Thrombin induced cytoskeletal change in cultured bovine corneal endothelial cells mediated via protein kinase C pathway.

We studied the participation of the protein kinase C pathway in thrombin-induced cytoskeletal alterations in confluent cultured bovine corneal endothelial (BCE) cells. Cultured BCE cells were exposed to alpha-thrombin (0.1-10 U/ml for 15-60 min) and the distribution of F-actin and vinculin plaques was examined using immunofluorescent staining and electron microscopy. Phorbol 12-myristate 13-acetate (PMA, 10 nM for 15 min), the broad spectrum protein kinase inhibitors staurosporine (10 nM) and H-7 (10 nM), and highly specific PKC inhibitor calphostin C (10 nM) were used to evaluate the role of PKC/phosphorylation in this phenomenon. HA-1004 (10 nM) was used as a negative control for these inhibitors. In a parallel experiment, PKC activity of cytosol and membrane of BCE cells was also evaluated. In control samples, F-actin was distributed mainly at the periphery of cells, where it formed dense peripheral bundles; vinculin plaques were also present at the cell boundary. Exposure of BCE cells to thrombin changed the distribution of F-actin and vinculin into a diffuse pattern; a similar alteration was also induced by incubation with PMA. These phenomena were blocked by incubation with H-7, staurosporine and calphostin C. Both cytosolic and membrane PKC activity was increased after 5 to 30 min exposure of alpha-thrombin and returned to the control level after 1 h. alpha-Thrombin induces alteration in the cytoskeleton of BCE cells, and this message is transduced at least in part by PKC dependent pathways. PKC/phosphorylation may thus play an important role in physiological processes that involve alterations of the cytoskeleton.

[Recurrence of pleomorphic xanthoastrocytoma six years after total removal of mural nodule: a case report].

A case of recurrent pleomorphic xanthoastrocytoma (PXA) was reported. A 46-year-old male underwent total removal of mural nodule from cystic tumor in the left temporal lobe in 1986. Histological diagnosis was astrocytoma. There were no abnormal neurological signs or symptoms after the operation. However, 6 years after the operation he complained of strong unpleasant smell as of rotten onions and was admitted to our hospital. MRI revealed a cystic tumor with a mural nodule which was enhanced markedly by Gd-DTPA in the same location as previous tumor site. He was operated upon again and tumor was removed with the cyst wall. Histological diagnosis was PXA. Surgical procedures for removal of cystic PXA has been controversial; removal of tumor only, tumor removal with biopsy of cyst wall, or tumor removal with resection of cyst wall. Real reason for recurrence of this tumor in our case is unknown. However, since astrocytoma cells were found in the biopsy specimen of the cyst wall at time of the first operation, the tumor might have recurred from these cells. Therefore, this time we removed not only the mural nodule but also the cyst wall, because the cyst wall peeled off easily. We would like to propose that PXA cyst walls should be resected, whenever possible, in order to accomplish total removal of tumor.

Bystander effect caused by suicide gene expression indicates the feasibility of gene therapy for hepatocellular carcinoma.

In the field of gene therapy using retroviral vectors, it appears impossible to introduce a foreign gene into all target cells. Therefore adjacent cell killing, the socalled bystander effect, caused by genetically modified cells provides therapeutic advantages for gene therapy against cancers. We retrovirally transduced the herpes simplex virus thymidine kinase (HSV-tk) gene into murine and rat hepatocellular carcinoma (HCC) cells. These HSV-tk gene-transduced HCC cells were cocultured with the corresponding parental cells in the presence of ganciclovir, at a concentration not at all cytotoxic to the parental cells. When parental HCC cells were cocultured with their HSV-tk gene-transduced counterparts at a high density at which most cells were in contact with one another, they were markedly eliminated. Conversely, when cocultured at a low density at which none of the cells were in contact, a weak but statistically significant bystander effect was observed. Addition of lysates of HSV-tk gene-transduced cells in the presence of ganciclovir did not cause and killing of parental cells. Furthermore, media conditioned by transduced cells with ganciclovir exhibited weak cytotoxic effects on parental cells. These results indicate that cell-cell contact plays a major causative role in the bystander effect and that minor contributors to this phenomenon are some cytotoxic substance released from transduced cells. Importantly, the bystander effect was induced in vivo as well as in vitro. When mixtures of transduced and untransduced HCC cells were implanted into the flank region of mice, intraperitoneal ganciclovir administration considerably inhibited tumor development, indicating the feasibility of gene therapy with HSV-tk gene and ganciclovir against HCC.

White matter changes caused by chronic solvent abuse.

PURPOSE: To examine the brain damage of solvent abusers in Japan, where pure industrial toluene is frequently abused. METHODS: Twenty solvent abusers 17 to 33 years of age with 7.2 +/- 4.0 years of abuse were examined with a 1.5-T MR imaging system. RESULTS: White matter hyperintensities in cerebrum, brain stem, and cerebellum on T2-weighted images were found in seven cases. The extent of white matter change was most clearly shown on proton density-weighted images. The patients with restricted white matter change and intermediate white matter change showed white matter hyperintensities in the brain stem and cerebellum on T2-weighted images, in some cases, with additional hypointensities in the corresponding T1-weighted images. These patients had mainly abused pure toluene. The patients with diffuse white matter change showed obvious brain atrophy, including hippocampal atrophy and thinning of the corpus callosum. These patients had mainly abused lacquer thinner. CONCLUSION: There are some patients with restricted but severe enough change to cause the neurologic symptoms in specific regions, such as the brain stem and/or cerebellum, before the brain atrophy becomes apparent. This suggests that the restricted white matter change represents not only an early change of diffuse white matter change, but at least in some cases also represents a qualitatively different change than that of diffuse white matter change. We suggest that pure toluene has a possible relation to this qualitative difference.

Effect of tecogalan sodium on angiogenesis in vitro by choroidal endothelial cells.

PURPOSE: To examine the possible inhibitory effect of tecogalan sodium, derived from bacteria, on three important components of in vitro angiogenesis (endothelial proliferation, migration, and tube formation in a collagen gel) using bovine choroidal endothelial cells (CECs). METHODS: The effects of tecogalan sodium (1, 5, 25, 125, and 250 micrograms/ml) on cultured CECs were examined when basic fibroblast growth factor (bFGF, 10 ng/ml), vascular endothelial growth factor (VEGF, 50 ng/ml), a combination of bFGF (10 ng/ml) and VEGF (50 ng/ml) (bFGF/VEGF) and 10% fetal calf serum (FCS) were used as angiogenic stimulants. For the proliferation assay, CECs were cultured and the cell numbers counted on days 1, 3, and 5. For migration assay, CECs were seeded in the upper half of a Boyden chamber while an angiogenic growth factor was loaded in the lower half. After 6 hours of incubation, cell migration was evaluated by counting the numbers of migrated cells per microscopic field on the lower side of the filter. For the tube-forming assay, CECs were seeded in a type I collagen gel, and the length of the tube-like structures (an indicator of angiogenesis) formed by CECs per microscopic field was quantified by image analysis. The effect of neutralizing antibody for bFGF also was tested in these three assays. RESULTS: All tested angiogenic stimulants induced CEC proliferation. The stimulatory effect of bFGF and bFGF/VEGF was reduced by tecogalan sodium (IC50 for bFGF effect, 26.1 micrograms/ml). However, the effect of VEGF and of 10% FCS was not altered by low doses of tecogalan sodium (< 25 micrograms/ml). Chemotaxis of CECs was stimulated by bFGF alone and by bFGF/VEGF, and this effect was inhibited by tecogalan sodium (IC50 for bFGF, 3.2 micrograms/ml). Stimulation of chemotaxis by VEGF alone and by 10% FCS was not affected by tecogalan sodium in low doses but was inhibited by high doses. Tube formation was stimulated by administration of each of the factors. Stimulation of tube formation by bFGF and by bFGF/VEGF was inhibited by tecogalan sodium (IC50 for bFGF, 18.2 micrograms/ml). High doses of tecogalan sodium (125 and 250 micrograms/ml) also inhibited 10% FCS-induced proliferation, migration, and tube formation. CONCLUSION: bFGF, VEGF, and a combination of bFGF and VEGF stimulated proliferation, migration, and tube formation by CECs in vitro. These stimulatory effects, but especially those of bFGF, were inhibited by tecogalan sodium. If tecogalan sodium can be shown to have a similar effect in vivo, it might have the potential for pharmacologic control of subretinal neovascularization.

[Two cases underwent patch closure and mitral annuloplasty for ventricular septal perforation and mitral regurgitation caused by acute inferior myocardial infarction].

Two cases underwent successful operation for the ventricular septal perforation (VSP) and mitral regurgitation (MR) caused by acute myocardial infarction (AMI) are reported. Patch closure of VSP and mitral annuloplasty (MAP) by Kay-Suzuki method were performed. Location of VSP were posterior ventricular septum and moderate MR were detected in both cases. Case 1 was a 74-year-old man. Four weeks after AMI, patch closure of VSP through right ventricular approach and MAP through trans-atrial approach and also aorto-coronary bypass to left anterior descending artery were performed. Doppler echocardiography was effective to evaluate the severity of VSP and MR. Case 2 was a 69-year-old man. Eight days after AMI, patch closure of VSP and MAP were performed through biventricular approach. Postoperative examination revealed the disappearance of left-to-right shunt and MR in both cases. These cases operated successfully were thought to be rare.

[Detection of IgE antibody to Pityrosporum orbiculare in patients with atopic dermatitis].

A lipophilic yeast, Pityrosporum orbiculare, found in normal microflora of the human skin, is considered to be a pathogenic allergen of atopic dermatitis (AD). We measured IgE antibody to P. orbiculare using a CAP RAST FEIA kit in 44 patients with AD (AD alone 17, AD+bronchial asthma (BA) 27). The incidence of positive RAST was 88.2% in the patients with AD alone and 74.1% in the patients with AD+BA. The IgE RAST index was 2.77 +/- 1.34 (mean +/- SD) in severe AD patients (n = 23), which was significantly higher than that in mild AD patients (1.88 +/- 1.41, n = 21). The patients with eczematous skin on the face, neck and scalp (n = 27) showed higher IgE titers (2.79 +/- 1.26) than those without skin lesions (1.64 +/- 1.44, n = 17). These results suggest that P. orbiculare is a pathogenic allergen of severe AD. IgE levels for P. orbiculare showed a statistically significant relation to those for Candida albicans (r = 0.62, p < 0.01). In RAST inhibition tests on five patients' sera, C. albicans extract significantly inhibited IgE RAST to P. orbiculare in only one serum, indicating that P. orbiculare shares some IgE binding epitopes with C. albicans, but that P. orbiculare-specific epitopes also exist.

[Vasoreactivity of normal brain tissue in cases of arteriovenous malformations--evaluation by 123I-IMP SPECT using acetazolamide].

Arteriovenous malformations (AVMs) are usually associated with diminished cerebral blood flow (CBF) in the parenchyma surrounding AVMs because of the steal phenomenon. Vasoreactivity in the parenchyma surrounding AVMs may change because of ischemia and abnormal hemodynamics. The aim of this study was to investigate vasoreactivity in parenchyma surrounding AVMs. Quantitative single-photon emission computed tomography using N-isopropyl-p-I-123 iodoamphetamine (IMP-SPECT) was performed before and after intravenous administration of acetazolamide (Diamox). Diamox is a potent cerebral vasodilator which causes tissue acidosis, and IMP-SPECT was performed 10 min after intravenous infusion of a 1g dose. The subjects were 8 patients with AVMs. The AVM was in the frontal lobe in 3 patients, the temporal lobe in 1 patient, the lateral ventricle in 1 patient, the midbrain in 1 patient, the basal ganglionic region in 1 patient, and the vein of Galen in 1 patient. A rotating gamma camera with two heads was used to perform SPECT. IMP was injected into an arm vein, and a 5-min period of blood withdrawal, 1 ml/min, was started simultaneously from a small catheter placed in the radial artery. Kuhl's method was used to quantify cerebral blood flow (CBF). Multiple contiguous 6-pixel-diameter region of interest (ROI) circles along the cerebral cortex and cerebellar hemisphere were used, and ROIs which included the AVM nidus, draining veins or infarctions were excluded from the study. Vascular territories containing arteries feeding the AVM were defined as near-ROIs on SPECT, and all other ROIs were defined as far-ROIs. (ABSTRACT TRUNCATED AT 250 WORDS)

[Clinicopathological study on atherosclerosis of the right gastroepiploic arteries].

We evaluated the extent of atherosclerosis of the right gastroepiploic arteries (RGEAs) used as a graft material in coronary bypass grafting for ischemic heart diseases, with analyzing the presence of arteriosclerotic lesions and the site of predilection and studying their relationship to clinical risk factors of ischemic heart disease. The subjects were 32 cases, 18 males and 14 females, ranging in age from 32 to 80 years (mean 61 years), in which gastrectomy was performed for early gastric cancer. RGEA 20 cm in length was taken from the junction of the gastroduodenal artery of the stomach and divided into A, B, C and D at intervals of 5 cm proximally, and each part was subdivided into the length of about 5 mm to make ultra-thin sections for HE and Elastica van Gieson staining. The area of intima (I), the area of media (M) and circumference of media (L) were calculated. The arterial section was transformed to a hypothetical state in which the internal elastic lamina was stretched to make an exact circle. The degree of stenosis "I" was expressed by the following formula. [formula: see test] The maximum value of "I" in the sections A, B, C and D in individual cases was designated as Ar, Br, Cr and Dr, and the maximum value of "I" among the four sections in each case was defined as R. Atherosclerosis was graded according to R value as follows. I; R < 25%, II; 25% < or = R < 50%, III; 50% < or = R < 75%, IV; 75% < or = R.(ABSTRACT TRUNCATED AT 250 WORDS)

A new model of subretinal neovascularization in the rabbit.

PURPOSE: To establish a new model of subretinal neovascularization (SRN) in the rabbit by implanting basic fibroblast growth factor (bFGF)-impregnated gelatin microspheres beneath the retina. METHODS: Basic fibroblast growth factor-impregnated gelatin microspheres were prepared by forming a polyion complex between gelatin and bFGF. The microspheres, containing 2.5 micrograms of bFGF, were injected into the subretinal space of rabbit eyes (n = 29). Control eyes (n = 10) received bFGF-free gelatin microspheres. Eyes were followed up for 3 days to 8 weeks by ophthalmoscopy, photography, fluorescein angiography, light microscopy, and transmission electron microscopy. RESULTS: Twenty of 24 experimental eyes (83%) showed fluorescein leakage from SRN 2 weeks after implantation of the bFGF-impregnated microspheres. This leakage continued for 2 to 6 more weeks. In striking contrast, control eyes showed no fluorescein leakage. Histologic examination revealed SRN in all the experimental eyes but in none of the control eyes. CONCLUSIONS: Subretinal implantation of bFGF-impregnated gelatin microspheres induces reproducible SRN in the rabbit.

MR in temporal lobe epilepsy: early childhood onset versus later onset.

PURPOSE: To study the relationship between the MR findings and the clinical features in temporal lobe epilepsy in childhood (less than 10 years of age). METHODS: MR studies were performed with a 1.5-T imager on 38 temporal lobe epilepsy patients receiving drug therapy at the psychiatric department. These patients were divided into two groups according to their age at onset (10 years or less, 11 years or more). The two groups were compared in terms of the MR findings and clinical features. RESULTS: The 11 younger-onset patients included 5 with a high-signal area attributed to mesial temporal sclerosis. Clinically, all of these 5 patients had a history of "complex" febrile convulsions, which sharply distinguished them from the older-onset group. CONCLUSION: The analysis suggests that complex febrile convulsions in infancy can be associated with high-signal areas on MR attributed to mesial temporal sclerosis.

[Surgical repair of valvular diseases in patients over 75 years of age].

From January 1985 through August 1993, 41 patients older than 70 years underwent valvular surgery. In them, 10 patients were older than 75 years old. We evaluated those operative results. First, we classified them into 3 groups; Group A: over 75 y.o. (10 cases), Group B: 70-74 y.o. (31 cases) and Group C: 60-69 y.o. (39 cases, from January 1992 through August 1993). Surgical deaths were 1 case in Group A, and 1 in Group B. Distant deaths were 1 case in Group B, and 2 in Group C. Preoperative cardiac catheterization data showed the progression of cardiac failure in proportion to aging. But postoperative data showed no difference among 3 groups, and the condition was reversible. NYHA classification and cardiothoracic ratio (CTR) showed the same results. In summary, in valvular diseases in patients over 75 years of age, the progression of the condition was recognized but it was reversible. So we conclude that the surgical repair of no influence of age is necessary.
