepoxytrichothecene metabolites from 3'-hydroxy HT-2 toxin and T-2 tetraol in rats.

3'-Hydroxy HT-2 toxin and T-2 tetraol, in vivo metabolites of T-2 toxin, were orally administered to Wistar rats, and four metabolites having a trichothec-9,12-diene nucleus, which were termed deepoxytrichothecenes, were newly found in the excreta. Their structures were confirmed as 3'-hydroxy-deepoxy HT-2, 3'-hydroxy-deepoxy T-2 triol, 15-acetyl-deepoxy T-2 tetraol, and deepoxy T-2 tetraol on the basis of mass and nuclear magnetic resonance spectroscopy. Resolution of T-2 metabolites and corresponding deepoxytrichothecenes by gas-liquid and thin-layer chromatography was also described.

The effects of Ba-wei-wan (hachimijiogan) on plasma levels of high density lipoprotein-cholesterol and lipoperoxide in aged individuals.

Ba-wei-wan was administered for 7 months to 24 aged, non-hospitalized subjects. Laboratory examinations showed an improvement in serum or plasma levels of total lipid, LPO and HDL-C. Other blood examinations were shown to be unaffected by the treatment. To reconfirm the effects of Ba-wei-wan on lipid metabolism, the three step administration of the drug was employed. The elevation of HDL-C levels and the decrease of LPO levels were obtained by this treatment. The discontinuation of this drug induced the converse effect on the plasma levels; decreased HDL-C and increased LPO. It could be concluded from these results that Ba-wei-wan administration improved the lipid metabolism in the aged subjects.

Effect of divalent cations on gastrointestinal hormone release and exocrine pancreatic secretion in dogs.

The purpose of this study was to examine the effect of three common divalent cations (Ca2+, Mg2+, and Zn2+) on the release of cholecystokinin (CCK-33), pancreatic polypeptide (PP), and gastrin. Five dogs with pancreatic and gastric fistulas were given 1-h intraduodenal infusions of calcium (5 mmol X kg-1 X h-1), magnesium (4 mmol X kg-1 X h-1), or zinc (1 mmol X kg-1h-1). At another time the same dogs were given an intravenous bolus followed immediately by a 1-h infusion of calcium (0.36 mmol/kg [bolus], 0.36 mmol X kg-1 X h-1), magnesium (0.25 mmol/kg [bolus], 0.25 mmol X kg-1 X h-1), or zinc (0.03 mmol/kg [bolus], 0.03 mmol X kg-1 X h-1). Intraduodenal infusions of calcium, magnesium, and zinc significantly stimulated CCK-33, PP, and gastrin release. Intravenous calcium stimulated CCK-33, PP, and gastrin release to 245, 193, and 155% of basal levels, respectively. Intravenous magnesium increased CCK-33 to 123% of basal levels but did not stimulate PP and gastrin levels. Intravenous zinc stimulated release of CCK-33, PP, and gastrin to 126, 185, and 124%, respectively. This study shows that calcium, magnesium, and zinc can stimulate release of CCK-33, PP, and gastrin in much the same manner. We suggest that these cations may have a nonspecific electrical action that results in an alteration of membrane permeability, which leads to release of gastrointestinal hormones.

Comparison of hepatic elimination of different forms of cholecystokinin in dogs. Bioassay and radioimmunoassay comparisons of cholecystokinin-8-sulfate and -33-sulfate.

The influence of hepatic transit on the ability of exogenous cholecystokinin-8-sulfate and -33-sulfate (CCK-8 and CCK-33, respectively) to stimulate gallbladder contraction and exocrine pancreatic secretion, as well as on the peripheral plasma concentration of each agent, was evaluated in five conscious dogs with pancreatic and gallbladder fistulas and complete portacaval transposition. The gallbladder pressure increments after portal administration of CCK-8 (0.125, 0.25, 0.50, and 1.0 microgram/kg per h for 5 min) were diminished by 36, 45, 39 and 25%, respectively, in comparison with those obtained with systemic administration of identical doses of CCK-8 (P less than 0.05). In a subsequent experiment, the integrated pancreatic juice volume, bicarbonate, and protein secretion were diminished by 22, 32, and 48%, respectively, during a 30-min infusion of CCK-8 (0.10 micrograms/kg per h) into the portal venous system, in comparison with the results obtained with systemic administration of CCK-8 (P less than 0.05). In contrast, the gallbladder pressure and pancreatic exocrine secretory responses to portal administration of CCK-33 did not differ significantly (P greater than 0.05) from the results obtained with systemic administration of CCK-33. Radioimmunoassay for CCK-8 in plasma showed that the integrated CCK-8 value during portal administration was significantly lower (P less than 0.05) than it was during systemic administration. The results for CCK-33, however, did not vary, whether it was given by a systemic or portal route (P greater than 0.05). Thus, the present study demonstrates that CCK-8 is partially inactivated by the liver whereas CCK-33 is not, which suggests that CCK-3 in the circulation may play a significant role in the physiologic regulation of the gallbladder and exocrine pancreas.

[Changes of peripheral lymphocyte subsets in patients with oral squamous carcinoma].

The distribution of peripheral lymphocyte subsets of 32 patients, who had undergone surgical removal of squamous carcinoma and have evaded recurrence was studied using OK series monoclonal antibodies. The T-lymphocyte subpopulation was reduced in patients who were tumor free for more than 3 years. The helper/inducer T-cell subpopulation was reduced in most of the patients studied, whereas little change was observed in the subpopulation of suppressor/cytotoxic T cells. Consequently the ratio of OKT 4+/OKT 8+ was reduced in accordance with the tumor free period.
