[Clinical effects of granisetron and methylprednisolone against nausea, vomiting and anorexia induced by cisplatin].

We investigated the usefulness of concurrent therapy of granisetron with methylprednisolone sodium for on nausea, vomiting and anorexia induced by chemotherapy, including cisplatin, in patients with oral cancer. Group A: 10 patients who were on concurrent therapy of granisetron (3.0 mg/body) with methylprednisolone sodium (750 mg/body) (19 courses). Group B: 13 patients who were given coktail therapy of methylprednisolone sodium (750 mg/body) as the base drug plus metoclopramide (46.7 mg/body on average) and domperidone (82.9 mg/body on average) (21 courses). Efficacy rates for nausea, vomiting and anorexia were 89.4%, 100% and 100% in group A, and 33.3%, 47.6% and 42.9% in group B, respectively. Statistical significance was found with nausea, vomiting and anorexia (p < 0.01). The results suggest usefulness of concurrent therapy of granisetron with methylpredonisolone sodium for cisplatin-induced nausea, vomiting and anorexia.

[A case report of malignant mesothelioma].

A case of successful surgical removal of a primary malignant pericardial mesothelioma was reported. The patient was a 79-year-old man with a complaint of dyspnea at rest. Chest X-ray study showed enlargement of upper mediastinum. Echocardiogram, computed tomography and MRI revealed a giant mass occupying the pericardial space anterior to the heart with marked pericardial effusion. The right ventricle was severely compressed with this tumor. Total resection of the tumor was achieved because the tumor was localized without invading surrounding tissue. The tumor was 17 x 12 x 10 cm in size and 900 g in weight. Rare growth fashion as a distinct isolated mass was a key of successful outcome.

[Successful documentation by coronary angiography of spontaneous simultaneous multivessel coronary spasm in a variant angina patient: a case report].

A 59-year old female who complained of chest discomfort was admitted to our hospital. Electrocardiogram (ECG) on admission and treadmill exercise test showed negative for ischemia. She underwent coronary arteriography. Initial angiography showed there was no significant coronary arterial stenosis. However, when we were preparing the spasm provocation test, she complained of the same kind of chest discomfort as she had felt before. We found that ST segment was elevated in both the anterior and inferior leads on the ECG. Coronary arteriography showed that severe spasm occurred in both the left anterior descending artery (Seg. 6) and the right coronary artery (Seg. 1). Heart rate decreased and electromechanical dissociation occurred. She temporarily lost consciousness. After cardiopulmonary resuscitation, she recovered and the elevation of ST segment returned on the ECG. This is the first case report which has documented spontaneous simultaneous multivessel coronary spasm by coronary arteriography.

[A successful surgical repair of ventricular septal perforation, whose perforated position could be marked by Swan-Ganz catheter before the operation].

The patient was a 72-year-old woman who was admitted with AMI. On the next day, a systolic murmur of Levine II/VI became audible on the left sternal border of the 5th intercostal space and VSP was diagnosed by the echocardiogram. The hemodynamics was comparatively stable, and so we scheduled the elective operation in the expectation of the healing of the infarction tissue. At the cardiac catheterization just before the operation, L-R shunt ratio was 64.9% and main PA was 41/18 (26) mmHg. Before the operation we marked the VSP position by Swan-Ganz catheter through the left ventricle and VSP closure and 2 CABG was performed. We concluded that this method was useful for the diagnosis and therapy of VSP.

Effects of volume loading, propranolol, and heart rate changes on pump function and systolic time intervals of the left atrium in open-chest dogs.

Left atrial (LA) pressure-dimension curve was constructed and the area of A-loop (AAL, in mm.mmHg) which reflected LA stroke work was measured in 14 open-chest dogs before and after saline infusion, in 14 dogs before and after propranolol administration under fixed heart rate and in 12 dogs while atrial pacing rate was increased stepwise. The peak +dP/dt of LA systolic pressure (LA +dP/dt, in mm Hg/s) and LA percent fraction shortening (LA %FS) were obtained as indices of LA pump function. The peak -dP/dt of X descent in LA pressure (LA -dP/dt, in mm Hg/s) was measured as an index of LA relaxation. From pulsed Doppler mitral flow velocities, A-wave acceleration (AC, in cm/s2), LA pre-ejection period corrected by P-wave duration (LA PEPc), and LA ejection time (LA ET) were measured. AAL, LA +dP/dt, AC, LA %FS, and LA -dP/dt increased after saline infusion (AAL; 2.2 +/- 1.6 to 5.1 +/- 3.3 (p < 0.05), LA +dP/dt; 34 +/- 16 to 52 +/- 23 (p < 0.05), AC; 320 +/- 90 to 472 +/- 98 (p < 0.05), LA %FS; 9 +/- 4 to 12 +/- 4 (p < 0.05), and LA -dP/dt; 38 +/- 28 to 60 +/- 32 (p < 0.05)) and decreased after propranolol administration (AAL; 2.9 +/- 1.9 to 1.2 +/- 0.7 (p < 0.05), and LA +dP/dt; 42 +/- 19 to 28 +/- 11 (p < 0.05), AC; 332 +/- 92 to 254 +/- 102 (p < 0.05), LA %FS; 14 +/- 5 to 8 +/- 5 (p < 0.05), and LA -dP/dt; 36 +/- 19 to 26 +/- 18 (p < 0.05)), but did not change significantly during heart rate increments. LA PEPc decreased after saline infusion (114 +/- 11 to 104 +/- 14 (p < 0.05)) and increased after propranolol administration (108 +/- 8 to 129 +/- 13 (p < 0.05)). As pacing rate was increased, LA PEPc and LA ET showed no significant changes. Both AAL and LA +dP/dt correlated significantly with LA PEPc (r = -0.76 and -0.66), but not with LA ET (r = -0.08 and -0.04). In conclusions, LA pump function was augmented by volume loading and decreased by propranolol, but unchanged by heart rate increments. These changes in LA pump function were reflected by LA PEPc, but not by LA ET. LA PEPc is a useful index of LA pump function which is not heart rate dependent.

[A case of spontaneous rupture of the ascending aorta].

We report a rare case of spontaneous rupture of the ascending aorta without any evidence of aneurysm formation or aortic dissection. A woman aged 64 was admitted to our cardiac care unit as an emergency patient with severe chest pain. Her face was pale and systolic blood pressure was 70 mmHg in spite of intravenous administration of dopamine (10 micrograms/kg/min). She had a history of hypertension for two years under good medical control. No trace of the chest trauma was noted before her admission. Physical examination revealed neck vein engorgement and distant heart sounds. Chest X-ray film showed enlargement of the cardiac silhouette. ECG showed no evidence of acute coronary syndrome. Pericardial effusion with a floating hematoma-like mass was detected by 2-dimensional echocardiogram. Pericardiocentesis revealed bloody pericardial fluid (Ht: 26%). Aortagraphy was performed resulting in a clinical diagnosis of acute aortic dissection, but there were no signs of a false lumen, aneurysm formation or extravasation of the contrast medium. Although continuous pericardial drainage was performed, she suddenly lost consciousness, collapsed and died. A longitudinal intimal laceration 5 cm long was observed in the ascending aorta. Pathological examination revealed cystic medial necrosis and irregularity of the elastic fibers in the media. No atheromatous plaque was noted in the intima. Spontaneous rupture of the aorta is a life-threatening condition that requires urgent surgery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Establishment and characterization of a cell line (H-1) from squamous cell carcinoma of human gingiva--effect of temperature].

We established a cell line (H-1) from human oral cancer. It was derived from a biopsy specimen of squamous cell carcinoma in the lower gingiva of a japanese man. The cell line has now been growing in vitro for more than 86 passages. The cells had an epithelial morphology and grew as adherent monolayers. PAS stain was positive and activity of ALP also was positive. They had a doubling time of 37 hours and colony formation of 32%. Chromosomal numbers ranged from 49 to 75 and the modal number was 61. The cell line was tumorigenic in nude mice, its histologic examination showed identification with that from the original tumor. The cells were sensitive to hyperthermia.

[A case of pulmonary alveolar proteinosis--evaluation of a Swan-Ganz catheter in bilateral lung lavage].

Lung lavage, known to be effective for alveolar proteinosis, brings about significant hypoxemia immediately after evacuation of lavage fluid. Impeding blood flow of even a distal branch of a pulmonary artery by using a Swan-Ganz catheter (SGC) successfully prevented this hypoxemia. And this SGC procedure could be safely carried out to make lavage of each lung successively in one day.

[An occlusion and stenosis of the venous drainage system in cerebral AVMs--angiographical investigation].

We investigated angiographically occlusion or stenosis of draining veins in cases of arteriovenous malformations (AVMs). The subjects were 51 cases who were diagnosed as cerebral AVMs by angiography in our hospital between April 1973 and January 1991. Those consisted of 8 dural AVMs, 3 mixed pial-dural AVMs, and 40 pial AVMs. Angiography was stereoscopically done in all cases. Angiograms obtained prior to treatment were retrospectively analyzed. Cases of dural AVMs or arteriovenous fistulae in the cavernous sinus were excluded from the subjects. Eight of 51 cases (16%) had occlusion or stenosis of draining veins; i.e., 2 dural AVMs, 1 mixed pial-dural AVM, 5 pial AVMs. One of 2 dural AVMs was located in the sigmoid sinus, and another involved the superior sagittal sinus. One mixed pial-dural AVM was a vein of Galen aneurysm. Occlusion or stenosis was recognized at the junction of the vein of Galen to the straight sinus (3 cases), in the straight sinus (2 cases), at the junction of the vein of Labbe to the transverse sinus (1 case), in the superior sagittal sinus (1 case), and in the sigmoid sinus (1 case). Two of 8 cases with occlusion or stenosis of draining vein presented with hemorrhage at the site of AVMs. In cases of pial AVMs, occlusion or stenosis of draining vein seemed be related to endothelial changes due to turbulent and irregular flow. On the other hand, in cases of dural AVM with dural sinus occlusion, sinus abnormalities may precede the formation of fistulous communications.(ABSTRACT TRUNCATED AT 250 WORDS)

[Myocardial revascularization with bovine internal thoracic artery graft (BIOFLOW): a case report].

Bovine internal thoracic artery graft (BIOFLOW) was used for coronary artery bypass grafting (CABG). A 64-year-old woman who had undergone triple CABG with saphenous vein five years ago was referred to our hospital again with anginal attack. Coronary angiogram showed the obstruction of the graft for LAD and new stenosis at right coronary artery (RCA). Gastroepiploic artery was not available because the patient had a history of gastric ulcer several times, and saphenous vein had harvested initial operation. We planned re-CABG for LAD with left internal thoracic artery and RCA with BIOFLOW. But at the operation, LAD was not graftable because of severe calcification. So that we performed single CABG to RCA with BIOFLOW. The BIOFLOW was patent at the 28 th postoperative day. From our experience, BIOFLOW can be expected as a graft for CABG when saphenous vein, internal thoracic artery and gastroepiploic artery are not available.

[A case report: surgical repair of bilateral coronary artery-pulmonary artery fistula associated with mitral regurgitation, tricuspid regurgitation and severe renal dysfunction].

The patient was a 72-year-old female who was admitted with evaluation of dyspnea on effort. On cardiac catheterization, coronary angiography showed the fistula from both RCA and LAD to the pulmonary artery and L-R shunt ratio was 37.4%, and MR and TR were found. The preoperative examination showed renal dysfunction, BUN: 61.6 mg/ml, Cr: 21. mg/ml, 24 hr Ccr: 15.2 ml/min. At the operation, the fistula was closed from the inside of the pulmonary artery and MVR (27 SJM) and TAP (Kay-Reed method) were performed associated with the intra-operative hemodialysis. The peritoneal dialysis was used for 9 days after the operation. The hemodynamics and the urination were well controlled. A successful surgical repair of bilateral coronary artery-pulmonary artery fistula associated with valvular disease and severe renal dysfunction was reported.

The effect of thrombin on actin filament and vinculin of corneal endothelial cells.

PURPOSE: The authors examined the effect of thrombin on confluent bovine corneal endothelial (BCE) cells in vitro, especially on cellular integrity and on the redistribution of F-actin and vinculin. METHODS: Immunofluorescent stainings against F-actin and vinculin were carried out on confluent BCE cells, and the effect of thrombin was evaluated. RESULTS: F-actin was distributed at the cytoplasmic peripheries of confluent BCE cells, forming dense peripheral bands (DPB), whereas vinculin was linearly located at the cell borders. Enzymatically active thrombin caused the loss of DPB and an increase of central microfilament bundles, associating the dissociation of vinculin-cell plaques and the formation of intercellular gaps. However, enzymatically inactive thrombin did not induce such changes. The thrombin effect was reversible and occurred in a concentration-dependent manner. The pre-incubation of BCE cells with disrupting agents of microtubules, such as colchicine and demecolcine, or voltage-dependent Ca2+ channel blockers did not affect these thrombin-induced changes, whereas forskolin and energy blockers such as oligomycin AB and C and antimycin A inhibited these changes. CONCLUSIONS: Enzymatic-active thrombin affects the arrangement of the cytoskeletal structure of BCE cells and cell-substratum interaction and plays an important role in the re-integrity or repair processes of the monolayer of BCE cells.

[Blood purification after aorto-coronary bypass grafting for dialysis patients].

Four dialysis patients received aorto-coronary bypass grafting (CABG) at Fukuyama cardiovascular hospital from April 1989 to March 1992. We employed continuous ambulatory peritoneal dialysis (CAPD) in two cases, hemodialysis with extracorporeal ultrafiltration method (ECUM + HD) in one case and hemodiafiltration with continuous veno-venous hemofiltration (CVVHF + HDF) in one case for perioperative management. In each cases, intermittent HD was undergone during 4 days before operation. Intraoperatively, HD was used together with extracorporeal circulation. Blood pressure in the postoperative course changed more frequently in CAPD cases than ECUM + HD or CVVHF + HDF case. Especially, CVVHF + HDF stabilized the movement of circulation and kept balance of serum BUN, Cr after the operation (BUN; 27.2 +/- 6.3, Cr; 5.0 +/- 0.6). We concluded that CVVHF + HDF would be the most favorable method to maintain the fluid balance stably for the uremic patients after CABG.

[Allelic losses of tumor suppressor gene on chromosome 17 in ovarian cancer].

Twenty-one human ovarian epithelial carcinomas (13 serous, 3 mucinous, 2 endometrioid and 3 clear cell type) were examined for allelic losses of p53 tumor suppressor gene, pYNZ22 or pTHH59 on chromosome 17. High-molecular-weight DNA was extracted from ovarian tumor tissues and normal ones obtained from the same patients, and was subjected to Southern blot analysis. The filters were hybridized with p53 cDNA probe or two VNTR probes on chromosome 17(pYNZ22, pTHH59). By comparing the hybridized band pattern of the normal tissue with that of the tumor, loss of heterozygosity (LOH) was detected. Frequent LOHs were detected in 5 of 14 informative cases (36%) on p53 and in 9 of 20(45%) on pYNZ22, but LOH on pTHH59 was observed less frequently than those of p53 and pYNZ22 (one of 7 cases: 14%). These results suggest that allelic loss of p53 may play an important role in the development and/or progression of ovarian carcinomas.

ET-1 released histamine from guinea pig pulmonary but not peritoneal mast cells.

Endothelin(ET)-1 triggered histamine release of mast cells from pulmonary tissue but not from the peritoneal cavity of guinea pigs. The observed difference in response to ET-1 was attributable to a quantitative difference in ET-1 binding sites between both cells. The concentrations of ET-1 required for half maximal release of histamine and half maximal binding of [125I]ET-1 were approximately 0.05 and 0.08 nM, respectively. The release of histamine by ET-1 was a Ca(2+)-dependent but not a cytotoxic process. These observations, taken together, suggest that ET-1 induces histamine release from mast cells in a receptor-dependent fashion.
